〔总论〕 血症有吐血。唾血。咳血。衄血之别。大抵多属于热。盖脏腑有热火逼之。
然后血涌溢而出也。吐血即呕血。属胃经兼肝经。多因酒过。或食热物太过。或劳伤用力太
过。也有瘀血紫黑色者。吐之无妨。若鲜红者。则火太盛。若劳伤而吐者。未尽属火也。唾
血咯血。出于肾经兼脾经。因色欲太过者。多咳血。嗽血。出于肺经兼肝经。乃酒色怒气。
种种不节。咳嗽有痰。痰中带血也。 血鼻中出血也。属肺经兼肝经。治法须究其因。若饮
食过饱。负重伤胃而吐者。调胃安血。思虑伤心。积热而吐衄者。须补益心志。积热则清之。
如气郁则顺之。脉宜沉细。不喜浮数。吐而不咳易治。唾中带红丝者难医。为有内损也。有
因怒气伤肝。肝木火旺。侵克脾土。致肝不能藏血。脾不能统血。忽然血晕。不省人事。吐
血至升斗者。一时切勿用寒凉之剂。及骤用补药。先服童便二三碗。得血止瘥安。次用归身。
丹参。丹皮。生地。熟地。远志。枣仁。茯苓。（各一钱半）炙甘草。（五分）水煎浓。和童
便
一盏服。临卧用广西真山羊血。每服三分。引血归源。不过三服。其血自止。然后徐用气血
兼补之剂。宿疾自霍然矣。
〔吐血不止〕 白茅草根水煎服之。（按白茅草根除肠胃中客热。止吐血。消瘀血。利
小便。凡劳伤而中气虚者。常服最有益。味甘平无毒。） 白薄纸五张烧灰。水服立效。此
方甚奇。 蒲黄。生地。（各一两）忌铁器。水煎。再用藕节七个。捣汁入药。食后温服。 韭
菜根于净石臼内木杵捣烂。入童便在内。布帛滤去渣。只将汁与便以碗盛之。置火边令热。
浊者俱下不用。只取汁便之轻者。服之立效。 嫩荷蒂七个。擂水服之甚佳。 经霜败荷叶
烧存性。研末。新汲水调服二钱。（按本草荷叶灰止吐血。莲房灰止泻血。）
〔血热妄行〕 生荷叶。生艾叶。生侧柏叶。鲜生地各等分捣烂。丸如鸡子大。每一丸
水煎去渣服。 不拘吐血鼻血。妄行不止。鲜生地二三斤。取汁服即止。如病久者。用干生
地煎浓。日饮三五次即效。
〔酒伤吐血〕 饮酒过多。热积胸膈。以致吐衄。葛花。（二两）川黄连。（四两）俱为
末。以大黄末。（熬膏为丸。如桐子大。）每服百丸。温汤下。
〔妇女吐血不止〕 益母草捣汁一盏。和童便半盏。顿热服。如无鲜者。以干者二两。
煎汁和服。（或用韭菜连根取汁。合服同效。）
〔血喷成升斗者〕 花蕊石。（不拘多少。 存性研如粉。）每用二三钱。用童便一盏顿
温。食后调下。止后。用人参煎汤补之。须熟睡一觉即安。 如吐出瘀血至升斗。及跌打损
伤瘀血冲心。与女人血崩等症。用真广西山羊心血。每服三分。用无灰陈酒下。立见神效。
〔久嗽以致吐血〕 鲜桑白皮。（一斤米泔水浸刮去黄皮）锉细。入糯米四两焙干。同
捣为细末。每服二钱。米饮调下。以好为度。
〔吐血咳嗽〕 藕节汁。梨汁。茅草根汁。萝卜汁。鸡冠油。麻油各四两。煎膏早晚服
即愈。 真阿胶。（五钱蛤粉炒）天冬。（去心一两）川贝母。（水洗去心五钱）白茯苓。（五
钱）杏仁。（汤泡去皮尖炒黄五钱）甘草。（五钱）俱为末。炼蜜丸如龙眼核大。不时噙化一
丸。
〔夜热咳嗽口燥吐血初起〕 枯黄芩四两。用猪胆三个。取汁拌匀。饭锅上蒸透晒干三
次。为末。米糊为丸。每服二钱。空心白汤下。轻者四两，重者一斤。无不全愈。
〔痰中带血〕 肺热咳嗽初起。萝卜汁半盏。入盐少许。服之立效。如因虚损而起者。
童便和竹沥服之即止。
〔咯血〕 （喉中常有血腥。一咯血即出是也。或鲜或紫。又如细屑者皆是也。） 白
芨
一两。藕节五钱。各切片焙燥为末。每服一钱。白汤调下立效。 新丝绵烧灰五分。好酒下
即效。 嗽久虚劳咯血。五倍子炒研细。每服五分。渐加至一钱。茶调匀。米饮下。
〔心肺受热受暑吐血〕 真桑黄每用三五钱煎汤服。以好为度。 枯黄芩。（二两）桑
皮。（一两）水法为丸。每服二钱白汤下。日二服。以好为度。
〔妊娠吐血〕 马屁勃研末。浓米汤调服五分即止。
〔鼻血不止〕 好陈酒烫热。自足浸至膝腕。以汗出为度。凶者浸至半身。再无不止。 栗
壳。（烧存性研末）米饮调下二钱立止。 刀刮指甲细末。吹入鼻中立止。 本人头发烧灰。
吹入鼻中立止。（或乱发烧灰存性。去火毒。井水调下一钱五分。并用少许吹入鼻即止。） 纹
银一锭。烧红烫足心。或用红线一根。紧扎手上中指中节。右鼻扎左指。左扎右。皆立止。 大
蒜头捣烂。浓涂足心即止。附子末酒调涂足心亦效。纸燃蘸麻油入鼻取嚏即止。（有人一夕
衄血盈盈。用此而效。） 白纸一张八折。水湿铺于头顶上。用熨斗微熨之立止。 用小豆
大艾丸灸项后发际两肋中间。即常人括痧处。三壮立止。（盖血自此入脑注鼻中出也。） 久
衄虚者。用栗壳灰。（二钱）人参末。（五分）黄连末。（一钱）人乳调服立效。 人乳。童
便。各一杯。和酒少许。顿热服立止。好后多服润肺之剂。或用枇杷叶刷去毛蜜炙泡汤当茶
吃。久服不再发。
〔九窍出血〕 黄荆叶捣汁。和酒服之。墙上青苔揉匀塞之。 生大蓟一握。捣汁和酒
服。（如无生者。以干者为末。冷水调下三钱。）或以冷水浸足。或以冷水喷面。皆可救止。
然此症名曰上厥下竭。百中救一耳。
【总论】血症分为吐血、唾血、咳血、衄血四种。大抵多属于热证，因为脏腑内有火热之邪逼迫，然后血液涌溢而出。吐血即呕血，归属于胃经兼涉肝经，多因饮酒过量，或进食过多热性食物，或劳力过度所致；也有排出紫黑色瘀血的情况，此时吐出来也无妨；若血色鲜红，则是火气太盛；若是因劳伤而吐血，则不完全是火邪所致。唾血和咯血，出自肾经兼涉脾经，多因房事过度引起。咳血和嗽血，出自肺经兼涉肝经，乃是由于饮酒、色欲、怒气等种种生活起居不节制，导致咳嗽有痰，痰中带血。衄血（鼻中出血），归属于肺经兼涉肝经。治疗必须探究病因：如果是饮食过饱、负重损伤胃部而导致的吐血，应调理胃气、安固血液；如果是思虑过度损伤心神、积热而导致吐血或衄血，须补益心志；积热则需清热，气机郁滞则需顺气。脉象宜沉细，不宜浮数。吐血但不咳嗽者易治，唾液中带有红丝者难医，这是因为体内有损伤。有的因怒气伤肝，导致肝木火旺，侵犯脾土，致使肝不能藏血，脾不能统血，突然发生血晕，不省人事，吐血多达升斗。此时千万不要立即使用寒凉药剂，也不要骤用补药。应先服用童便（童子尿）二三碗，待血止后病情安定。接着用当归身、丹参、牡丹皮、生地黄、熟地黄、远志、酸枣仁、茯苓（各一钱半），炙甘草（五分），水煎浓汁，混合一盏童便服用。临睡前服用广西产的真山羊血，每次三分，以引血归源，不过三服，其血自止。然后慢慢使用气血双补的药剂，宿疾自然痊愈。
【吐血不止】白茅草根水煎服用。（按语：白茅草根能清除肠胃中的客热，止吐血，消瘀血，利小便，凡是劳伤导致中气虚的人，常服最有益处。味甘平，无毒。）五张薄白纸烧成灰，用水送服，立刻见效。此方很奇特。蒲黄、生地黄（各一两），忌用铁器，水煎。再用藕节七个，捣汁加入药中，饭后温服。韭菜根在干净的石臼内用木杵捣烂，加入童便在内，用布帛过滤去渣，只取汁和便用碗盛好，放在火边加热，浑浊的部分全部倒掉不用，只取汁便中较轻清的部分，服用后立刻见效。嫩荷蒂七个，研磨后用水送服很好。经霜的败荷叶烧至存性，研成末，用新汲水调服二钱。（按语：本草记载荷叶灰止吐血，莲房灰止泻血。）
【血热妄行】生荷叶、生艾叶、生侧柏叶、鲜生地黄各等分，捣烂，制成如鸡蛋大的丸剂，每一丸水煎去渣服用。不拘是吐血还是鼻血，妄行不止，取鲜生地黄二三斤，榨汁服用即可停止。如果病久，用干生地黄煎浓汁，每天饮用三五次即有效。
【酒伤吐血】饮酒过多，热积于胸膈，以致吐血或衄血。葛花（二两）、川黄连（四两），都研为末，用大黄末（熬膏作为粘合剂制成丸剂，如梧桐子大）。每次服用一百丸，温开水送下。
【妇女吐血不止】益母草捣汁一盏，混合童便半盏，顿热后服用。如果没有新鲜的，用干品二两，煎汁混合服用。（或者用韭菜连根取汁，合服同样有效。）
【血喷成升斗者】花蕊石（不拘多少，烧至存性研如粉）。每次用二三钱，用一盏童便顿温，饭后调服。血止后，用人参煎汤补益身体。必须熟睡一觉才能安定。如果吐出瘀血多达升斗，以及跌打损伤导致瘀血冲心，与妇女血崩等症状，用真广西山羊心血，每次三分，用无灰陈酒送下，立刻见到神效。
【久嗽以致吐血】鲜桑白皮（一斤，用米泔水浸泡刮去黄皮），锉细，加入糯米四两焙干，共同捣为细末。每次服用二钱，米饮调下，以好转为度。
【吐血咳嗽】藕节汁、梨汁、茅草根汁、萝卜汁、鸡冠油、麻油各四两，煎成膏状，早晚服用即愈。真阿胶（五钱，蛤粉炒）、天冬（去心一两）、川贝母（水洗去心五钱）、白茯苓（五钱）、杏仁（汤泡去皮尖炒黄五钱）、甘草（五钱），都研为末，炼蜜制成如龙眼核大的丸剂，不时含化一丸。
【夜热咳嗽口燥吐血初起】枯黄芩四两，用猪胆三个，取汁拌匀，在饭锅上蒸透晒干三次，研为末，米糊为丸。每次服用二钱，空腹白开水送下。轻症四两，重症一斤，无不痊愈。
【痰中带血】肺热咳嗽初起，萝卜汁半盏，加入少许盐，服用后立刻见效。如果是因虚损引起的，童便和竹沥混合服用即止。
【咯血】（喉中常有血腥味，一咯血即出，颜色或鲜或紫，又如细屑状等都是。）白及一两，藕节五钱，各切片焙干研为末。每次服用一钱，白开水调下立刻见效。新丝绵烧灰五分，用好酒送下即效。咳嗽日久导致虚劳咯血，五倍子炒后研细，每次服用五分，逐渐加至一钱，茶调匀，米饮送下。
【心肺受热受暑吐血】真桑黄每次用三五钱煎汤服用，以好转为度。枯黄芩（二两）、桑白皮（一两），用水泛为丸。每次服用二钱，白开水送下，每天两次，以好转为度。
【鼻血不止】好陈酒烫热，从脚浸至膝盖手腕，以出汗为度。凶险者浸至半身，再也没有不止的。栗壳（烧至存性研末），米饮调下二钱立刻停止。刀刮指甲细末，吹入鼻中立刻停止。本人头发烧灰，吹入鼻中立刻停止。（或者乱发烧灰存性，去除火毒，井水调下一钱五分，并用少许吹入鼻中即止。）纹银一锭，烧红烫足心，或者用红线一根，紧扎手上中指中节，右鼻扎左指，左鼻扎右指，都立刻停止。大蒜头捣烂，浓涂足心即止。附子末酒调涂足心也有效。纸燃蘸麻油入鼻取嚏即止。（有人一夜衄血盈满，用此法有效。）白纸一张八折，水湿铺于头顶上，用熨斗微熨之立刻停止。用小豆大的艾丸灸项后发际两肋中间，即常人刮痧处，三壮立刻停止。（盖血由此入脑注鼻中出也。）久衄且体虚者，用栗壳灰（二钱）、人参末（五分）、黄连末（一钱），人乳调服立刻见效。人乳、童便各一杯，混合少许酒，顿热服用立刻停止。好后多服润肺的药剂，或者用枇杷叶刷去毛蜜炙泡汤当茶喝，久服不再发作。
【九窍出血】黄荆叶捣汁，和酒服用。墙上青苔揉匀塞之。生大蓟一握，捣汁和酒服用。（如果没有生的，以干的研为末，冷水调下三钱。）或者用冷水浸足，或者用冷水喷面，都可以救止。然而此症名为上厥下竭，百人中只能救一人罢了。
[General Discussion] Blood disorders are categorized into hematemesis (vomiting blood), spitting blood, hemoptysis (coughing up blood), and epistaxis (nosebleed). Generally, these belong to heat syndromes, as internal fire in the Zang-Fu organs forces blood to surge and overflow. Hematemesis is also known as vomiting blood; it belongs to the Stomach Meridian involving the Liver Meridian. It is often caused by excessive alcohol consumption, overeating hot-natured foods, or excessive physical exertion. There are cases where dark purple-black stasis blood is vomited, which is harmless if expelled. If the blood is bright red, it indicates excessive fire. If caused by labor injury, it does not solely belong to fire. Spitting blood and coughing up blood originate from the Kidney Meridian involving the Spleen Meridian, often due to excessive sexual activity. Coughing blood and clearing phlegm with blood originate from the Lung Meridian involving the Liver Meridian, resulting from various immoderacies such as drinking, sexual indulgence, and anger, leading to coughing with phlegm containing blood. Epistaxis (nosebleed) belongs to the Lung Meridian involving the Liver Meridian. Treatment must investigate the cause: if vomiting blood results from overeating or heavy lifting damaging the stomach, regulate the Stomach Qi and stabilize the blood; if vomiting or nosebleeding results from excessive worry damaging the Heart Spirit and accumulating heat, tonify the Heart Spirit; accumulate heat requires clearing, and Qi stagnation requires smoothing. The pulse should be deep and fine, not floating and rapid. Vomiting blood without coughing is easy to treat; spitting blood with red threads is difficult to cure because there is internal damage. Some cases involve anger injuring the Liver, causing Liver Wood Fire to flourish and invade Spleen Earth, preventing the Liver from storing blood and the Spleen from controlling blood. This leads to sudden syncope due to blood loss, unconsciousness, and vomiting blood amounting to sheng or dou units. At this moment, do not immediately use cold-cooling formulas or suddenly use tonic medicines. First, administer two or three bowls of Tongbian (Child's Urine). Once bleeding stops and the condition stabilizes, next use Dangshen (Radix Codonopsis), Danshen (Radix Salviae Miltiorrhizae), Mudanpi (Cortex Moutan), Shengdihuang (Radix Rehmanniae Preparata), Shudihuang (Radix Rehmanniae Preparata), Yuanzhi (Radix Polygalae), Zaoren (Semen Ziziphi Spinosae), Fuling (Poria) (each 1.5 qian), Zhigancao (Radix Glycyrrhizae Preparata) (0.5 qian). Decoct in water until concentrated, mix with one bowl of Tongbian, and take. Before sleep, take Sanxiyangxue (Goat Blood from Guangxi), 3 fen per dose, to guide blood back to its source. Within three doses, bleeding will stop naturally. Then slowly use formulas that tonify both Qi and Blood, and chronic diseases will resolve spontaneously.
[Unceasing Hematemesis] Decoct Baomaogen (Rhizoma Imperatae) in water and take. (Note: Baomaogen clears guest-heat in the intestines and stomach, stops hematemesis, resolves stasis, and promotes urination. It is most beneficial for those with labor injuries and deficiency of Middle Qi. Its taste is sweet and neutral, non-toxic.) Burn five sheets of thin white paper to ash, take with water for immediate effect. This formula is very unique. Puhuang (Pollen Typhae) and Shengdihuang (Radix Rehmanniae Recens) (each 1 liang). Avoid iron vessels. Decoct in water. Add juice from seven Oujie (Nodus Nelumbinis Rhizomatis), crushed, to the medicine. Take warm after meals. Grind Jiugencui (Radix Allii Tuberosi) in a clean stone mortar with a wooden pestle until pasty. Add Tongbian (Child's Urine) inside. Filter through cloth to remove dregs. Keep only the liquid and urine in a bowl. Heat near fire. Discard all turbid parts. Use only the clear part of the liquid and urine. Immediate effect upon taking. Grind seven Neneodi (Petiole Nelumbinis) in water and take; very good. Burn Jianshuang Baiheye (Lotus Leaf after frost) until preserved character remains, grind into powder, mix with fresh well-water, and take 2 qian. (Note: Bencao records that Lotus Leaf Ash stops hematemesis, and Lotus Pod Ash stops bloody diarrhea.)
[Reckless Bleeding Due to Blood Heat] Shengheye (Folium Nelumbinis), Shengaiye (Folium Artemisiae Argyi), Shengcebaiye (Folium Platycladi), and Xianshengdihuang (Radix Rehmanniae Recens) in equal parts. Crush and make pills the size of chicken eggs. Decoct one pill in water, strain, and take. Regardless of hematemesis or epistaxis, if bleeding is reckless and unceasing, take juice from 2-3 jin of Xianshengdihuang (Radix Rehmanniae Recens) to stop it. For chronic illness, decoct dry Shengdihuang (Radix Rehmanniae Preparata) until concentrated, drink 3-5 times a day for effect.
[Hematemesis Due to Alcohol Injury] Excessive drinking causes heat accumulation in the chest and diaphragm, leading to hematemesis or epistaxis. Gehua (Flos Speranskiae) (2 liang) and Chuanhuanglian (Rhizoma Coptidis) (4 liang) are ground into powder. Use Dahuang (Radix et Rhizoma Rhei) powder (boiled into paste to form pills, size of Sophora seeds). Take 100 pills each time with warm water.
[Unceasing Hematemesis in Women] Crush Yimucao (Herba Leonuri) to extract one bowl of juice, mix with half a bowl of Tongbian (Child's Urine), heat thoroughly, and take at once. If fresh herbs are unavailable, use 2 liang of dried herbs, decoct, and mix. (Alternatively, use juice from Jiucailu (Rhizoma Allii Tuberosi) including roots; combined administration has the same effect.)
[Blood Spraying Amounting to Sheng or Dou Units] Huaruishi (Lapis Arseniti Rubri) (any amount, burn until preserved character remains, grind to powder). Use 2-3 qian each time. Warm one bowl of Tongbian (Child's Urine) thoroughly. Mix and take after meals. After bleeding stops, decoct Renshen (Radix Ginseng) soup to tonify. Must sleep soundly to stabilize. If vomiting stasis blood amounts to sheng or dou units, or if bruising injuries cause stasis blood rushing to the heart, or women suffer from metrorrhagia, use Zhen Guangxi Shan Yangxue (True Goat Blood from Guangxi), 3 fen per dose, taken with Wuhui Chenjiu (Aged Wine without Ash). Divine effect is seen immediately.
[Hematemesis Due to Chronic Cough] Xiansangbaidi (Cortex Mori Radicis) (1 jin, soak in rice wash water, scrape off yellow skin). Cut finely. Add Nuomi (Oryza Sativa Glutinosa) 4 liang, bake dry. Grind together into fine powder. Take 2 qian each time, mixed with Mi Yin (Rice Water). Continue until improvement.
[Hematemesis with Cough] Oujiezhi (Juice of Nodus Nelumbinis Rhizomatis), Lizhi (Juice of Pyri Fructus), Maocaogenzhi (Juice of Rhizoma Imperatae), Luobozhi (Juice of Raphani Fructus), Jiguanyou (Oil of Gallus Galli Crista), Mayou (Sesamum Oil) each 4 liang. Decoct into paste. Take morning and evening for cure. Zhen Ejiao (Colla Corii Asini) (5 qian, stir-fried with Ge Fen), Tiandong (Radix Asparagi), Chuanbeimu (Bulbus Fritillariae Cirrhosae) (washed, core removed, 5 qian), Baifuling (Poria) (5 qian), Xingren (Semen Prunus Armeniacae) (soaked in water, peel and tip removed, stir-fried until yellow, 5 qian), Gancao (Radix Glycyrrhizae) (5 qian). All ground into powder. Honey pills the size of longan seeds. Hold one pill in mouth occasionally to dissolve.
[Initial Hematemesis with Night Fever, Cough, Dry Mouth] Ku Huangqin (Fructus Scutellariae) 4 liang. Use three pig gallbladders, extract juice, mix evenly. Steam over rice cooker until penetrated, sun-dry three times. Grind into powder. Rice paste pills. Take 2 qian each time on an empty stomach with plain boiled water. Mild cases 4 liang, severe cases 1 jin. All recover completely.
[Blood in Phlegm] Initial lung-heat cough: Luobozhi (Juice of Raphani Fructus) half a bowl, add a little salt. Take for immediate effect. If caused by deficiency injury: Tongbian (Child's Urine) and Zhuli (Nodulus Bambusae in Massa) mixed and taken to stop bleeding.
[Coughing Up Blood] (Constant metallic taste in throat, blood appears upon coughing, color fresh or purple, or like fine particles.) Baiji (Rhizoma Bletillae) 1 liang, Oujie (Nodus Nelumbinis Rhizomatis) 5 qian. Slice and bake dry, grind to powder. Take 1 qian each time with plain boiled water for immediate effect. Xin Simian (New Silk Cotton) burn to ash 5 fen, take with good wine for effect. Chronic cough due to deficiency consumptive disease causing hemoptysis: Wupeizi (Fructus Terminaliae Chebulae) stir-fried and ground fine. Take 5 fen each time, gradually increase to 1 qian. Mix with tea, take with rice water.
[Hematemesis Due to Heart and Lung Heat and Summerheat] Zhen Sanghuang (Conk of Morus Alba) 3-5 qian each time, decoct in water and take. Continue until improvement. Ku Huangqin (Fructus Scutellariae) (2 liang), Sangpi (Cortex Mori Radicis) (1 liang). Water pills. Take 2 qian each time with plain boiled water, twice daily. Continue until improvement.
[Hematemesis During Pregnancy] Mapibo (Moss on Horse Dung) grind to powder. Mix with thick rice soup, take 5 fen to stop.
[Unceasing Epistaxis] Heat good aged wine. Soak feet from toes to knees and ankles until sweating occurs. In severe cases, soak up to half body. No case remains uncontrolled. Liqiao (Shell of Castanea) (burn until preserved character remains, grind to powder). Mix with rice water, take 2 qian for immediate stop. Scrape fingernails into fine powder. Blow into nostril for immediate stop. Burn patient's own hair to ash. Blow into nostril for immediate stop. (Or burn messy hair until preserved character remains, remove fire toxicity, mix with well water, take 1.5 qian, and blow a little into nostril to stop.) Red silver ingot, heat red-hot, scald sole of foot. Or tie a red thread tightly around the middle joint of the middle finger. Right nostril tied to left finger, left nostril to right finger. All stop immediately. Crush garlic head. Apply thick paste to sole of foot to stop. Fuzi (Radix Aconiti Lateralis Preparata) powder mixed with wine applied to sole of foot is also effective. Burn paper dipped in sesame oil, insert into nostril to induce sneezing to stop. (Someone had epistaxis filling the night, used this method with effect.) Fold white paper eight times, wet with water, place on top of head. Gently iron with a clothes iron for immediate stop. Moxibustion with Ai Wan (Moxa Pill) the size of adzuki bean on the nape of neck between the hairline and the two ribs (where people usually scrape Sha). Three moxa cones stop immediately. (Blood enters brain and flows out through nose here.) For chronic epistaxis with deficiency: Liqiao Hui (Ash of Castanea Shell) (2 qian), Renshen Mo (Ginseng Powder) (5 fen), Huanglian Mo (Coptis Powder) (1 qian).
