The Phlegm-Fire Syndrome is what is commonly known as Acute Convulsive Disorder (Ji Jing Feng). In children, whether due to contraction of Wind-Cold or accumulation of milk and food, phlegm is generated; when phlegm accumulates, it transforms into Fire. Alternatively, exposure to Summer-Heat also generates Fire; if not cleared and resolved in time, the Fire flares upward and phlegm is simultaneously produced. When Phlegm and Fire obstruct upwards, closing the Lung orifices, all orifices become blocked. The symptoms include fixed staring eyes, panting, coma and unresponsiveness. Moreover, excessive Fire causes Liver dryness and tendon tension, leading to convulsions, tremors, retraction of the eyeballs (Fan Yin Cuan Shi), thereby giving rise to the "Eight Signs" (Ba Hou) of convulsion. This is fundamentally caused by the stagnation of Phlegm and Fire, which stirs up internal Liver Wind. During periods of spasms and opisthotonos (body arching backward), one should only support with hands gently; do not hold tightly with force, lest meridians and collaterals be damaged, resulting in disability. Initially, Tong Guan San (Pass-the-Passage Powder) can be used to induce sneezing for opening the orifices; once sneezing occurs, consciousness returns. For mild cases, Li Huo Jiang Tan Tang (Fire-Reducing and Phlegm-Descending Decoction) is indicated; for severe cases, Qing Xin Jian (Clear Heart Decoction - *note: original text missing character, inferred*) with Shigao (Gypsum Fibrosum), Shichangpu (Acori Tatarinowii Rhizoma), and Zhuru (Bambusae Caulis in Recens) is used, or Bao Long Wan (Dragon-Hugging Pill). After waking up, Qing Re Yang Xue Tang (Heat-Clearing and Blood-Nourishing Decoction) is administered.
The Wood-Insulting-Earth Syndrome is what is commonly known as Chronic Convulsive Disorder (Man Jing Feng). Children contracting Summer-Heat or Cold, or injuring themselves with milk and food, may experience vomiting and diarrhea, or alternating vomiting and diarrhea. Over time, Spleen Earth becomes deficient, allowing Liver Wood to overact upon it. The stool gradually turns greenish, the face appears sallow with a bluish tint, hands and feet twitch slightly with weakness, and spirit is listless and depressed, thus forming Chronic Convulsive Disorder. At the onset, Yi Gong San (Benefiting Labor Powder) is appropriate; if vomiting occurs, Huoxiang (Pogostemonis Herba) and Wei Jiang (Zingiberis Rhizoma Preparatum) are added. If the disease has persisted for several days and the stool shows green color, Muxiang (Aucklandiae Radix) or Rougui (Cinnamomi Cortex) should be added. If hands and feet are cold, pulse is faint and fine, and lips and tongue are sallow and pale, this indicates impending collapse of Yang Qi; Fuzi Lizhong Tang (Aconite Root Decoction to Warm the Middle) must be urgently administered to warm the middle and restore Yang, offering hope for rescue. Pathologies of the five Zang organs are generally slow, but changes in Spleen pathology are extremely rapid; there are cases where vomiting and diarrhea lasting only one day lead to sudden collapse. Do not overlook this lightly.
Pediatric fingerprint diagnosis has traditionally lacked orthodox theory, being erratic and unverifiable. Some argue it should not be used; others use it to such absurd extremes that they deceive the public and confuse the world. These are theories that miss the mark, hence the divergent opinions on its utility. Permit me to offer my humble insights to clarify the essentials. Essentially, this fingerprint is the same type of pulse as the Cun, Guan, and Chi positions. According to the *Neijing* (Classic of Internal Medicine), the twelve meridians begin with the Hand-Taiyin Lung Meridian. The fingerprint is a branch collateral beside the Guan position, emerging from behind the wrist to the tip of the index finger, facilitating the communication of Ying and Wei Qi between the Hand-Yangming Large Intestine Meridian and the Hand-Taiyin Lung Meridian. Even Zhang Jingyue, a figure of clarity, still regarded this line as a superficial collateral of the Hand-Yangming Meridian. He failed to realize that the Hand-Taiyin Lung Meridian begins at Zhongfu (LU-1) and ends at Shaoshang (LU-11); the Hand-Yangming Large Intestine Meridian begins at Shangyang (LI-1). They do not directly meet. Without this branch to communicate Ying and Wei, would not the exterior-interior relationship between Taiyin and Yangming be severed? Furthermore, although this pulse can be diagnosed, people are unaware that its characteristics of slow/fast, intermittent/rapid are identical to those of the Taiyuan pulse (LU-9). It is merely smaller because it is a branch. Fingerprint diagnosis originated with Qian Zhongyang (Qian Yi) of the Song Dynasty, who divided the index finger into three sections: the first joint is the Wind Section (Feng Guan), the second is the Qi Section (Qi Guan), and the third is the Life Section (Ming Guan). His mantra states: Wind section is light, Qi section is heavy, Life section is critical. While his words may not be entirely accurate, the principle is acceptable. The position goes from bottom to top, pathogenic factors from shallow to deep, and conditions from mild to severe, making it credible to all. I only regret the emergence of bizarre fallacies (within the Convulsive Disorder category), causing later scholars to lose their way through too many divergent paths, turning it into a controversial case. I believe Qian Zhongyang, a renowned Song figure known for filial piety, would never have created such misleading practices. Most likely, these were fabricated by vulgar practitioners later, falsely attributing them to him. Only those with insight, recognizing the vulgar language and absurd arguments, discard them. Figures like Zhang Jingyue and Xia Yuzhu stated they need not be used. It is not that they cannot be used, but rather that they reject them due to their absurdity and lack of utility. Modern physicians, unaware of truth and falsehood, right and wrong, practice them habitually, thereby confusing the world and harming infants. Medicine concerns life and death; listening to baseless rumors and spreading harm is unacceptable. Though I am not wise, having a basic knowledge of meridians, I often feel hair-raising indignation wanting to correct pediatric fingerprint theories, regretting that I am not the right person to speak out. Unresolved in my heart, now that I have leisure, I will explain the essentials. This fingerprint is essentially a branch of the Taiyuan pulse; thus, changes in the fingerprint reflect changes in the Taiyuan pulse. There is no need to establish異說 (heterodox theories) to dazzle the mind. One should simply use floating/sinking to distinguish exterior/interior, red/purple to differentiate cold/heat, and pale/stagnant to determine deficiency/excess, and it will be endlessly useful. If one ignores this and clings to fallacies, failing to investigate the disease source during clinical practice, and mistakenly diagnosing human vs. animal fright, confusing the ignorant, I fear it is like a blind man riding a blind horse, falling into an abyss from which there is no escape. Zhou Luzhong said: "The precision of clarifying the fingerprint's meaning has been unmentioned for over seven hundred years since Zhongyang." Reading this now is like waking from a dream or sobering from drunkenness, refreshing the spirit, truly a unique insight of the ages. Since people may not fully trust this initially, I will summarize the erroneous traditions to correct them. Briefly: Blue lines indicate fright from four-legged animals; blue veins at the tiger's mouth indicate fright from pigs, dogs, or horses; black indicates fright from water immersion; red indicates fright from fire; purple often leads to diarrhea; yellow indicates fright from thunder. This implies that blue, red, and black fingerprints are all caused by fright. Consequently, all pediatric conditions—whether due to constitution, prenatal cold/heat, internal injury, external contraction, miscellaneous diseases, or hundreds of types of measles and smallpox—would be labeled as fright disorders and treated as such. Otherwise, excluding blue, yellow, red, and black lines, how else would one distinguish non-fright disorders? No error is greater than this. If yellow signifies Spleen deficiency, it refers to insufficient Spleen Earth due to injury; yet saying "yellow is thunder fright" suggests excess due to hearing thunder. If a child's fingerprint is yellow, should one diagnose Spleen distress or Thunder fright? Should treatment target the Spleen or the Thunder? Should they be treated together or separately? Spleen distress is deficiency; Thunder fright is excess. Treating deficiency while neglecting excess, or treating excess while neglecting deficiency, makes combined or separate treatment impossible, leaving one at a loss. Regarding blue indicating fright from humans or four-legged animals, the difference in spirituality and nature between humans and animals is vast, like ice and charcoal. Their qi transformations are pure and mixed differently; they absolutely should not be grouped together. If both are considered, then any blue fingerprint could be either human or animal fright, implying humans and animals are equivalent. Even distinguishing by curve size, assuming large curves are human fright and small curves are animal fright, how can we know humans don't have small curves or animals don't have large ones? What evidence confirms human fright always has large curves and animal fright always has small ones? If a large curve contains small parts, or a small curve has large parts, does one say "animal fright within human fright"? This is baseless nonsense. Regarding treatment, it is even more flawed. If human and animal fright are the same and require no differentiation or distinct treatment, then distinguishing sizes and sources is delirious. If they are different, what exactly are "human fright" and "animal fright" diseases? What treats which? Having named these frights, one should establish corresponding treatments, yet no method exists, only empty words to deceive. While quacks are blameless, compiling such errors into authoritative texts misleads future generations, causing outrage among the discerning. Furthermore, blue indicates cold, white indicates Gan (Malnutrition/Wasting). Blue fright repeats previous points. As for white indicating Gan, this is indeed absurd. Gan syndrome corresponds to Deficiency Consumption (Xu Lao) in general medicine; in children, it is Gan Accumulation, representing early damage to Spleen and Kidney. Prolonged, it damages all five Zang organs, exhausting Middle Qi, causing the face, eyes, and skin to turn white, resembling a skeleton. Thus, "white is Gan" refers to appearance. Applying this to fingerprints is severely erroneous. In cases of dual Qi-Blood injury and chronic Spirit depletion, the line must be pale, as with all deficiency syndromes, not just Gan. One can say the color is pale, but never white, as white lines never appear on fingers. In forty years of practice, I have never encountered one. Do not assume ancient words are immutable. Waiting for a white fingerprint to diagnose Gan means no child with Gan would ever be saved. Thus, following diagrams blindly is not true understanding; divine illumination constitutes mastery. Fingerprint differentiation is detailed below.
