The ancient texts state: "When the Yin pulse beats distinctly different from the Yang, it is called having a child." This refers to the harmonization of Qi and Blood, where Yang Qi exerts its influence and Yin receives transformation. If the pulse of the Hand-Shaoyin (Heart) channel moves vigorously upon palpation, it indicates pregnancy.
The Hand-Shaoyin corresponds to the Heart pulse. The Heart governs the blood vessels. It is also said that the Kidney is referred to as the "Gate of the Womb" (Baomen Zihu), and the Chi (Cun-Guan-Chi) pulse position corresponds to the Kidney. A Chi pulse that remains unbroken upon deep pressure indicates pregnancy. When the pulses at the three positions (Cun, Guan, Chi) are equal in floating and sinking qualities, and continuous upon pressure, it signifies conception. Furthermore, a deep and solid pulse on the left hand suggests a male fetus, while a floating and large pulse on the right hand suggests a female fetus. If both hands show deep and solid pulses, twins will be boys; if both show floating and large pulses, twins will be girls. Additionally, if the Chi pulse is larger on the left side, it indicates a boy; if larger on the right, a girl. If both Chi pulses are large, two children will be born. If both Chi pulses are floating, two boys will be born; otherwise, girls (or vice versa depending on specific variations). If both Chi pulses are deep, two girls will be born; otherwise, boys. Also, a floating and large Chi pulse on the left hand indicates a boy, while a deep and fine Chi pulse on the right hand indicates a girl. Obtaining a Taiyin pulse (deep) suggests a boy, while obtaining a Taiyang pulse (floating) suggests a girl. To determine the gender, one method involves the pregnant woman walking with her back to her husband, who calls out to her from behind; if she turns her head to the left, it is a boy; if to the right, it is a girl. Another method involves calling out suddenly when she is using the toilet; turning left indicates a boy, turning right indicates a girl. Another sign is observing the breasts: a nodule on the left breast indicates a boy, while one on the right indicates a girl.
In the first month of pregnancy, termed "Beginning of Form," diet should be refined and well-cooked, favoring sour and pleasant flavors, with barley being suitable. Avoid raw, pungent, or spicy foods. This stage is called "Initial Purity." It is nourished by the Foot-Jueyin Liver channel. The Foot-Jueyin is the Liver meridian, which governs blood. During this month, blood flow is sluggish, so the Foot-Jueyin nourishes the fetus. The acupoint of the Foot-Jueyin is located at the junction of white flesh between the big toe of the right foot.
In the second month, termed "Beginning of Paste," avoid raw, pungent, or spicy foods, and reside in quiet places. Men should avoid exertion, and the mother may experience pain in all joints. This stage is called "Beginning of Storage." It is nourished by the Foot-Shaoyang Gallbladder channel. The Foot-Shaoyang is the Gallbladder meridian, governing essence. In the second month, the fetal essence forms in the womb, hence the Foot-Shaoyang nourishes it. The acupoint is located one cun above the metatarsophalangeal joint of the little toe, in the depression of the tarsal bone.
In the third month, the fetus begins to take shape. At this time, blood does not flow freely, and the form begins to transform. It is nourished by the Xinbao (Pericardium) channel, also known as Shou-Xinzhu. The Pericardium contains spirit and connects internally to the Heart. One should avoid sorrow, worry, and shock. The acupoint is located at the transverse crease of the palm. Palpating the pregnancy pulse, if it is slippery and rapid, and disperses under heavy pressure, it indicates three months of pregnancy.
In the fourth month, the fetus begins to receive the essence of Water to form blood vessels. Diet should include rice and glutinous rice, with soups made from fish or wild geese. This stage is called "Abundant Prosperity," facilitating the ears, eyes, and meridians. Bathing should avoid extreme cold and heat. It is nourished by the Hand-Shaoyang Sanjiao (Triple Burner) channel. The Hand-Shaoyang is the Sanjiao meridian, connecting internally to the Fu organs. In the fourth month, the six Fu organs of the fetus develop smoothly, hence the Hand-Shaoyang nourishes it. The acupoint is located two cun proximal to the metacarpophalangeal joint of the little finger. To determine gender at four months, a rapid pulse on the left indicates a boy, on the right a girl; rapid pulses on both sides indicate two girls. At this stage, purgative methods must be avoided to prevent postpartum disasters. This is because the Hand-Shaoyang Sanjiao meridian connects to the Sanjiao, requiring physical stillness, mental harmony, and dietary moderation.
In the fifth month, the fetus receives the essence of Fire to form its Qi. The mother should sleep late, wash clothes, stay in deep rooms, wear thick clothing, absorb morning sunlight to avoid cold evils, and eat rice, wheat, beef, and mutton, seasoned with cornel and five flavors. This is called "Nourishing Qi" to stabilize the Five Zang organs. Alternatively, fish and turtle are recommended. It is nourished by the Foot-Taiyin Spleen channel. The Foot-Taiyin Spleen meridian governs the four seasons. In the fifth month, the fetal limbs are formed, hence the Foot-Taiyin nourishes it. The acupoint is three cun above the medial malleolus. Palpating the pulse, if it is rapid but not slippery under heavy pressure, it indicates five months. A rapid pulse suggests blood accumulation; a tight pulse suggests uterine obstruction; a slow pulse suggests abdominal fullness and asthma; a floating pulse suggests edema.
In the sixth month, the fetus receives the essence of Metal to form tendons. Mild labor is encouraged, avoiding complete rest. Outdoor activities like watching running dogs and horses are beneficial. Eating meat from startled birds or fierce beasts helps transform the fascia and tendons, nourishing nails and strengthening the back. It is nourished by the Foot-Yangming Stomach channel. The Foot-Yangming is the Stomach meridian, governing the mouth and eyes. In the sixth month, the fetal mouth and eyes are formed, hence the Foot-Yangming nourishes it. The acupoint is two cun above Taichong (LR3).
In the seventh month, the fetus receives the essence of Wood to form the body. Physical activity and limb movement are encouraged, avoiding stillness. Living environments should be dry, and food should avoid cold. Eating rice and glutinous rice helps tighten the pores, nourishing bones and teeth. It is nourished by the Hand-Taiyin Lung channel. The Hand-Taiyin Lung meridian governs skin and hair. In the seventh month, fetal skin and hair are formed, hence the Hand-Taiyin nourishes it. The acupoint is in the depression at the radial border of the thenar eminence, proximal to the thumb's metacarpophalangeal joint. A strong, large, and firm pulse indicates survival; a deep and fine pulse indicates danger. Frequent epistaxis and tetanus suggest "uterine bleeding"; frequent sneezing suggests fetal movement. Sudden profuse watery discharge indicates threatened miscarriage due to premature rupture of membranes.
In the eighth month, the fetus receives the essence of Earth to form skin. Mental peace and calm breathing are required to avoid Qi exhaustion, tightening pores and brightening complexion. It is nourished by the Hand-Yangming Large Intestine channel. The Hand-Yangming is the Large Intestine meridian, governing the nine orifices. In the eighth month, fetal orifices are formed, hence the Hand-Yangming nourishes it. The acupoint is in the depression proximal to the index finger's metacarpophalangeal joint. A strong, wiry, and tight pulse indicates survival; a deep and fine pulse indicates danger.
In the ninth month, the fetus receives the essence of Stone to complete skin and hair. All six Fu organs and joints are complete. Sweet wine and foods are recommended, loosening belts to await delivery. This nourishes hair and increases strength. It is nourished by the Foot-Shaoyin Kidney channel. The Foot-Shaoyin is the Kidney meridian, governing vessel connections. In the ninth month, fetal vessels are complete, hence the Foot-Shaoyin nourishes it. The acupoint is posterior to the medial malleolus, near the anterior tibial artery. By the tenth month, the Five Zang and Six Fu are complete, absorbing Heaven and Earth Qi into the Dantian, preparing for birth. Methods for promoting delivery can be prepared in advance.
If menstruation stops for three months and the Chi pulse remains active, it indicates pregnancy.
Yantai San (Fetal Verification Powder): Cinnamomi Ramulus powder, one qian, taken with mugwort decoction on an empty stomach. Abdominal movement indicates pregnancy; no movement after one day indicates amenorrhea.
Aijia Tang (Mugwort Vinegar Decoction): Mugwort stir-fried with vinegar. Severe abdominal pain after ingestion indicates pregnancy; lack of pain indicates no pregnancy.
Irregular menstruation with slippery, large, and uniform pulses, normal spirit, aversion to food odors, cravings, vomiting, or spitting clear fluid indicates "Ezu" (Morning Sickness). Do not treat as cold. Baosheng Tang (Life Preservation Decoction) is suitable: Ren Shen (2 qian), Baizhu (5 qian), Juhong (5 qian), Xiangfu (5 qian), Wuyao (5 qian), Gancao (1 qian). Decoct with ginger. Add Dingxiang and Shengjiang if vomiting occurs.
Yangwei Tang (Stomach Nourishing Decoction) treats Ezu: Danggui, Baishao, Baizhu, Baifuling, Chenpi, Huoxiang, Sharen, Shenqu, Banxia (processed with Xingren), Xiangfu, Gancao. Decoct with ginger and jujube.
For persistent vomiting, Erchen Tang with processed Banxia and Shengjiang stops it immediately.
Menstrual bleeding during early pregnancy with normal appetite indicates excess blood; the fetus will consume it, stopping the bleeding.
Zifan (Fetal Irritability) causes restlessness and mental disturbance. Zhuye Tang (Bamboo Leaf Decoction) is used: Baifuling, Fangfeng, Maidong, Huangqin.
Zixian (Fetal Epilepsy) involves phlegm surging, eye deviation, jaw lock, and unconsciousness. Lingyang San (Antelope Horn Powder) is used: Danggui, Chuanxiong, Fangfeng, Duhuo, Baifuling, Wujiapi, Yiyiren, Suanzaoren, Xingren, Muxiang, Gancao, Lingyangjiao.
To stop fetal crying in the womb, hold a piece of soil from an old mouse hole in the mouth.
Zixuan (Fetal Suspension) causes heart-stomach pain and abdominal distension. Zisu Heqi Yin (Perilla Harmonizing Qi Decoction) treats this and other pre-labor issues: Danggui, Chuanxiong, Baishao, Renshen, Zisu, Chenpi, Dafupi, Gancao. Modify based on symptoms: add Xiangfu/Muxiang for pain, Zhike/Sangbaipi for cough, Huangqin for heat, Sharen for vomiting, Baizhu/Baifuling for diarrhea, Qianghuo/Mahuang for cold, Shanzha/Xiangfu for food stagnation, Xiangfu/Wuyao for anger.
Zizhong (Fetal Edema) causes facial swelling and limb edema in the 7th-8th month. Fuling Tang is used: Danggui, Chuanxiong, Baishao, Shudihuang, Baizhu, Baifuling, Zexie, Tiaohuangqin, Zhizi, Houpo, Gancao, Maidong.
Ziqi (Fetal Qi) causes leg swelling from the feet up, difficulty walking, and shortness of breath, resembling water retention. Tianxianteng Tang is used: Tianxianteng, Chenpi, Xiangfu, Wuyao, Mugua, Gancao. Add Cangzhu and Zisu for better effect.
Zilin (Fetal Strangury) causes painful, frequent urination. Zilin San is used: Maidong, Chifuling, Dafupi, Mutong, Gancao. Decoct with bamboo leaves, take on an empty stomach.
Zhuanbao (Displaced Bladder) causes sudden inability to urinate due to fetal pressure on the bladder. Differentiate from Zilin: Zilin is painful and scanty; Zhuanbao is painless and scanty. Wuling San plus fried Ejiao or Dongkui San can be used. Dongkui San: Dongkuizi, Shanzhizi, Mutong, Huashi. Apply external paste of these herbs with snail meat or ginger juice to the umbilicus. For Zhuanbao, Bayi Tang plus Banxia and Chenpi (Sanhe Tang) induces vomiting to raise Qi and open the lower orifice.
Tailouxia (Fetal Leakage) involves vaginal bleeding with a viable fetus, caused by Qi/Blood deficiency and heat. Xiong Gui Tang (Dangguiwei and Chuanxiong) decocted with good wine and Tongbian (child's urine) stops bleeding instantly. Jiao Ai Siwu Tang is also effective.
For fetal movement with bleeding and perineal pain, Huanglian powder mixed with wine is taken three times daily.
For trauma causing fetal death, lochia discharge, severe pain, and jaw lock, Foshou San (Danggui and Chuanxiong) is used to expel the dead fetus or save the mother in cases of difficult labor, retained placenta, postpartum hemorrhage, or stroke-like symptoms. Induce vomiting if necessary. Alternatively, cooked carp soup helps settle fetal movement.
For threatened miscarriage with bleeding, Siwu Tang combined with Sijunzi Tang plus Tiaohuangqin, Ejiao, Sharen, Baizhi, and Sangjisheng is used.
There are two types of An Tai (Fetal Stabilization): treating the mother's illness stabilizes the fetus, or directly stabilizing the Qi calms the mother. An Tai Yin (Fetal Stabilization Decoction) is used for general discomfort, mild pain, or poor appetite in mid-pregnancy: Dangguishen, Chuanxiong, Baishao, Tiaohuangqin, Baizhu, Sharen, Chenpi, Sugeng, Gancao, Shudihuang. Modify for bleeding or pain.
Ban Chan (Miscarriage) results from Qi/Blood deficiency failing to nourish the fetus. Xiong Gui Buzhong Tang tonifies Qi and Blood: Huangqi, Renshen, Baizhu, Danggui, Chuanxiong, Baishao, Ganjiang, Ejiao, Wuweizi, Muxiang, Duzhong, Gancao.
Pregnancy forms through Yang exerting influence and Yin transforming. Deficiency leads to miscarriage. Emotional stress and internal fire can also cause miscarriage. A case study describes a woman miscarrying around three months due to weak blood. Treating with Baizhu decoction and Huangqin powder stabilized the pregnancy. The third month corresponds to Ministerial Fire, making it vulnerable.
An Tai Wan (Fetal Stabilization Pills) is for regular use: Danggui, Chuanxiong, Baishao, Tiaohuangqin, Baizhu. Pill form with wine paste. Suitable for thin women with heat or history of miscarriage.
Another formula for preventing miscarriage uses Lotus seeds and Sharen.
Qianjin Baoyun Dan (Thousand Gold Pregnancy Preservation Pill) treats habitual miscarriage: Danggui, Shudihuang, Renshen, Baizhu, Tiaohuangqin, Chenpi, Xiangfu, Xuduan, Duzhong. Pill form with glutinous rice.
