Ma Pi Feng (Horse Spleen Wind) is Pulmonary Distension (Lung distension). It presents with upper body wheezing and rapid breathing, flapping of the two hypochondriac regions, nasal flaring with labored breathing, hoarse wheezing sounds, and obstruction by phlegm and saliva. This syndrome is dangerous and ominous; it requires urgent purgation. Niu Huang San (Calculus Bovis Powder) is the primary prescription.
Black and white Qian Niu Tou Mo (Head powder of *Pharbitidis Semen*, 1 liang), Da Huang (*Rhei Radix et Rhizoma*, 2 liang), Bing Lang (*Arecae Semen*, 5 qian), Mu Xiang (*Aucklandiae Radix*, 3 qian), Qing Fen (Calomel, a small amount). The above are ground into powder and mixed evenly. Each dose is taken mixed with cold water or Jiang Shui (fermented rice water).
Someone asked: Why is it called Ma Pi Feng? I replied: Wu (noon) corresponds to the Horse and Shaoyin Jun Huo (Ministerial Fire). The Heart governs Heat, and the Spleen governs Deficiency. Heart Fire attacks Lung Metal, and Phlegm from the Spleen rises, causing Lung distension and wheezing. Hence it is called Ma Pi Feng.
A four-year-old child suddenly developed wheezing, with rebellious Qi and obstructed phlegm, and open nostrils. I said: This is Ma Pi Feng. If the chest is high and shoulders shrug, with sweating that wets the hair, it is incurable. It must be treated urgently. Use Ting Li Wan (Descurainiae Fructus Pill), removing Fang Ji (*Akebiae Caulis*), adding Da Huang (*Rhei Radix et Rhizoma*) to clear Lung Heat, combined with Xiao Xian Xiong Tang (Minor Chest Trap Decoction) to clear Lung Phlegm. Grind into fine powder and administer mixed with Zhu Li (Bamboo Sap), resulting in recovery.
Due to post-illness vomiting or diarrhea, Spleen and Stomach deficiency damage occurs, leading to coldness of the whole body and cold mouth and nose, occasional convulsions of the hands and feet, lethargy with eyes half-open. This indicates Yanglessness. It should be treated before symptoms manifest. Diao Yuan Tang (Regenerate Origin Decoction) combined with Xiao Jian Zhong Tang (Minor Construct the Middle Decoction) is the primary prescription. If these signs appear, even with divine pills, it is incurable. Someone asked: Why does vomiting and diarrhea generate Wind, and why is it incurable? I replied: The principle of Five Elements states that excess Qi attacks what it overcomes, while deficiency allows what overcomes it to attack. Vomiting and diarrhea damage Spleen Earth. Spleen belongs to Earth, and Wind is generated by Liver Wood. When Spleen Earth is deficient, Liver Wood attacks it, meaning Wood overcomes Earth. Such illness is difficult to cure. In the human body, grain and water are the root. Excessive vomiting prevents intake; excessive diarrhea prevents retention. Vomishing injures Qi, and purging injures Blood. When the source of grain and water is exhausted, and Qi and Blood are ruined, how can one not die?
Someone asked: Wind syndromes should be treated by addressing Wind, so why do the prescribed formulas not use Wind-dispelling herbs? I replied: *Nei Jing* (Inner Canon) says: "The Liver dislikes tension; soothe it with Sweet flavors, gather it with Sour flavors, and disperse it with Pungent flavors." It also says: "The Spleen desires relaxation; urgently eat Sweet flavors to relax it." In Diao Yuan Tang, Ren Shen (*Ginseng Radix*) and Gan Cao (*Glycyrrhizae Radix*) are Sweet. Sweetness can relieve Liver tension and is the holy medicine for treating Wind, while also tonifying the Spleen. Shao Yao (*Paeoniae Radix*) and Gui Zhi (*Cinnamomi Ramulus*) are Bitter and Pungent, following the nature of the Spleen, to construct the Middle Jiao. Combining these two formulas to treat Man Jing Feng (Slow Convulsion Wind) is the secret transmission of Elder Dong Yuan (Li Dongyuan).
Because of suffering from Jing Feng (Convulsion Wind), doctors used too many drugs to drain shock, damaging the Spleen and Stomach, further depleting Yuan Qi, transforming into Man Jing Feng. This is when external Wind has not receded, and internal Wind arises from central deficiency. Wind and deficiency fight each other; righteous Qi departs while pathogenic factors remain, leading to imminent death. This Man Jing Feng syndrome especially requires caution at the beginning.
A five-year-old child suffered from dysentery. Doctors used medication, but the dysentery worsened due to sinking of the Middle Qi of the Spleen and Stomach. I used Shen Ling Bai Zhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) to regulate him. After ten days, he recovered. I took my leave. A skeptic told the father: "No accumulation, no dysentery. Sons of wealthy families often have meat accumulation. I have As Wei (Ferula resin), which I have used effectively for dysentery." The father was confused and listened. He made As Wei into pills, size of small beans, and administered three pills continuously. The son fell into lethargy. Fortunately, I arrived again. Upon hearing about the As Wei pills, I exclaimed: "Although As Wei removes meat accumulation, it severely damages Yuan Qi. Your son's Spleen and Stomach are already weak; how can he take this?" The father said: "The sick child lies quietly and likes to sleep, not yet awake." I said: "Have the wet nurse call him. If his eyes are half-open, his Qi is already extinct."
A child with inherently weak Spleen and Stomach suffered diarrhea one day. He was given Li Zhong Wan (Regulate the Middle Pill), but the diarrhea did not stop, and ulcers formed in the mouth. I reasoned that the previous medicine was hot and aided fire. Administering more medicine resulted in slight fever and upward-gazing eyes during sleep. Seeing this, I said: This child is developing Man Jing Feng. The Spleen and Stomach are originally deficient; diarrhea increases deficiency. Ulcers in the mouth indicate Spleen deficiency heat. Misusing cold medicines further damages Middle Qi, causing lethargy and refusal to nurse, representing extreme deficiency. Urgently prepare Diao Yuan Tang with doubled Ren Shen (*Ginseng Radix*). Regulate for half a month, and he recovered.
A girl experienced convulsions within five months. I administered Xie Qing Wan (Drain Green Pill). After three or four doses, convulsions persisted and worsened. I pondered that phlegm obstruction and Qi stagnation caused the convulsions. The pill and powder were coarse, sticking to the throat with phlegm, blocking Qi flow, thus worsening convulsions. I decocted Zhu Ye (Bamboo Leaf) as soup, filtered it through cotton paper to remove dregs, clarified it, and administered it. Convulsions stopped. The father sighed: "The value of a doctor lies in such adaptability."
A child had Zhen Chu (True Convulsion). I said it was incurable. The family invited a Tuina practitioner to pinch acupoints. The child protected against pain, stared with wide eyes, and moved his mouth. The family rejoiced. The child looked sideways; the practitioner said he was looking at his mother. The child opened his mouth; the practitioner said he was seeking his mother's milk. I sighed: "Mistaken. Seeing the child's eyeballs turn is called 'looking at mother'; urgently opening the mouth is called 'seeking milk.' Both are death omens." That night, the child indeed died.
The Liver governs Wind. Ji Jing Feng (Rapid Convulsion Wind) involves spasms and tremors, generated by the movement of Qi in the Liver meridian. It requires urgent treatment. If Heart Heat develops, use Xie Qing Wan (Drain Green Pill) and Dao Chi San (Direct to the Heart Powder) decocted together to send down the medicine, resulting in recovery. At initial onset of convulsions with lethargy and unresponsiveness, pinch Ren Zhong (GV26), or pinch Da Ling (PC7), or moxa Zhong Chong (JE9). Wait for awakening before administering medicine. Alternatively, use Bai Jiang Can (Bombyx Batryticatus), Zhu Ya Zao Jiao (Gleditsiae Sinensis Spine), Xi Xin (Asari Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), and Li Lu (Veratri Rhizoma) in equal parts, ground to powder, insufflated into the nose. Those who sneeze can be treated; those who do not cannot.
For stubborn phlegm obstruction, use Bai Jiang Can powder to induce vomiting, or Meng Shi Gun Tan Wan (Mica Pill to Roll Phlegm) to induce vomiting. Use family-prescribed San Huang Wu Se Wan (Three Yellows Five Colors Pill) to purge.
When children develop convulsions, treat according to this method. If convulsions stop, it is auspicious. If they come and go, with lethargy, unresponsiveness, and refusal to nurse, it is fatal.
A one-year-old child had continuous convulsions. Warm breath from mouth and nose indicates Zhen Chu (True Convulsion), which is incurable. If the child wakes easily after convulsions and breath is hot, it is Jia Chu (False Convulsion), which is treatable. Qian Yi said: Warm temperature indicates internal generation, showing the true organ disease of the Liver, hence "True." Hot temperature indicates external origin, hence "False." My late father treated an infant who developed convulsions after one month using Zhi Sheng Bao Ming Dan (Supreme Sage Preserving Life Pill), resulting in peace.
Ancestral instructions for treating Ji Jing Feng use only Xie Qing Wan (Drain Green Pill) and Dao Chi San (Direct to the Heart Powder).
The eldest son of Zhang Yueshan from Jiuxian suffered from Ji Jing Feng, unconscious for seventeen days. By the time I was summoned, his tongue color was black. I once saw my father reading *Yu Han Jing* (Jade Box Classic): "If the tongue is black and washing doesn't make it red, medicine washing clearly shows fortune or misfortune." I asked: What medicine to wash with? Father said: Bo He Tang (Mint Decoction). So, urgently following the method, I washed the tongue with Bo He Tang. The tongue turned red. I said: Treatable. Administered Xie Qing Wan (Drain Green Pill), 2 qian, decocted and drunk. One swallow, and dry mouth/thirst stopped. That night, convulsions ceased, heat retreated, and he was peaceful. This child would have nearly died without me.
Wang Yuanjin's young son, in July, contracted malaria due to food injury, developing convulsions on the seventh day. I examined him: Liver Wind was severe, but Spleen was not yet exhausted. Drain Liver and tonify Spleen. Used Xie Gan San (Drain Liver Powder); convulsions stopped after three doses. Later used Diao Yuan Tang (Regenerate Origin Decoction) to tonify Spleen, and Hu Po Bao Long Wan (Amber Embracing Dragon Pill) to calm Liver. The child liked to sleep, eyes unable to open. I reasoned that liking to sleep is not Spleen exhaustion, but spirit confusion and lack of alertness. Eyes belong to Liver, eyelids to Spleen. Closed eyes are not loss of Hun (Soul), but spiritual fatigue. Now the child's eyes want to open and close. Use the previous formula. Two days later, had people who usually played with him bring small drums and cymbals, singing and dancing before the bed in the room. Before half a day passed, eyes opened, and he recovered. Peaceful after ten days.
Hu Fengya's son had food injury and malnutrition after smallpox, emaciated with sparse hair. A medical assistant saw him and said: Not malnutrition, but Blood deficiency. The family disagreed. Initially effective, then another illness arose, resembling epilepsy but not quite. Quiet by day, dreaming by night. Holding the wet nurse, crying "I'm afraid, I'm afraid," like being hunted. Inquiry revealed the child refused medicine. The assistant forced medicine, threatening with needles and cautery, causing this disease. Stomach is Wu Earth, Kidney is Gui Water, combining to transform into Fire. Kidney governs Fear; fear injures Kidney. Because Spleen and Stomach are weak, failing to generate Lung Metal, Kidney lacks transformation source and becomes deficient. Kidney stores Zhi (Will); Kidney deficiency leads to restless spirit and will, generating fright. Awake, spirit resides in Heart; asleep, spirit resides in Kidney. Spleen is the portal for will. Must primarily tonify Spleen, secondarily calm spirit. Use Bu Pi Fei Er Wan (Tonify Spleen Enlarge Child Pill) and Qian Shi An Shen Wan (Qian's Spirit-Calming Pill). Regulated for half a year, then peaceful.
Tiao Diao (Sky Hook) and Nei Diao (Internal Hook) involve the Foot Jueyin Liver Meridian. It starts at the big toe, circles the genitalia, crosses left to right and right to left, enters the lower abdomen, meets below, runs along the spine to the vertex. If Wind injures Liver, Tiao Diao occurs: eyes roll up, head tilts back, body arches backward, completely like bow-string tension. Qian Yi said: Severe Liver Wind causes opisthotonos. Tiao Diao belongs to Wood, suitable for dispersing. In Xie Qing Wan (Drain Green Pill), remove Da Huang (*Rhei Radix et Rhizoma*), add Tian Ma (*Gastrodiae Rhizoma*), Quan Xie (Scorpio), Jiang Can (Bombyx Batryticatus), Gou Teng (Uncariae Ramulus cum Uncis).
Nei Diao: Liver receives Cold, causing lower abdominal pain, loud crying, fixed gaze, but no convulsions. Urgently warm the interior. Use Dang Gui Zhu Yu Tang (Angelica and Evodia Decoction) and Mu Xiang Wan (Aucklandia Pill).
Dang Gui Zhu Yu Tang
Dang Gui (*Angelicae Sinensis Radix*), Wu Zhu Yu (*Evodiae Fructus* (roasted and dried)), Xiao Hui Xiang (*Foeniculi Fructus* (stir-fried)), Gan Cao (*Glycyrrhizae Radix*), Mu Xiang (*Aucklandiae Radix*).
Mu Xiang Wan See Convulsion Syndromes category.
Xiao Er Chang Tong (Child Intestinal Pain) is also in the lower abdomen and waist flexion, crying hollowly without tears. Named Pan Chang Tong (Coiled Intestine Pain), similar to Nei Diao, but without fixed gaze. Use Jin Ding Zi San (Golden Ingot Powder), see Syndromes category.
Shan (Hernia) is pain in the Jueyin Liver Meridian, unrelated to Kidney, caused by Cold. Swollen but painless is named Jin Shan (Sinew Hernia); painful but not swollen is named Qi Shan (Qi Hernia); swollen and painful is named Shui Shan (Water Hernia). Use Zhu Yu Nei Xiao Wan (Evodia Internal Resolving Pill).
The eldest son of Grand Administrator Liang in our county suffered from hernia. Right testicle swollen like a chicken egg, length about 5 cun, connecting to umbilicus side, reaching scrotum, straight and hard, with blocked urination and defecation. Urgently summoned Quan. I formulated: Dang Gui (*Angelicae Sinensis Radix*), Chuan Xiong (*Ligustici Chuanxiong Rhizoma*), Mu Xiang (*Aucklandiae Radix*), Qing Pi (*Citri Reticulatae Pericarpium Viride* (remove pith)), Shan Zhi Ren (*Gardeniae Semen*), Shan Zha (*Crataegi Fructus*), Xiao Hui Xiang (*Foeniculi Fructus*), Chuan Lian Zi (*Toosendan Fructus*), Ze Xie (*Alismatis Rhizoma*). Two doses brought peace.
Scrotal swelling occurs in irritable children who cry much. I once treated a child, formulating: Xiang Fu (*Cyperi Rhizoma*), Chuan Xiong (*Ligustici Chuanxiong Rhizoma*), Mu Xiang (*Aucklandiae Radix*), Qing Pi (*Citri Reticulatae Pericarpium Viride*), Shan Zhi Zi (*Gardeniae Fructus*), Mai Ya (*Hordei Fructus Germinatus*) in equal parts, made into pills.
Another case of Child Intestinal Pain: I used another formula from *Zhu Zheng Bian Yi*: Wu Ling San (Five Ingredient Powder with Poria) plus Chuan Lian Zi (*Toosendan Fructus*), Xiao Hui Xiang (*Foeniculi Fructus*), with a pinch of salt decocted. Miraculous effect.
Liver opens into the eyes. Red and painful eyes indicate Liver Heat. Use Xie Qing Wan (Drain Green Pill) plus Huang Lian (*Coptidis Rhizoma*), made into pills.
White membrane covering pupils indicates Liver Deficiency. Use Xie Qing Wan (Drain Green Pill), remove Da Huang (*Rhei Radix et Rhizoma*) and Zhi Zi (*Gardeniae Fructus*), add Gan Ju Hua (*Chrysanthemi Flos*), Mu Zei (*Equiseti Herba*), Chan Tui (*Cicadae Periostracum*), made into pills.
Classic says: When Liver has Heat, urine turns red first. Use Dao Chi San (Direct to the Heart Powder) plus Zhi Zi (*Gardeniae Fructus*), Tiao Qin (*Scutellariae Radix*), Dan Cao (*Gentianae Radix*), Gan Cao Shao (*Glycyrrhizae Radix et Rhizoma caulis*).
Miss San Shou in our prefecture excreted urine like green water, causing ulceration where it touched flesh. Her mother worried. She asked me: Could her organs be damaged? I answered: Bladder receives fluids from five Zang organs to store them, which is urine. Color follows the respective Zang. Green is Liver color. Ulceration at contact site indicates盛 Liver Fire. Fire burns and causes ulceration. I treated her alone with the previous formula for red urine, adding Huang Bo (*Phellodendri Cortex*) as pills. Regulated for five days, then peaceful. Mother greatly delighted, ordered Miss out to pay respects.
Miraculous Formula for Treating Yin Odor
Dang Gui (*Angelicae Sinensis Radix*), Dan Cao (*Gentianae Radix*), Shan Zhi Zi (*Gardeniae Fructus*), Mu Tong (*Akebiae Caulis*), Che Qian Zi (*Plantaginis Semen*), Ze Xie (*Alismatis Rhizoma*), Gan Cao Shao (*Glycyrrhizae Radix et Rhizoma caulis*), Tiao Qin (*Scutellariae Radix*). Decoct in water and drink.
When Liver is diseased, bowel movements are difficult. Use Xie Qing Wan (Drain Green Pill) and Mu Tong San (Akebia Powder).
Qian Yi said: When Liver has Heat, hands rub clothes and pick at things randomly, or teeth grind.
A fifty-day-old infant cried day and night incessantly. I used Xie Qing Wan (Drain Green Pill), 5 li, mixed with Zhu Ye (Bamboo Leaf) decoction and a little rock sugar. Stopped immediately.
Continuous cough leading to convulsions indicates Lung decline with Liver Wood insulting it. First tonify Lung with E Jiao San (Donkey-Hide Gelatin Powder), then drain Liver with Xie Qing Wan (Drain Green Pill). Treat convulsions first; if cough persists, it is incurable. Water is scarce and fears [Fire]. Qian Yi stated: Three times tonifying Lung fails to stop cough; three times draining Liver fails to stop Liver convulsions.
Cai Shajiang, a Juren scholar from Qishui, had a son with persistent cough. Invited me. I saw him coughing continuously, face turning blue during coughs, right hand moving constantly. To Shajiang, I said: Your son is incurable. Shajiang asked why. I said: Cough is Lung disease, Lung belongs to Metal. Blue face is Liver color, Liver belongs to Wood. Hand waving is sign of impending Liver Wind. Wood insults Metal; scarcity fears [Metal]. Currently October, time when Metal is sick and Wood generates. Order of four seasons: incoming advances, successful retreats. Wood generates and advances, Metal sick and retreats. Convulsions are incurable. Aggravates on Jia/Yi days. Indeed died convulsing on Jia/Yi days.
Daughter of Scholar Gan in Huangzhou Prefecture, after fright, right thumb flexed and unable to extend. Doctors used Quan Xie (Scorpio) and Jiang Can (Bombyx Batryticatus) without effect. Asked me for treatment. Liver governs sinews; sinews rely on Blood nourishment. Thus, palm bends when receiving Blood. Blood dryness causes sinew withering, inability to bend/extend. Thumb is exit of Hand Taiyin Lung Meridian. Metal nature is dry; using Wind drugs worsens dryness. Liu Zongnong said: Do not treat Wind, do not treat Dryness. When Fire is treated, Wind and Dryness resolve. Prescribed: Ren Shen Gu Ben Wan (Ginseng Solidify Root Pill) plus Huang Qin (*Scutellariae Radix*), Huang Bo (*Phellodendri Cortex*), Zhi Mu (*Anemarrhenae Rhizoma*), made into pills.
Convulsions in infants within 100 days are most malignant, called Tai Jing (Fetal Fright). Qian Yi's discussion is detailed.
