Sores in the mouth and tongue are all caused by heat stagnation in the Upper Jiao (Upper Burner). It is appropriate to use Rusheng Tang or Koukeng Tang with one qian of Huangqin (Scutellariae Radix). Additionally, apply Xinghua San externally. If there is no effect, then combine with Sixiang Yin. For those who cannot produce sound, taking cold-natured medicines leads to loss of voice; this is due to deficiency-cold of the Kidney Meridian. Several doses of Fuzi (Aconiti Lateralis Radix Praeparata) preparations must be administered to cure this. This point must not be overlooked.
Deficiency in the Lower Yuan (Lower Origin) and excess in the Upper part lead to sores in the mouth and tongue. If using sedative drugs to descend Yang, administer Yangzheng Dan or Heixi Dan with Yangjiang Tang taken with salt water. Furthermore, before sleep, wash feet with hot water, take a small pinch of stir-fried, clean Wuzhuyu (Evodiae Fructus), wipe the sole of the foot, then immediately apply stir-fried Wuzhuyu to the sole of the foot and bind it tightly. Apply to the left foot for men and the right foot for women.
There was once a patient with sores on the tongue that eroded into ulcers over time. Multiple courses of cooling medicines were ineffective. Later, someone advised taking Heixi Dan, leading to gradual recovery. This is also due to deficiency in the lower causing excess in the upper.
Additionally, sudden swelling of the tongue is called Chongshe (Double Tongue). Use only Zhen Puhuang (Polygoni Typhonii Pollen) powder applied externally.
There are cases where the tongue feels numb spontaneously without cause; this should not be treated as Wind syndrome. It is due to insufficiency of Heart Blood. Use Lizhong Tang with half a qian of Shufu (Prepared Aconite) and 1.5 qian of Danggui (Angelicae Sinensis Radix).
Toothache, see Toothache Syndrome in the Chapter on Various Pains. Other dental ailments vary and have their own specialized departments. Gingival bleeding (Yanu) see Hemorrhage Chapter.
(Attached: Back Carbuncle)
To disperse various toxic pathogens, grind Bai Furong Ye (Hibisci Mutabilis Folium) extensively, add a little Caowu Ye (Aconiti Kusnezoffii Radix Folium), and mix with honey for external application. For severe cases, add Nanxing (Arisaematis Rhizoma) powder. For all kinds of toxic pathogens, if using plasters to disperse them, rub in Shexiang (Moschus); if intending to induce suppuration, rub in Xionghuang (Realgar).
For all toxic syndromes, if thirst has already appeared, or if pus discharge is excessive, to prevent thirst due to fluid depletion, besides Qianjin Neibu San, concurrently administer Bawei Wan (Eight-Ingredient Pill with Rehmannia Root), preferably containing Wuweizi (Schisandrae Chinensis Fructus).
Fever and aversion to cold, resembling Taiyang disease but accompanied by pain, indicate that the painful area is developing into a carbuncle or deep abscess. Carbuncles belong to the Fu organs, thus they develop superficially, with thin skin and high swelling. Abscesses belong to the Zang organs, thus they develop deeply, with thick skin and firm swelling. There are also milder forms than carbuncles and abscesses, named Jiedu (Boil/Carbuncle). "Yong" means congestion; "Ju" means obstruction; "Jie" means nodule. It refers to Qi and Blood congestion and obstruction at joints. Treat all with Weilingxian Yin to mildly promote bowel movement, or Wuxiang Liaoqiao San, assisted by Fuyuan Tongqi San. If pus and blood have formed, do not use dispersing methods. Whether ulcerated or not, Xiaotuoli San is appropriate, or Qianjin Neibu San added with Muxiang (Aucklandiae Radix), Qianghuo (Notopterygii Rhizoma et Radix), Baishao (Paeoniae Radix Alba), and Wuyao (Linderae Radix) in equal parts. Add Gualougen (Trichosanthis Radix) for thirst. These herbs treat Yang syndromes. For Yin syndromes, start with Qianjin Neibu San, reduce Fangfeng (Saposhnikoviae Radix) and Jiegeng (Platycodonis Radix) by half, and double Baizhu (Atractylodis Macrocephalae Rhizoma). If ineffective, add Shufu. Decocting these together yields better results.
Additionally, those who ingest mineral pills to aid libido, when essence and Qi are depleted and drug toxicity becomes dominant, may suffer from Back Carbuncle. Minerals share affinity with the Lung. The Lung attaches to the Kidney, hence such diseases often arise.
Those fond of eating turtle meat may also develop Back Carbuncle. This is because the Yang Qi of the turtle gathers in the upper shell, which is why the upper shell can be used in medicine. Treatment is the same as above. If the toxin manifests in the Yin aspect and Fuzi/Rougui preparations cannot yet be used, presenting with external pain and internal cold, Wuxiang Tang is appropriate.
At the initial onset of toxins, generally administer Fuyuan Tongqi San mixed with wine. Or if the disease is located in the lower body, make Tongqi San into pills with wine paste, and swallow with Muxiang Tang.
To treat forming toxic nodules, use mulberries from the Soapberry tree (Zaojiao Shu), ground and mixed with black vinegar for topical application. These mulberries must be stored in advance in a smoky room for emergency use.
Patients with back sores and other toxic ulcers who have lost excessive blood and have reduced body fluids should use Huanji San. For initial onset of toxins with headache, body ache, alternating chills and fever, suspecting exterior pathogen involvement, use Xiangsu Yin added with Chuanxiong (Chuanxiong Rhizoma), Baizhi (Angelicae Dahuricae Radix), Fangfeng (Saposhnikoviae Radix), Danggui (Angelicae Sinensis Radix), Chenpi (Citri Reticulatae Pericarpium), etc.
Patients with Xiao Ke (Wasting-Thirst) often develop toxic sores. This is due to consumed body fluids, floating deficient Yang, and deficiency inside and out. This is an extreme condition. Sores developing after Xiao Ke resolves, or Xiao Ke recurring after sores heal, are generally untreatable.
Yong, Ju, and Jie are general terms for toxins, listing only the broad categories. Other symptoms and names vary. Before ulceration, if there is increased chills and high fever, delirious speech, seeing ghosts, and persistent high fever despite significant pus discharge, it seems difficult to treat. Essentially, toxin producing pus is like Taiyang disease producing sweat. If high fever persists after sweating, it is a deteriorated Taiyang disease. Washing and dressing belong to surgery. For ulcerated Back Carbuncle with excessive pus discharge and emaciation, use Xiong Gui Tang. None of the taken medicines should contain Baizhu, as it promotes pus formation. However, it is suitable for non-ulcerated cases and initial Yin syndromes.
If the patient vomits and lacks appetite, do not use Qianjin Neibu San, as it may obstruct Spleen Qi. Suspecting exterior pathogen involvement, for non-ulcerated cases, use Erchen Tang added with Chuanxiong, or Buhuanjin Zhengqi San. For ulcerated cases, use Gushen Jiahe Yin.
Another prescription for toxic patients lacking appetite adds Baifuling (Poria) and Banxia (Pinelliae Rhizoma) in equal parts to Buhuanjin Zhengqi San, with Ren Shen (Ginseng Radix) and Muxiang halved. Also called Zhengqi San.
For patients with toxic ulcers who have ulcerated and lost excessive pus/blood, resulting in dried-up fluids and frequent thirst, even if there are fever and restlessness, do not treat conventionally. Use formulas that nourish Ying and generate fluids. If high fever persists, it is difficult to treat.
Chang Yong (Intestinal Abscess) is an abscess growing in the intestine. Abdominal pain occurs. Initially, there is fever and aversion to cold, making symptoms hard to distinguish. It is recognized only after passing pus and blood. Or there is lower abdominal swelling and fullness, or dysuria, or pus discharging from the umbilicus. Swallow Taiyi Gao.
For chronic toxic sores that fail to heal and become fistulas, drink wine infused with Rendongcao (Lonicerae Japonicae Caulis) regularly.
For toxic sores with dry and hard stools, use Shengma Heqi Yin combined with Xiaodu Yin. If accompanied by severe body fever, combine Xiaodu Yin with Baidu San.
For Fire Evil forcing its way through the body, or red nodules appearing all over the body after moxibustion, with swelling, pain, and vortex-like patterns, wash with Jingfang Tang infusion.
Lu (a type of skin lesion) is called Yanghe. Take fresh Shichangpu (Acori Tatarinowii Rhizoma), grind thoroughly, and apply. Also, take about one liang of Shishoujiao (Corean Shellfish Glue), calcine until charred on the outside but raw inside, grind into powder, and take with wine.
Some cases experience severe pain without medication, which stops upon application. Others feel no pain without medication, but pain increases upon application. This is because Cold, Heat, Dampness, Wind, along with emotions like anger, accumulate and attack internally. When the drug draws the pathogen out, pain ceases. Conversely, if dead blood and Yin toxin remain inside, continued medication drives the toxin deeper, causing less pain. When the drug pushes the toxin outward, pain increases.
