(The symptoms are the same for diarrhea and dysentery)
From *Huiji Lun* (Treatise on Compassionate Relief), the song regarding the syndrome of generalized edema after pediatric dysentery has subsided (should be used in conjunction with the Taci San formula below):
Cold-type dysentery, if left untreated for a long time, results in generalized edema as if inflated like a balloon. A surging pulse indicates that qi stagnation has transformed into water retention, while a deep and solid pulse suggests the presence of internal accumulation. Promoting qi circulation to reduce swelling is the superior treatment method; once qi regulation is achieved, urine output will likely increase within two days. Avoid overfeeding to prevent stagnation; the root cause of this syndrome lies in accumulation damaging the Spleen.
*Taci San* (Qi-Settling Powder) from *Huiji Lun*:
Fennel Seed (Foeniculi Fructus), White Morning Glory Seed (Baiqianniu, Semen Ipomoeae Praepratae), Licorice Root (Gancao, Radix Glycyrrhizae Preparatae) [all stir-fried], Costus Root (Muxiang, Radix Aucklandiae). (Each ingredient: 1 qian).
Grind the above ingredients into fine powder. Take 0.5 qian per dose, administered with Perilla Decoction (Zisu Tang).
*Zhike Shengxiao San* (Thirst-Relieving Miraculous Effect Powder) from *Wangshi Shouji*: Treats pediatrics cases where vomiting and diarrhea lead to Qi deficiency and fluid depletion, resulting in infantile malnutrition (Gan syndrome), incessant thirst and drinking, facial and foot edema, a large head with a thin body, clear and white urination, and complete anorexia.
Kudzu Root (Gegen, Radix Puerariae Lobatae), Angelica Dahurica (Baizhi, Radix Angelicae Dahuricae) [each 2 liang; 1 liang stir-fried until yellow, 1 liang raw]. Fine Ink Stick (Ximo, Mo), Yellow Lead Oxide (Huandan, Minium). (Each 2 liang; for both Ximo and Huandan, 1 liang is calcined/fire-treated, 1 liang is raw).
Grind all ingredients together into fine powder. Take 0.5 qian per dose, mixed with "flowing-back" water (Dao Liu Shui).
(Stagnant Dysentery with Red and White Stool) - Twelve Categories
Mr. Mao states: Children from birth up to ten years old may suffer from eight types of dysentery, each described individually. Red dysentery is caused by accumulated heat in the Zang-Fu organs. White dysentery is caused by accumulated cold in the Zang-Fu organs. Accumulation-injury dysentery presents with stool half resembling soil color, originally due to injury from milk or food. Shock-accumulation dysentery features stool mixed with green mucus-like substance, occurring when shock was not addressed promptly, leading to shock accumulation. Spinal drip dysentery presents with unpredictable colors of five hues, refusal to nurse, also known as Five-Flower Closed-Mouth Dysentery; this is due to accumulated toxins in the Five Zang organs blocking the orifices. Medicinal toxicity dysentery discharges stool resembling fish brain paste, resulting from prolonged dysentery treated incorrectly by physicians, hence named medicinal toxicity accumulation dysentery. Lock-mouth dysentery involves total inability to eat, constant craving for water, and post-autumn Spleen deficiency, also known as Regulated Diarrhea. Whenever treating dysentery leads to diarrhea, or treating diarrhea leads back to dysentery, this indicates slipping of the Large Intestine and Spleen deficiency with heat, also known as accumulation toxins in the Zang organs. Heat-toxin and Wind-toxin dysentery discharges resemble grass juice or red bean juice, dripping continuously; this is caused by the Spleen receiving wind-heat toxins. Such dysentery has a mortality rate of nearly 100%, representing a malignant prognosis.
Upon observing any of these eight types of dysentery, regardless of color or quantity, first regulate Qi using Yunqi San (see Section on Disharmony of Stomach Qi) combined with Xingpi San (two formulas available: one in Section on Disharmony of Stomach Qi, one in Section on Slow Spleen Wind), Xianglian San, and Ruxiang San for two days. (Both formulas seen in Section on All Diarrheas.) Once yellow stool begins to appear, administer Pianyin Wan once (see Section on Accumulation Diarrhea) to expel three to five movements of dysenteric accumulation. Then use Yunqi San and Xingpi San to harmonize Qi. Subsequently, regularly take Xianglian Wan (see Section on All Diarrheas) combined with Tiaozhong Yin to achieve recovery. (Tiaozhong Yin see Section on Disharmony of Stomach Qi.) If extreme thirst, refusal to eat, oral ulcers, dry nose, and abdominal distension occur, it is a death sign and untreatable.
Mr. Wang Handong in *Jiabao* states: The eight types of pediatric dysentery arise from eight pathogenic factors. Uneven cold and heat, or wind-heat entering the Zang organs, causes dysentery. Heat dysentery presents with red stool, cold dysentery with white stool, mixed cold and heat with red and white stool, food dysentery with sour odor, shock dysentery with green stool, Spleen dysentery with undigested food in stool, epidemic dysentery with blood in stool, and Gan dysentery with intermittent scrotal discharge. These constitute the eight dysenteries.
*Wukai Guan Zhenzhunang* records eight pediatric dysentery syndromes: 1. White pus dysentery; 2. Fish brain dysentery; 3. Five-color dysentery; 4. Blood mass dysentery; 5. Watery diarrhea dysentery; 6. Abdominal distension dysentery; 7. Mass accumulation dysentery; 8. Red and white dysentery.
Mr. Mao’s Song on Untreatable Death Signs for Eight Pediatric Dysenteries:
Frequent dysentery with complete anorexia, abdominal distension with coarse panting, stool completely black as ink, whole-body fever accompanied by extreme thirst.
