Wei Syndrome (Flaccidity Syndrome), in the medical literature of the Tang and Song dynasties, rarely saw discussions on its formulas and theories. Since the Ming Dynasty, views have largely been similar to those of Liu Wansu (Liu Hejian) and Zhang Congzheng (Zi Hizi), Zhu Zhenheng (Danxi), etc. Therefore, the content collected in this compilation is only a few pages; readers should not suspect it is omitted or simplified.
**Nomenclature**: "Wei" refers to flaccidity and weakness, with insufficient strength for movement. (*Suwen Cizhu*)
**Diagnosis Methods**: The pulse at the cun position (radial artery distal segment) is weak, appearing slow, choppy, and tight. The disease presents as foot pain or Wei syndrome. (*Cui Shi Maijue*)
In Wei syndrome due to Lung deficiency, the pulse is often slightly slow, or choppy, tight, fine, or soggy. (*Siyan Juyao*)
**General Discussion on Etiology and Pathogenesis**: The human body forms its structure through skin, hair, blood vessels, sinews, muscles, and bones, which are governed internally by the Liver, Heart, Spleen, Lung, and Kidney. If one indulges in emotional excesses without moderation, combining labor with rest improperly, it leads to the depletion of essence and blood in the Zang organs. This causes an imbalance of Ying (Nutritive) and Wei (Defensive) Qi, manifesting as alternating chills and fever. Consequently, the skin, blood, sinews, bones, and muscles become flaccid and weak, unable to move, resulting in Wei syndrome. Its appearance is similar to Roufeng (Soft Wind) and Hongkong Foot (Jiaoxie). It must be distinguished by pulse, symptoms, and etiology, and not confused. Roufeng and Hongkong Foot are caused by external factors, whereas Wei syndrome belongs to internal factors, specifically insufficiency of Zang Qi. This must be carefully examined. (*Sanyin Fang*)
**Etiology: Lung Heat**: Generally, the Lung governs Qi. Qi is Yang, and Yang is light, clear, and ascending; thus, the Lung resides in the upper jiao. When diseased, its Qi becomes full and rushing, unable to ascend, leading to flaccidity and weakness of the hands and feet, unable to grasp or hold. Since Lung Metal is inherently dry, dryness pathology causes a decline in blood, failing to nourish the entire skeleton. The Classics state: "Fingers receive blood to grasp, palms receive blood to hold, feet receive blood to walk." Thus, when Autumn Metal Qi is prosperous, mist gathers, and grass and trees wither; this is the image of disease. "Wei" is like "Wei" (withered). (*Yuanbing Shi*)
The manifestation of Wei syndrome is flaccidity and weakness of both legs, inability to walk. This occurs because Kidney Water fails to generate Heart Fire, causing Heart Fire to scorch Lung Metal. Lung Metal, controlled by fire, has its six lobes scorched. The skin and hair become weak, contracting and adhering tightly, generating Wei syndrome. "Bi" refers to the inability to extend the feet while walking. Generally, Wei syndrome arises from exogenous heat. Excessive desire and sexual indulgence, forcing strength beyond limits, gradually lead to Wei disease. Therefore, Rou Wei (Flesh Wei) belongs to the Lung, Mai Wei (Vessel Wei) to the Heart, Jin Wei (Sinew Wei) to the Liver, Rou Wei (Flesh Wei) to the Spleen, and Gu Wei (Bone Wei) to the Kidney. Ultimately, it is due to Lung heat scorching the leaves, transmitting to the other four Zang organs, forming Wei syndrome. It is directly stated that Wei syndrome has no cold pattern. Thus, patients with Wei syndrome have a floating and large pulse. (*Shiqin*)
Lung Metal is dry and resides above, governing Qi, and fears Fire. Spleen Earth is damp and resides in the center, governing the four limbs, and fears Wood. Fire nature is flaming upward. If desires are unregulated, Water loses nourishment. Fire, lacking restraint, insults the metal it overcomes, causing Lung heat. Wood nature is rigid and urgent. When Lung receives heat, Metal loses nourishment. Wood, lacking restraint, insults the earth it overcomes, injuring the Spleen. Lung heat cannot control the whole body; Spleen injury prevents the four limbs from functioning, causing various Wei syndromes. (*Jufang Fahui*)
Wei syndrome refers to emaciation and exhaustion of form and Qi, with inability to raise hands and feet. This is due to insufficiency of Middle Qi. Earth fails to nourish Metal; Metal fails to generate Water; Water fails to control Fire. Thus, Lung leaves wither and dry, Kidney loses generative Qi, causing the Five Spirits to lose their positions. Water dries, Fire roasts, Blood empties, Qi becomes dry, causing Zongjin (tendons) to lose moisture. Sinews, vessels, bones, flesh, blood, and Qi all become flaccid. (*Xintong*)
**Etiology: Damp-Heat (Large sinews contract and shorten, small sinews relax and lengthen)**: This symptom of Wei syndrome lies entirely in Damp-Heat. Due to excessive wine and sex, Qi and Blood become empty. Adding labor damages sinews and bones. Damp-Heat then takes advantage. Heat injures Qi, preventing it from smoothing the sinews, causing large sinews to contract and shortening, leading to spasms. Dampness injures Blood, so Blood fails to nourish sinews, and sinews fail to bind bones, causing small sinews to relax and lengthen, leading to flaccidity. (*Shengmo*)
The Classics say: "If Damp-Heat is not removed, large sinews contract and shorten, small sinews relax and lengthen. Shortening causes spasm; lengthening causes flaccidity." This is the general description of sinew diseases in *Neijing*, offering examples to inspire self-reflection, not implying that large sinews never relax or small sinews never contract. Just as flaccidity must result from relaxation, how could large sinews be unrelated? The intent of the Classics can be understood here. Therefore, outside of Wei syndrome, when encountering convulsions, one should know that spasms result from shortening, and paralysis results from lengthening. This aligns with *Neijing* and aids in distinguishing Cold, Heat, Dryness, and Dampness. (*Jingyue Quanshu*)
Although the Zang-Fu etiologies of Wei syndrome vary, they mostly originate from Damp-Heat in the Yangming channel, turbid internally. This causes the Lung to suffer heat invasion and dry up daily, and the Spleen to suffer dampness overflow and swell daily, forming a condition of upper dryness and lower dampness. The world universally focuses on Kidney deficiency, never mentioning Yangming Damp-Heat. (*Yitong*)
Wei syndrome refers to softness and laxity of hands and feet, unable to hold themselves (commonly known as limp hands and weak feet). This is caused by Heat combined with Dampness. Observing objects: cold and dry things are hard and contractile; hot and damp things are soft and lax. Dampness belongs to Earth. The Stomach is the sea of water and grain, moistening sinews and vessels. Stomach disease fails to transform water and grain; dampness stagnates in sinews, becoming flooding rather than moistening. Combined with Heat, treatment of Wei syndrome exclusively targets the Yangming channel. However, while this syndrome is Heat without Cold, the presence of Dampness requires careful differentiation. If there is no Dampness but drying diuretics are used indiscriminately, Fire becomes more arid, and sinews/vessels become dry and contracted, making relaxation impossible. Physicians must examine this. (Note: There are cases of Wei without Dampness, where fluids are scorched by heat but not yet dry. Though not lax or contracted, they are weak and soft, unable to move. Like plants under the scorching sun, leaves wilt.) (*Yique*)
**Not All Cases Are Fire Patterns**: The meaning of Wei syndrome is detailed in *Neijing*. The five Zang patterns listed all mention Heat, summarized as Lung Heat scorching leaves, leading to Metal dryness and Water deficiency. Danxi’s treatment theory is indeed correct. However, examining the text closely, it also says: extreme sorrow closes the Bao Luo (Pericardium collaterals), transmitting to Mai Wei; endless thoughts and unfulfilled desires cause Jin Wei; gradual exposure to dampness and water work cause Rou Wei. Thus, not all are Fire patterns. There are cases where Fire is generated from this, and cases where Yuan Qi (Original Qi) is damaged. Damaged Yuan Qi leads to Essence deficiency failing to irrigate, and Blood deficiency failing to nourish. If treated solely as Fire, those with True Yang failure and Spleen/Water depletion may not withstand it. One must weigh the depth of Cold/Heat and urgency of Deficiency/Excess to treat comprehensively. (*Jingyue Quanshu*)
**Associated Factors**: Internal Heat causing Wei discusses the root. If triggered, it depends on associated factors: Damp-Heat, Damp-Phlegm, Qi Deficiency, Blood Deficiency, Yin Deficiency, Stasis (Dead Blood), Food Stagnation obstructing ascent/descent (based on Danxi). These must be clearly distinguished.
* **Damp-Heat Wei**: Rain/dampness invades, evil steeps the Spleen, flows to limbs. Patients feel heat rising from shins, limb soreness/swelling/numbness/itching, dark/scanty urine, deep/soggy/rapid pulse. This is Damp-Heat below. Treat with raising Yang and drying Dampness; avoid tonifying agents.
* **Damp-Phlegm Wei**: Common in obese people. Original Qi cannot move Phlegm, causing stagnation in meridians. Waist/knees numb, limbs weak, deep/slipping pulse. Caused by rich food/wine. Spleen Earth excess causes limb immobility. Treat with drying Spleen and moving Phlegm.
* **Qi Deficiency Wei**: Caused by hunger/fatigue. Stomach Qi fails, Lung Qi first perishes. Body parts lose nourishment, Zongjin relax, joints empty. Post-illness limb weakness is Qi Deficiency. Spleen fails to transport fluids; limbs lack Grain Qi. Treat with Bu Zhong Yi Qi Tang.
* **Blood Deficiency Wei**: Postpartum blood loss, sallow complexion, limb weakness, inability to move. Treat with nourishing Ying Blood. Note: Blood is born in Spleen; if tonifying blood fails, Spleen is deficient. Tonify Spleen to restore Blood.
* **Yin Deficiency Wei**: Excessive wine/sex, Lower Jiao Liver/Kidney Fire scorches sinews/bones. Feet extremely hot, rushing to knees. Sore/weak/flaccid, difficulty walking, unable to stand long. Pulse choppy/weak or left pulse large but empty upon pressure. Treat with severe tonification of Essence/Blood to support Liver/Kidney.
* **Blood Stasis Wei**: Postpartum lochia not cleared, flowing to waist/knees, or trauma causing stasis. Limb pain, inability to move, choppy/hollow pulse. Treat with nourishing Blood and moving Stasis.
* **Food Stagnation Wei**: Overeating obstructs pathways, disrupting ascent/descent. Spleen Qi fails to reach limbs. Limbs weak, abdominal distension/pain, nausea/belching. Right hand pulse surging/string-tight/slipping. Treat with moving Spleen and resolving stagnation. After resolution, tonify Spleen. (Note: Danxi mentions Wei from food stagnation or stasis should be purged.)
* **Post-Dysentery Weakness**: Soft legs/shin pain/knee swelling after dysentery is due to excessive downward flow depleting Yin. Tonify Spleen and lift. Treating as Wind dries Yin and worsens Wei. Rarely, premature astringency in dysentery leaves stasis flowing into channels, causing Wei. Treat with moving Qi and expelling Stasis, distinct from previous cases. (*Huibu*)
**Bone Wei**: Bones are governed by the Kidney. Congenital insufficiency leads to soft, non-solid bones. Marrow does not fill bones; sinews lack blood to bind bones. Ying/Wei are weak, causing Bone Wei and deformation. Like young plants broken by wind, bending and twisting. Ancient and modern treatments for bone disease often fail. I use "Return to Origin" herbs and Kidney-tonifying/Lung-clearing herbs; 20-30% recover. (*Nuke Baibing Wenda Buyi*)
**Treatment Principles**: Use Huangqin, Huanglian, Danggui, Shengdi, Duhuo, Niuxi, Qinjiao, Xuduan. Do not bias towards Wind herbs or treat as Wind. Do not bias towards tonics and treat as Deficiency. Promote urination to remove Damp-Heat; use cooling herbs to unblock Qi/Blood. For Kidney Qi emptiness, spine cannot rise, marrow exhausted, Bone Wei: add Wuweizi, Gouqi, Shanzhuyu, Shudi, Tiger Bone, Tortoise Shell. (*Shengmo*)
For Wei due to Damp-Heat, surging/slipping pulse, irritability/heat: First clear Fire. Use Ermiao San, modified. For Yin Deficiency with Heat: Zhengchuan Modified Siwu Tang, Hu Jinggu Wan, Danxi Buyin Wan, Ziyin Bawei Wan. For no Fire, only Kidney Water/Liver Blood deficiency: Avoid cooling herbs that damage life Qi. Lujiao Jiao Wan is best. Or Jiawei Sijin Wan, Bawei Dihuang Wan, Jingang Wan. If Yin Deficient without Dampness or with sweating: Avoid Cangzhu. Wei syndrome forbids exterior-releasing herbs to prevent damaging Yin. (*Jingyue Quanshu*)
Wei belongs to Damp-Heat. Classics say: "Treat Wei by exclusively targeting Yangming." Avoid breaking Qi, lifting, acrid-hot dispersing. Use major Qi/Blood tonification, clearing Heat/drying Dampness, Sweet-Cold, Sweet-Warm, Bitter-Cold, Sour-Cold. (*Bencao Jingshu*)
**Treatment Should Not Be Hot/Drying Yang Tonification (Can Fuzi be used?)**: Physicians seeing weak legs/knees or one leg unable to extend often treat as Cold-Damp Hongkong Foot, using Wutou, Fuzi, Ruxiang, Moyao, Ziran Tong, Weilingxian, moxa, hot compresses. This worsens Wei and causes death. Is this fate? Treating Wei differs from Bi (Painful Obstruction). Bi (Wind-Cold-Damp) may respond to heat/moxa. Why does Wei worsen? Because Lung Heat is the root, leaves scorch, transmitting to five Zang. How can it be Cold? (*Shiqin*)
Chen Wuye said Wei is due to five internal deficiencies, which is true. But the deficiency is Yin and Blood. Most formulas tonify Yang and Qi. Can this avoid the error of reinforcing Excess and depleting Deficiency? (*Weiyi*)
Some ask: If Wei is Damp-Heat, why do ancient formulas use acrid-hot Fuzi successfully? Unknowingly, long-standing Damp-Heat stagnation requires acrid-hot power to open meridians; it was not originally for Kidney Cold deficiency. If True Yang is not depleted, indiscriminate warming tonification without clearing Heat/dampness will surely assist Damp-Heat. (*Yitong*)
**Case Study Using Purging**: Three military officers in Wanchou suffered Wei for years, unable to rise, swollen/painful below waist, red sores, dim eyes, dry lips/tongue. They sought Dai Ren (Zhang Congzheng). Dai Ren wanted to purge. Two refused, were seduced by warm-tonifying physicians, and died. Another patient, Song Ziyu, realized they died from heat, invited Dai Ren. Dai Ren said: "You took hot herbs long ago. Remove the drug pathogen first, then the disease pathogen. Purge 300 times." Ziyu agreed. Used Zhouche Wan, Xunchuan San, purged a basin. Next day improved. Legs felt pain/itch (previously numb). After 300 purges, recovered. Dai Ren said: "Treat Wei exclusively via Yangming. Yangming is Stomach and Large Intestine. This applies to needles and medicine alike." (*Suliao*)
Zhu Xiuzhi, a Taixue student, paralyzed for 8 years, seen by hundreds of doctors, no effect. Read my *Yisheng Weilun*, summoned me. Pulse strong, diet normal. Internal Steaming Heat, isolated Heart Yang hyperactivity. Diagnosis: Mai Wei. Used Chengqi Tang, purged 6-7 times. Left foot moved. Used Da Chengqi Tang, purged 10+ times. Hand could hold objects. Then Huanglian, Huangqin (1 jin each), Jiuzheng Dahuang (8 liang), honey pills, 4 qian/day with Renshen soup. Within a month, expelled countless accumulations. Limbs relaxed. I said: Accumulations gone. Prepared Santai Gao (10 jin). Taken, he resumed duties. (*Bixu*)
