Bao Feng Ke Re (Violent Wind and Guest Heat) suddenly flares up, with eyelid swelling, headache, and tears flowing like soup. There are alternating chills and fever, nasal congestion, severe sandiness in the eyes, and unbearable pain.
This condition is not Tian Xing Chi Re (Epidemic Red Eye), which involves mutual infection, nor is it a condition resembling malaria where eye pain and cup-like swelling occur at onset and recede slowly over time. It arises because of internal accumulation of damp-heat that has not been cleared, combined with irritability, fatigue, and labor, leading to an external contraction of wind-heat pathogenic factors, resulting in sudden onset. The presence of swelling is due to wind-heat attacking together, with fire residing in the blood level. Treatment is relatively easy to resolve. It is suitable to take:
Ju Fang Xi Xin San (Official Formula Washing Heart Powder). Indicated for cases where heat pathogen predominates. It treats wind obstruction causing high fever, dizziness and pain in the head and eyes, stiffness and tension in the shoulders and back, vexatious pain in the limbs and joints, rising heat qi, bitter taste in the mouth, dry and cracked lips, sore and swollen throat, accumulation of phlegm and saliva, thick and sticky sputum and nasal discharge, mental restlessness, dry and painful eyes, as well as imbalance of cold and heat, nasal congestion with heavy voice, dry throat with excessive thirst, five-palm heat, red and scanty urination, and constipation with difficult defecation. All these symptoms are suitable for this formula.
Ingredients: Jing Jie Sui (Schizonepetae Spica), Gan Cao (Licorice Root), Dang Gui (Angelicae Sinensis Radix), Da Huang (Rhubarb Root), Chi Shao Yao (Red Peony Root), Ma Huang (Ephedrae Herba) (each 6 qian); Bai Zhu (Atractylodis Macrocephalae Rhizoma) (5 qian).
Grind the above ingredients into powder. Take 2–3 qian each time, decocted with ginger and mint water.
Because Bai Zhu combined with Da Huang enters the Heart meridian, it is named "Washing Heart"; assisted by Ma Huang and Jing Jie, it acts as a formula addressing both exterior and interior.
Xi Gan San (Liver Washing Powder). Indicated for cases where both wind and heat pathogen predominate. It treats wind-toxin attacking upward, causing sudden redness of the eyes, severe swelling and pain making it difficult to open the eyes, itching and grittiness, and profuse discharge and tears.
Ingredients: Bo He Ye (Menthae Haplocalycis Folium), Dang Gui (Angelicae Sinensis Radix), Qiang Huo (Notopterygii Rhizoma et Radix), Gan Cao (Licorice Root, prepared), Shan Zhi Ren (Gardeniae Semen), Fang Feng (Saposhnikoviae Radix), Da Huang (Rhubarb Root), Chuan Xiong (Ligusticum Chuanxiong Rhizoma).
Grind the above ingredients equally into powder. Take 2–3 qian each time, mixed with boiling water shortly after meals.
Qiang Huo Sheng Feng Tang (Notopterygium Victory-over-Wind Decoction) (Indicated for cases where wind pathogen predominates; see Volume 2).
Wind and fire blaze fiercely, invading the Six Yang meridians. The face swells, the brain feels swollen, tears flow like soup. There is photophobia, redness and grittiness in the eyes, headache, and no relief day or night, making the suffering unbearable.
This condition presents with red and painful eyes, swelling of the head and face, and dry, red skin internally. The presentation resembles Da Tou Shang Han (Big Head Cold). It occurs frequently in summer. If caused by damp-heat, there is excessive tearing and skin erosion. If caused by wind-heat, there is distending pain and aversion to cold. If untreated, blood stasis will occur internally. Even if the swelling subsides, eye complications will inevitably arise. It is suitable to take:
Pu Ji Xiao Du Yin (Universal Benefit Detoxification Decoction). Luo Qianfu said: When my master supervised the tax collection in Jiyuan, in April, many people suffered from epidemic diseases. Initially, they felt aversion to cold and heaviness in the body, which then progressed to severe swelling of the head and face, inability to open the eyes, upper respiratory distress, throat dysfunction, dry tongue, and dry mouth. Commonly known as "Big Head Epidemic," relatives avoided visiting each other, and most infected individuals did not survive.
My master said: The upper half of the body corresponds to the Qi of Heaven; the lower half corresponds to the Qi of Earth. This pathogenic heat resides between the Heart and Lungs, attacking upward to the head and eyes, causing severe swelling.
Ingredients: Huang Lian (Coptidis Rhizoma), Huang Qin (Scutellariae Radix) (each 5 qian), Bai Jiang Can (Bombyx Batryticatus) (stir-fried, 1 qian), Niu Bang Zi (Arctii Fructus), Lian Qiao (Forsythiae Fructus), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Ban Lan Gen (Isatidis Radix), Xuan Shen (Scrophulariae Radix), Chai Hu (Bupleuri Radix), Jie Geng (Platycodonis Radix), Gan Cao Shao (Licorice Root tip, raw), Pi Po Bo (Ageratum Conyzoides Herb), Sheng Ma ( Cimicifugae Rhizoma) (each 2 qian), Ren Shen (Ginseng Radix et Rhizoma) (3 qian).
Grind the above ingredients into powder. Half is taken mixed with boiling water, consumed frequently; the other half is made into pills with refined honey for sucking.
In the above formula, Huang Qin and Huang Lian are bitter and cold, clearing heat from the Heart and Lungs, serving as the Sovereign drugs; Ju Hong is bitter and neutral; Xuan Shen and Chai Hu are bitter and cold, resolving various toxins; Raw Gan Gan Cao is sweet and cold, clearing fire; Ren Shen is sweet and warm, tonifying Qi, serving as the Minister drugs; Lian Qiao and Niu Bang Zi are acrid and neutral; Ban Lan Gen is bitter and cold; Pi Po Bo, Bai Jiang Can, and Sheng Ma are bitter, neutral, and slightly cold, unblocking the Qi stagnation in the Shaoyang and Yangming meridians; Jie Geng is bitter, acrid, and warm, acting as a boat and oar to prevent downward movement of the medicine.
Alternatively, add Fang Feng (Saposhnikoviae Radix), Su Bo He (Menthae Haplocalycis Folium var. pterocarpa), Chuan Xiong (Ligusticum Chuanxiong Rhizoma), Dang Gui Shen (Angelicae Sinensis Radix radix portion), chopped like hemp seeds. Take 5 qian each time, with two zhong of water, decocted to one zhong. Strain and drink warm, taking frequently after meals. If stool is hard, add Jiu Zhi Da Huang (Wine-processed Rhubarb Root) 1–2 qian to promote bowel movement; if swelling is severe, it is appropriate to use Bian Ci (stone puncture/bloodletting).
Commentary: Epidemic diseases, although caused by heat-toxin infection, can be cured by purgation in patients with excess constitution. However, applying purgation indiscriminately to patients with Qi deficiency rarely fails to cause danger. Therefore, Mr. Dongyuan formulated this prescription to save those with Qi deficiency, showing great benevolence.
Zhu Tong Jie Du Wan (Pain-Relieving Detoxification Pill)
Ingredients: Peng Sha (Borax), Mo Yao (Myrrha), Chuan Xiong (Ligusticum Chuanxiong Rhizoma), Jing Jie Sui (Schizonepetae Spica), Pu Xiao (Natrii Sulfas), Bai Zhi (Angelicae Dahuricae Radix), Shi Gao (Gypsum Fibrosum), Jia Ju Hua (Chrysanthemi Flos) (each 1 qian), She Xiang (Moschus).
Grind the above ingredients into fine powder, bind with rice paste to make pills the size of paulownia seeds. Take 1.5 qian each time, at any time, swallowed with warm water.
