Zhi Mahuang (Prepared Ephedrae Herba, added later, 4 fen) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Sigua Zi (Trichosanthes Kirilowii Fruit, crushed, 4 qian) Lianqiao (Forsythiae Fructum, 3 qian) Zhi Qiao (Aurantii Fructum, 1 qian) Wei Shigao (Calcined Gypsum, 4 qian) Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma, 2 fen) Zi Danshen (Salviae Miltiorrhizae Radix et Rhizoma, 2 qian) Jiegeng (Platycodonis Radix, 1 qian) Yujin (Curcumae Radix, 1.5 qian)
Mr. Yang (Right side): External contraction of wind pathogen, internal retention of food stagnation, leading to fever, headache, and abdominal pain. This is the typical presentation of seasonal diseases. On the third day, the condition is at its peak.
Chi Ju (Chrysanthemi Flos, 1.5 qian) Sang Ye (Mori Folium, 1 qian) Zhi Shi (Aurantii Fructus Immaturus, 1.5 qian) Fan Zhi Qu (Medicated Leaven, stir-fried, 2 qian) Niu Bang Zi (Arctii Fructum, 3 qian) Lai Fu Zi (Raphani Semen, stir-fried and crushed, 3 qian) Jiegeng (Platycodonis Radix, 1 qian) Jiao Zha Tan (Charred Crataegi Fructum, 3 qian) Su Bohe (Menthae Haplocalycis Herba, 1 qian) Xingren (Prunus Armeniaca Seed, crushed, 3 qian) Guang Yujin (Curcumae Radix, 1.5 qian)
Second visit: Fever has subsided, but there is pain above the umbilicus and constipation. The pulse is deep and wiry. This is obstruction by cold and dampness internally; it is appropriate to use Xiao Cheng Qi Tang (Minor Order the Qi Decoction) to descend the qi.
Chuan Po (Magnoliae Officinalis Cortex, 1 qian) Ju Pi (Citri Reticulatae Pericarpium, 1 qian) Suo Sha Ren (Amomi Fructus Rotundus, added later, 5 fen) Wu Yao (Linderae Radix, 1.5 qian) Jiao Zha Tan (Charred Crataegi Fructum, 3 qian) Zhi Shi (Aurantii Fructus Immaturus, 1 qian) Fu Ling (Poria, 3 qian) Zhi Xiang Fu (Cyperi Rhizoma processed, 2 qian) Sheng Jin Wen (Rhei Radix et Rhizoma, added later, 2 qian) Wei Sheng Jiang (Gingeris Rhizoma Recens calcined, 2 slices) Foshou (Citrus Sarcodactylis Fructus, 1 qian)

Mr. Weng (Left side): Initially due to cough and sore throat, leading to persistent fever, sweating that is not thorough, chest tightness, mental restlessness, and insomnia. The pulse is rapid and slippery, the tongue is red with a white, greasy coating. This is wind-heat pathogen combining with dampness, steaming between the Lung and Stomach. The syndrome belongs to wind-dampness, fearing transformation into heat.
Pao Shegan (Belamcandae Rhizoma soaked, 7 fen) Yujin (Curcumae Radix, 1.5 qian) Hei Shan Zhi (Gardeniae Fructus charred, 3 qian) Lianqiao (Forsythiae Fructum, 3 qian) Fan Zhi Qu (Medicated Leaven, stir-fried, 2 qian) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Zhi Shi (Aurantii Fructus Immaturus, 1 qian) Ma Bo (Polyporus, 1 qian) Jiegeng (Platycodonis Radix, 1 qian) Lai Fu Zi (Raphani Semen, raw and crushed, 3 qian) Da Li Zi (Arctii Fructum, 3 qian) Dong Sang Ye (Mulberry Leaf, 1.5 qian)
Second visit: Heat intensity has greatly reduced, and the tongue coating has slightly transformed, but cough remains unsmooth. Dampness lingers in the Lung and Stomach. Again, disperse and resolve dampness.
Guang Xingren (Prunus Armeniaca Seed, crushed, 3 qian) Yujin (Curcumae Radix, 1.5 qian) Jiegeng (Platycodonis Radix, 1 qian) Zhi Qiao (Aurantii Fructum, 1 qian) Chi Bai Ling (Red and White Poria, 2 qian each) Sheng Yi Ren (Coix Seed raw, 4 qian) Fen Hu (Picrorhizae Rhizoma, 1 qian) Bo Ju Hong (Tangerine Peel thin, 1 qian) Chao Lou Pi (Trichosanthis Pericarpium fried, 3 qian) Huashi (Talcum, 3 qian) Pipa Ye (Loquat Leaf de-haired, 4 pieces)
Mr. Zhang (Left side): Initially injured by food, leading to vomiting and diarrhea. The Wind-Warmth pathogen took advantage of the situation to erupt. Phlegm latent in the body normally combined with heat, steaming between the Lung and Stomach, causing heat not to diffuse outward, resulting in cough and thick sputum. The Upper and Middle Jiao are obstructed by phlegm and qi, preventing clear fluids from ascending, leading to thirst, dry tongue with little fluid, gray soot-like center on the tongue, and painful urination. The pulse is rapid and slippery. The syndrome is Wind-Warmth with phlegm, transforming into heat and injuring Yin. Now that ten days have passed, there is fear that during the transition of the disease, phlegm-heat closing internally may lead to coma and convulsions. Propose clearing and resolving phlegm-heat, assisted by saving Yin. Please consult and decide.
Tian Hua Fen (Trichosanthis Radix, 2 qian) Guang Xingren (Prunus Armeniaca Seed, tip removed and crushed, 3 qian) Hai Fu Shi (Spongilla Spiculum, 3 qian) Zhen Chuan Bei (Fritillariae Cirrhosae Bulbus, fried yellow, 2 qian) Bei Sha Shen (Glehniae Radix, 4 qian) Dong Gua Zi (Benincasae Semen, crushed, 4 qian) Da Tian Dong (Asparagi Radix, 3 qian) Bai Luo Bo (Radish sliced, 1.5 liang) Fei Zhi Mu (Anemarrhenae Rhizoma fried, 2 qian) Xian Lu Gen (Imperatae Rhizoma fresh, nodes removed, 1 liang) Chen Guan Zhe (Marina Corpusculum washed and desalted, 1 liang) Gan Pipa Ye (Loquat Leaf dried, de-haired, 4 pieces)
Second visit: After clearing and resolving phlegm-heat, the soot-like tongue coating has receded, turning from dry to moist, and the moldy black center has also resolved. Painful urination has ceased, rebellious qi has flattened, and the pulse has slowed slightly. However, internal heat is not yet cleared; it is still appropriate to clear and resolve.
Bei Sha Shen (Glehniae Radix, 3 qian) Ju Hong (Tangerine Peel salt-water fried, 1 qian) Xian Zhu Ru (Bambusa Textilis Caulis fresh, 1.5 qian) Dong Gua Zi (Benincasae Semen, 3 qian) Tong Cao (Tetrapanacis Medulla, 1 qian) Qing Yan Ban Xia (Pinelliae Rhizoma green salt processed, 1.5 qian) Fu Ling (Poria, 1 qian) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Yi Ren (Coix Seed, 4 qian) Pipa Ye (Loquat Leaf de-haired and roasted, 4 pieces)
Third visit: Cough and rebellious qi have stabilized, and stomach appetite has slightly recovered. However, phlegm-heat in the Lung and Stomach has not resolved, qi does not circulate, and fluids do not move, leading to dry mouth and tongue wanting to drink. The right pulse is slippery and large; deficient fire carries phlegm, steaming the stomach opening, fearing the onset of oral ulcers. Again guide fluids upward and conduct heat downward.
Xi Sheng Di (Rehmanniae Radix Preparata fine, 4 qian) Xi Mu Tong (Akebiae Caulis fine, 4 fen) Tian Hua Fen (Trichosanthis Radix, 2 qian) Zhu Li Ban Xia (Pinelliae Rhizoma bamboo sap processed, 1.5 qian) Hai Ge Fen (Concha Meretricis et Cyprinae powder, bagged, 3 qian) Xi Gancao (Glycyrrhizae Radix et Rhizoma fine, 3 fen) Zhen Chuan Bei (Fritillariae Cirrhosae Bulbus fried yellow, 2 qian) Dong Gua Zi (Benincasae Semen crushed, 3 qian) Bai Fu Ling (White Poria, 3 qian) Xian Zhu Ru (Bambusa Textilis Caulis fresh, salt-water fried, 1 qian) Huo Shui Lu Gen (Imperatae Rhizoma living water, nodes removed, 1 liang) Qing Zhu Ye (Bambusa Leaves green, 10 pieces)
Fourth visit: Nourishing Yin and descending fire, clearing and resolving phlegm-heat, sputum is smooth, dry tongue turns moist. This indeed belongs to dysfunction of Jin-Qi (fluids and qi). It differs from cases of fluid exhaustion.
Nan Sha Shen (Adenophorae Radix, 4 qian) Zhu Li Ban Xia (Pinelliae Rhizoma bamboo sap processed, 1.5 qian) Hai Ge Fen (Concha Meretricis et Cyprinae powder, bagged, 3 qian) Ju Hong (Tangerine Peel salt-water fried, 1 qian) Chuan Shi Hu (Dendrobii Caulis, 4 qian) Chuan Bei Mu (Fritillariae Thunbergii Bulbus fried yellow, 1.5 qian) Sheng Yi Ren (Coix Seed raw, 4 qian) Fu Ling (Poria, 3 qian) Chao Zhu Ru (Bambusa Textilis Caulis fried, 1 qian) Pipa Ye (Loquat Leaf de-haired, 4 pieces)
Fifth visit: Phlegm and qi gradually resolve, fluids circulate, thirst stabilizes, and stomach function gradually improves. This shows that the "dryness" was false dryness, while "dampness" was true dampness. The variations of the disease are confusing enough.
Bai Fu Ling (White Poria, 3 qian) Zhi Ban Xia (Pinelliae Rhizoma processed, 1.5 qian) Hai Ge Fen (Concha Meretricis et Cyprinae powder, bagged, 5 qian) Chao Lou Pi (Trichosanthis Pericarpium fried, 3 qian) Tian Xingren (Sweet Prunus Armeniaca Seed fried, 3 qian) Bo Ju Hong (Tangerine Peel thin, 1 qian) Chuan Chuan Bei (Fritillariae Cirrhosae Bulbus fried, 1.5 qian) Sheng Yi Ren (Coix Seed raw, 3 qian) Sheng Shu Gu Ya (Hordei Fructus Germinatus raw and cooked, 1 qian each) Pipa Ye (Loquat Leaf de-haired, 4 pieces)
Mr. Gu (Right side): Winter-Warmth for nine days, fever, restlessness, no sweat, red macules erupted on the chest and upper back but could not be exuded, mental confusion, finger joint twitching. Pathogen constrained and unable to reach the exterior, carrying warmth and steaming, divine mechanism becomes diffuse. Pulse is fine and rapid, tongue coating white with yellow center and greasy texture. There is danger of internal penetration leading to coma and convulsions. Tentatively propose venting and resolving pathogenic dampness, aromatic orifice-opening method. Please consult and decide.
Dou Chi (Sojae Semnis Preparatum, 3 qian) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Ban Xia (Pinelliae Rhizoma mixed with bamboo sap, 1.5 qian) Chao Zhi Qiao (Aurantii Fructum fried, 1 qian) Chi Fu Ling (Red Poria, 3 qian) Sheng Yi Ren (Coix Seed raw, 3 qian) Bai Jie Geng (Platycodonis Radix white, 1 qian) Xian Shi Chang Pu (Acori Tatarinowii Rhizoma fresh, 4 fen) Niu Huang Qing Xin Wan (Calculus Bovis Clear Heart Pill, one pill dissolved in boiling water)
Someone: Winter-Warmth for eleven days, pathogen transforms into heat, refining fluids into phlegm, constrained and obstructing Lung and Stomach, causing Liver Wood (Jia) not to descend, Liver Wood (Yi) rising alone,烦热 (restless heat) and fire rising causing malar flush. Qi rushes upward, causing nausea and desire to vomit, extreme chest oppression. Difficulty sleeping, startle, deafness, occasional limb tremors. Pulse is rapid, wiry, and large; tongue red with white coating and gray center. Seasonal pathogen triggers underlying disease, fearing internal rotation of wind-fire leading to coma and convulsions. The Classic says: If the Upper Jiao does not function, the Lower Stomach will not unblock. Propose promoting Qi transformation, retaining the meaning that clearing Metal can pacify Wood.
Dou Chi (Sojae Semnis Preparatum) Xingren (Prunus Armeniaca Seed) Zhi Shi (Aurantii Fructus Immaturus) Zhu Ru (Bambusa Textilis Caulis) Gou Gou (Uncariae Ramulus cum Uncis) Pipa Ye (Loquat Leaf) Shan Zhi (Gardeniae Fructum) Yujin (Curcumae Radix) Dan Pi (Moutan Cortex) Jie Geng (Platycodonis Radix) Hai Zhe (Marina Corpusculum)
Mr. Zuo: Lung heat and fluid deficiency should logically cause dryness and thirst. Yesterday's diagnosis showed no thirst, clearly indicating that although the Lung is dry and hot, the Spleen and Stomach still have dampness obstructing. Therefore, flowing and resolving dampness allows clear fluids to ascend. Yu Jiayan thus spoke of "flowing dampness can moisten dryness." Unfortunately, wind transforms into fire, all congesting in the Lung. Repeated administration of Lung-clearing and Yin-nourishing herbs was like pouring a cup of water on a cartload of burning fuel. Heat follows the Lung channel inward attacking the Danzhong, causing occasional delirium. Fire constrained internally manifests externally as malar flush, crimson lips and mouth, turning purple when extreme. Pulse is rapid, exceeding six beats per breath. At this time, clearing Metal, venting heat, and preserving Yin is indeed the standard principle. However, one must not ignore that fire arises from wind transformation; its heat is fire in the pot, its wind is firewood under the pot. Although the steaming heat intensity has slightly declined, the firewood under the pot has not been removed. As long as the wood is there, the fire is there. Hence, the symptoms improved noticeably before, but I still said I couldn't grasp the situation firmly because of this. Currently, the pulse is rapid; the Qi Kou (radial artery) shows floating, but pressing feels less intense. If we wish to dissipate the wind and remove the firewood to slow the prairie fire, the healer knows their ability is insufficient. Reluctantly, I again propose Qing Fei Yin (Clear Lung Drink) combined with Qing Gong Tang (Clear Palace Decoction) to exert my limited effort.
Xi Jian (Rhino Horn tip ground and taken with liquid, 5 fen) Lian Qiao Xin (Forsythiae Fructum heart, 3 qian) Da Mai Dong (Ophiopogonis Radix with heart, 3 qian) Chi Fu Ling Shen (Red Poria with hostwood, 2 qian each) Chuan Bei Mu (Fritillariae Thunbergii Bulbus, 2 qian) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Guang Yujin (Curcumae Radix, 1.5 qian) Bei Sha Shen (Glehniae Radix, 5 qian) Sang Bai Pi (Mori Cortex roasted, 2 qian) Pipa Ye (Loquat Leaf de-haired, 1 liang) Bai Mao Gen (Imperatae Rhizoma, 1 liang) Lian Zhu (Pearl, 3 fen)
Chuan Bei (Fritillariae Cirrhosae Bulbus 4 fen, two ingredients ground extremely fine and taken mixed)
Original note after the prescription says: What is currently feared is Lung heat following the channel entering the Heart, causing simultaneous disease of Heart and Lung, asthma, and coma, which would be a critical state. Someone asked: A few days ago, if drugs like Ejiao for saving the Lung were used, treatment should have been effective. Why not continue them to let Water control Fire? Answer: Tongue coating is white and greasy, no thirst. If dampness is abundant generating phlegm, it is even harder to handle. Reluctantly stepping back is not shrinking from battle. Furthermore, this case is Winter-Warmth lingering into Spring, unable to heal for a long time. It was likely treated by mediocre doctors using Bai Shao (Paeoniae Radix Alba) up to 4 qian, Chuan Lian (Coptidis Rhizoma) up to 5 qian, and Wu Wei Zi (Schisandrae Fructum) up to 2 qian. By Jingzhe (Awakening of Insects solar term), the disease worsened, as Jingzhe is when Yang moves. Initially used Yu Shi's Qing Zao Jiu Fei Tang (Clear Dryness Save Lung Decoction), later used Zhu Ye Shi Gao Tang (Bamboo Leaf and Gypsum Decoction). Previous cases are lost, so recorded here (Qing Ru Zhi).
Mr. Qiu (Right side): Syndrome exceeded two weeks, first red rash appeared, then white milia (white grains), followed by menstruation. Pathogen force did not fail to drain slightly through rash and milia, so a few days ago the disease had signs of retreat,烦热 (restless heat) and thirst were largely stabilized. However, since both rash and milia appeared, Ying Qi (Nutritive Qi) and Yin fluids must have secretly depleted. Moreover, mental worry causes Liver Wood and Fire to rise, pathogenic heat thereby intensifies,烦热 (restless heat) returns strongly, tinnitus and ear blockage, phlegm sound in throat, resembling dream talk, finger twitching, lower abdomen qi sinking and distension. Pulse is rapid, slippery, and wiry; tongue red with yellow coating. Pathogenic dampness not resolved, Liver Wood and Fire secretly rising, refining fluids into phlegm, divine mechanism not operating, danger of coma and convulsions. Tentatively propose venting heat, cooling Liver, resolving phlegm, and opening orifices.
Ling Yang Pian (Antelope Horn slices, 3 qian) Chi Fu Ling (Red Poria, 3 qian) Zhu Ru (Bambusa Textilis Caulis salt-water fried, 1 qian) Yi Yuan San (Benefit Origin Powder, 3 qian plus Cinnabar 7 li wrapped in silk) Da Lian Qiao (Forsythiae Fructum large, 3 qian) Chen Dan Xing (Areca Seed with bile aged, 6 fen) Hei Shan Zhi (Gardeniae Fructus charred, 3 qian) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Yujin (Curcumae Radix, 1.5 qian) Ju Ye (Citrus Reticulata Leaf, 1.5 qian) Yin Hua Lu (Honeysuckle Dew, 1 liang)
Second visit: Venting heat, cooling Liver, resolving phlegm, opening orifices,烦ao (restlessness) greatly reduced, sleep slightly peaceful, limb twitching more stable, lower abdominal distension relaxed, red rash slightly faded. Pulse wiry and slightly soft, tongue red with thinning yellow coating. This morning expectorated three mouthfuls of sputum, feeling quite relieved, self-perceiving sticky phlegm in chest. Indeed, Liver-Gallbladder fire and pathogenic heat fiercely compete, refining fluids into phlegm, causing phlegm-fire to fan each other. Coma and convulsions are imminent and alarming. After taking previous medicine, less than a week has passed, cannot operate too aggressively. Propose clearing and resolving pathogenic heat, assisted by resolving phlegm.
Lian Qiao (Forsythiae Fructum, 3 qian) Fen Dan Pi (Moutan Cortex powdered, 2 qian) Guang Yujin (Curcumae Radix, 1.5 qian) Chen Dan Xing (Areca Seed with bile aged, 5 fen) Bai Ji Li (Tribuli Fructus, 3 qian) Shan Zhi (Gardeniae Fructum, 3 qian) Gua Lou Pi (Trichosanthis Pericarpium, 3 qian) Guang Xingren (Prunus Armeniaca Seed, 3 qian) Yi Yuan San (Benefit Origin Powder, 3 qian plus Cinnabar 7 li wrapped in silk) Qing Zhu Ye (Bambusa Leaves green, 20 pieces) Huo Shui Lu Gen (Imperatae Rhizoma living water, 1 liang) Yin Hua Lu (Honeysuckle Dew, 1 liang)
Mr. Feng (Left side): Warm pathogen for seven days, intense heat, mental confusion, limb joint twitching, pulse with pauses, black tongue with red substance, malar flush, loud rattling phlegm in throat, coarse and short breathing. This is intense heat internally, with damp-phlegm depression, heat unable to vent, turning to internal penetration, danger of phlegm surging, coma, and asthma.
Ling Yang Jiao (Antelope Horn) Chen Dan Xing (Areca Seed with bile aged) Tian Zhu Huang (Calami Turbidae Resina) Zhu Ru (Bambusa Textilis Caulis) Jiu Jie Shi Chang Pu (Acori Tatarinowii Rhizoma nine-sectioned) Da Lian Qiao (Forsythiae Fructum large) Xing Ren Ni (Prunus Armeniaca Seed paste) Xuan Fu Hua (Inulae Flos) Zhi Shi (Aurantii Fructus Immaturus) Zhi Bao Dan (Treasure Pill, one pill divided into two doses)
Mr. Zhou (Left side): After administering Lung-draining and phlegm-resolving methods, conducting heat downward, the ascending qi has retreated by three or four parts out of ten, sputum is slightly smoother, but heat intensity still does not decline. Left rib pain decreased, but sputum is yellow and thick, frequent thirst wanting to drink, mental restlessness, chest tightness, tongue substance turned red, edges and tips showing significant crimson tendency. Like dream talk, malar flush. Pulse rapid, slippery, large; left part small and soggy, right part pounding. Good reason: accumulated Qi division gradually opens, but loud rattling phlegm indicates Lung-Stomach pathogenic heat is still intense. Fire agitates phlegm, hence the loud sound. Heat burns fluids, causing deficiency, hence frequent thirst and desire to drink. Disease trend is slightly stabilized compared to before, but under the soaring momentum, without major improvement, the overall situation cannot be secured. What is urgent now is phlegm rattling and rebellious qi, most critically important. However, if pathogenic force does not vent, fluids become phlegm. The ancients said: Phlegm is tangible fire; fire is intangible phlegm. This is exactly the meaning. Propose within clearing Metal and肃肺 (purifying Lung), assisted by heavy辛凉 (pungent-cool) agents. Must achieve effectiveness to be proper.

The Wind-Warmth category, attached with Winter-Warmth, Warm-Thermal, and Autumn-Dryness, although stated in the example remarks, is actually due to lost prescriptions and insufficient copies kept, so only appended consecutively afterwards, unable to list item by item. Proofreading makes me feel ashamed like Boyu, deeply regretting the omission. Asked my friend Mr. X: Compiling cases differs fundamentally from writing books. Authors categorize strictly, allowing no disorder. Compiling prescriptions is merely arranging based on sequence. If one desires clear uniformity and comprehensive perfection, it is difficult amidst many challenges. Han believes Mr. X has experience and gains, knowing well the complexity of compilation, and sees the value of flexibility in reading. Tentatively record this. If old classmates have preserved drafts, hope you provide them. When republishing, I will supplement and revise. (Wen Han Zhi)
