Translation:
It is discussed that when the fetus is in the womb, if the mother delights in consuming hot and toxic foods, heat generates wind, leading to dysfunction of the Spleen and Lung, thereby producing Wind-Phlegm. Although this may not yet damage the limbs or body, Phlegm and Wind attack each other; if there is much phlegm and wind fails to dissipate due to intense heat, phlegm regenerates. Moreover, both wind and phlegm can cause fever in the infant. How can one add warm blankets, placing the infant as if in a red furnace? If the mother consumes roasted, fried, pickled, or stored foods that stir up wind, it will immediately harm the newborn's life. Medicines are not ineffective; the fault lies in the breast milk. If the infant suffers from Wind-Phlegm, there is no need to use harsh wind-dispelling and phlegm-resolving medicines. Instead, one should clear the Heart and Lung, cool the Diaphragm, and unblock the Triple Burner, and the infant will naturally recover. One must not administer great cold medicines, fearing injury to the Zang-Fu organs. The suitable approach is to swallow Zhiliang Wan (Excellent Medicine Pill) with Xiaofeng San (Wind-Dispensing Powder), made into small pills for consumption.
It is discussed that when the fetus is in the womb, if the mother neglects regulation, indulges in diet without proper care, and allows heat to accumulate for a long time, upon birth, hidden heat transmits inward, resulting in closure and constipation. This closure is caused by extreme heat, manifesting as difficulty in defecation, heat rushing to the Heart, abdominal distension, protruding umbilicus, mouth pulling (crouching mouth), and straining. In urgent cases, Sanhuang Wan (Three Yellows Pill) or Sishun Qibao Dan (Four Smooth Seven Treasures Pill) should be used urgently to restore bowel movement, without exceeding the necessary dose. One must absolutely avoid using Xiaji Wan (Accumulation-Draining Pill); such drugs are not only useless but may cause harm.
It is discussed that when the fetus is in the womb, if the mother contracts Cold pathogenic factors,烦蒸 (disturbing steaming heat) transmits into the fetal viscera, which the fetus receives. Upon birth, this causes blockage of the orifices, turbid spirit, fever with steaming sensation, weak crying voice, and terrifying wailing. Uninformed physicians may mistakenly attribute this to convulsive wind and recklessly administer heart-calming sedatives. This is because the contracted Cold initially remains at the exterior but transmits inward over time. As stated, pathogenic factors disrupt true Qi, causing blood vessels and skin/hair to lose their proper state, constraining the Couli (pores/interstices), preventing pathogen resolution, disrupting the Bianzheng (variable steaming) process, injuring Qi and Blood, and increasing irritability without self-relief. At this stage, Xiao Chaihu Tang (Minor Bupleurum Decoction) should be administered first; once symptoms slightly improve, Renshen Qianghuo San (Ginseng and Notopterygium Powder) should follow. However, since the pathogenic factor acquired in the womb has not been differentiated into exterior or interior, and after birth under maternal protection, a Harmonizing method should be discussed as the best approach.
It is discussed that when the fetus is in the womb, if the mother has an increased appetite and favors consuming Ginger and Melons. Ancients said, "Repairing the stove leads to iron lips; eating ginger leaves surplus." This implies Ginger is extremely hot in nature and, being pickled/stored, attacks blood vessels and transmits into meridians. Mildly, it causes the infant to develop maggot sores, spreading heat-toxic sores, erysipelas-like conditions, recurring annually, or nodules on the forehead. Severely, it develops into large carbuncles and furuncles; upon ulceration, skin rots and decays, with ten deaths for every one survival. How can one bear to see a newborn's tender flesh suffer intensified toxin accumulation? Treatment should involve Shen (Ginseng) and Zhu (Atractylodes) to support the interior, and Gouqi (Lycium Bark/Digut皮的 root bark, here referring to Digupi/Gouqipi) and Bo (Phellodendron Bark) to support the muscles ("Gou" refers to Digupi/Gouqipi [Cortex Lycii], "Bo" refers to Huangbo [Cortex Phellodendri]). Regularly combine methods to clear the Heart and harmonize blood vessels. The mother must guard her diet, the father holds and nurses the infant, and physicians follow principles to support the critical condition.
