Mr. Tang stated: For this type of diarrhea, tonification should not be applied immediately. First, use food-resolving medicines, or use Zishuang Wan (Purple Frost Pill) to eliminate accumulation, and only after the accumulation is cleared can tonification be used. The Classics state: In cases of severe food-induced diarrhea, one must consider deficiency due to malnutrition (Gan syndrome) and employ methods to tonify deficiency. To treat food-induced diarrhea, Xiangju Bingzi (Fragrant Orange Cake) may be modified, or Guanyin San (Guanyin Powder), Tiaozhong Tang San (Regulate the Middle Decoction Powder) may be used with modifications based on clinical judgment. Food-induced diarrhea is difficult to stop and is not suitable for immediate tonification. If diarrhea recurs due to further dietary injury, diet should be regulated. At this stage, Jinshi Wan (Advance Meal Pill) should be used to purge accumulated food, followed by Qian's Modified Yihuang San (Yellow Tiger Powder). Using a single herb from this formula can stop diarrhea; this is a crucial therapeutic principle. Subsequently, Yigong San (Yi Gong Powder) and Sijunzi Tang (Four Gentlemen Decoction) [indicated for vomiting and diarrhea] should be used for regulation to ensure complete recovery. For patients with borborygmus (rumbling in the abdomen) and dysentery, Shengjiang Xiexin Tang (Fresh Ginger Xiexin Decoction) is indicated. For diarrhea caused by cold accumulation, Xiangpo San (Fragrant Magnolia Bark Powder) is used to stop diarrhea, Baizhu San (Atractylodes Macrocephala Powder) is used to harmonize Qi and regulate the Stomach, Tiaozhong San (Regulate the Middle Powder), Bao'an Wan (Security Pill), and others can also treat food-induced diarrhea. Yinggan Wan (Responsive Pill), Chenxiang Jian (Agarwood Infusion), and Sanleng Wan (Sparganium Pill) [for fixed masses] are also applicable.
Li Dongyuan stated: Food injury leads to aversion to food. In children with food-induced diarrhea, it is because dietary intake has injured the Spleen, causing the Spleen Qi to fail in its transportation and transformation functions, resulting in undigested milk and food being expelled. If there is belching with a foul odor, acid regurgitation, chest and diaphragm fullness, and abdominal pain that worsens upon pressure, although diarrhea is present, the stagnant material has not yet been digested; Baohe Wan (Harmonize the Stomach Pill) should be used. If abdominal pain does not worsen upon pressure, it indicates that the milk and food have already been digested; Yigong San (Yi Gong Powder) should be used. If Spleen Qi is injured and has not recovered, leading to loss of appetite, Liujunzi Tang (Six Gentlemen Decoction) should be used. If the injury was caused by raw/cold foods or if the patient prefers warmth, add Muxiang (Costus Root) and Ganjiang (Dried Ginger). If the milk and food have been digested, abdominal pain has ceased, but diarrhea persists, this is due to the Spleen losing its clear Yang ascending Qi; Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. For other syndromes, refer to respective chapters.
Qian Yi recorded: The two-year-old son of Huang Chengwu suffered from food-induced diarrhea. Other physicians administered antidiarrheal agents for over ten days. The stool became greenish-white, milk and food remained undigested, the body temperature dropped, accompanied by hiccups and lethargy. All considered the condition critical and summoned Qian Yi. Qian Yi first prescribed Yihuang San (Yellow Tiger Powder) and Bufei San (Tonify Lung Powder), three doses each. After three days, the body temperature normalized, and hiccups ceased. Then, Baixingzi (White Pill) was used to mildly purge, followed by three doses of Yipi San (Strengthen Spleen Powder), which stopped the diarrhea. Why was this approach taken? The diarrhea originated from Spleen deficiency combined with food stagnation. Initially, purgatives were withheld. After ten days, this led to excess above and deficiency below. Weak Spleen Qi drew the Lung Qi into deficiency as well, due to the mother-son relationship between Spleen and Lung. By tonifying the Spleen first, the Lung pathology receded naturally, resulting in normalized body temperature and cessation of hiccups. Only then was the accumulated food purged. One might ask: Why not purge first and tonify later? The answer is that green stool indicates cold in the lower Jiao. Purging first would cause severe deficiency. By strengthening the Spleen first and then purging, deficiency is avoided. Subsequent tonification ensures stability.
