Regarding headache as a disease, *Ling Shu* states: For Jue Tou Tong (Rebellious Headache), select acupoints from the six meridians of the foot; Zhen Tou Tong (True Headache) of the Hand Shaoyin meridian involves pain throughout the brain, with cold hands and feet extending to the joints, which is a fatal condition untreatable. *Nan Jing* states: When the meridians of the three Yang channels of the hand are affected by wind-cold that lingers without dissipating, it is named Jue Tou Tong (Rebellious Headache). If the pain extends to connect with the brain, it is named Zhen Tou Tong (True Headache). *Nei Jing* states: A short pulse at the Cun Kou (Radial Artery) indicates Tou Tong (Headache). *Mai Jing* states: A Yang-chord-like pulse indicates Tou Tong (Headache). It further states: A floating pulse at Cun Kou with wind-stroke fever and headache, or a tight pulse indicating headache, refers to Feng Tou Tong (Wind Headache) where the tight pulse of Shanghan (Cold Damage) reaches the Cun Kou. *Mai Jue* states: Headache with a short and choppy pulse indicates death, while floating and slippery pulses indicate wind-phlegm which is easy to eliminate. *Nei Jing* states: East wind arises in spring, affecting the Ganshu (Liver Transporter) point, located at the head and neck; thus, spring qi pathology affects the head. Furthermore, all Yang meridians converge on the head and face. For instance, the Foot Taiyang Bladder Meridian originates from the inner canthus, ascends to meet at the vertex, enters to network the brain, emerges to descend separately at the nape; pathological rebellion causes headache. The Foot Shaoyang Gallbladder Meridian originates from the outer canthus, ascends to reach the temporal angle; pathology causes pain in the temporal angle and forehead. Wind pathogen is received from the upper part; wind-cold damages the upper part, pathogen enters from outside, lodges in the meridians, causing chills, headache, heavy body, and aversion to cold. Treatment should focus on Fengchi (GB20) and Fengfu (GV16) acupoints, regulating Yin and Yang; deficiency requires tonification, excess requires diaphoresis for recovery. This is Shanghan Tou Tong (Headache due to Cold Damage). Headache with tinnitus and dysfunction of the nine orifices arises from the stomach and intestines, constituting Qi Deficiency Headache. Heart irritability with headache, with stagnation in the chest, involving the Hand Taiyang and Hand Shaoyin meridians, constitutes Damp-Heat Headache. If Qi rebels upward without descending, causing headache and disorders of the vertex, it is Lower Deficiency and Upper Excess. Involvement of the Foot Shaoyin and Foot Taiyang meridians, progressing to the Kidney, constitutes Cold-Damp Headache. If only one side of the head suffers cold pain, first select Hand Shaoyang and Hand Yangming points, then Foot Shaoyang and Foot Yangming points; this is Pian Tou Tong (Hemicrania). True Headache, when severe, connects to the brain. With cold hands and feet extending to the joints, morning onset leads to evening death, evening onset leads to morning death. Jue Ni Tou Tong (Rebellious Headache due to Reversal) results from exposure to severe cold, penetrating to the bone marrow; since the brain is the ultimate extent of marrow, brain Qi rebellion causes headache, and teeth also hurt. Li Dongyuan stated: Treating all headaches with Wind-dispersing herbs is a general principle. At the high peak, only Wind herbs are applicable; light and mild herbs belong to Yang within Yin, rising from earth to heaven. However, there are distinctions among the Three Yin and Three Yang syndromes. Taiyang Headache presents with aversion to wind, floating and tight pulse, treated mostly with Xiong (Ligusticum Chuanxiong), Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Mahuang (Ephedrae Herba), etc. Shaoyang Headache presents with string-taut and moderate pulse, alternating chills and fever, treated mostly with Chaihu (Bupleuri Radix). Yangming Headache presents with spontaneous sweating, fever, aversion to cold, floating, moderate, long, and full pulse, treated mostly with Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Lobatae Radix), Shigao (Gypsum Fibrosum), Baizhi (Angelicae Dahuricae Radix). Taiyin Headache necessarily involves Phlegm, heavy body, abdominal pain, known as Tan Pi (Phlegm Stagnation), with deep and moderate pulse, treated mostly with Cangzhu (Atractylodis Rhizoma), Banxia (Pinelliae Rhizoma), Nanxing (Arisaematis Rhizoma). Shaoyin Headache involves non-circulation of the Three Yin and Three Yang meridians, cold feet, Qi rebellion as Cold Jue (Reversal), with deep and fine pulse, treated mostly with Mahuang (Ephedrae Herba), Fuzi (Aconiti Lateralis Radix Praeparata), Xixin (Asari Radix et Rhizoma). Jueyin Headache presents with neck pain, or vomiting saliva/phlegm and cold limbs, with floating and moderate pulse, treated primarily with Wuzhuyu Tang (Evodia Decoction). Various Blood Deficiency Headaches are treated mostly with Danggui (Angelicae Sinensis Radix) and Chuanxiong (Ligustici Chuanxiong Rhizoma); various Qi Deficiency Headaches are treated mostly with Renshen (Ginseng Radix) and Huangqi (Astragali Radix). "Used mostly" refers to the principal herbs for treating excess conditions; auxiliary and guiding herbs are added based on concurrent syndromes, which is the major principle of formula construction. For Headache with both Qi and Blood Deficiency, add small amounts of Chuanxiong (Ligustici Chuanxiong Rhizoma), Manjingzi (Vernoniae Semen), and Xixin (Asari Radix et Rhizoma) to Tiao Zhong Yi Qi Tang (Regulate the Middle and Boost Qi Decoction); the efficacy is beyond description. Banxia Baizhu Tianma Tang (Pinellia, Atractylodes Macrocephala, and Gastrodia Decoction) treats Tan Jue Tou Tong (Phlegm-Reversal Headache); Qingkong Gao (Clear Empty Paste) treats Feng Shi Re Tou Tong (Wind-Damp-Heat Headache); Qianghuo Fuzi Tang (Notopterygium and Prepared Aconite Decoction) treats Jue Ni Tou Tong (Reversal Headache). For dampness in the head, use bitter herbs to induce vomiting; do not adhere rigidly to one formula. The discussed principles of headache treatment are very detailed. Worldly formulas mention the name Tou Feng (Head Wind), why is it not elucidated here? Please explain briefly. Pathogens causing headache are essentially the same, differing only in newness, duration, retention, or departure. Superficial and recent is named Tou Tong (Headache), sudden in onset, easy to disperse and resolve quickly; deep and distant is named Tou Feng (Head Wind), pain comes and goes irregularly, recurs upon trigger after recovery. One must verify the origin of the pathogen to apply treatment. Observing *Shi Xiao Fang* (Trial Effect Formulas): Elder Jiegu suffered from periodic headache in his prime. Upon onset, cheeks turned blue-yellow, dizzy, eyes unwilling to open, reluctant to speak, heavy body, drowsy wanting to vomit food, resolving after several days. He said this was a combined disease of Taiyin and Jueyin, named Feng Tan (Wind-Phlegm). Decocted Jinhua Wan (Golden Flower Pill) in water, moxaed Xiaxi (GB43) acupoint 14 times (two sets of seven), recovered in less than ten days. From this, is it not Tou Feng (Head Wind)? Here, "wind" is not mentioned because it refers to the root of the meridian; worldly mention of "wind" refers to the branch. Why not clarify that Shaoyang and Jueyin headaches cause hemicrania, as Liver-Gallbladder Wind-Wood is the pathogen? Later generations named it Tou Feng (Head Wind), arguably failing to seek the root. Jiegu stated: Da Tou Feng Zheng (Big Head Wind Syndrome) is caused by excessive Yangming pathogenic heat nourishing excess, and Shaoyang Minister Fire acting upon it. It often occurs in Shaoyang, or Yangming, or transmits to Taiyang. View the swelling in which part, select points according to the meridian. Damp-heat causes swelling, Wood excess causes pain. This pathogen appears in Tou Feng (Head Wind), around both ears, governing the disease. Treatment should not be too fast; speed exceeds the disease site, meaning Upper Heat remains while Middle Cold arises, harming life. This disease spreading from outside inward involves Blood pathology. Moreover, the head receives pathogen in formless regions; treatment should be slow first, then urgent. Slow first means pathogen is upper, lodging in formless divisions; once lodged in formless, it is everywhere. Using heavy agents for rapid purgation exceeds the disease site, making it hard to cure. Even using slow agents, if taken urgently, before meals, or in one dose, it loses the nature of slowness, so medicine cannot remove disease. One must slowly steep the formless pathogen, or ensure the herb's nature, flavor, and form align with slowness. Regarding slow and urgent: when slow agents purge, pathogen enters middle, reaching Yin parts, coming to substantial forms. If not removed quickly, Yin is damaged. This final treatment of guest pathogens requires urgency; treat guests with urgency. Treating main diseases slowly means Yang pathogen is upper, Yin pathogen is lower, being the home disease. Urgent treatment cannot resolve division but increases chaos. Treating guests urgently means Yang division receives Yin pathogen, Yin division receives Yang pathogen; these guest pathogens must be removed urgently. Suppose Shaoyang and Yangming are pathogens emerging before and behind ears. Yangming pathogen causes large head swelling. First use Huangqin Huanglian Gancao Tang (Scutellaria, Coptis, and Licorice Decoction), stir-fried and decocted, taken frequently in small amounts. If unresolved after finishing, stir-fry Niubangzi (Arctii Fructus) on a new tile until fragrant, decoct with Dahuang (Rhei Radix et Rhizoma), filter dregs, add Mangxiao (Natrii Sulfas), equal parts, also sip occasionally, avoid food. Mild diarrhea in front indicates pathogen departure. Yangming channel-draining herbs include Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Lobatae Radix), Shaoyao (Paeoniae Radix), Gancao (Glycyrrhizae Radix et Rhizoma); Taiyang channel-draining include Duhuo (Angelicae Pubescentis Radix), Fangfeng (Saposhnikoviae Radix), etc. Also, Dongyuan's clinical experience shows his formula construction has origins. Four seasons of heaven, people suffer many epidemics. Initially feel increased chills and heavy body, then transmit to swollen head and face, eyes unable to open, upper panting, throat dysfunction, dry tongue and mouth. Commonly called Da Tou Tian Xing (Big Head Epidemic), relatives avoid visits fearing contagion. After five or six days, doctors used Chengqi Tang (Harmonize the Interior Decoction) plus Lan Gen (Isatidis Radix) to purge, slightly relieved. Next day, disease remained. Purged again, slightly relieved, but ultimately not cured. Jiegu told him: Upper half of body is Heaven Qi. Lower half is Earth Qi. This pathogenic heat lodges between Heart and Lung, attacks head and eyes causing swelling. Using Chengqi Tang to purge clears Stomach excess heat, punishing the innocent.
