Weisheng San (Hygiene Powder), Buzhong Yiqi Tang (Decoction for Replenishing the Middle and Augmenting Qi), Shiquan Dabux Tang (Decoction for Perfect Tonification of All Deficiencies), Shenqi Wan (Kidney Qi Pill), Bazhen Tang (Eight Treasures Decoction), Ligong Dingzi (Ligong Paste), Gan Zhu Dan (Crimson Pearl Pill) (all see Volume 1), Rongwei Fanhun Tang (Decoction for Returning Ying and Wei to the Soul) (see Chapter on Swelling and Toxins), Bawei Xiaoyao San (Eight-Ingredient Free Wanderer Powder) (see Chapter on Scrofula), Badingsan (Boil-Pulling Powder) (see Chapter on Carbuncles), Wuxiang Liuqi Yin (Five-Aromatic Flowing-Qi Drink) (see Chapter on Flowing Abscesses).
Meridians: The dorsum of the foot belongs to the Liver, Gallbladder, and Stomach meridians. The great toe belongs to the Liver and Spleen meridians. The second and third toes both belong to the Stomach meridian. The fourth toe belongs to the Gallbladder meridian. The little toe belongs to the Bladder meridian.
Dorsal Foot Inflammation: Also known as "Foot Back Inflammation." It occurs at the convergence of the Liver and Stomach meridians, caused by damp-heat pathogens sinking downward due to deficiency. Initially, Weisheng San should be used, with Mugua (Papaya), Binglang (Areca Seed), Zisu (Perilla Leaf) etc., added as guiding herbs. In robust individuals, it is formed by wind-phlegm and heat accumulation. Niuhuang Jinhudans (Ox Gallstone and Tiger Bone Pills) should be used, followed by Shiquan Dabux Tang (Decoction for Perfect Tonification of All Deficiencies) and Tuoli Wenzhong Tang (Decoction for Supporting the Interior and Warming the Middle). Yang abscesses occurring on the dorsum of the foot are treatable if the pus is thick; they are difficult to treat if the pus is thin. If purple depression and black blood appear, it is untreatable.
Abscess of the Sole: Also known as Yongquan Carbuncle. Fever at the Yongquan acupoint in the early stage is its sign. It is caused by Kidney Yin deficiency. Garlic-separated moxibustion should be applied, Huoming Yin (Life-Saving Drink) taken, accompanied by Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). If exterior or interior syndromes exist, sweating or purging methods should be used appropriately. Once suppuration has occurred, large doses of Shiquan Dabux Tang (Decoction for Perfect Tonification of All Deficiencies) and Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill) should be administered. Smoke from old sheep horn lamps should be used to fumigate the area. If black depression appears without swelling, no foul pus discharges, irritability and thirst occur, vomiting and regurgitation happen, dysuria with pain persists, pain penetrates the bone, and the pulse is faint or intermittent, this indicates a failure of the Three Yin meridians and is untreatable.
Gangrene of the Toe: Occurs on the great toe of the foot and also the thumb of the hand. It begins as a yellow blister, then resembles a cooked red date. Over time, black gas spreads, potentially affecting all five toes. It is caused by excessive consumption of rich foods and hot medicinal pills containing cinnabar and sulfur. This toxin accumulates in the bone marrow. Survival is possible if there is no purple-black discoloration; treatment is feasible if the joint is not involved. If blackness spreads to all five toes and ascends to the dorsum of the foot, with emaciation, tendon retraction, severe pain, and foul odor, death ensues. If the lesion is in the flesh, it should be excised; if on the toe, amputation is required. The goal is to allow tendons to emerge with the bone to drain the toxin, causing minimal pain. If one waits for the toxic tendons to break internally, even after bone removal, ulceration will persist, rendering it untreatable. Some die because they cannot bear to harm the body, which is even more tragic. However, treatment decisions must not be made unilaterally; consultation with others and consideration of the patient's wishes are necessary, as the disease progression is variable.
Amputation Method: Before the spread, wrap the proximal interphalangeal joint of the affected toe with about ten strands of hair, tying it tightly. Place Chansu Pie (Toad Venom Cake) on the initial rice-grain-sized lesion and apply moxibustion until the tissue necroses. The next day, if the toe is entirely black, use a sharp knife to sever it at the joint line. If bleeding occurs, stop it with Jindao San (Golden Knife Powder). Apply Ligong Dingzi (Ligong Paste) to reduce swelling. If black gas remains, apply powdered Chansu and cover with ointment. The black gas will recede, and pus will form. Use Honghei Er Gao (Red and Black Ointments) to promote granulation and wound closure according to standard methods. Internally administer herbs that nourish the Kidney, strengthen the Spleen, calm the Spirit, tonify Qi, and nourish Blood. Treatment is successful in three out of ten cases if there are no internal complications or external mixed infections. If after amputation, black gas spreads further, swelling and pain worsen, there is foul odor without pus, dry mouth, stiff tongue, coma, and anorexia, death occurs. Moxibustion on relevant acupoints may help. Cases caused by nail trimming injuries, bites, frostbite, or needle pricks in women require etiology-specific treatments. These resemble true lesions but are not caused by specific acupoint pathologies.
Tun Ju (Dun Abscess): Only the great toe suffers from Gangrene; other toes suffer from Tun Ju, also called Toe Inflammation. Symptoms include swelling, pain, and redness. Spontaneous ulceration indicates deficient Yuan Qi and abundant damp-toxin. Apply garlic-separated moxibustion or Yangsui Dingzi (Yang Mirror Paste) moxibustion, and apply Ligong Dingzi (Ligong Paste). Administer Wuyou San (Carefree Powder) and Tuoli San (Interior-Supporting Powder). For ulcerated cases, use Shiquan Dabux Tang (Decoction for Perfect Tonification of All Deficiencies) and Jiaban Bawei Wan (Modified Eight-Ingredient Pill). Moxibustion on the ankle bone tip is beneficial. If thirst precedes pain, or pain precedes thirst, and the area is black, non-ulcerated, and painless, it indicates Spleen damage from rich diet and Kidney damage from sexual excess, with exhaustion of Essence and Qi. It is untreatable, though prior methods may occasionally yield survival.
Onychocryptosis (Ingrown Nail): Caused by nail trimming injuring the skin or long nails invading the flesh, obstructing Qi and Blood circulation. Prolonged obstruction leads to rot, ulceration, blister formation, or hyperplastic tissue wrapping the nail, causing swelling and pain. This is a muscle disorder, not following meridians, and not visible in pulse diagnosis. Generally, removing the nail alone can cure it without medication. Soak in Chenpi Decoction (Tangerine Peel Decoction), insert folded cotton wool under the nail gradually. When the nail lifts, cut it off with scissors. Apply Wubei San (Galla Powder).
Heel Carbuncle: Occurs below the heel bone, also known as Rabbit Bite, resembling a rabbit bite. This area is the source of the Du Meridian and passes through the Kidney Meridian. If the wound does not heal, Qiao Meridian Qi cannot generate, and Kidney Qi leaks. Major tonification is essential, otherwise it becomes a lifelong ailment. Diffuse swelling with chills and fever, fatigue, and poor appetite indicate Spleen Qi sinking; use Buzhong Yiqi Tang (Decoction for Replenishing the Middle and Augmenting Qi). Evening fever with pain and unclear head/eyes indicate Spleen deficiency with Yin Fire; use Buzhong Yiqi Tang combined with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). Rising phlegm-saliva and mouth ulcers indicate Kidney Water depletion; use Buzhong Yiqi Tang combined with Jiaban Dihuang Wan (Modified Rehmannia Pill). All aim to nourish the source of transformation. Externally wash with salt water and sprinkle Baizhu Powder (Atractylodes Macrocephala Powder), changing every two days.
Li Yong (Severe Carbuncle): Occurs on the little toe, extending to the side of the foot. The shape is small.
Si Yin (Four Licentious Ones): Occurs on the upper and lower parts of the foot. The shape is a large carbuncle.
Differentiate these two conditions like Gangrene; treat them like Tun Ju. Additionally, for swollen, itchy, weeping toe webs, hot itching soles, emaciation, thirst, and expectoration, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) and Buzhong Yiqi Tang (Decoction for Replenishing the Middle and Augmenting Qi) to nourish the source. For hard, white, non-ulcerated swelling caused by Yin deficiency attacked by external cold, use Dafangfeng Tang (Great Saposhnikovia Decoction). Attacking only the toxin rarely results in survival. For toe web rot extending to the sole, with thick skin peeling off, superficially shallow but deeply painful, boil four taels of white bark from mulberry roots growing eastward in raw white wine, drink the decoction, and follow with alcohol to intoxication.
Stinking Snail Ulcer: Occurs between toes, presenting as white blisters that rot, itch, hurt, and weep. Severe cases involve chills, fever, and foot dorsum swelling. Caused by damp-heat sinking from the Stomach Meridian. Wash with Gancao Tang (Licorice Decoction), apply crushed fine tea, or burn Goose Palm Skin to ash and apply. Alternatively, apply raw Tung Oil. Severe cases use Chansu Pie (Toad Venom Cake). After three days, change to Zhenzhu San (Pearl Powder) and apply Jinhuang San (Golden Yellow Powder) to swollen areas.
Niu Cheng Jian (Ox Journey Lameness): Heel swelling with hard skin and flesh, caused by walking barefoot in water after heating feet and exposing to wind, leading to Qi stagnation and Blood exhaustion. Initially, place a brick in a basin, pigeon dung on the brick, cover with a basket, and step on the basket. Pour boiling water alongside to steam and soak. Replace when cool. Alternatively, heat a new brick, pour leek juice and vinegar on it, and step on it to fumigate. Puncture internal abscesses, cut cracked hard skin, apply Niujiao San (Ox Horn Powder) for draining pus, Jiangzhu Gao (Crimson Pearl Ointment) for non-healing flesh, and Zhenzhu San (Pearl Powder) for non-healing skin.
Tu Li (Earth Chestnut): Occurs beside the heel, yellow and swollen like glass, without pus. Caused by rough walking damaging bones and tendons. Use Huoming Yin (Life-Saving Drink). Puncture when pus matures.
Cold Ding (Cold Thorn): Occurs on the heel. Begins as purple-white blisters, penetrating bone pain, progressing to purple-black rot, deep holes bleeding fluid, foul odor, and chronic non-healing. Use Shendeng Zhaozhao (Divine Lamp Illumination) and tie Tie Fen San (Iron Powder Powder).
Hong Kong Jiao Chuang (Athlete's Foot Ulcer): Ulcers between foot and knee, caused by Kidney deficiency and Wind-Damp interaction. Progressively increases swelling, pain, yellow discharge, fever, and chronic non-healing. Take Xijiao San (Rhinoceros Horn Powder), wash with Lulutang (Reed Catkin Decoction), apply Longgu San (Dragon Bone Powder).
Tian Luo Pao (Snail Blister): Appears suddenly like fire burning on the foot, generating purple, white, or yellow blisters with distending pain. Caused by Wind-Heat in the Spleen Meridian. Take Jiedu Xiapi Tang (Detoxifying and Draining Spleen Decoction). Externally puncture and apply plaster.
Mei Ci (Flesh Thorn): Commonly known as Corn. Break with a needle, dissolve 5 fen of Chansu (Toad Venom) in soup, mix with 1 qian of Lead Powder, apply and bandage. Alternatively, trim thick skin with a knife, apply Pufferfish Gall on paper. Repeat 2-3 times to prevent recurrence. Can also be dried and stored for use.
