[Qian Yi] In infants within three days after birth, if vomiting, diarrhea, and high fever occur, with a refusal to suckle or eat, and stools containing undigested milk or appearing white, this is a Cold Damage (Shanghan) syndrome. It should be treated with purgation and stomach harmonization. For purgation, use Baibing Zi (White Pill) to treat accumulations; for stomach harmonization, use Yihuang San (Yellow-Protecting Powder) to treat Spleen deficiency. These are the primary treatments.
From three days to ten days after birth, if vomiting, diarrhea, and a lukewarm body temperature occur, with a refusal to suckle or eat, and stools appearing greenish-white with undigested milk, this indicates an Excess in the Upper and Deficiency in the Lower pattern. Additionally, there may be concurrent manifestations of the Five Zang organs: Lung involvement presents as sleeping with eyes half-open and panting; Heart involvement presents as palpitations and thirst; Spleen involvement presents as lethargy and excessive sleepiness; Liver involvement presents as yawning and irritability with chest oppression; Kidney involvement presents as muteness and photophobia. One must first observe the concurrent organ symptoms, first draining the Excess and then tonifying the Deficiency. For example, for Spleen deficiency, Yihuang San is used. These two patterns often occur in autumn and summer.
After the Summer Solstice in May, if vomiting, diarrhea, and high fever occur, this is a Heat syndrome, because infants have nine parts heat out of ten in their internal organs. This may be due to dietary injury from hot milk/food causing indigestion, resulting in deep yellow diarrhea. Yulu San (Jade Dew Powder) is used.
Yulu San Formula (also known as Ganlu San):
Hanshuishi (Calcitritum, half tael, soft and slightly dark green-black with fine lines inside)
Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma, one qian)
Shi Gao (Gypsum Fibrosum, half tael, hard, white, and textured like a wall, cannot be split by hand; if unavailable, Fangjiexie [Arsenolite] can substitute, which should be hard, white, similar to Gypsum, and break into square fragments when struck).
Grind all ingredients into a fine powder. Take one zi or five fen to one qian per dose, mixed with warm water after meals. (Haizang stated: Do not use this unless there is severe Fire of the Ministerial Heat in the Kidney.)
After Major Heat in June, if vomiting, diarrhea, and a significantly warm body resembling fever occur, indicating six parts heat and four parts cold in the internal organs, with vomiting of undigested milk, yellowish-white diarrhea, apparent thirst, variable appetite for milk, take small doses of Yihuang San before meals and large doses of Yulu San after meals. The Fifth Taiwei of Guangqin Palace suffered from incessant vomiting and diarrhea with undigested rice and grain. Physicians used warming medicines, but after one day, wheezing and irregular vomiting worsened. Qian Yi said, "It should be treated with cooling medicines because heat is injured internally." Use Shigao Tang (Gypsum Decoction) for three doses. Other physicians argued that since vomiting and diarrhea were severe with undigested food, the Spleen should be tonified, questioning why cooling medicines were used. Wang Xin agreed. The other physicians then used Dingxi San (Clove Powder) to tonify the Spleen for three doses. When Qian's physician arrived later, he said, "Do not take this. In three days, abdominal distension, body heat, thirst, and vomiting/regurgitation will occur." Three days later, it happened exactly as predicted. This is because the heat in June penetrates deeply into the abdomen, causing thirst; heat injures the Spleen and Stomach, leading to severe vomiting and diarrhea. The physicians' use of warming medicines caused heat in the Upper Jiao, leading to wheezing and thirst, worsening in three days. The senior teachers could not cure it. Qian Yi was summoned again, saw the heat signs, administered Baihu Tang (White Tiger Decoction) for three doses, followed by Baibing Zi for purgation. On the first day, reduce the dosage by two fen; on the second and third days, give Baihu Tang twice each. On the fourth day, give Shigao Tang once, along with a freshly prepared pill of Maidong (Ophiopogonis Radix), Huangqin (Scutellariae Radix), Bingpian (Borneolum Syntheticum), Niuhuang (Calculus Bovis), Tianzhuhuang (Spongilla Calcarea), and Fuling (Poria), coated with Zhusha (Cinnabaris). Take five pills, washed down with Zhuyetang (Bamboo Leaf Decoction). The heat subsided, and the patient recovered.
After Start of Autumn in July, if vomiting, diarrhea, and a moderate body temperature occur, indicating three parts heat and seven parts cold in the internal organs, inability to suckle, excessive sleepiness, irritability, sighing with long exhalations, sleeping with eyes half-open, white lips with frequent hiccups, desire to defecate without thirst, take more Yihuang San before meals and less Yulu San after meals. The Seventh Taiwei of Guangqin Palace, aged seven, suffered from vomiting and diarrhea in July. His symptoms included complete anorexia and coma, waking up irritable, sighing with dry retching, intermittent stools, and no thirst. Other physicians treated it as Wind-Stroke, suspecting it was due to sleep. Qian Yi said, "First tonify the Spleen, then clear heat." Give Shijunzi Wan (Quisqualis Pill) to tonify the Spleen and Shigao Tang to clear heat. The next day, mix Shuiyin (Mercurius) and Liuhuang (Sulfur) powders, taken with ginger water. Qian Yi stated: For vomiting and diarrhea, in May, use nine parts purgation and one part tonification; in August, use nine parts tonification and one part purgation. This case was Spleen-deficiency diarrhea, mistreated by physicians, leading to deficiency damage. Purgation would cause death; thus, Spleen tonification is required. Using Shijunzi Wan might be too slow, so warming and Spleen-tonifying medicines were retained. Physician Li Sheng asked, "What food causes hiccups?" Qian Yi replied, "Spleen deficiency and scanty fluids cause vomiting and regurgitation." Asked, "Why green-brown watery stool?" Answered, "Extreme emptiness of the intestines and stomach, and extreme cold." Qian Yi cured him.
After Autumn Equinox in August, if vomiting, diarrhea, and a cold body lacking Yang Qi occur, inability to suckle, dry retching/hiccups, green-brown watery stool, tonify the Spleen using Yihuang San. Do not use purgation.
[Tian Shi] In general, for pediatric vomiting and diarrhea during intense summer heat, if pathogenic heat is in the Lower Jiao, it causes diarrhea; if in the Upper Jiao, it causes vomiting. Loss of fluids leads to thirst. Use Yulu San. Even if vomiting occurs, administer frequently in small sips; recovery is expected within three days. If body heat and a large pulse are present with yellow urine, use half Wuling San (Five-Ingredient Powder with Poria) and half Yiyuan San (Benefiting Origin Powder), mixed with hot water and taken warm. If the body is cool with a fine pulse and blue-green urine, take Yihuang San in the morning and Yulu San in the afternoon. If weakness persists after four or five days, use Yigong San (Extraordinary Power Powder), Hezhong San (Harmonize the Middle Powder), or Kaiwei Wan (Open the Stomach Pill).
[Zhang Congzheng] If diarrhea is caused by Wind, use Fangfeng (Saposhnikoviae Radix) and Qianghuo (Notopterygii Rhizoma et Radix)—indicating vomiting/diarrhea accompanied by Liver Wind spasms and stiffness. If caused by Heat, use Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), and Dahuang (Rhei Radix et Rhizoma)—indicating vomiting/diarrhea accompanied by Heart disease and body heat. If caused by Cold, use Fuzi (Aconiti Lateralis Radix Praeparata)—indicating vomiting/diarrhea accompanied by Kidney disease, body coldness, or cold/reversal in the lower legs. If caused by Dampness, use Baizhu (Atractylodis Macrocephalae Rhizoma) and Fuling (Poria)—indicating vomiting/diarrhea accompanied by Spleen disease, excessive sleep, heaviness, and lethargy. If diarrhea is due to Lung disease, use Shaoyao (Paeoniae Radix) and Guixin (Cinnamomi Ramulus); for wheezing, use Maidong (Ophiopogonis Radix) and Renshen (Ginseng Radix et Rhizoma); for severe cases, add Bolang (Arecae Semen); if constipation exists, add Dahuang. Also, examine whether the disease is new or chronic. New diseases should be stopped immediately; chronic diseases risk Intestinal Wind, requiring elimination of stale matter and promotion of new growth. Use Xuanfeng San (Wind-Releasing Powder) to guide out pathogens, then regulate with Zang-targeting Jun-Chen medicines, preferably Yihuang San.
[Zeng Shirong] In mid-summer to early autumn, if children sleep in the wind at night, drink water when thirsty, and overeat raw/cold fruits, irritating the intestines and stomach, sudden vomiting and diarrhea may occur, progressing to paralysis of hands and feet, tendon contraction, and pain. During pain, spirit and consciousness become unsettled. Treating this as a Wind-Stroke syndrome is erroneous. Tendons contract when exposed to cold. The Yangming channel nourishes the Zongjin (major tendons), belonging to the Stomach and Large Intestine. Internal injury from raw/cold diet and external invasion of Wind cause alternating vomiting and diarrhea. Stomach Qi becomes deficient and fails to nourish the Zongjin, leading to contraction. Signs include warm breath, pale complexion, deep-slow pulse, and visible tendon retraction under the skin upon pressing the knees and wrists. Treat with Lizhong Tang (Middle-Warming Decoction) plus Fuzi (half raw, half processed), decocted with water and fresh ginger, taken warm on an empty stomach. Adjust treatment based on deficiency/excess and cold/heat. Another case involved a child of several years suffering sudden vomiting and diarrhea after playing in extreme heat during the day, removing clothes to cool off, and sleeping in the wind at night. This resulted from initial heat exposure followed by cold exposure, where Yin and Yang contend, Qi strikes the Middle Jiao, named Huoluan (Cholera-like disorder). *Huoren Shu* recommends Xiangru San (Elsholtzia Decoction) to separate fluids and grains and regulate Yin/Yang ascent/descent. It states: For heat-dominant cases with thirst, use Wuling San; for cold-dominant cases without thirst, use Lizhong Wan. These methods aim to calm the Middle Jiao.
[Xue Ji] In general, during summer heat, if vomiting and diarrhea are accompanied by hot finger tips, thirst, and desire for cold drinks, it belongs to Yang syndromes; use cooling agents. If finger tips are cold, thirst, and desire for hot drinks occur, it belongs to Yin syndromes; use warming and tonifying agents. If Yin syndromes are mistakenly treated with cold/cooling herbs, death results with cyanotic/dark hands and feet, or generalized discoloration.
