Indicated for cases where, after profuse sweating in Taiyang disease (Taiyang syndrome), the exterior syndrome has resolved, yet there is extreme restlessness and thirst in the chest and heart, desiring to drink water, accompanied by vomiting or following diarrhea. After seven or eight days, if pathogenic factors remain unresolved, heat accumulates internally while the exterior shows heat with occasional aversion to wind, severe thirst, a dry tongue, irritability, and a desire to drink several sheng of water, this formula is appropriate. It is also indicated for summerheat-stroke caused by toxic summerheat during summer, presenting with sweating, aversion to cold, fever, and thirst.
Zhimu (Anemarrhenae Rhizoma, 6 liang, bitter, cold) Shigao (Gypsum Fibrosum, 1 jin, crushed, sweet, cold) Gancao (Glycyrrhizae Radix et Rhizoma Preparatum cum Melle, 2 liang, honey-fried, sweet, neutral) Jingmi (Oryza Sativa, 6 he, sweet, neutral)
Take the above four ingredients, boil in one dou of water until the rice is cooked and the decoction is ready, strain, and take one sheng warm, three times daily.
Commentary: Cheng Wuji stated that Baihu corresponds to the Metal spirit of the West, aligning with autumn and relating to the Lung. When heat is excessive internally, use cold purgatives; when heat is excessive externally, use cooling dispersants. However, when there is heat both inside and out, unable to be vented internally or released externally, no other formula can resolve it but this one. He further cited the *Neijing*: "When heat is rampant, assist with bitter and sweet." The bitter and sweet properties of Zhimu and Shigao dissipate heat. Since heat damages Qi, the sweet property moderates it; the sweetness of Gancao and Jingmi tonifies Qi.
This is Baihu Tang added with Renshen (3 liang), indicated for Taiyang Zhongshu (summerheat-stroke affecting the Taiyang channel), presenting with sweating, aversion to cold, fever, and thirst. This treats active summerheat injury with severe thirst, known as Huo Tang.
This is Baihu Tang added with Cangzhu (2 liang), indicated for passive summerheat injury without thirst.
This is Baihu Tang added with Guizhi, indicated for summerheat-stroke with spontaneous sweating and slight aversion to cold. It is also indicated for Wenmian (warm malaria), where the pulse appears normal, there is no chills but only fever, joint pain and irritability, occurring intermittently.
This is Baihu Tang added with Shanzhizi, indicated for strangury with restlessness and thirst.
This is Baihu Tang added with Wuweizi, indicated for cough with scanty body fluids.
This is Baihu Tang minus Zhimu, plus Zhuye (2 bundles), Banxia (0.5 sheng, washed), Maidong (1 sheng, heart removed), and Renshen (3 liang). It treats post-febrile weakness, emaciation, shortness of breath, and rebellious Qi leading to retching. The sweet and acrid properties of Zhuye, Shigao, and Gancao disperse residual heat; the sweet properties of Maidong, Renshen, and Jingmi tonify deficiency; Banxia disperses rebellious Qi.
This is Baihu Tang minus Gancao and Jingmi, added with Baizhi (1 liang), indicated for malaria with more heat than chills, where slight pathogenic factors remain after purgation/diarrhea.
This is Baihu Tang minus Gancao and Jingmi, added with Guizhi and Huangqin, indicated for malaria without other symptoms, alternating every other day, starting with chills followed by fever, with less chills and more heat.
