(Manifesting only as vomiting and diarrhea, rather than cholera-like syndrome)
*Zhu Bing Yuan Hou Lun* (Treatise on the Pathogenesis and Symptoms of All Diseases), Syndrome of Vomiting and Diarrhea in Children: Vomiting and diarrhea result from intestinal deficiency and rebellious stomach qi. After a newborn's umbilical cord is cut, if wind-cold pathogens invade the intestines and stomach, the deficiency of the intestines and stomach leads to diarrhea, while rebellious stomach qi causes vomiting. This condition resembles cholera-like syndrome in general presentation but is milder and less severe, accompanied by mental restlessness and fatigue; therefore, it is directly termed "vomiting and diarrhea" (Tu Li) and not referred to as "cholera-like syndrome" (Huo Luan).
Qian Yi discussed cases where newborns within three days exhibit vomiting, diarrhea, high fever, anorexia, and undigested milk/food in stool, or white stool. This is food stagnation (Shang Shi), requiring purgation followed by harmonizing the stomach. For purgation, use *Baibing Zi* (White Pill Decoction) (see this section); for harmonizing the stomach, primarily use *Yihuang San* (Yellow-Protecting Powder) (see Stomach Qi Disharmony section).
Qian Yi discussed cases from three days to ten days after birth presenting with vomiting, diarrhea, normal or slightly cool body temperature, anorexia, greenish-white stool, and undigested milk/food. This indicates excess above and deficiency below (Shang Shi Xia Xu). Accompanying symptoms include: Lung governs sleep with eyes partially open (Lu Jing) and wheezing; Heart governs palpitations and thirst; Spleen governs lethargy and excessive sleep; Liver governs yawning and dizziness; Kidney governs muteness and photophobia. Purging methods should be applied based on the involved viscera. To tonify the spleen, use *Yihuang San*. These two syndromes often occur in autumn and summer. (*Yihuang San* formula see Stomach Qi Disharmony section).
Qian Yi discussed summer and autumn vomiting and diarrhea: Vomiting and diarrhea with high fever occurring after May 25th indicate heat syndrome, where nine-tenths of the child's viscera are hot. Due to heat injury, vomited milk is undigested, and diarrhea is deep yellow; treat with *Yulu San* (Jade Dew Powder). Vomiting and diarrhea occurring after June 15th with normal body temperature, appearing slightly hot, indicate six-tenths heat and four-tenths cold in the viscera. Undigested milk in vomit, yellowish-white diarrhea, apparent thirst, variable feeding habits; administer *Yihuang San* before meals and more *Yulu San* after meals. Vomiting and diarrhea occurring after July 7th with normal-to-cool body temperature indicate three-tenths heat and seven-tenths cold. Inability to feed, excessive sleepiness, mental confusion, gasping, long sighs, sleeping with eyes open, white lips, frequent hiccups, urge to defecate without thirst; administer more *Yihuang San* before meals and less *Yulu San* after meals. Vomiting and diarrhea occurring after August 15th indicate coldness without yang, inability to feed, dry heaving, and greenish-brown watery diarrhea; tonify the spleen using *Yihuang San*, and do not use purgatives. (*Yulu San* see this section, *Yihuang San* see Stomach Qi Disharmony section).
Qian Yi debated treatments for vomiting and diarrhea: The Fifth Taiwei of the Guangqin Residence suffered from persistent vomiting and diarrhea with undigested grain. Physicians used tonic medicines, advising administration with ginger juice. In June, warm medicines were given, worsening wheezing and vomiting daily. Qian Yi stated: "It should be treated with cooling medicines because there is internal heat injury." He prescribed *Shigao Tang* (Gypsum Decoction) three times. Other physicians argued: "Severe vomiting and diarrhea with undigested grain requires spleen tonification; why use cooling medicine?" The Prince believed them and administered *Dingxiang San* (Clove Powder) three times. Qian Yi later arrived and said: "Do not take this. Within three days, abdominal distension, fever, thirst, and vomiting will occur." Three days later, it happened exactly as predicted. This occurred because the intense heat of June penetrated the abdomen, causing thirst; heat injuring the spleen and stomach led to severe vomiting and diarrhea. Other doctors' use of warm medicines caused upper jiao heat, leading to wheezing and thirst, resulting in death within three days. Unable to cure, they summoned Qian Yi again. Observing heat signs, he administered *Baihu Tang* (White Tiger Decoction) three times, followed by *Baibing Zi* to purge. On day one, reduce dosage by two fen; on days two and three, give *Baihu Tang* twice each. On day four, give one dose of *Shigao Tang*, compounded with *Mendong* (Ophiopogon Rhizome), *Huangqin* (Scutellaria Root), *Naozi* (Borneol), *Niuhuang* (Calculus Bovis), *Tianzhuhuang* (Calamus Rhizome), and *Fuling* (Poria), coated with *Zhusha* (Cinnabar), five pills per dose, dissolved in bamboo leaf decoction. Fever subsided, and the patient recovered.
Qian Yi discussed purgation based on deficiency and excess: A five-year-old son of Feng Chengwu presented with vomiting, diarrhea, high fever, and anorexia. Qian Yi noted: "Few black pupils and many white sclerae, pale complexion; this child is prone to illness. Pale complexion indicates deficient spirit; few black pupils indicate kidney deficiency. Black pupils belong to Water; inherently deficient and weak, hence prone to illness. Even upon growth, muscles will not be robust, and tolerance to cold and heat will be poor. Prone to shifting between deficiency and excess, spleen and stomach are also weak; further indulgence in desires is forbidden. Without care, lifespan will not reach adulthood. A lack of spiritual luster on the face resembles blood loss in women. Current vomiting, diarrhea, anorexia, and high fever indicate food stagnation. Do not purge, as it induces deficiency. Pathogens entering Lung cause wheezing; entering Heart cause fright; entering Spleen cause diarrhea; entering Kidney worsen deficiency. Use *Xiaoji Wan* (Accumulation-Dissolving Pill) to grind away accumulation, as it is mild food stagnation. If food stagnation is severe, purgation is required; otherwise, it becomes a mass (Pi Ji). Only when solid food stagnation exists internally can purgation be used. After purgation, tonify the spleen for recovery. Following deficiency/excess principles ensures efficacy."
Qian Yi addressed a difficult case: The Seventh Taiwei of Guangqin Palace, aged seven, suffered vomiting and diarrhea in July. Symptoms included total anorexia, lethargy, waking with confusion, gasping, dry heaving, intermittent stools, and no thirst. Physicians treated for fright due to sleep. Qian Yi said: "Tonify spleen first, then clear heat. Give *Shijunzi Wan* (Quisqualis Pill) to tonify spleen, and *Shigao Tang* to clear heat." The next day, mercury and sulfur were used to purge, mixed with ginger water. Qian Yi stated: "For vomiting/diarrhea in May, use nine parts purgation and one part tonification. In August, use ten parts tonification and zero parts purgation. This is spleen-deficiency diarrhea; erroneous treatment caused deficiency. Purging would be fatal. Only tonify spleen; *Shijunzi Wan* is gentler." Qian Yi also retained warming stomach and strengthening spleen herbs to stop symptoms. Physician Li Sheng asked: "What caused this?" Qian Yi replied: "Spleen deficiency prevents eating; low fluids cause rebellious qi." Asked about greenish-brown watery diarrhea, he answered: "Intestines and stomach are extremely deficient, extreme cold." Qian Yi cured him.
Qian Yi's attached formulas state that pediatric vomiting and diarrhea generally require warming and tonifying. I commonly use *Lizhong Wan* (Regulate the Middle Pill) to warm the middle jiao, and *Wuling San* (Five-Ingredient Powder with Poria) to guide rebellious qi downward; *Wuling San* is most effective for pediatric vomiting. Continuous administration combined with warming agents like *Yigong San* (Extraordinary Efficacy Powder) often cures the condition. If already deficient, rapidly generate stomach qi using *Fuzi Lizhong Wan* (Aconite Regulate the Middle Pill) mixed with powdered *Jinye Dan* (Golden Liquid Pill), administered with ginger and rice drink decoction. High doses are necessary for effect (up to 2-3 liang is safe). Once stomach qi returns, hands and feet warm, yin recedes, and yang returns. However, if convulsions persist, reduce *Jinye Dan* by 1-2 fen and increase *Qingzhou Baiwan Zi* (Qingzhou White Pills) by 1-2 fen, grinding together and administering as before. Adjust gradually, adding *Yigong San*, *Qianghuo Gao* (Notopterygium Paste), *Wenbai Wan* (Warm White Pill), *Gouteng Yinzi* (Uncaria Drink), etc., until stable. Following this regimen with frequent gruel allows survival even in critical cases, saving 8-9 out of 10.
*Yingtong Baojian*: If a wet nurse drinks alcohol or engages in licentious behavior, the milk becomes pathological. Symptoms include vomiting/diarrhea of green/yellow water, fever, and crying as if frightened. Untreatable.
