In the postpartum period, obstruction of both urination and defecation occurs because the intestines and stomach originally harbor heat. After childbirth, fluids and blood are simultaneously discharged downward, leading to exhaustion of body fluids, dryness and astringency of the intestines and stomach, and accumulation of heat causing dryness and binding, thus resulting in obstruction. Some patients find relief after drinking human or cow's milk; therefore, tonifying the Kidney is preferable, as the Kidney governs the five fluids and opens into the two yin passages (the anterior and posterior orifices).
Postpartum urinary incontinence is caused by insecurity of Kidney Qi. It should be treated with Wuweizi Wan (Schisandra Berry Pill). If there is Spleen and Kidney deficiency, Buzhong Yiqi Tang (Decoction for Boosting the Middle and Augmenting Qi) should be used to deliver Huanshao Dan (Return to Youth Pill). If there is Spleen and Kidney deficiency-cold, Bawei Wan (Eight-Ingredient Pill) and Sishen Wan (Four Spirits Pill) should be used as adjuvants.
Postpartum hematuria occurs due to Qi and Blood deficiency allowing heat to invade. Heat causes blood to flow and seep into the uterus, so blood is expelled with urine. In cases where a parturient presents with hematuria, sallow complexion, hypochondriac distension, and poor appetite, this indicates Liver Wood overacting on Spleen Earth. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Buzhong Yiqi Tang should be taken concurrently for recovery. Postpartum hematochezia may result from improper diet and lifestyle, or from excess Six Evils (Wind, Cold, Summerheat, Dampness, Dryness, Fire) and Seven Emotions (Joy, Anger, Worry, Pensiveness, Sadness, Fear, Fright), leading to depletion of Yuan Qi (Primal Qi) and injury to the Yin collaterals. If caused by accumulation of heat from rich foods or damp-toxin from alcohol, clearing heat is indicated. If caused by anger stirring Liver Fire, stagnation injuring the Spleen, or excessive thinking injuring the Heart, harmonizing the Liver and regulating the Heart and Spleen is indicated. For Wind-Blood-Heat in the Large Intestine, cooling Blood and dispelling Wind is indicated. For weakness of the intestines and stomach with sinking of Yuan Qi, major tonification combined with lifting is indicated. Moreover, after the severe deficiency of Qi and Blood following childbirth, if one suffers from collateral injury and blood loss, how can one not urgently consolidate the Spleen's Yuan Qi and Middle Qi to serve the function of controlling and holding blood?
Around half a month after delivery, sudden carbuncles and swellings appearing on the limbs, chest, and abdomen indicate that lochia has not been completely expelled, flowing and stagnating in the meridians, or that Qi and Blood are weak, causing Ying Qi (Nutritive Qi) to fail to follow its course and reverse into the muscle tissue. If due to lochia stasis, symptoms include swelling, redness, pain, and a wiry, surging, and forceful pulse. Treatment should focus on nourishing and moving Blood, assisted by guiding out stasis and dispersing Qi stagnation. If due to Qi and Blood deficiency with stagnant Ying Qi and reversed Wei Qi (Defensive Qi), symptoms include flat, diffuse swellings without heads, and a deficient, minute, and forceless pulse. Treatment should primarily involve major tonification of Qi and Blood, using formulas such as Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) or Bazhen Tang (Eight Treasure Decoction) to secure the root Yuan Qi and support Stomach Qi. When Qi is strong and Blood is harmonious, the toxin resolves spontaneously. If toxic diseases are treated with cooling and detoxifying herbs, it will inevitably fail to suppurate and rupture, turning into a disastrous condition.
