(Attachment: Distension)
Edema is caused by the deficiency of the Spleen and Stomach, which fail to control water metabolism. As Qi Bo stated, Shui Zhang (Water Distension), Fu Zhang (Skin Distension), Gu Zhang (Drum Distension), Chang Qin (Intestinal Polyp), and Shi Jia (Stone Fibroma), although named differently, are essentially this same condition. At the initial stage of edema, the eye sockets swell slightly like a sleeping silkworm, gradually spreading to the whole body with puffiness of the face and eyes, coughing and rapid breathing, chest and diaphragm distension, swelling of the shins and dorsum of the feet, and difficult urination; these are signs of Edema.
The *Qianjin Fang* states that there are ten types of edema with different names and causes, which can be categorized into five main types. First is Feng Zhong (Wind Edema), starting from the limbs, migrating frequently, with numb skin; second is Qi Zhong (Qi Edema), characterized by fluctuating severity and remission; third is Xue Zhong (Blood Edema), presenting with thread-like blood stasis marks; fourth is Yang Shui (Yang Water), showing bluish-yellow complexion, fever, thirst, and constipation or dysuria; fifth is Yin Shui (Yin Water), showing bluish-white complexion, loose stools, clear and profuse urination, and cold extremities. Treatment should follow the specific symptoms. A floating and large pulse indicates an easy-to-treat case; a deep and thin pulse indicates a difficult case. In cases of edema, if muscles collapse, fluid oozes from the shins, the supraclavicular fossa becomes flat and full, the umbilicus protrudes, lips turn black, and palms and soles flatten, it indicates internal injury to the Five Zang organs and is incurable. The *Su Wen* states: Facial swelling is due to Wind; foot swelling is due to Water. Swelling above the waist should be treated with dispersing methods; swelling below the waist requires promoting urination. This is the key method for treating edema. When Kidney Water overflows, one must tonify the Spleen Yuan to prevent flooding; using bland and leaching herbs while carefully regulating Stomach Qi ensures success.
Distension (Gu Zhang) is also caused by Spleen and Stomach deficiency, excessive consumption of raw/cold foods, and fishy/meaty toxic substances, leading to stagnation in the middle jiao. Over time, clear and turbid qi fail to separate, and ascending/descending functions fail, causing distension. Patients with distension must avoid salt, soy sauce, wheat products, and fishy/meaty foods for medication to be effective.
Chronic malaria, chronic diarrhea, chronic dysentery, or long-term illness with poor appetite can lead to extreme Spleen Qi deficiency and subsequent edema. Treatment should focus on tonifying the Spleen and Stomach and supporting Yuan Qi. Tonification is preferred over purgation. Formulas such as Si Jun Zi Tang (Four Gentlemen Decoction) or Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) minus Gan Cao (Licorice Root), plus herbs that promote differentiation of yin/yang and warm the interior, are suitable. Once Spleen Yuan recovers, edema subsides. Treatment should be gentle and does not fall under standard edema protocols.
