Zhu Tongdao from Chi'an, aged forty, on the Mid-Autumn Festival (August 15th), caught cold after rain. After midnight, severe abdominal pain, sweating like rain, kicking through walls with his feet. Pain in the lower abdomen, intolerable to touch. Six pulses were wiry, thin, and solid; heavy pressure felt like touching blades. He reported bathing in a deep pool during the intense heat of June, suggesting this as the cause. Given Dachengqi Tang (Major Order the Qi Decoction), slight bowel movement occurred, and pain stopped immediately. At You hour (5-7 PM) the next day, pain recurred in the lower abdomen, hard and intolerable to touch. Given another two doses of Dachengqi Tang plus Rougui (Cinnamon twig), with crushed Taoren (Peach kernel) decocted together. More than a sheng of dark purple-black blood was passed, and pain stopped immediately. At You hour the next day, pain recurred as before. Pulses slightly decreased but still felt sharp. Given two doses of Dachengqi Tang plus Fuzi (Aconite lateral root), with crushed Taoren (Peach kernel) decocted together. Passed five stools, obtaining about two sheng of black-purple blood like broken cotton. Pain stopped immediately, allowing sleep through the night. At You hour the next day, pain recurred as before. Inquiry revealed the lower abdomen was soft, pain located below the umbilicus. Pulses were harmonious, appearing normal, yet groaning continued. Inquiry revealed he had eaten two bowls of radish green soup. Given one dose of Xiao Jianzhong Tang (Minor Construct the Middle Decoction), and he recovered. (From *Bencao Gangmu* citing *Danxi Xinfa*) [Note: Wang Shishan had a case of Xiaochengqi Tang plus Taoren, with similar intent, not recorded here.]
I believe Zihe's treatment of the Seven Hernias, although varied, relies heavily on purgatives, thus having bias. Liu Zongnong and Xu Yongcheng doubted and criticized it, saying it was too extreme, not that it was completely unusable. Looking at Danxi's method, he said treating hernia strongly contraindicates purgatives, which is a great contradiction. I examined Zihe's medical cases: treating Cai Canjun, who sat on damp ground causing unbearable hernia pain, cured by purging with Daoshui Wan (Water-Guiding Pill). Treating another person who drank too much sweet water due to malaria thirst, developing hernia; doctors gave Jiang (Ginger) and Fu (Aconite), causing blockage by dry heat, leading to heavy, swollen scrotum the size of a measuring cup. First given Daoshui Wan, then Zhufu San (Pig Skin Powder), purged heavily, and cured. Treating another person with sudden hernia, red, swollen, and extremely painful for days, unresponsive to other medicines. Given Daoshui Wan, followed by Tongjing San (Unblocking Meridian Powder), purged heavily, and cured. Are such cases truly untreatable by purgation? Therefore, one must weigh deficiency/excess and urgency. If blocked by pathogenic heat, or sudden illness in youth, or extreme swelling/hardness/redness/pain, then Daoshui Wan (Water-Guiding Pill), Sanhua Shenwan (Three-Flower Divine Pill), Yugong San (Yu Gong Powder), etc., are appropriate. When pathogen is abundant and urgent,势头 unstoppable, and beyond the reach of Qi-moving or diuretic agents, attack is necessary. Zihe's methods have their indispensable value, which must be understood. (From *Jingyue Quanshu*)
