According to *Shenghui Fang* (Prescriptions from the Holy Healer): Slow convulsion in infants and children (Man Jing Feng) arises from irregular breastfeeding, leading to stagnation of Qi and blood in the Zang-Fu organs, accumulation of internal heat, and invasion by wind-pathogens, which then lodge in the Heart. The manifestations include: alternating periods of quietness and agitation, restlessness of the mind-spirit, vomiting of phlegm and saliva, high fever, stiffness and difficulty in movement of the sinews and vessels, frequent startles during sleep, and if the wind-heat is not resolved, the condition changes unpredictably, fluctuating in severity over a prolonged period; hence it is called slow convulsion. It requires prompt treatment.
Mr. Mao states: Some infants suffer from slow convulsion immediately after birth, presenting with upward deviation of the eyes, spasms of both hands, rapid breathing, rattling of phlegm in the throat, alternating quietness and agitation, mental confusion, and loss of awareness. This syndrome results from fright invading the Heart residence; it can also arise from vomiting and diarrhea, severe dysentery, or spleen deficiency following chronic diarrhea/dysentery where wind-pathogens invade due to emptiness. Upon observing these signs, immediately administer one dose of Shuijing Gao (Sleeping Convulsion Paste) (see this section) mixed with urine and wine. After a moment, the spasms will cease, and the child will vomit two cups or more of greenish frothy phlegm, or three to five cups, after which they may slightly revive. Immediately follow with two doses of Yunqi San (Harmonizing Qi Powder) (see Stomach Qi Disharmony section). If there remains a small amount of phlegm in the throat, administer Zhusha Gao (Cinnabar Paste) (see Fright and Accumulation section). The next day, administer Xingpi San (Awakening Spleen Powder) three doses (two formulas available: one in Stomach Qi Disharmony section, one in Slow Spleen Wind section), and Zhenxin Wan (Calming Heart Pill) two doses (see All Fright sections). If latent heat persists, administer Dafu San (Great Aconite Powder) (see this section); peace should be restored within three days. Signs such as turning away from the mother, shaking the head, protruding and retracting the tongue, biting at the breast, ptosis with tearing, and unilateral spasms indicate a fatal prognosis and are incurable.
Qian Yi states: Slow convulsion occurs after serious illness or vomiting and diarrhea, resulting in damage to the Spleen and Stomach. Symptoms include coldness throughout the body, cold air exhaled from the mouth and nose, intermittent spasms of the limbs, lethargy, and sleeping with eyes open (exposing the pupils). This indicates a lack of Yang. It is treated with Gualou Tang (Trichosanthes Decoction) (see this section). In all cases of acute and slow convulsion, the Yin and Yang syndromes differ; careful differentiation is essential. Acute convulsion requires cooling and purging methods, while slow convulsion requires warming and tonifying methods. Many folk prescriptions fail to distinguish between them, causing much harm to children. Furthermore, if a child contracts wind-cold leading to vomiting and diarrhea, and the physician treats it as Spleen deficiency with warming tonics without success, then switches to cooling herbs without success, this is termed: originally wind injury, but mishandled by the physician's aggressive attacks, leading to Spleen Qi deficiency. Internally unable to disperse and externally unable to release, after ten-plus days, symptoms include excessive sleep, exposed pupils during sleep, warm body, and wind pathogen in the Spleen and Stomach causing loose stools. One must remove wind from the Spleen; once wind recedes, diarrhea stops. Xuanfeng San (Dispersing Wind Powder) is indicated (see this section). Subsequently, Shijunzi Wan (Quince Pill) is used to tonify the Stomach (formula not seen). There are also cases of prolonged vomiting and dysentery that do not heal, where Spleen deficiency generates wind, forming slow convulsion.
Qian Yi discusses Yin Epilepsy and deteriorating conditions: The son of Mr. Wang in Dongdu suffered vomiting and diarrhea. Various physicians used purgatives until he became extremely deficient, transforming into slow convulsion. Symptoms included sleeping with eyes open and spasms of hands and feet with a cold body. Qian Yi said: "This is slow convulsion." He administered Gualou Tang. Because the child's Stomach Qi was still substantial, his eyes opened and his body warmed. Mr. Wang suspected urinary and fecal retention and asked other physicians to use diuretics/purgatives. They prescribed Bazheng San (Eight-Normal Powder) etc., but multiple doses were ineffective, and the body became cold again. When asked to promote urination, Qian Yi said: "One should not promote urination. Doing so will inevitably cause coldness in the body." Mr. Wang replied: "The body is already cold." Upon examining the child, Qian Yi said: "Inability to eat indicates Stomach emptiness. Promoting urination and defecation will lead to death. Prolonged deficiency leads to simultaneous Spleen and Kidney deficiency, manifesting as cold body and closed eyes. Fortunately, prenatal Qi is strong and hard to deplete." Qian Yi used Yihuang San (Yellow Protecting Powder) (see Stomach Qi Disharmony section) and Shijunzi Wan four doses (formula not seen). He advised light diet; by noon, the child could indeed eat. The reason is that promoting urination/defecation causes Spleen-Stomach deficiency-cold; one must tonify the Spleen, not attack elsewhere. Later, the child lost speech. Physicians treated it as aphonia. Qian Yi said: "If truly aphonic, how can he open his eyes and eat? Also, why are the teeth not clenched and lips not tight?" The physicians did not understand. Qian Yi used Dihuang Wan (Rehmannia Pill) to tonify the Kidney (see Deficiency-Cold section). The rationale: using clearing/draining herbs for urination caused Spleen-Kidney deficiency. Now the Spleen is solid, but the Kidney is still deficient; thus, tonifying the Kidney ensures recovery. After half a month, he could speak; after one month, he recovered.
Qian Yi states: Slow convulsion arises after major illness, vomiting, or diarrhea, or due to mismanagement causing Spleen-Stomach damage, allowing wind-pathogens to invade. (Note: Persistent vomiting/diarrhea in infants leads to slow convulsion; treat promptly). Symptoms resemble spasms but are mild (this is called chi zong [spasmodic contraction]), resembles sleep but with sluggish spirit, cold limbs and breath, sleeping with eyes open, or stomach pain with crying sounds like those of evil spirits. This syndrome is critical due to Spleen-Stomach deficiency.
Zhang Huan states: If infant acute convulsion is not resolved, fluctuating over time, with alternating quietness/agitation, vomiting phlegm, and severe tidal spasms, it is named slow convulsion disease and requires prompt treatment.
*Miyao Zhimi* (Secret Essentials Pointing to the Mystery) states: For any infant newly born, regardless of days, if sudden slow convulsion occurs, waking with cold extremities and unconsciousness, but slight warmth in the heart region, immediately administer Cezi Tang (Side Aconite Decoction).
*Wuguan Guanzhenzhunang* (Five Passages Piercing the True Pearl Sac) describes the slow convulsion syndrome: Body is not significantly hot, appears fatigued but cannot sleep, intermittent uncontrollable crying, refusal to nurse. These are signs of slow convulsion, generated by flourishing wind.
Mr. Mao's Song on Infant Slow Convulsion Signs:
Eyes looking up indicates wind-fright,
Hands and feet frequently spasm uncontrollably.
Phlegm rattling in the throat requires first resolving,
If phlegm transformation fails, judge the situation carefully.
Mr. Mao's Song on Fatal Signs of Infant Slow Convulsion:
Slow convulsion is truly difficult to cure,
When the whole body is heavy with high fever,
Sleeping often with startles, mind unstable,
Hand pulling, foot twitching, accompanied by panting.
Protracted duration characterizes this sign,
Quickly administer medicine, do not hesitate.
*Yujue* (Jade Decisions) Song on Infant Slow Convulsion Deficient Wind Syndrome:
Long sighs, grinding teeth, face greenish-yellow,
High milk-phlegm injures Stomach Qi,
Flourishing wind and high phlegm generate spasms,
Frequent diarrhea/dysentery with abnormal color.
For this affliction, first regulate Qi, then expel wind, subsequently drain phlegm, and harmonize Zang-Fu. Then there will be no error.
*Shibi Jing* (Stone Wall Classic) Song on the 36 Types of Impending Slow Convulsion:
Before onset, the child sleeps well, (48 signs say: impending slow convulsion precedes with heavy sleep). Tongue protrudes and head shakes during sleep, Face is green, hair stands like needles, ( *Fengsui Jing* says: upon fright onset, face red, hair stands). High fever throughout, legs curved like hooks. (*Fengsui Jing* 48 signs say: additionally hands and feet like hooks. This sign often results from untreated typhoid or erratic behavior of the wet nurse, causing fever or vomiting/diarrhea, compounded by cold pathogen distress, gradually leading to more sleep, less sweat, refusal to nurse, weak limbs, or stiff curvature. This requires sweating, awakening the Spleen, and removing wind for recovery.) Vomited milk smells fishy, generating foul breath, Sweat beads flow from the forehead, Unblocking intestines and releasing exterior sweat cures the child, If purged, diarrhea continues endlessly. (Still, must sweat first, then purge gently; purging must not be too fierce, and use mild warming medicines to generate Stomach Qi.)
This disease is injured by cold Qi, interstices blocked, accumulating into major affliction. Before onset, fatigue prevails; upon onset, face is green, head/forehead sweaty, vomited milk smells fishy. If this disease exists, first resolve the exterior, then use Lang Wan (Mantis Egg Case Pill) (see All Dysentery section) one or two doses. For robust children, mildly purge, then regulate Qi for peace. If this does not resolve, it becomes Slow Spleen Wind.
*Fengsui Jing* agrees with this song, with notes: First give HT Jian San (decoction powder) to release the exterior (see Complicated Fright Typhoid section), then Wuxi Gao San (Black Rhinoceros Horn Paste Powder) (see Acute/Slow Convulsion section).
*Shibi Jing* Song on the 36 Types of Active Slow Convulsion:
Nausea arises, then fright follows, Frequent vomiting/diarrhea, Qi invades upward, Tongue red, lips crimson, both eyes closed, ( *Fengsui Jing* these three lines say: Vomiting/diarrhea arises then nausea, Cholera vomiting milk sweat invades, Tongue out, lips crimson, both eyes closed.) Head shakes, hair straight like needles. (This sign also relates to exhaustion injury. Cold Qi has manifested outward, toxic vapors internally invade and injure the Stomach Meridian, initial internal heat sensation, thus frequent vomiting or diarrhea, unquenchable thirst. Untreated, it leads to exhaustion; severe exhaustion triggers spasms. Not severe/tight, just occasional mild limb movement. Endures half a month to ten days without healing, but not fatal. Mostly no sweat, dry body. Should awaken Spleen, induce sweat, remove fright.) Suffocation, angry Qi, rapid breathing, spasms of both flanks, White ulcers in mouth, life cannot bear it. This sign should be treated early before arising; Phlegm flooding lungs and viscera is harder to control. (*Fengsui Jing* this line says: Ulcers, Stomach closure, life heavy. If Qi urgent, flanks move slightly; movement means shortness of breath and panting. Another note: Using cold medicines excessively forces toxic vapors to injure Spleen/Stomach, causing mouth ulcers. If red, still treatable. If white ulcers fill mouth like pearls, eyes fixed, pupils unresponsive, face black without luster,必定 fatal.)
*Fengsui Jing* this passage says: Infant slow convulsion and slow spleen wind form after vomiting/diarrhea. Note: If no mouth ulcers, it is fine to first give Baihe Dan (White Crane Pill) (see this section), then Shenbai San (Divine White Powder) (see Excessive Sleep section).
*Xiao'er Xingzheng Lun* (Discussion on Infant Forms and Syndromes) 48 Signs: Post-song commentary on vomiting/diarrhea transmitting to slow convulsion says: Upon onset, vomiting/regurgitation occurs first. Physicians fail to recognize it, instead administering Rongwei-regulating medicines, causing mouth ulcers and bad signs. First use Wan (Pill) (see All Epilepsy section) to purge chest/diaphragm wind-phlegm; if not vomiting, then regulate Qi.
*Huiyan Guanzheng* (Wisdom Eye Observing Syndromes) states: Slow convulsion signs include alternating quietness/agitation, vomiting or diarrhea, or due to trauma, or dysentery, Spleen-Stomach acting due to emptiness. If flesh halos are seen, crying with tears, slight limb tremors, urgently administer Manpi Tang (Slow Spleen Decoction) and Tang/Wan to drain. (See Acute/Slow Convulsion section). Next day, regulate Qi, concurrently advance Xingpi Pingwei Huaxian Tang (Awaken Spleen, Level Stomach, Transform Phlegm Decoction) for conditioning. If tidal fever first, then mouth pulls to one side, only one side limbs spasm, sawing sound in throat, no crying, no tears, this sign is incurable, death within three days, eye membrane forms after death, Five Viscera perish.
Li Gangzhong, a physician from Changsha, says: Yin is quiet and slow, Yin is sluggish and late. Qian described slow convulsion arising after major illness, vomiting/diarrhea, or mistaken ingestion of cold medicines causing purging, leading to weak intestines/stomach, wind-pathogen invasion. Resembles spasms but mild, resembles sleep with exposed pupils, limb spasms, or crow-like sounds. This syndrome is critical due to Spleen-Stomach deficiency. The Foot-Taiyin Spleen and Foot-Yangming Stomach interior-exterior pairs are both deficient, jointly affected by disease, overwhelmed by wind excess. People regard Stomach Qi as the root. The Stomach is the sea of water and grain, the great source of the Spleen. It receives water and grain; clear parts become Ying (Nutritive) Qi, turbid parts become Wei (Defensive) Qi. Spleen Qi resembles Earth, centered in the middle. Qi and Blood circulate to irrigate the four directions. Infants' Qi and Blood are unsettled, Five Viscera just formed. Again, irregular breastfeeding, cold-heat clashing causes vomiting/diarrhea. Prolonged non-healing leads to dual Spleen-Stomach deficiency, internal wind invasion, thus green face, dull eyes/sleep, cold mouth/nose breath, limb spasms. Physicians using Iron Powder, Mercury, Borneol, Cinnabar etc. is like carrying firewood to throw into a fire. Thus Qian used Qingzhou Baiwanzi (Qingzhou White Pills) powder and Jin Ye Dan (Golden Elixir Pill) powder, measuring weight and proportion, mixing the two, adjusting with rice drink to govern Spleen-Stomach. Wait for limbs/warmth, then gradually reduce, then administer medicines to awaken Spleen and drive out wind. This is Qian's teaching and mirror. There is also Slow Spleen Wind, similar to slow convulsion, differing only in severity. It also arises from infant dual Spleen-Stomach deficiency and internal wind invasion.
