(Attached: Leg Pain, Huantiao Abscess, Hong Kong Foot [Gouty Arthritis], Heel Abscess, Leg Carbuncle)
If the pulse at the cun position is tight and powerless accompanied by pain, employ the supporting method (Tuo Fa).
If swelling is hard and pain is severe, apply garlic-separated moxibustion, and further administer internal supporting formulas.
If pus does not form but there is pain, prioritize detoxification.
If pus does not form, prioritize internal supporting methods.
If it does not ulcerate or ulcerates but fails to heal, prioritize internal supporting methods.
A patient suffered from a gluteal carbuncle with swelling, hardness, and pain. The chi pulse was floating and tight, but powerless upon deep pressure. After one dose of Neituo Qianghuo Tang (Internal Supporting Notopterygium Decoction), the pain ceased. Four doses of Jinyinhua San (Honeysuckle Powder) were then administered, leading to suppuration and recovery.
Another patient presented with a gluteal carbuncle characterized by severe swelling, hardness, and pain. Garlic-separated moxibustion was applied, followed by two doses of Xianfang Huoming Yin (Immortal Recipe for Life-Saving Decoction), which stopped the pain and reduced the swelling. Subsequently, Tuoli Xiaodu San (Supporting Interior and Detoxifying Powder) was administered with added Huangbai (Phellodendron Bark), Cangzhu (Atractylodes Rhizome), and Qianghuo (Notopterygium Rhizome). The abscess head ulcerated, and the patient recovered.
A third patient had a gluteal carbuncle with forming pus and pain. Two doses of Xianfang Huoming Yin stopped the pain. This was followed by Tuoli Xiaodu San, resulting in suppuration and recovery.
One patient suffered from a gluteal carbuncle without suppuration, decreased appetite, and poor desire for food. Liu Junzi Tang (Six Gentlemen Decoction) plus Chuanxiong (Sichuan Lovage Rhizome) and Danggui (Angelica Sinensis) was given first. As diet gradually improved, Tuoli San (Supporting Interior Powder) was administered, leading to suppuration.
Another patient’s gluteal carbuncle ulcerated with clear, thin pus that failed to heal. Douchi Bing Moxibustion (Moxibustion with Fermented Soybean Cake) was applied, and Shiquan Dabu Tang (Complete Great Tonifying Decoction) was drunk. Recovery took over two months.
Generally, when sores fail to suppurate, fail to ulcerate, or fail to heal after ulceration, it is due to deficiency of Qi and Blood. If the pus is clear and thin, the deficiency is even more severe.
A patient with a gluteal carbuncle had continuous pus discharge, gradual muscle emaciation, decreased appetite, and a faintly wiry stomach pulse. Several doses of Liu Junzi Tang added with Huoxiang (Pogostemon Herb) and Danggui improved dietary intake. Shiquan Dabu Tang was then used along with Douchi Bing Moxibustion, leading to recovery after over two months.
A frail patient had a gluteal carbuncle where pus formed but did not ulcerate. Several doses of Shiquan Dabu Tang supported the interior sufficiently to allow needle drainage. Over twenty additional doses led to recovery. The buttocks are located in a remote area where Qi and Blood rarely reach. For elderly or weak patients, tonifying Qi and Blood is essential for recovery.
A patient developed a carbuncle on the medial aspect of the leg. Before suppuration, there was swelling and pain. Two doses of Neituo Huangqi Chaihu Tang (Internal Supporting Astragalus and Bupleurum Decoction) brought slight improvement, and two more doses resolved the swelling.
A patient presented with diffuse gluteal swelling, unchanged skin color, and an active, rapid, yet powerless pulse. Pus was forming internally. The intention was to use interior-supporting medicine to bring it to the surface before needling. The patient desired internal resolution and took attacking/clearing drugs, worsening the deficiency. Upon seeking treatment again, the previous formula was used to support the interior and bring out the pus, followed by needling, and recovery was achieved with great tonification.
When toxin qi has condensed but cannot emerge, only tonifying Qi and Blood should be done to hasten suppuration for drainage. Internal resolution methods should not be discussed. Once pus forms, needling should depend on maturity and depth. Deep pressure causing pain indicates deep pus; light pressure causing pain indicates shallow pus; slight pain indicates no pus yet; immediate rebound indicates pus; lack of rebound indicates no pus. High, soft swelling indicates involvement of blood vessels; low, hard swelling indicates involvement of sinews and bones; unchanged flesh color indicates involvement of bone marrow.
A patient had a carbuncle on the lateral aspect of the leg with diffuse swelling and severe pain. Neituo Huangqi Tang decocted in wine was taken for two doses, bringing slight relief. Several doses of interior-supporting medicine caused ulceration and recovery.
A woman suffered from a chronic leg carbuncle with purple, sunken wound edges and clear, thin pus. I diagnosed this as deficiency. She disbelieved and took toxic-clearing drugs, causing numerous deficiency symptoms to arise. Seeking treatment again, I applied Fuzi Bing Moxibustion (Moxibustion with Aconite Cake) and administered over a hundred doses of Shiquan Dabu Tang, leading to recovery.
Generally, clear pus, failure to heal, or sunken wounds indicate extreme deficiency of Qi and Blood. Great tonification is most appropriate; otherwise, it becomes a defeat syndrome. If other diseases occur simultaneously, cure is difficult.
A patient had an internal rupture of a leg carbuncle. Needling yielded four to five bowls of pus. Symptoms included aversion to cold, fear of eating, and a thread-like pulse, indicating minute Yang Qi. Si Junzi Tang with fried Shengjiang (Dried Ginger) was taken; aversion to cold subsided slightly. After four more doses, it stopped. Liujunzi Tang with Rougui (Cinnamon Bark) was taken for several doses, improving diet. Finally, Shiquan Dabu Tang and Fuzi Bing Moxibustion were used for two months until recovery.
An elderly patient with a leg carbuncle had spontaneous rupture followed by sudden coma and a fine, minute pulse, indicating extreme deficiency of Qi and Blood. Great tonification restored consciousness.
A frail patient with internal rupture of a flowing abscess (Liuzhu) expelled five to six bowls of foul pus and exhibited facial deviation. The pulse was extremely deficient, indicating severe deficiency rather than true stroke. Dushen Tang (Single Ginseng Decoction) with one qian of Fuzi (Aconite Lateral Root) was taken for two doses, followed by great tonifying medicines, leading to recovery in over a month.
Generally, after massive loss of pus and blood, tonifying Qi and Blood must be prioritized. Even if other symptoms exist, they should be treated secondarily. After large ulceration of carbuncles, fever and aversion to cold belong to Qi deficiency. If the left pulse is insufficient, blood-tonifying herbs should exceed Qi-tonifying ones; if the right pulse is insufficient, Qi-tonifying herbs should exceed blood-tonifying ones. Surface-releasing methods must strictly be avoided.
A woman suffered from lower back pain, weak and flaccid legs unable to walk. Renshen Baidu San (Ginseng Resolving Toxins Powder) added with Cangzhu, Huangbai, and Zexie cured her. (This case of leg weakness and flaccidity belongs to Damp-Heat, so treatment was based on symptoms.)
A woman had pain at the Huantiao acupoint with unchanged skin color and a tight, rapid pulse, diagnosed as Agut Ju (Bone-Adhering Abscess). With pus not yet formed, Neituo Huangqi Tang decocted in wine was used, adding Qingpi (Green Tangerine Peel), Longdancao (Gentiana Root), and Shanzhi (Gardenia Fruit). Several doses stopped the pain.
A patient had a leg carbuncle with tendon spasm and pain, and a wiry-tight pulse. Wuji San (Five Accumulations Powder) added with Huangbai, Chaihu, and Cangzhu led to recovery. (Treatment was based on pulse and symptoms for Damp-Heat.)
A woman suffered from chronic Agut Ju with continuous pus, itchy skin, and flaccid limbs. I considered it deficiency and proposed tonification. Confused by wind-disease theories, she took wind-dispelling drugs and died.
Chen Wuyue stated: The human body consists of skin/hair, blood vessels, fascia, muscles, and bone marrow, governed internally by the Heart, Liver, Spleen, Lung, and Kidney. Reckless use of emotions (joy, anger) and labor/rest leads to depletion of essence and blood in the five zang organs, causing weakness in skin, blood, sinews, bones, and muscles, resulting in Wei Syndrome (Flaccidity). Its appearance resembles Roufeng (Soft Wind) and Hong Kong Foot. These latter two are external causes, whereas Wei Syndrome is due to internal insufficiency of the five zang organs.
A patient with Agut Ju had swelling, hardness, fever, bone pain, and tendon spasms with a rapid, deep pulse. Danggui Niangong Tang (Angelica Joint-Pain Relieving Decoction) cured him.
A patient had pain penetrating to the bone near the Huantiao acupoint at the root of the leg, with normal outer skin and a rapid, slippery pulse. This indicated forming pus in Agut Ju. Six doses of interior-supporting medicine caused swelling and pain, with a rapid-slippery pulse indicating mature pus. Needling released about one bowl of pus. Additional tonics led to recovery in over a month.
A patient had a leg carbuncle with diffuse swelling and pain, cold limbs, throat obstruction, and alternating chills and fever. Previous treatments failed because pathogenic factors were depressed in the meridians. Xianglianqiao Tang was taken for one dose, slightly relieving symptoms. Continued administration resulted in two bowel movements, complete resolution of symptoms, and recovery. (Since previous treatments failed, it was treated as depression using aromatic/wood-based herbs.)
A woman had left leg pain preventing extension, with a wiry-tight pulse becoming choppy upon pressure. Wuji San for two doses slightly stopped the pain, and another two doses stopped it completely. Shenyang Yangzhen Dan restored health. (Wiry-tight-choppy pulses indicate Cold, so the pungent-warm nature of Wuji San disperses it.)
A patient had leg pain with slight knee swelling. Superficial pulse was floating; deep pulse was wiry-tight. This was Hexi Feng (Crane Knee Wind). Dafangfeng Tang for two doses relieved it. Some claimed Fuzi is toxic and advised against it, suggesting toxic-resolving drugs instead. The patient wasted away. Upon return, I explained that diet fails to nourish flesh, fluids fail to transform essences, and spirit lacks foundation. Shortness of breath indicates Qi damage; lethargy indicates Spleen Qi deficiency; urinary incontinence indicates Bladder failure to store; occasional heat/epigastric fullness indicates Stomach Qi failing to rise; forgetfulness indicates chaotic Spirit. Without treatment, death would follow. This condition often affects those with deficiency. Xiaoxuming Tang modified and Dafangfeng Tang are effective. Toxic-resolving drugs would be erroneous.
A woman suffered from Hong Kong Foot with intermittent leg swelling, tendon spasms, and abdominal pain. Medications failed, and condition worsened. Texts state Bawei Wan treats Shaoyin Kidney channel, leg/abdominal pain, and asthma. One dose relieved it immediately; subsequent doses cured it. Those with extreme Kidney Jing deficiency often suffer this, where Water overacts on Fire. Urgent administration is required.
A patient had leg pain with tendon spasms and bone pain, wiry-tight pulse. Dafangfeng Tang for six doses improved spasms; two more doses reduced swelling. Occasional internal pain remained, but pulse was no longer wiry-tight, indicating Cold pathogen had left and stagnant turbid matter was preparing to suppurate. Interior-supporting medicine for several doses raised swelling; rapid-slippery pulse confirmed pus. Needling was performed. Shiquan Dabu Tang ensured stability in over a month. (Previous cases showed wiry-tight pulses with tendon/bone pain, indicating Wiry-Tight often governs Cold Pathogens.)
A woman had knee swelling/pain worsening with cold, lasting over a month despite medications. Pulse was wiry-tight, indicating deep-seated Cold. Dafangfeng Tang and Huolong Dan cured it.
Generally, although swelling depth and pathogen severity vary, all result from True Qi deficiency allowing deep invasion. Thus, Agut Ju and Hexi Feng are common in Kidney Deficiency. Ancients used Fuzi to warm and tonify Kidney Qi and promote herb action to dispel Cold. Weak individuals exposed to cold air may develop this condition with immobility, intermittent heat without sweat, and bone-pain upon pressure. If unresolved, Extreme Yin generates Yang, transforming Cold into Heat and ulcerating. If severely injured by thief-wind, local heat is mild with aversion to cold and sweating; moxibustion relieves pain. Dafangfeng Tang and Huolong Gao are required. Contracture, hemiplegia, and stone-hard lumps are called Shi Ju (Stone Abscess). Slow-healing, non-ulcerating red-purple rotting flesh is called Huan Ju (Slow Abscess). All stages benefit from the aforementioned decoctions to dispel Cold and tonify deficiency/support interior.
A patient had red, swollen, painful right leg with a deep-rapid pulse. Danggui Niangong Tang was used, but limb pain increased. This indicated Damp-Toxin stagnation, exacerbated by difficulty reaching lower body parts. It was not incorrect prescription. Phlebotomy removed toxic blood. The previous medicine was resumed, reducing symptoms significantly in one dose and resolving them in four.
A patient initially had leg pain, then generalized wandering pain worsening at night. Damp-resolving/toxic-clearing drugs failed. Pulse was slippery and choppy, indicating turbid blood pathology. Erchen Tang added with Cangzhu, Qianghuo, Taoren, Honghua, Niuxi, and Caowu cured him. (Slipperiness and choppy nature seem contradictory; why?)
For Damp-Phlegm, Damp-Heat, or dead blood flowing into joints, only pungent-warm agents can open pores, unblock tunnels, move Qi, and harmonize Blood.
A patient with leg pain experienced increased phlegm and gastric discomfort during attacks. Pulse was slippery-rapid. Erchen Tang added with Shengma, Er Zhu, Zexie, Qianghuo, and Nanxing brought peace. (Diagnosis based on pulse.)
A patient with chronic Hong Kong Foot had hypochondriac pain, fever, dizziness, vomiting, and leg numbness. Detoxifying/heart-protecting drugs failed. Left Guan pulse was tight; Right Guan pulse was wiry. This was also Hong Kong Foot. Banxia Zuojing Tang cured him.
A patient had soft, swollen, painful feet, fever, craving cold drinks, and constipation. Right Guan pulse was rapid, indicating Damp-Heat pouring down the Foot-Yangming Channel. Dahuang Zuojing Tang cured him.
A patient had shin, knee, and foot pain. Jiawei Baidu San cured him.
A patient had bilateral leg pain with active-slow pulse, indicating Damp-Phlegm. Erchen Tang added with Zhu, Huangbai, Qianghuo, and Zexie cured him. (Based on pulse.)
A patient had bilateral leg swelling/pain with slippery-slow pulse, indicating Damp-Phlegm. Wuling San added with Cangzhu and Huangbai for two doses brought slight improvement. Erchen Tang added with Er Zhu, Binglang, Zisu, Qianghuo, Duhuo, Niuxi, and Huangbai cured him. (Based on pulse.)
For Damp-Phlegm syndromes, moving Qi, promoting diuresis, and strengthening the Middle must come first. If Middle Qi harmonizes, Phlegm dissipates and Dampness has no place to reside.
A woman had bilateral leg pain with choppy-rapid pulse, indicating Blood Deficiency combined with Damp-Heat. Cangzhu, Huangbai, Zhimu, Longdancao, Fuling, Fangfeng, Jiangcan, and Qianghuo for several doses reduced swelling/pain. Siwu Tang added with Er Zhu, Huangbai, Niuxi, and Mugua cured her in over a month. (Based on pulse.)
A patient had joint swelling/pain with slow-rapid pulse, indicating Damp-Heat. Jingfang Baidu San added with Mahuang for two doses halved pain. Binglang Baidu San for four doses removed swelling. Siwu Tang added with Er Zhu, Niuxi, and Mugua cured him. (Slow and rapid pulses are contradictory; 'slow' might be a typo for 'fine'.)
A woman had calf swelling/pain, chills/fever, and floating-rapid pulse, indicating Damp-Heat pouring down the Three Yang channels. Pathogen was still in the exterior. Jiawei Baidu San failed because of blood stasis condensation preventing drug efficacy. Phlebotomy removed stasis. Previous medicine for two doses relieved symptoms rapidly. Danggui Niangong Tang for four doses cured her.
Ancient texts state Hong Kong Foot is a stagnation disease; treatment must promote circulation to prevent stagnation. Severe stagnation requires phlebotomy to remove bad blood and reduce severity. Classics say accumulation causes swelling/heat; after phlebotomy, medicinal treatment follows.
A woman had bilateral leg pain with tendon spasms upon cold exposure and wiry-tight pulse, indicating Cold Pathogen. Wuji San combined with Siwu Tang for several doses stopped pain. Siwu Tang added with Mugua, Niuxi, and Zhike cured her in over a month.
A patient had leg swelling and tendon spasms preventing movement. Jiajiao San for two doses cured him.
A woman had leg inability to flex/extend, worsening pain with wind-cold. Medications failed. Huoluo Dan one pill provided immediate relief. Recurrence next year responded to one pill. Duohuo Jisheng Tang for four doses cured her.
A patient with chronic Hong Kong Foot and Agut Ju pain recovered both conditions with one pill of Huoluo Dan.
A patient with chronic hernia and leg pain recovered both with one pill.
A Tai Anren suffered arm pain for years; two pills cured her.
A girl with critical convulsions, untreatable by others, recovered immediately with one pill and did not relapse.
Diseases deeply embedded internally require this medicine for penetration. Recently, some claim it draws wind into bones like oil into flour, causing reluctance. However, if the disease exists, the corresponding medicine should be used. Clinging to such words hinders cure.
A woman had wandering leg pain, shortness of breath, and spontaneous sweating. Chi pulse was wiry-rapid, indicating Cold-Damp flowing into the Kidney channel. Fuzi Liuwu Tang cured her.
People fear Fuzi toxicity. However, processed with child's urine, what harm is there? Infrequent use poses little risk. I took Buzhong Yiqi Tang with Fuzi for three years for Middle Qi deficiency without toxicity issues. Classics state: treat the disease with the appropriate medicine.
A woman had joint swelling/pain, worse in calves/feet, with sweating/headache, indicating Damp-Heat in the Taiyang channel. Mahuang Zuojing Tang for two doses cured her. (Previous case had wiry-rapid pulse with Cold-Damp; perhaps Damp generated Heat.)
A woman with hemorrhoids and leg soreness resembling bi-syndrome suffered from Yin Blood Deficiency failing to nourish sinews. Blood tonification was primary. Jiawei Sijin Wan cured her.
An elderly man with tendon spasms, bone pain, and leg weakness used Jufang Huantuan Wan.
A woman with tendon spasms, bi-pain, and leg weakness used Sanyin Shengsuan Wan.
Hejian stated Hong Kong Foot arises from Kidney Deficiency. Women also suffer it due to Blood Sea Deficiency and Seven Emotions. Blood Sea Deficiency parallels Male Kidney Deficiency. Medication is similar, but treating Seven Emotions is also necessary for efficacy.
A woman with leg pain and calf spasms worsened by releasing drugs had a wiry-tight pulse, indicating Kidney/Liver weakness with internal Wind-Damp invasion. Duohuo Jisheng Tang stopped pain; Shenyang Yangzhen Dan prevented spasms.
A patient with chronic leg pain worsened by overeating, fatigue, and weak pulse recovered with Liujunzi Tang added with Shanzha, Shenqu, Cangzhu, Xiong Gui, Shengma, and Chaihu.
An elderly drinker with frequent leg swelling/pain and surging-slow pulse improved with Danggui Niangong Tang. Later, Liujunzi Tang added with Cangzhu, Huangbai, and Zexie cured him.
A patient with slight dietary excess had chest fullness, acid regurgitation, and leg pain. Daoyin Wan cured him quickly. Liujunzi Tang added with Shenqu, Maiya, and Cangzhu for over twenty doses prevented recurrence.
Classics state: Doubling food intake injures intestines/stomach. Stomach Qi fails to circulate; Spleen Qi fails to distribute. Downward flow exploits Liver/Kidney deficiency, causing leg swelling. Irregular sexual activity leads to Yang Deficiency and Yin Excess, causing Hong Kong Foot. Internal injury from diet causing Spleen/Stomach Qi deficiency prevents upward movement, leading to downward pouring. Dongyuan's Kaijie Daoyin Wan opens knots and guides water, transforming Spleen Qi. If Spleen Dampness blocks causing facial swelling/pain, Daozhi Tongjing Tang guides and unblocks.
The above nineteen cases concern Hong Kong Foot. Though not sores/toxins, they are recorded due to successful treatment.
The buttocks are posterior to the lower abdomen, Yin within Yin, low, distant, and remote. Taiyang has much blood but little Qi. Little Qi reaches there, so Blood also arrives sparingly. Abscesses appearing after middle age require preventive tonification. Without accumulated tonification, disaster often occurs after scabbing or half a year later, leading to neglect.
A patient with leg carbuncle had weak pulse/symptoms, a critical sign. Treatment focused on supporting interior and urgent needling. Patient refused. Pus drained continuously, failed to heal, and patient died.
A patient over fifty suffered winter leg carbuncle with rapid pulse, irritability, craving cold, constipation, and severe swelling/pain. Heat rampant internally required bitter-cold medicine. Qingliang Yin with doubled Huangqin rapidly reduced severity. Siwu Tang added with Huangqin cured him.
Medication was based on symptoms and pulse.
A thirty-year-old patient suffering worry, labor, and lust developed millet-grain-sized red swelling on the lateral left leg. A physician diagnosed Fu Organ Excess and used Chengqi Tang twice to purge. Another physician gave Dahuang, Zhusha, and Xuemei for three doses, making pulse excessively excess. Patient died. This is the Jueyin channel, abundant in Qi but scarce in Blood.
A official suffered gluteal swelling/pain and dysuria, suspecting Guange (Obstruction/Reversal). Moxibustion on navel did not help bowels. Fire-clearing/diuretic drugs failed. Pulse indicated mature pus. Needling released several bowls of pus; bowels opened immediately.
Five days later, scrotal swelling and urinary retention occurred. Needling released urine and pus. Breath was weak. Large doses of Renshen, Danggui, Baizhu were too slow. Dushen Tang up to two jin restored Qi slightly.
Generally, for sores with massive pus/blood loss, tonifying Qi/Blood is primary. Other diseases are secondary.
A patient with leg carbuncle had mature pus drained (two bowls). One dose of interior-supporting medicine caused high fever. Shengyu Tang for two doses stopped fever. Next day... [text incomplete] ...ping (peaceful).
A patient with leg swelling, fever, and aversion to cold was treated with Buzhong Yiqi Tang. Patient thought it too slow, took Huangqin/Huanglian, worsening heat. Sought treatment again.
