Women whose bodies have disease but whose pulses show no signs of pathogenic factors are pregnant; those with slippery and rapid pulses who have missed their menstruation are also pregnant. A deep and rapid pulse indicates fetal heat, a floating and rapid pulse suggests the presence of wind, while a choppy or weak pulse implies that the fetus may fail to develop properly.
If there is fetal movement, palpate it (to assess deficiency or excess); if the fetal foundation is weak, nourish and protect it. If heat is congested and qi is rebellious, clear heat, nourish blood, regulate qi, and strengthen the spleen. If the fetus threatens to miscarry, urgently employ lifting and raising methods. Whether dealing with cold damage or miscellaneous diseases, follow the classic methods strictly, prioritizing not to injure the fetal qi.
The primary formula is An Tai Yin (Stabilize Fetus Decoction). For rebellious qi, add Zisu Geng (Perilla Stem) and Hei Zhike (Black Bitter Orange Peel); for abdominal distension, add Dafupi (Areca Peel) and Hei Zhike; for abdominal pain, add Muxiang (Aucklandia Root) and Baishao (White Peony Root); for lower back pain, add Shengdi (Rehmannia Root) and Duzhong (Eucommia Bark); for hypochondriac pain, add Chaihu (Bupleurum Root) and Jiuye (Tangerine Leaf); for heart pain, add Baiko (Cardamom Seed) and Sharen (Amomum Fruit); for body heat, use Zisu Tang (Perilla Decoction); for nausea and vomiting, add Banxia (Pinellia Tuber); for diarrhea, add Baizhu (Atractylodes Macrocephala Rhizome); for dysentery, add Xianglian Powder; for bleeding, add Lian Shao Decoction; for unresolved cold damage, use Yangdan Tang; for heat pathogens sinking inward, use Heigao Tang. For routine regulation, An Tai Yin is most appropriate.
During pregnancy, if the spleen and stomach are weak, clear qi fails to ascend while phlegm-fluids stagnate internally, causing nausea, dizziness, vomiting of saliva/phlegm, anorexia, or selective eating; this is called E Zu (Morning Sickness/Obstruction). If blood is weak and cannot control qi, leading to rebellious qi and vomiting, this is also termed E Zu. These symptoms typically appear in the second or third month of pregnancy.
A deep pulse indicates qi stagnation, a slippery pulse indicates phlegm obstruction, a tight pulse indicates cold invasion, and a rapid pulse indicates heat congestion. If the pulse is wiry and deficient, it signifies Liver Wood overacting on the Spleen.
To treat the branch, promote diuresis and transform qi; to treat the root, strengthen the spleen and harmonize blood. Cold patterns require warming regulation, heat patterns require clearing and descending, and qi or blood deficiency requires tonification and nourishment.
The primary formula is Er Chen Tang (Two-Cured Decoction). For chest fullness, add Zhike (Bitter Orange Peel); for spleen deficiency, add Baizhu; for qi depression, add Xiangfu (Cyperus Rhizome); for qi deficiency, add Renshen (Ginseng Root); for unsmooth qi, add Sugeng (Perilla Stem) and Wuyao (Lindera Root); for abdominal pain, add Muxiang and Sharen. For spleen deficiency, use Yigong San (Yi-Gong Powder); for stomach deficiency, use Renshen Jupi Tang (Ginseng and Tangerine Peel Decoction); for dual spleen-stomach deficiency, use Liu Junzi Tang (Six-Gentlemen Decoction); for dual qi-blood deficiency, use Banxia Fuling Tang (Pinellia and Poria Decoction).
[Banxia Fuling Tang]
Indicated for morning sickness with a soft and wiry pulse.
Banxia (Pinellia Tuber, processed), 1.5 qian; Shudi (Prepared Rehmannia Root), 3 qian, stir-fried with Amomum powder; Renshen (Ginseng Root), 8 fen; Baishao (White Peony Root), 1.5 qian, stir-fried; Chuanxiong (Sichuan Lovage Rhizome), 1 qian; Chenpi (Aged Tangerine Peel), 1.5 qian; Baizhu (Atractylodes Macrocephala Rhizome), 1.5 qian, stir-fried; Fuling (Poria), 1.5 qian; Quan Fuhua (Inula Flower, wrapped in silk), 1.5 qian; Jiegeng (Platycodon Root), 3 fen.
Decoct in water, strain, and take warm. Add Huangqin (Scutellaria Root) if accompanied by heat; add Shengjiang (Fresh Ginger) if accompanied by cold.
In the second or third month of pregnancy, qi and blood are deficient, and spleen-stomach transportation is impaired. Accompanied by phlegm-fluids, this causes vomiting of saliva/phlegm, dizziness, and fatigue, known as E Zu. Renshen supports primal qi and tonifies qi; Baizhu strengthens the spleen and generates blood; Chuanxiong moves qi within the blood; Baishao astringes spleen-stomach yin; Shudi tonifies yin and nourishes blood; Fuling leaches out dampness and warms the middle; Jiegeng clears the throat and benefits the diaphragm; Chenpi regulates qi and harmonizes the stomach; Banxia awakens the spleen-stomach to resolve phlegm; Xuanfuhua (Inula Flower) regulates wind-qi to eliminate fluid retention. Decoct in water and take warm. This harmonizes the spleen and stomach, filling the interior with qi and blood so that phlegm-fluids transform naturally. Why would E Zu not recede and pregnancy remain unsettled? Adding Huangqin for heat or Shengjiang for cold are methods of syndrome differentiation.
[Susha Erchen Tang]
Indicated for morning sickness with a wiry and slippery pulse.
Banxia (Pinellia Tuber, processed), 1.5 qian; Fuling (Poria), 1.5 qian; Chenpi (Aged Tangerine Peel), 1.5 qian; Gancao (Licorice Root, honey-fried), 5 fen; Wumei (Mume Fruit), 3 qian; Sharen (Amomum Fruit), 8 fen, stir-fried until yellow; Shengjiang (Fresh Ginger), 2 slices; Hongzao (Jujube), 4 pieces.
Decoct in water, strain, and take warm.
Phlegm-fluids stagnate internally and disturb the middle jiao during pregnancy, causing rebellious qi, vomiting, and poor appetite. This is the cause of E Zu due to phlegm-fluids. Banxia quickens the spleen-stomach to dissipate phlegm; Fuling leaches dampness to eliminate fluids; Chenpi regulates qi and harmonizes the stomach; Honey-fried Gancao benefits the stomach and moderates the middle; Wumei astringes rebellious qi to conserve fluids; Sharen awakens the spleen-stomach to calm fetal qi; Shengjiang and Hongzao warm the stomach, nourish the heart, and regulate Ying and Wei. Decoct in water and take warm. Ensure phlegm transforms and fluids dissipate, so rebellious qi calms and the spleen-stomach harmonizes. How could food intake remain poor and E Zu persist?
[Shanju San] (Also known as Renshen Jupi Tang)
Indicated for morning sickness with irritability, abdominal stuffiness, and a soft, wiry, stagnant pulse.
Renshen (Ginseng Root), 1 liang; Houpo (Magnolia Bark, processed), 1 liang; Baizhu (Atractylodes Macrocephala Rhizome, stir-fried), 2 liang; Fuling (Poria), 1.5 liang; Maidong (Ophiopogon Tuber, heart removed), 3 liang; Chenpi (Aged Tangerine Peel), 1.5 liang; Gancao (Licorice Root, honey-fried), 5 qian; Shengjiang (Fresh Ginger), 5 slices; Zhuru (Bamboo Shavings), 1 liang.
Process into a powder. Decoct 3 qian in water, strain, and take warm. Add Chao Hei Zhizi (Charred Gardenia Fruit) if heat is excessive.
Dual deficiency of the spleen and lungs during pregnancy impairs the transformation of damp-heat, causing vomiting, irritability, and abdominal fullness. This is E Zu due to qi deficiency and damp-heat. Renshen tonifies spleen-lung deficiency; Houpo removes fullness in the central region; Baizhu assists spleen transportation of dampness; Maidong moistens the lungs and clears the heart; Honey-fried Gancao moderates the middle and benefits stomach qi; Fuling leaches dampness and harmonizes spleen qi; Chenpi regulates qi and harmonizes the middle; Zhuru clears heat and resolves depression; Shengjiang disperses and resolves phlegm-fluids. Process into powder and decoct. If heat is severe, add Charred Zhizi to clear and descend. Ensure damp-heat dissipates instantly, filling the primal qi and restoring proper transportation. Why worry about persistent fullness, irritability, or vomiting after administration?
