Lianggan Shanghan (Double-Contracted Cold Damage) refers to the simultaneous pathogenic invasion of the Taiyang and Shaoyin meridians. On the first day, Ju Yang (Foot-Taiyang Bladder Meridian) and Shaoyin (Foot-Shaoyin Kidney Meridian) are simultaneously affected, presenting with headache, dry mouth, mental restlessness and fullness, and thirst. On the second day, Yangming (Foot-Yangming Stomach Meridian) and Taiyin (Foot-Taiyin Spleen Meridian) are simultaneously affected, manifesting as abdominal distension, generalized fever, anorexia, and delirious speech. On the third day, Shaoyang (Foot-Shaoyang Gallbladder Meridian) and Jueyin (Foot-Jueyin Liver Meridian) are simultaneously affected, resulting in deafness, retraction of the testes, cold extremities, inability to ingest fluids, and unconsciousness with loss of recognition. This condition is difficult to treat. Why? Because the exterior and interior aspects of the Zang-Fu organs are simultaneously invaded by pathogens, leading to internal injury of the five Zang organs, blockage of the six Fu organs, contention between Yin and Yang, and stagnation of Ying (Nutrient) and Wei (Defensive) Qi circulation. Under such circumstances, death may occur within three days.
In the initial one or two days of Lianggan Shanghan, diaphoresis (sweating) is absent, and emetic or purgative methods are contraindicated. The patient exhibits heavy limbs, mental confusion and oppression, cold hands and feet, inner irritability, desire for water, headache, aversion to cold with high fever, or immediate vomiting and regurgitation, pain in the abdomen and hypochondrium, worsening heaviness, and inability to sleep. This represents a critical state of Yin-Yang intermixture, generally considered incurable. However, by observing the severity of symptoms and the depth of pathogenic invasion, appropriate rescue measures can be taken, potentially leading to recovery.
The verse states: "Lianggan involves the Bladder meridian opposing the Shaoyin, with headache and dry mouth, and a pulse that is large and deep. By the second and third days, the Stomach, Spleen, Liver, and Gallbladder are involved; when pulses and symptoms transmit doubly, follow standard patterns."
Another discussion states: Among Shanghan conditions, only Lianggan is considered incurable. Lianggan involves half Yin meridians and half Yang meridians, affecting both Zang and Fu organs. First, Taiyang and Shaoyin simultaneous disease presents with headache, dry mouth, restlessness, thirst, and a large-deep pulse. Second, Yangming and Taiyin simultaneous disease presents with body heat, dry nose, delirium, abdominal fullness, anorexia, and a long-deep pulse. Third, Shaoyang and Jueyin simultaneous disease presents with deafness, testicular retraction, cold extremities, inability to drink, and a string-tight-deep pulse. Some become rigid in three days, others collapse in six days. Master Zhang provided no specific treatment but stated: "Treatment of Lianggan must prioritize sequence; releasing the exterior and attacking the interior have different principles." Shuang Zhong offered his interpretation: If there is incessant diarrhea and body pain, urgently save the interior with Sini Tang (Four Reversal Decoction); if there is no diarrhea but only body pain, urgently release the exterior with Guizhi Tang (Cinnamon Twig Decoction). This illustrates the principle of prioritized treatment and adaptive flexibility. Xie Fugu explained Zhang's view, agreeing that if Yang meridians are primarily affected at the exterior, release the exterior first; if Yin meridians are primarily affected at the interior, save the interior first. This is consistent logic. However, if releasing the exterior first, saving the interior cannot be delayed; if saving the interior first, releasing the exterior cannot be delayed either?
