When food stagnation persists for a long time, it stubbornly condenses into a mass (Pi Ji). Accumulation on the left transforms into phlegm, which arises from food stagnation; accumulation on the right transforms into Qi, which knots with phlegm. It should be eliminated quickly. For chronic cases, it takes seven to ten days to dissipate. The massage techniques should include separating Yin and Yang, moving the Eight Trigrams, moving the Five Meridians, pinching the Four Transverse Creases, pushing the Three Gates, and tonifying Spleen Earth. Long-term rubbing and pressing of the "string" area combined with rubbing and kneading techniques, lateral pushing of the Large Intestine meridian up to the Tiger's Mouth point, clearing Liver Fire, clearing Lung Meridian, and rubbing at Tianmen (Heavenly Gate), the line between Tianmen and Tiger's Mouth, and the elbow. The prescription involves crushing Field Snail (Cipraeum) and Plantago Seed Herb (Cheqiancao) and applying the paste to the Dantian (Sea of Qi) acupoint.
Swelling presents with ten types of symptoms, generally arising from damp-heat and Spleen deficiency. A floating pulse indicates wind or deficiency; a deep and hidden pulse indicates water disease. A deep pulse suggests weak meridians; a hidden pulse suggests difficulty in urination, which constitutes Zheng Shui (standard edema). A deficient and large Spleen pulse often indicates Spleen enlargement. If neglected and untreated, it develops into edema. This is because Spleen Earth prefers dryness and dislikes dampness. When Spleen Earth fails, it cannot control water, leading to fluid retention and stagnation in the skin, causing swelling. Initially, the eyelids may appear swollen in the morning and subside slightly by afternoon. However, this condition is relatively easy to treat in summer, autumn, and winter. In spring, when rainwater floods and Liver Wood flourishes to overact upon Spleen Earth, making Spleen Earth unable to transform water, it becomes difficult to treat. The progression and regression are irregular, requiring gradual regulation to achieve efficacy. Generally, for pediatric edema, first use dispersing methods, then draining methods. Massage with scallion white and ginger, add pure sesame oil, then one cup of wine, a little refined salt, powder from one piece of Gleditsia Sinensis Fruit (Zaojiao), and one bowl of yellow earth, stir-fried together. Wrap in cloth, invert onto the palm, pinch the thumb joint, and the swelling will subside. Techniques should include separating Yin and Yang, moving the Eight Trigrams, pushing the Three Gates, pushing Spleen Earth, Yellow Bee Entering the Cave, moving the Five Meridians, rubbing the Two Doors of Wind to stop diarrhea, tonifying Kidney Water, operating at the Tiger's Mouth to elbow area, tonifying Spleen Earth, and moving Earth into Water. Qi distension is specifically due to Spleen deficiency failing to generate Metal (Lung), resulting in empty Qi causing bloating in the Lungs. Suitable techniques include separating Yin and Yang, moving the Eight Trigrams, pushing the Three Gates, tonifying Spleen Earth, moving Water into Earth, rubbing at Tianmen, Tiger's Mouth to elbow, and pressing the string with rubbing and kneading. Light vinegar can also be used for massage here. Another type of edema occurs because children overeat, damaging dampness, and Qi fails to circulate, causing swelling; this is neither water nor Qi, but food stagnation. Once the food disperses, the swelling naturally subsides. Techniques should include separating Yin and Yang, moving the Eight Trigrams, rubbing Zhongwan (CV12), pressing the string with rubbing and kneading, rubbing HT Men (likely a specific point or typo in original text), Tianmen, Tiger's Mouth to elbow rubbing, tonifying Spleen Earth, and moxibustion on Guwei (GV1). (For males, operate on the left; for females, on the right.)
