Ke Ni (Cough with Qi Reversal) refers to coughing accompanied by upward reversal of Qi. Qi belongs to Yang, circulates through the Zang-Fu organs, and disseminates through the Cou Li (interstices); this is governed by the Lung. Cough diseases arise from Lung deficiency combined with slight cold invasion. Cold condenses with Qi, preventing its dissemination; Stomach Qi reverses and accumulates, rushing upward, causing distension in the lungs and lateral abdomen. The reversed Qi does not descend but again combats the Stomach. Due to weak Stomach Qi and latent cold in the Spleen, pathogenic Qi rushes into the Stomach tube. The Stomach fails to control it, causing Yin and Yang Qi to clash, resulting in Ke Ni. Symptoms include cough, chest fullness, Qi reversal, shoulder and back pain, sweating, and pain in the buttocks, yin region, thighs, knees, and feet. Whether in men or women, Ke Ni is colloquially called "Chi Te" (hiccup/hem), caused by disharmony between Yin and Yang. Fire desires to ascend but is suppressed by cold; since cold cannot overcome fire, it produces a stagnant sound. This syndrome can also occur in Shang Han (Cold Damage) and should be treated with Jiji San.
Youqi Tang: Treats Jue Ni (Reversal Cold/Extremity Cold). Caused by excess organ Qi, cold-induced consumptive disease, worry, and fright. The patient is prone to palpitations, feels cold in the chest intermittently, experiences unpredictable upper and lower sensations, often feels sad, has wandering Qi flowing through the four limbs and around the umbilicus, often presents with nodular wandering swellings, and suffers from constipation.
Houpo (Magnoliae Officinalis Cortex) (4 liang, processed), Renshen (Radix Ginseng), Gancao (Glycyrrhizae Radix et Rhizoma, processed), Muli (Ostreae Concha) (each 2 liang, calcined), Fuling (Poria) (4 liang), Jiang (Zingiberis Rhizoma) (8 liang), Huangqin (Scutellariae Radix) (3 liang). Grind the above herbs into powder. Add 9 sheng of water, decoct to obtain 3.5 sheng, strain, and divide into five doses. Take three times during the day and twice at night. If abdominal pain occurs, remove Huangqin and add Shaoyao (Paeoniae Radix).
Dingxiang Shidi San (Clove and Date Sepal Powder): Treats vomiting and dysentery, as well as post-illness Stomach deficiency-cold, where Ke Ni reaches 70-80 sounds continuously without returning breath; this is difficult to treat.
Renshen (Radix Ginseng), Fuling (Poria), Jupi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Liangjiang (Alpiniae Officinarum Rhizoma, stir-fried), Shidi (Kaki Calyx), Dingxiang (Caryophylli Flos) (each 1 liang), Shengjiang (Zingiberis Rhizoma Recens) (1.5 liang), Gancao (Glycyrrhizae Radix et Rhizoma) (0.3 fen). Grind the above herbs into powder. Take 3 qian per dose, decoct with one sheng of water, and take hot in one gulp. Alternatively, use this powder to administer Suhexiang Wan (Styrax Pill), which is also effective.
Wuweizi Tang (Schisandra Decoction): Treats cough with shortness of Qi, inability to breathe freely, intermittent chills and fever, chest distress and fullness, and inability to eat.
Wuweizi (Schisandrae Fructus) (2 liang), Guixin (Cinnamomi Cortex) (0.5 fen), Gancao (Glycyrrhizae Radix et Rhizoma, processed, 1 qian), Xixin (Asari Radix et Rhizoma) (1 liang), Ganjiang (Zingiberis Rhizoma) (0.3 fen), Ziwan (Asteris Radix) (2 liang, broken apart), Dazao (Jujubae Fructus) (30 pieces, broken apart), Mahuang (Ephedrae Herba) (2 liang, nodes removed). [Text missing standard preparation instruction here].
Shangqi Wan (Upper Qi Pill): Treats Ke Ni.
Ganjiang (Zingiberis Rhizoma) (4 liang), Guixin (Cinnamomi Cortex), Kuandonghua (Farfarae Flos) (each 1 liang), Fuzi (Aconiti Lateralis Radix Praeparata) (4 pieces, processed), Wuweizi (Schisandrae Fructus) (2 liang), Badou (Crotonis Fructus) (60 seeds, 30 for elderly/frail, remove heart and crush). Sieve the mixed herbs separately, crush Badou into a paste, mix with the herb powder, and bind with honey to form pills sized like sesame seeds. Place one pill on the teeth to chew/suck, preferably taken at bedtime or three times daily. Avoid raw green onions, pork, and asparagus.
For chronic cough and hiccups, with continuous coughing for 40-50 sounds (from *Zhouhou Fang*):
Use 0.5 he of fresh ginger juice and one spoonful of honey. Decoct until warm and take. Three to four doses yield immediate results.
For cough caused by cold attacking the Stomach (from *Shenghui Fang*):
Use 2 he of black beans. Place in a bottle, pour hot vinegar over them, seal the bottle mouth with paper leaving only a small hole. Let the patient inhale the vapor through the mouth into the throat to stop the cough.
Jupi Ganjiang Tang (Tangerine Peel and Dried Ginger Decoction) (from *Yifang Dacheng*): Treats Ke Ni due to cold in the Stomach.
