New-onset Gan syndrome is called Heat Gan, presenting with a sallow complexion and reddish cheeks, bone fever, night sweats, dry nose, foul breath, cracked lips, irritability, thirst, mental restlessness, palpitations, low mood, and unhappiness. If the Gan syndrome has persisted for a long time, it is called Cold Gan, presenting with swollen eyelids, abdominal distension, irregular bowel movements, greasy stools or those resembling oil droplets, accompanied by irritability, thirst, yellowish discoloration, and emaciation. Heat Gan typically involves pathology in the exterior (body surface), while Cold Gan typically involves pathology in the interior (internal organs). There are also cases where cold and heat syndromes interact, neither new nor old, and not attributable to internal or external causes.
Muxiang Wan (Aucklandia Pill) treats Cold Gan.
Ingredients: Muxiang (Costus Root), Qingdai (Indigo Naturalis, separately ground), Binglang (Areca Seed), Roudoukou (Nutmeg, peeled; each 1 fen), Shexiang (Musk, separately ground, 1.5 qian), Xuisui Zi (Euphorbia Seed, 1.5 liang, stir-fried), Hamo (Toad, three pieces, tied with string, dried, burned until carbonized but retaining efficacy).
Preparation: Grind the above ingredients into fine powder, mix with honey to form pills the size of mung beans. Take 3–5 pills up to 10–20 pills per dose, washed down with Bohe Tang (Mint Decoction), before meals.
Master Tang stated: Children with Cold Gan often experience excessive thirst, prefer lying on cool surfaces, exhibit irritability and crying, refuse food, and gradually become emaciated. The symptoms are difficult to clarify, resembling Heat syndromes, but characterized by slippery diarrhea unresponsive to various medications. A woman tried hundreds of remedies without success until she was given Qian Yi’s Muxiang Wan, resulting in recovery after just a few doses. Since then, this formula has proven effective for all such cases.
Huhuanglian Wan (Picrorhiza Pill) treats Heat Gan.
Ingredients: Huhuanglian (Picrorhiza Rhizome), Huanglian (Coptis Rhizome; each 0.5 liang), Zhusha (Cinnabar, 1 fen).
Preparation: Grind the first two ingredients into fine powder, insert the Cinnabar powder into pig gallbladders, suspend the gallbladders in light fermented grain water in a clay pot, and boil for the duration of one meal. Remove, then add Lvhui (Aloe) and Shexiang (Musk; each 1 fen). Mix with rice flour to form pills the size of sesame seeds. Take 5–7 pills up to 10–20 pills per dose, washed down with Mi Yin (Rice Drink), after meals. An alternative prescription adds Hamo (Toad, 0.5 liang) without burning.
Rusheng Wan (Like-Sage Pill) treats Cold-Heat Gan (see "Gan Diarrhea" section for the formula).
Chao Yuanfang stated: When children present with a sallow face, straight hair, intermittent high fever, inability of food intake to nourish muscles and skin, and eventually death after prolonged duration, it is called Wugu Gan (Innocent Gan). Legend says there is a bird named Wugu in the sky that hides by day and roams by night, washing children's clothes and mats. After dew falls overnight, the bird flies over, takes these clothes for the child to wear and mats for the child to sleep on, causing the disease. *Taiping Shenghui Fang* records that children with Wugu Gan have a nodule behind the head like a bean, which moves when pressed. This resembles the forbidden worm under the tongue of birds and beasts. If not removed quickly, it will endanger life. Initially, the nodule is soft and painless, containing worms like rice flour. With heat, it grows larger, leading to tendon binding. Once bound, the worms disperse with Qi and Blood, multiplying and eroding viscera, causing sores on muscles, or bloody purulent diarrhea, leading to gradual emaciation, large head with erect hair, weak limbs, and early death. So-called Wugu Gan Dysentery involves abdominal distension, bloody purulent diarrhea, withered hair and skin, progressive emaciation, deficient intestines and stomach, and irregular dysentery, hence the name. Master Wang from Handong stated: The ancient claim that a bird named Wugu urinates on sun-dried children's clothes causing illness and emaciation is incorrect. This condition is actually caused by Eight Evils: hunger, fullness, fatigue, labor, wind, fright, summerheat, and accumulation. Prolonged exposure leads to emaciation, failure to gain muscle from food, nighttime crying, slight fever, thirst, lack of appetite control, or skin sores/eczema. Zhu’s *Eight-Piece Brocade Song* sings: Child’s Wugu Qi arises from maternal misconduct. If care is prudent, emaciation is avoided. Fright and thin cold cause wind diseases; warmth and cold cause Qi flaccidity. Must remove sick milk, avoid heavy clothing. Do not feed too early; speech delay is not feared. Eight prohibitions in theory: frequent weakness in chest, shiny scalp, sparse hair. Liver congestion invades eyes; Spleen yellow enters limbs. Whole body rashes, widespread sores. Unceasing diarrhea, aversion to cold with no fixed time. Cannot play with embroidered balls or bamboo horses. White halos appear in pupils, clear saliva drips from mouth. Grinding teeth biting nails, raising hands to scratch brows. Nightly eating melons/fruits, morning eating soil/mud. Stomach injury causes intestinal bloating; Lung excess causes wheezing doubtlessly. Unceasing diet, sleeping drunk-like.
Fei'er Wan (Flesh-Nourishing Pill) treats Wugu Gan in children, characterized by a bean-sized object at the back of the neck/head, movable upon pressure, soft and painless. Long-term use yields miraculous effects.
Ingredients: Huhuanglian (Picrorhiza Rhizome), Shenqu (Medicated Leaven, stir-fried), Maifu (Wheat Bran; each 5 qian), Binglang (Areca Seed, 3 qian), Muxiang (Aucklandia Root, 2 qian), Roudoukou (Nutmeg, baked in flour coating), Shijunzi Rou (Quisqualis Fruit Flesh; each 2.5 qian).
