[Xue Ji's Commentary] The *Classic of Internal Medicine* states: "The Stomach is the sea of water and grain, and the great source of the six fu organs." The qi and blood of the human body, as well as the functions of the zang and fu organs, are all generated by Stomach Qi. Therefore, Li Dongyuan's medical approach focuses primarily on regulating the Spleen and Stomach. The saying "tonifying the Spleen is better than tonifying the Kidney" embodies this very principle. In infants and children, although they consume milk and food, the essence derived from water and grain is not yet fully established; they rely especially on Stomach Qi. If Stomach Qi becomes deficient, the other four zang organs (Heart, Liver, Lung, Kidney) will all lose their nourishment. Thus, Zhu Danxi stated that children often suffer from disorders of the Liver and Spleen.
If the child presents with a pale complexion, lackluster eyes, cold breath, refusal to eat accompanied by vomiting of clear fluid, emaciation, and abdominal pain, this is a syndrome of Deficiency-Cold of Stomach Qi. It should be treated primarily with Wuwei Yigong San or Liu Junzi Tang. If the stool is unformed, indicating concurrent Spleen deficiency, add Ganjiang (Dried Ginger) to warm the interior. If there is abdominal fullness and difficulty in defecation, it indicates failure of the Spleen to transport; add Muxiang (Aucklandia Root) to move qi and relieve obstruction. If the child prefers cold drinks and suffers from constipation, it indicates Excess-Heat in the Stomach; treat with Xiehuang San to clear heat and cool. If Mingmen Fire is deficient and unable to warm the Earth (Spleen), use Bawei Wan to tonify. For congenital insufficiency of Stomach Qi, this pill is also used. This is because when the True Yang of the lower jiao is abundant, it generates Yuan Qi of the Spleen upwards, naturally enabling the warming and steaming digestion of water and grain.
Pingwei San: Treats disharmony of the Spleen and Stomach, loss of appetite, distending pain in the chest and abdomen, bitter taste in the mouth, shortness of breath, nausea, belching, acid regurgitation, sallow complexion, emaciation, lethargy, body aches, and syndromes such as cholera-like vomiting and diarrhea.
Ingredients: Houpo (Magnolia Bark, processed with ginger juice, 5 liang), Chenpi (Tangerine Peel), Gancao (Licorice Root, honey-fried, 1 liang each), Cangzhu (Atractylodes Rhizome, soaked in rice wash water, dried, 8 liang).
Preparation and Administration: Grind the above ingredients into fine powder. Take 2 qian per dose, decocted with fresh ginger and jujube, or it can be taken as a suspension in boiling water. Regular consumption regulates qi, warms the stomach, resolves retained food, eliminates phlegm-fluids, and wards off pathogenic factors of the four seasons.
My Note: For the aforementioned syndromes, if caused by retention of milk and food, characterized by belching with a putrid odor, acid regurgitation, vomiting, and nausea, this formula is appropriate. However, if the food has digested but the Spleen and Stomach remain weak, leading to vomiting and nausea, Sizijunzi Tang is more suitable.
Tiaozhong Wan: Treats Deficiency-Cold of the Spleen and Stomach.
Ingredients: Baizhu (Atractylodes Macrocephala Rhizome), Renshen (Ginseng Root), Gancao (Licorice Root, stir-fried, 5 fen each).
Bawei Dihuang Wan: Consists of Liudihuang Wan plus Rougui (Cinnamon Bark) and Fuzi (Aconite Lateral Root Processed), each 1 liang. It treats congenital deficiency of Mingmen Fire failing to warm the Earth, resulting in Cold-Deficiency of the Spleen Earth, poor appetite, indigestion, peri-umbilical pain, and frequent nocturia. As stated in the *Inner Canon*: "Benefit the source of fire to dissipate yin shadows." This medicine exemplifies that principle.
Qian Shi Yihuang San (also known as Bupi San): Treats vomiting and diarrhea due to Cold-Deficiency of the Spleen and Stomach. (Formula see previous section)
My Note: For the preceding formulas, if the syndrome involves Cold-Deficiency of Spleen Earth, or Water insulting Earth causing vomiting and diarrhea, cold hands and feet, surging phlegm-saliva, sleeping with eyes partially open (exposed pupils), and refusal of milk/food, this formula is indicated. For vomiting and diarrhea due to Spleen Earth weakness, use Liu Junzi Tang added with Chaihu (Bupleurum Root). If ineffective or if both hands and feet are cold, it belongs to Deficiency-Cold; add Muxiang and Paogjiang (Processed Dried Ginger). If the condition arises from the nursing mother's Spleen deficiency leading to Liver Qi invading the child, treat with the aforementioned medicines. If the nursing mother's depression and anger cause these symptoms in the child, the mother should concurrently take Jiaweigui Pi Tang.
Shenling Baizhu San: Primarily treats Spleen and Stomach weakness, refusal of food, fatigue, and shortness of breath, abdominal fullness, stagnation, belching, vomiting, and counterflow qi. This medicine is neither cold nor hot, with a balanced nature and flavor. Regular consumption harmonizes the Spleen, promotes a healthy complexion, supports righteous qi, and expels evil.
Ingredients: Renshen (Ginseng Root, remove rhizome), Baifuling (Poria, remove skin), Fencao (Licorice Root), Baizhu (Atractylodes Macrocephala Rhizome), Baibiandou (White Hyacinth Bean, stir-fried, remove shell), Shanyao (Chinese Yam, remove black skin), Suosharen (Amomum Fruit), Yiyiren (Coix Seed), Jiegeng (Platycodon Root, crushed and stir-fried; nine ingredients above, 1 liang each), Lianzirou (Lotus Seed Embryo, remove heart).
Preparation and Administration: Crush and dry the above ingredients, then grind into powder. Take 0.5 to 1 qian per dose, mixed with jujube decoction on an empty stomach, or warm rice soup may also be used.
Jianpi Yin: Establishes Spleen function, nourishes Stomach qi, regulates vomiting, treats dysentery and diarrhea, and is suitable for post-illness weakness, phlegm-induced nausea, mild abdominal pain, reduced diet, and mental sluggishness.
Ingredients: Houpo (Magnolia Bark, remove coarse bark, crush, soak overnight in ginger juice, slow-fire dry, then quench with pure vinegar, slow-fire dry again), Renshen (Ginseng Root, remove rhizome, 1 liang each), Baifuling (Poria, remove skin), Roudoukou (Nutmeg), Banxia (Pinellia Tuber, boil thoroughly, filter, crush, dry), Yizhiren (Alpinia Oxyphylla Fruit), Jing Xiangfu (Cyperi Rhizoma, cleaned), Liangjiang (Galangal Rhizome, slice, stir-fry with earth from the east wall), Hezierou (Chebula Fruit Flesh, 2.5 qian each), Gancao (Licorice Root, honey-fried, 5 qian).
Preparation and Administration: Crush the ingredients. Take 2 qian per dose, with one cup of water, two slices of ginger, and one jujube, decoct to 70%, and take at any time.
Huoxiang Yin: Regulates deficiency, transforms phlegm, treats disharmony of the Spleen and Stomach, reduced food intake, strengthens righteous qi, and expels pathogens.
Ingredients: Renshen (Ginseng Root, remove rhizome), Banxia (Pinellia Tuber, boil thoroughly, filter, slice, dry), Chifuling (Red Poria, remove skin), Gancao (Licorice Root, honey-fried, 1 liang each), Cangzhu (Atractylodes Rhizome, remove coarse bark, soak overnight in rice wash water, drain, slice, stir-fry until slightly yellow, 2 liang), Chenpi (Tangerine Peel, remove white pith), Huoxiang (Patchouli Herb, remove stem, 7.5 qian each), Houpo (Magnolia Bark, remove coarse bark, processed, 1.5 liang).
