On Treating Fire in the Upper Jiao, On Treating Phlegm in the Upper Jiao, On Treating Cold Stomach, On Treating Cold Spleen, On Treating Lao Chong (Consumption Parasites/Bacilli), On Eliminating Lung Phlegm
The Heavenly Master said: The method of "treating after a full meal" refers to conditions located in the Upper Jiao (the chest and diaphragm area above), where medication should be taken after a full meal; this is one method. For conditions suitable for emesis, medication should be administered after a full meal to induce vomiting; this is another method. There are also cases that do not require emesis but are treated by inducing satiety through food intake; this is yet another method. Conditions in the Upper Jiao refer to diseases of the head and eyes. Medicines similar to Shangqing Wan (Upper Clearing Pill) are used. However, the formula for Shangqing Wan circulating in the world is often inappropriate. I have revised it after careful consideration to treat fire in the Upper Jiao, which can be taken universally. Su Ye (Perilla Leaf) two liang, Bo He (Mint) one liang, Bai Zhi (Angelicae Dahuricae Radix) five qian, Huang Qin (Scutellariae Radix) two liang, Gan Cao (Licorice Root) one liang, Jie Geng (Platycodonis Radix) three liang, Mai Dong (Ophiopogonis Radix) three liang, Tian Men Dong (Asparagi Radix) three liang, Ban Xia (Pinelliae Rhizoma Preparatum) one liang, Chen Pi (Citri Reticulatae Pericarpium) one liang, Man Jing Zi (Vittaginis Fructus) five qian, Chai Hu (Bupleuri Radix) one liang. Each is ground into fine powder. Form pills using water. Take three qian per dose, administered after a full meal. (Commentary: Shangqing Wan.) The brilliance of this formula lies in clearing fire without damaging Zhong Qi (Middle Jiao Qi/Spleen-Stomach Qi). Whether individuals of strong or weak constitution contract wind-stroke pathogens, or those with wind-fire in the Upper Jiao, taking this formula yields excellent results.
When phlegm-qi in the Upper Jiao is excessive while the Lower Jiao is deficient, purgative methods should not be used. Instead, allow the patient to eat until full, then administer medicine to induce vomiting immediately, stopping once food contents are expelled. This way, without affecting the Lower Jiao, the phlegm-fire in the Upper Jiao is cured in one instance of vomiting. This is a clever treatment method. The formula uses Gua Di (Melon Seed) seven pieces, Ren Shen (Ginseng Radix et Rhizoma) two qian, and three large bowls of water. Boil briefly and drink; it will induce severe vomiting immediately. (Commentary: Jia Shen Gua Di San [Gua Di Powder with Added Ginseng].) The brilliance of this formula lies in adding Ren Shen to Gua Di San. Since emesis inevitably damages Qi, using Gua Di for emesis while Ren Shen replenishes the Qi in the stomach ensures that even with severe vomiting, the Stomach is not injured. Thus, the condition stabilizes after one episode of vomiting.
For cases not requiring emesis but treated via satiety, the indication is coldness in the Stomach causing pain. If pressing the area provides slight relief, this method should be used. The formula consists of Ren Shen (Ginseng Radix et Rhizoma) one liang, Bai Zhu (Atractylodis Macrocephalae Rhizoma) one liang, Rou Gui (Cinnamomi Cortex) one qian, and one fat duck. Place the herbs inside the duck's cavity, cook until extremely tender, season externally with Five Flavors (Wu Wei), allowing all condiments such as scallions and pepper. Simmer further with tofu skin (Fu Pi). Allow the patient to eat heartily until finished. (Commentary: Wu Xiang Tang [Five Fragrances Decoction].) If the patient cannot finish it all, let it be; there is no need to eat rice afterwards. After one meal, the pain disappears as if lost. This method of treating via satiety truly has miraculous effects. Cold Stomach is invariably accompanied by deficiency of Stomach Qi. If decoctions are given, they inevitably fail to remain long in the Stomach, as Qi from various meridians comes to share and distribute the medicinal effect, making cure difficult. Now, by cooking the medicine in a fat duck and eating until full, the medicine remains long in the Stomach, acting independently without being distributed by other meridians. Naturally, one meridian receives its benefit exclusively, and the specific cold pathogen feels its sudden disappearance. When Zheng Qi (Upright Qi) remains long in the Stomach, Xie Qi (Pathogenic Qi) naturally avoids the exterior of the Stomach. Because Chen Zi did not understand this, I have thus widely revealed this secret.
Master Zhang said: Generally, diseases located in the upper part should be treated with medication taken after a full meal. Only the use of a full meal combined with duck to treat Stomach disease is truly unprecedented, a method worthy of immortals. Using a full meal to treat disease specifically targets Spleen disorders. Pain from Cold Stomach is located above the heart; pain from Cold Spleen is located below the heart and on the left and right sides. The formula uses one pig stomach, Lian Rou (Lotus Seed) one liang, Hong Zao (Jujubae Fructus) one liang, Rou Gui (Cinnamomi Cortex) one qian, Xiao Hui Xiang (Foeniculi Fructus) two qian, and Yi He (one unit of volume) of white glutinous rice. Put all herbs and rice into the stomach, tie the opening tightly with thread, and boil externally in clear water. Before putting herbs into the pig stomach, wash it thoroughly with clear water to remove odors. Cook with herbs until extremely tender. Eat all at once, dipping in sweet soy sauce. (Commentary: Lian Hua Du [Lotus Belly].) If not yet full, press down with rice, and the pain will disappear as if lost. This formula can be passed down alongside the Heavenly Master's formula. The Heavenly Master's formula treats the Stomach, while my formula treats the Spleen; the two do not interfere with each other.
Another method uses two jin of fat eel, Bai Wei (Cynanchi Atrati Radix) one liang, Xiao Hui Xiang (Foeniculi Fructus) three qian, Gan Cao (Licorice Root) one qian, Yi Yi Ren (Coicis Semen) five qian, and ten Fei Zi (Torreya Nuciferae Semen) shelled. Simmer together in a clay pot with water until tender. Season with Five Flavors and eat in one meal during moderate hunger or fullness. (Commentary: Zuo Xiang Man [Fragrant Eel].) Do not leave any behind; finish completely. Do not eat other food afterwards, and refrain from tea or water for half a day. All Lao Chong (consumption parasites/bacilli) will die. Due to Yuan Gong's inquiry, I greatly opened up this mechanism. I dare not conceal it, lest I incur heavenly retribution.
Master Hua said: Same as above. I have one more incomplete method. For patients suffering from chronic phlegm disease that does not heal, use one pig lung head. Grind Luo Bo Zi (Radish Seed) five qian and Bai Jie Zi (Mustard Seed) one liang into powder. Mix with Five Flavors, steam in a rice cooker until cooked, and eat in one sitting after a meal. One lung head cures it. This formula treats phlegm in the Upper Jiao that decoctions cannot cure, yielding miraculous results. Since the phlegm remains long in the lungs, it completely eliminates phlegm in the membranes and septa, representing an exceptionally clever treatment.
