Shenghui Fang records: Pediatric gan disease (a wasting disorder in children) presents with a variety of symptoms. Although the severity and clinical manifestations differ, upon careful investigation of the root cause, the treatment principles are identical. This condition arises from improper feeding, imbalance of cold and heat, pathogenic invasion of the zang-fu organs, and failure of blood and qi to nourish the body, resulting in gan disease. For gan diseases of the five zang organs and other types, the same prescription is now used for common treatment; hence it is called "Yiqie Gan" (All Gan Diseases).
Qian Yi discussed various gan diseases, stating: When gan is internal, it manifests as swollen eyes, abdominal distension, diarrhea with unpredictable stool color, sometimes with greenish-white froth in the stool, and gradual emaciation; this is a cold syndrome. When gan is external, it manifests as redness and ulceration under the nose with the child rubbing it, sores on the nasal tip that do not scab over, and sores spreading around the ears. For treating nasal sores and ulceration, Lanxiang San (Orchid Fragrance Powder) is used (see Men of Nasal Gan). For other sores, Baifen San (White Powder) is the primary treatment (see Men of Gan Sores). For Liver Gan, characterized by a white membrane obscuring the pupil, Bungan Dihuang Wan (Liver-Nourishing Rehmannia Pill) is indicated (see Men of Deficiency-Cold). For Heart Gan, characterized by sallow complexion, flushed cheeks, and high fever, Bu Xin Anshen Wan (Heart-Nourishing Calming Spirit Pill) is indicated (see Men of Deficiency-Heat). For Spleen Gan, characterized by yellowish skin, abdominal distension, and pica for dirt, Buti Yihuang San (Spleen-Nourishing Yellow-Regulating Powder) is indicated (see Men of Stomach Qi Disharmony). For Kidney Gan, characterized by extreme emaciation and skin sores/scratches, Bushen Dihuang Wan (Kidney-Nourishing Rehmannia Pill) is indicated. For Tendon Gan, characterized by bloody diarrhea and emaciation, Bungan Dihuang Wan is indicated. For Lung Gan, characterized by asthma and sores in the mouth and nose, Buti Fei Yihuang San (Spleen-Lung Nourishing Yellow-Regulating Powder) is indicated. For Bone Gan, characterized by a preference for lying on cool surfaces, Bushen Dihuang Wan is indicated. All gan diseases should be treated by tonifying the mother organ of the affected zang and administering gan-specific medicines. Cold-type gan is treated with Muxiang Wan (Costus Pill), and heat-type gan is treated with Huhuanglian Wan (Coptis Pill) (both formulas are found in the Men of Post-Illness Emaciation). Gan disease is fundamentally a spleen-stomach disorder caused by the depletion of body fluids. It results from major illnesses or excessive vomiting/diarrhea where doctors use emetics or purgatives, leading to spleen-stomach weakness and fluid loss. Moreover, pediatric gan is often iatrogenic, caused by incompetent physicians. For instance, tidal fever may indicate one deficient zang and one excess zang generating internal deficiency heat; the method should be to tonify the mother zang and drain the affected zang to achieve cure. If tidal fever occurs at noon, it is heart deficiency heat. Since liver is the mother of heart, one should first tonify the liver; after the liver is sufficient, drain the heart. Once the heart receives the qi of its mother, internal balance is restored, and tidal fever resolves. Physicians seeing tidal fever mistakenly assume excess and use cold-purgative drugs like Dahuang (Rhubarb Rhizome) and Yaxiao (Mirabilite) to purge. Excessive purging leads to uncontrollable fluid loss, resulting in gan. Another condition is pi accumulation (masses), which presents with alternating chills and fever, thirst, and hard painful masses under the ribs. The treatment for pi should be gradual dissolution, but doctors wrongly use harsh purgatives like Badou (Croton Seed) and Sharen (Amomum Fruit). Children's constitutions are prone to both deficiency and excess; excessive purging depletes stomach fluids, gradually leading to gan-related emaciation. In cases of cold damage lasting five to six days with occasional indications for purgation, excessive use of cold purgatives reduces spleen-stomach fluids, causing unquenchable thirst and internal heat. Internal consumption by heat and external wasting of muscle, combined with interference from other pathogens, leads to diverse symptoms and eventually gan. Prolonged vomiting/diarrhea or wrongful purgation causes severe deficiency and dry/fluid loss, also forming gan. Fei Gan (Fatty Gan) is synonymous with Pi Gan. It presents with emaciation, yellowish skin, dry skin with sores/scratches. Symptoms vary widely; briefly summarized: dry eyes or white membranes, red lips, yellow/dry/blackish skin, preference for cool surfaces, pica, skin sores, green/white/yellow frothy stools, variable watery diarrhea, abdominal fullness, sores on body/ears/nose, matted hair, large head with thin neck, extreme emaciation, and polydipsia are all signs. Generally, gan must be differentiated into cold/heat and fat/thin. Early-stage gan is Fat-Heat Gan; late-stage is Thin-Cold Gan. Cold gan uses Muxiang Wan; heat gan uses Huhuanglian Wan (see above). Both cold and heat gan are particularly suitable for Rusheng Wan (see Men of Gan Diarrhea).
Therefore, children's zang-fu are delicate and cannot withstand harsh attacks; strong purgation inevitably depletes fluids and causes gan. Where purgation is indicated, it must be measured according to age and deficiency/excess status to avoid causing gan. For early-stage gan with low fluids, one should generate stomach fluids using Baizhu San (Atractylodes Macrocephala Powder) (see Men of Stomach Qi Disharmony). More is better. See below for the rest.
Zhang Huan stated: After one hundred days, children have light bones and fine intestines/stomach; breastfeeding must be appropriate for natural harmony of zang-fu. If the nursing mother has poor cold/heat regulation, improper conduct, irregular diet, excessive sweet/greasy food, emotional turmoil, or nurses immediately after drunkenness/exertion, gan disease will definitely result. Between one year and five years old, as solid food intake increases, indiscriminate consumption of raw/cold foods and indulgence in greasy/sweet/sour foods will generate gan qi. Pediatric gan diseases are numerous and urgent.
Tonglun on Various Gans in Tongying Baojian: Pediatric gan presentations are described in many ways, leading to confusion and loss of the main point. Key signs include stiff yellow hair, emaciated body, abdominal pain, pica, irregular diarrhea/dysentery, persistent night sweats, abdominal distension with wheezing, thin legs difficult to walk, prolapsed rectum from loose stools, frequent high fever, sleeping face down, crying/restlessness, intestinal parasites, rice-like particles in stool, milky stools, intermittent vomiting, resembling consumptive fatigue. Accompanied by shivering, these constitute gan. There are twelve distinct types: Jing Gan (Startle Gan) in the Heart; Feng Gan (Wind Gan) in the Liver; Qi Gan (Qi Gan) in the Lung; Rou Gan (Flesh Gan) in the Spleen; Ji Gan (Urgent Gan) in the Kidney. Beyond these five zang gans, twelve other types are discussed separately. Gan Gan (Dry Gan) can eat and drink, craves sour/salty foods. Ji Gan presents with dysentery/prolapse, multi-colored stool, inability to gain weight despite eating, somnolence, and sweating. Feng Gan presents with tremors, eye deviation, erratic laughter/anger, blue nails, resembling spirit possession. Rou Gan presents with dry/painful eyes, indigestion, sallow emaciation, limb weakness, abdominal distension, and wheezing. Ji Gan (Spine Gan) involves parasitic attack, itching of spine/back/fingers, stiff yellow hair, dry skin/muscle, hypochondriac fullness, frequent diarrhea, and serrated spine appearance. Kou Gan (Mouth Gan) presents with black lips/gums, oral ulcers, gum ulceration, and emerging worms. Nao Gan (Brain Gan) presents with red/ulcerated nose base, self-rubbing, fever, dry body, and red eyes. Shi Gan (Food Gan) presents with nocturnal tidal fever, aversion to cold, cold limbs, emaciation after feeding, and night sweats. Nai Gan (Milk Gan) occurs post-illness due to maternal toxicity rising or ingestion of toxic milk; initially shortness of breath, progressing to emaciation, chronic diarrhea, triple-jiao heat, and five-zang exhaustion. Hui Gan (Roundworm Gan) presents with prone sleeping, shortness of breath, sallow face, loud crying, cardiac pain-like symptoms, and periodic attacks at the start of the lunar month. Pi Gan (Spleen Gan) involves pica for dirt/rice/salt, depression, yellow body, salivation, and worms in stool. Qi Gan may result from hot milk or severe illness; sudden cough, worsened by cold medicines, leading to whole-body fever, icy feet, wheezing, and blurred vision.
Huiyan Guanzheng states: Gan etiology stems from consuming fruits, melons, grease, and excessive sweet/sticky foods. Signs include emaciation, protuberant abdomen, thin legs, and zang-fu dysfunction (obstruction or diarrhea). Treatment begins with regulating qi; after two days, administer decoctions/pills for purgation based on size (see Men of Acute/Chronic Convulsions). If pulse is fine and solid, heavily purge to remove evil matter, then follow with Taqi (Settle Qi), Yunqi (Harmonize Qi), Xingpi (Awaken Spleen), and Pingwei (Peace the Stomach) regimens (see respective men). Subsequently, continuously administer Wuxijiao Wan (Black Rhinoceros Horn Pill), Lushui Wan (Aloe Pill), and Shenling San (Ginseng and Poria Powder) for a month until weight gain indicates recovery. Chronic cases treated repeatedly may transform into Guzhen Gan Lao (Bone-steaming Gan Consumption) with oral ulcers and upper-jiao deficiency heat, progressing to Dingxi (Stiff Neck/Wasting) and Bulu (Nursing Dew). This is due to severe spleen deficiency. Initial treatment settles qi; after two days of qi movement, administer Xingpi. The goal is qi sufficiency and small meals, followed by Yunqi with Chaihu Wan to reduce bone-steaming heat. If heat subsides, use Aloe for consumption. Regulation with Yunqi is key for Dingxi and Bulu. Dingxi can be purged to remove gan worms. Bulu presents with visible abdominal veins and undigested food; regulate first, wait a month for qi sufficiency before purging.
