According to *Chao Shi Bing Yuan* (Source of Diseases by Master Chao) regarding the syndrome of pediatric Gu poison dysentery: When the seasonal cold and heat are disordered, there exists toxic and pestilential Qi. Children with loose pores are injured by it. The pathogenic factor contends with blood and Qi, entering the intestines and stomach. Toxic heat accumulates; when the large intestine is deficient, it transforms into bloody dysentery. The appearance of the dysentery involves blood clots resembling slices of chicken or duck liver, which are passed with the stool. This occurs because toxic heat is abundant and attacks the zang organs, presenting symptoms similar to being poisoned by Gu, hence it is called Gu poison dysentery.
The song from *Shi Bi Jing* (Book of Stone Walls) regarding the thirty-six types of internal spleen poison dysentery:
Poison in the spleen is called Pure Yang, originally caused by physicians' use of hot medicines. This leads to frequent constipation and dryness in the large intestine, with much dripping blood in the withered intestines. It resembles wind causing abdominal closure and difficulty opening eyes, body heat, warm head, and cooling feet. Red tongue and high chest indicate this syndrome; frequent crying and rapid breathing require careful examination. (Forty-eight syndromes state: even resembling madness.)
First, one must clear heat and open the stomach; this is the good medication of a wise physician. This syndrome arises from spleen heat accumulation and lack of treatment; latent poison should be treated by cooling the spleen, followed by removing accumulation. If the bone above the chest suddenly protrudes, accompanied by urgent crying, it is incurable.
The song in *Feng Sui Jing* (Phoenix Marrow Classic) is identical. A note states: It is suitable to administer Jinhua San (Golden Flower Powder) and Xianglian Wan (Cinnamon and Coptis Pill). (Xianglian Wan formula is seen in the Cold-Heat Dysentery section, same as Ji's formula. Jinhua San formula is seen in the Excess Heat section.)
The forty-eight syndromes spleen poison dysentery song in *Xiao Er Xing Zheng Lun* (Discussion on Pediatric Signs and Symptoms) is also identical. It further states: This syndrome involves accumulation in the zang-fu organs, or red dysentery; hot medicines are not suitable. One should use Ka Wei San (Stomach-Opening Powder) (formula seen in the Red Dysentery section) combined with purgatives, assessing deficiency and excess, slightly inducing evacuation, generally aiming for unblocking and rotation.
The forty-eight syndromes wind poison dysentery song in *Xiao Er Xing Zheng Lun*:
Among eight types of dysentery, wind type is hardest to treat, appearing like various shades of green grass juice. Poisonous wind and bean juice add evil heat, stomach failure results in complete pieces like chicken liver. Adding red does not require prior evacuation of accumulation; closed eyes and no food intake are unbearable to look at. If returning to white dysentery, evacuation is still needed; frequently warming the zang-fu organs retrieves the source.
