The Five Zang organs can all develop Ding sores (Ding ulcers), which manifest on the body and require careful examination. Regarding the division of Yin and Yang into upper and lower, to know the meridian and organ involvement, one must distinguish the location.
Heart Meridian Ding is named Flame Ding (Flame Ding), often occurring in the lips and fingers. When the heart feels vexed, the spirit becomes confused; pain is accompanied by numbness, itching, and yellow-red discoloration.
Toxin affecting the Liver Meridian is named Purple Swallow Ding (Purple Swallow Ding). This condition mostly appears on tendons and bones. Upon rupture, it bleeds fluid and rots the tendons; the fingers turn cyan, the tongue becomes stiff, and the spirit is chaotic.
Yellow Drum Ding (Yellow Drum Ding) arises from Spleen toxin. It mostly occurs at the corners of the mouth and zygomatic bones. The color is yellow and glossy, surrounded by red, accompanied by numbness, itching, stiffness, and vomiting.
Toxin affecting the Lung Meridian is named White Blade Ding (White Blade Ding). The white top is hard, and the root protrudes sharply. It easily rots and sinks, often damaging the cheeks, with coughing up of phlegm and severe shortness of breath.
Historically, Black Crust Ding (Black Crust Ding) originates from the Kidney Meridian. It presents as black spots, purple and hard like a nail. The toxicity is extreme, causing pain radiating to the bones, palpitations, deep coma, and exposed eyeballs.
The above five Dings correspond to the Five Zang organs. There is also Red Thread Ding (Red Thread Ding), which initially resembles a small sore that gradually turns red. It is most dangerous if the red thread attacks the heart.
In treating Ding conditions, early intervention is crucial. One must understand the Three Yin and Three Yang meridians. Moxibustion is suitable for the lower part, while acupuncture is suitable for the upper part. Prompt treatment leads to recovery; delayed treatment makes preservation difficult.
[Note] These conditions are collectively called Ding. "Ding" refers to the shape of a nail: small in form but deep in root, capable of arising anywhere. It results from indulging in rich flavors, poisoning from snakes or gu, poisoning from epidemic-dead cattle, horses, pigs, or sheep, or contracting abnormal epidemic qi from the four seasons.
Ding sores belong to Fire syndromes and are rapid diseases. Some die within the same day or night; some die immediately upon onset. If not dead within three to five days, death is inevitable within a month or half a month. This is due to disharmony of the Zang-Fu organs, sudden changes in temperament, seasonal cold/heat/severity, and varying depths of toxicity. If treated late at the initial stage, life and death are decided instantly.
Flame Ding (Flame Ding) mostly occurs in the lips, mouth, and between finger joints of the palms. Initially, a small red-yellow spot appears with pain, itching, numbness, and anesthesia. Severely, alternating chills and fever occur, with restlessness, stiff tongue, and slurred speech. This is caused by Fire-toxin of the Heart Meridian.
Purple Swallow Ding (Purple Swallow Ding) mostly occurs between the hands, feet, waist, ribs, and tendons/bones. It turns purple upon onset, bleeding fluid the next day, and rotting tendons and bones after three days. Severely, eyes turn red, nails cyan, vision is distorted, spirit is comatose, sleep-talking and fright occur. This is caused by Fire-toxin of the Liver Meridian.
Yellow Drum Ding (Yellow Drum Ding) initially presents as yellow, bright and moist, surrounded by red. It mostly occurs at the mouth corners, cheeks, zygomatic bones, upper and lower eyelids, and temples. Onset brings numbness and itching. Severely, nausea and vomiting occur, limbs feel wooden and painful, alternating chills and fever, thirst, dry retching. This is caused by Fire-toxin of the Spleen Meridian.
White Blade Ding (White Blade Ding) initially presents as white, with a hard top and protruding root, leaking fatty fluid upon rupture, with both itching and pain. It mostly occurs in the nostrils and hands. It easily rots and sinks. Severely, cheeks are damaged, throat scorched, coughing up phlegm, nasal flaring, and shortness of breath. This is caused by Fire-toxin of the Lung Meridian.
Black Crust Ding (Black Crust Ding) mostly occurs in ear canals, tooth gaps, chest, abdomen, and remote areas of the waist and kidneys. It initially presents as black spots, purple and toxic, spreading through skin to muscles, hard like a Ding, with pain penetrating the bone marrow. Severely, hands and feet turn cyan, palpitations and deep fatigue occur, soft sinking with deep holes, and eyeballs protrude. This is caused by Fire-toxin of the Kidney Meridian. The above five Dings originate from the Five Zang organs.
There is also Red Thread Ding (Red Thread Ding), appearing on palms and joints. Initially resembling a small sore, it gradually develops red threads attacking the arm, causing alternating chills and fever, and severely nausea/vomiting. Delayed treatment allows the red thread to attack the heart, often causing harm.
There is Dark Ding (Dark Ding). Before manifestation, the axilla becomes firm and swollen without a head, followed by swelling of the scrotum and testicles, protruding like a tendon knot, causing alternating chills, fever, stiffness, heat, and pain.
There is Internal Ding (Internal Ding). It initially presents with alternating chills, fever, and abdominal pain. After a few days, a lump suddenly swells up, resembling an accumulation.
There is Wool Ding (Wool Ding). The body develops alternating chills and fever, resembling Taiyang syndrome. However, there are red spots on the anterior and posterior chest, resembling rashes. Viewing the spots, purple-black indicates old severity, while pale red indicates mild severity.
For these conditions, initially服用 Chan Su Wan (Toad Venom Pill) to induce sweating. If toxicity remains and aversion to cold with high fever persists,服用 Wu Wei Xiao Du Yin (Five-Ingredient Decoction for Eliminating Toxin) to induce sweating. If fever, thirst, constipation, and deep-solid pulse exist, the pathogen is in the interior;服用 Huang Lian Jie Du Tang (Coptis Decoction for Resolving Toxicity) plus 1.5 qian of raw Da Huang (Rhubarb Rhizome) and five green onion heads to clear heat. For mild cases,服用 Hua Ding Nei Xiao San (Ding-Resolving Internal Dissipation Powder). If Ding-toxin is about to spread internally (Zouhuang), urgently服用 Ding Du Fu Sheng Tang (Ding-Toxin Revival Decoction). If Zouhuang has occurred, causing mental confusion, urgently use Qi Xing Jian Tang (Seven-Star Sword Decoction) to rescue. If extremities are cold and six pulses vanish suddenly, this is due to toxin blocking Yuanqi circulation; first服用 Chan Su Wan, then follow with Mu Xiang Liu Qi Yin (Aucklandia Flowing-Qi Decoction) to move Qi, and the pulses will reappear. If Ding-toxin is mistakenly moxibusted, causing restlessness and delirium, this is forced toxin attacking inward;服用 Jie Du Da Qing Tang (Detoxifying Daqing Decoction). If after ulceration residual toxin remains causing five-palm heat,服用 Ren Shen Qing Shen Tang (Ginseng Calming-Spirit Decoction). If Qi deficiency causes palpitations during pus discharge post-acupuncture,服用 Nei Tuo An Shen San (Internal Support Spirit-Calming Powder). If purgation was excessive causing thirst and large-deficient pulses,服用 Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). If after sweating, sweat does not stop, fever does not recede, sore is painless, and bowels are unsmooth, this is interior deficiency;服用 Ba Zhen Tang (Eight-Treasure Decoction) plus Huang Qi (Astragalus Root) and Mai Dong (Ophiopogon Tuber). Generally, tonification should not be used early after Ding ulceration; even if true deficiency is seen, only mild tonification is allowed, avoiding warm-tonifying herbs.
External treatment with medicines, acupuncture, and moxibustion must follow the sequence. Books state: Ding sores first require bloodletting, internal toxins should be released via sweating and purging, moxibustion is forbidden but acupuncture is allowed, fear cotton but not iron. Early treatment yields ten cures out of ten; slightly delayed yields five or six; missed treatment results in eight or nine failures.
For Ding starting above the neck, the Three Yang meridians receive the toxin. Use a Pi needle (scalpel-like needle) to pierce the center of the sore 4-5 fen deep,挑断 (cut off) the Ding root to release foul blood. Immediately insert Li Ma Hui Ding Dan (Horse-Returning Ding Pill) or a Chan Su strip into the hole, covered externally with Ba Gao (Ba Ointment).
For Ding starting below the neck, the Three Yin meridians receive the toxin. Apply moxibustion to suppress its momentum. If moxibustion causes no pain, still puncture to bleed, insert Chan Su strip, and apply Li Gong Ding (Li Palace Paste) to surrounding swelling.
If surrounding swelling is stubborn and immobile, stab the hard area randomly with a needle to release much foul blood, otherwise Zouhuang (internal spread) will inevitably occur.
Picking method: First use a dry needle to scrape away the scorched skin at the toxin peak. Insert needle into the Ding root. Hardness like a needle indicates favorable prognosis; softness like melon pulp with no pain indicates unfavorable prognosis (almost fatal).
When picking the Ding root, purple-black blood comes out first. Continue picking until fresh blood appears and pain is felt; stop here. Fill with Ba Ding San (Ding-Pulling Powder) and cover with Wan Ying Gao (Ten-Thousand-Response Ointment). After 3-4 hours, remove old medicine and apply new. If medicine dries without water and causes no pain, the root was not cut; pick deeper until pain is felt and liquid flows. After 3-4 days, when the sore top is dry, apply Hu Po Gao (Amber Ointment) to push out the root, sprinkle Jiu Yi Dan (Nine-One Powder), smear Huang Lian Gao (Coptis Ointment), and cover with Bai Gao Yao (White Plaster) to generate flesh and close the wound.
If initial treatment was missed, or sexual exhaustion/emission damaged Qi, causing toxin to attack inward and Zouhuang to persist, the sore will collapse. Urgently locate the Zouhuang area along the meridian. Find a single erect thorn-like projection, which is the Ding seed. Urgently prick with an iron needle to release foul blood, then apply three moxa cones at the pricked site to vent residual toxin.
If body and face are diffusely swollen, spirit is confused and irritable, dry retching, thirst, and whole-body convulsions occur, these are all unfavorable signs.
Only for Red Thread Ding, at onset, urgently use a porcelain needle to cut the end of the red thread to bleed. Find the original sore, pick it open, insert Chan Su strip, cover with Wan Ying Gao, and orally administer Huang Lian Jie Du Tang.
For Dark Ding and Internal Ding, do not use picking. First dissolve Chan Su Wan in the mouth completely, rinse with cold water to remove toxic saliva. Then chew three pills with 3 cun of green onion white, wrap medicine, swallow with yellow wine, cover and lie down to sweat. If no sweat shortly, drink hot wine to induce it. If still no sweat, it is toxin-heat stagnation; urgently use Pi Li Huo (Thunderfire) method to induce sweating, resolving toxin-heat. Follow with Shuang Jie Gui Jin Wan (Double-Resolving Noble Gold Pill) to purge.
If Dark/Internal Ding starts with locked jaw, use 3-5 Chan Su Wan, grind with green onion head decoction, and force-feed. Wait for slight recovery, then treat as before.
For Wool Ding, first prick purple-black spots with a clothing needle to extract wool-like strands. Prick several spots on front and back chest. Apply powder of black beans and buckwheat. Sweat immediately ensues and cure follows.
Another method: Use 2 qian of refined Realgar powder wrapped in blue cloth, dip in hot liquor, rub on anterior chest from outside inwards. The "wool" moves to the posterior chest. Rub again to extract all wool onto the cloth. Bury the cloth. Avoid tea and water for one day.
Regarding locations and appearances of Dings, urgency varies. Those on head/neck/chest/back are most urgent; those on hand/foot joints are slightly less urgent. A small sore appearing besides one Ding is called Ying Hou (Responding Sentinel); red swelling around without spreading is called Hu Chang (Protecting Field); many small sores around are called Man Tian Xing (Sky-Filling Stars). Presence of these indicates slower progression; absence indicates urgency.
From onset to 4-5 days, if color changes from white to cyan-purple, Ding head suppurates resembling a honeycomb, and internal Seven Evils signs are absent, it is favorable. If initially resembling but not Ding, gray sunken top like fish navel or silkworm spots, cyan-purple-black, soft/sunken without pus, and internal Seven Evils appear, it is unfavorable.
All Ding toxins arise from Fire-toxin. Avoid acrid-hot herbs fearing they aid evil; avoid cold-cooling topical applications fearing they force toxin inward. Plasters should not be applied early; only apply when about to ulcerate or after ulceration to drain pus, generate flesh, and avoid wind-cold. At initial ulceration, avoid flesh-generating drugs fearing toxin remains and increases ulceration.
Dings above the neck belong to Three Yang meridians and are unsuitable for moxibustion. Dings born on Fire days are also forbidden for moxibustion; violation may cause inversion or Zouhuang.
Avoid pepper, alcohol, chicken, fish, seafood, goose meat, pig head, spicy/cold foods. Avoid anger, sexual activity, fragrant substances, mourning clothes, menstruating women, monks/nuns, chickens/dogs. Violation causes recurrence; be cautious.
