When an infant reaches one hundred days of age, the Ren Mai (Conception Vessel) develops, and the child gains the ability to roll over. The wet nurse should regulate emotions of joy and anger, ensuring the infant adapts to cold and warmth. At half a year of age, the sacral bones are formed; the child should be taught to sit. After two hundred days, the metacarpal bones develop; the wet nurse should teach the child to crawl on the ground. At three hundred days, the patellar bones form, enabling independent standing. Upon reaching one year of age, the knee bones are formed; the wet nurse should teach the child to walk.
These are fixed developmental laws. People often fail to follow these rules, holding infants for excessive periods, which damages the sinews and bones; this must be strictly avoided.
For the initial stage of variola (smallpox) or exanthema, where fever has not yet subsided, whether it is variolous toxin or pathogenic factor, amidst uncertainty, this mild formula nourishing Ying (Nutritive Qi) is appropriate as a base treatment. It can support the expulsion of toxins in cases of toxicity, and disperse pathogens in cases of external invasion. In excess conditions, it will not aid the pathogen; in deficiency conditions, it will not damage Zheng Qi (Upright Qi). (In cases of Yangming channel Excess Heat with vigorous pathogenic factors, Baihu Tang [White Tiger Decoction] is indicated. If there is no Excess pathogen, modifications of this formula are suitable. Symptoms such as high fever, yawning, sudden lethargy, frequent palpitations, vomiting or diarrhea, redness of hands, feet, cheeks, and malar region, coughing, and sneezing indicate variola. Yawning and sudden lethargy correspond to Liver symptoms; frequent palpitations correspond to Heart symptoms; vomiting/diarrhea with cold extremities correspond to Spleen symptoms; red cheeks with cough/sneeze correspond to Lung symptoms; lower back pain with preference for sleep corresponds to Kidney symptoms. When symptoms of all five Zang organs appear fully, the eruption will be profuse. If only one or two symptoms appear, the eruption will be sparse. The root cause lies in the toxin of hyperactive fire.)
Dang Gui (Angelicae Sinensis Radix, 2-3 qian), Shao Yao (Paeoniae Radix, raw or stir-fried, 1.5 qian), Jing Jie (Schizonepetae Spica, stir-baked with honey, 1 qian), Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma, 0.7-1 qian), Chai Hu (Bupleuri Radix, 1 qian), Sheng Jiang (Zingiberis Rhizoma Recens, 3 slices), Rou Gui (Cinnamomi Cortex, a small amount).
If an infant presents with intense fever in the early stage, distinguishing between Wind-Cold and variola/exanthema may be difficult; this formula is safe and reliable.
(See Formula in Volume III, Section on Exterior-Releasing Formulas)
If exterior heat is intense, this indicates Pathogenic Factor Excess at the exterior. The Classic states: "Light substances can dispel excess." Therefore, Sheng Ma (Cimicifugae Rhizoma) and Ge Gen (Puerariae Lobatae Radix) are used to release the exterior. Gan Cao (Glycyrrhizae Radix et Rhizoma) assists in harmonizing the exterior.
For treating fever at the initial stage of variola/exanthema. For patients with strong Qi and Blood who do not require tonification, the primary approach is to resolve the pathogen. This formula serves as a substitute for Shengma Gegen Tang [Cimicifuga and Kudzu Root Decoction] or Suge Tang, being the most reliable option.
Shao Yao (Paeoniae Radix, wine-stir-fried), Chai Hu (Bupleuri Radix, doubled dose), Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma, stir-baked), Zi Su Ye (Perillae Folium), Jing Jie Sui (Schizonepetae Spica, halved dose). Decoct in water and take hot. For those without internal fire, add Sheng Jiang (Zingiberis Rhizoma Recens, 3 slices); for those with excessive fire and internal heat, add Huang Qin (Scutellariae Radix); for thirst, add Ge Gen (Puerariae Lobatae Radix).
Removing Zi Su Ye and Jing Jie Sui, and adding Gan Ge (Puerariae Radix), Huang Qin (Scutellariae Radix), and Lian Qiao (Forsythiae Fructus), creates the name Chaige Jian [Bupleurum and Kudzu Decoction]. It treats exterior heat of variola/exanthema and epidemic febrile diseases.
