29．陈幼。始下赤白痢，将及两旬，继则溏薄中夹有粘液。因营养缺乏太甚，目疏无所见，两足亦有浮肿状。此脾阳大虚之候。炮附片9克，潞党参9克，赤石脂12克，肉豆蔻9克，生艾叶5克，乌梅丸9克，炮姜炭3克，淮山药9克，云苓12克。共研细末，每取5克，稍加白糖，和入饮食中。[按]赤白痢急性期多系湿热为患，但延久则脾阳受伤，不宜投苦寒清肠之品再伤其阳。方以附子理中(去甘草)加味，峻补脾阳、收涩止泄为主；乌梅丸能调肠中寒热，善治久痢，因以佐之。

3．王小。面部透布红点，视疹为小，视痧而大，既有热为之前驱，亦是皮肤发疹病之一种。小溲短，最堪注意。
牛蒡子9克，浮萍草6克，白薇9克，银花9克，赤苓9克，通草3克，地龙9克，白茅根1扎，绿豆衣9克。
4．王幼。丹痧发于遍身，骨节酸痛异常，喉痛，此喉痧重症。舌红起刺如杨梅，是其特征。
浮萍草5克，前胡5克，板蓝根9克，紫草2．4克，山栀皮9克，蒲公英9克，薄荷6克，大力子9克，射干2．4克，丹皮6克，连翘9克，六一散9克(包)，白茅根30克(打)。另：玄明粉30克，水冲多次漱口。
二诊：喉痧重症，表之后当清之。
小蓟9克，玄参9克，麦冬9克，连翘9克，升麻2．4克，板蓝根9克，知母9克，银花9克，生山栀9克，通草3克，鳖甲24克(先煎)，藏青果5枚。另：陈莱菔英120克，煎汤代茶。外吹锡类散。
三诊：再投养阴凉血之属。
鲜生地12克，小蓟9克，白薇9克，麦冬9克，夏枯草9克，梗通1．5克，玄参9克，浮萍草5克。
四诊：喉痧寻愈，一身关节疼痛，不利转侧。
浮萍草6克，西河柳9克，稀莶草9克，桃仁泥9克，丹皮9克，薄荷6克，白芍9克，汉防己12克，海桐皮6克，晚蚕沙9克(包)。
[按]喉痧即现代医学所称之猩红热，临床以发热，咽喉炎和全身起细小密集的猩红色皮疹，以及杨梅舌为特征。治疗初起宜辛凉解表，解毒利咽；表解后宜清营、泄热、养阴。锡类散吹喉，能清热消肿。
山豆根5克，桔梗3克，菊花9克，大贝母9克，生草节3克，板蓝根6克，射干2．4克，前胡5克，炙僵蚕9克。
二诊：咽头白腐消失，乳蛾仍肿，尚有中度发热、便难，再予前法，兼通腑气。
板蓝根6克，射干3克，杭菊花6克，黄芩6克，紫花地丁9克，玄参9克，桔梗3克，玄明粉6克(分冲)，生甘草3克。
2．高幼。热两候，作起伏型，入暮有寒意，主因在喉蛾双发。
玄参9克，黄芩9克，大青叶6克，青蒿9克，山豆根5克，菊花9克，连翘9克，紫地丁12克，桑白皮9克，杜红花5克。另：玄明粉18克，溶于温开水漱口。
[按]此二案均为急性扁桃体炎。方药均从散风、解毒、清热、消肿人手。其中甘桔汤为治疗咽痛之效方；山豆根、板蓝根、射干、僵蚕等均为治疗咽痛的常用药。大凡此证一有便难见症，即需投硝、黄以泻火通便，顿挫其火炎之势。高案用玄明粉溶于温开水中漱口，取其局部消炎消肿作用。
3．陈幼。其热起于昨夜，喉蛾双发，耳下腺亦发炎。风热上乘之候。
大贝9克，薄荷6克，射干6克，大力子9克，僵蚕9克，玄参9克，苏子6克，大青叶30克，板蓝根9克，白茅根30克(打)。
5．谢幼。喉头掀红上罩白腐，身热，此不可忽。

31．徐幼。慢性痢疾日久，大便稀溏且频繁。此方温补脾脏兼有杀虫之意。

二诊：丹痧已入恢复期，周身皮肤皆脱，其舌前光红起刺，解毒利溲最为重要。
金银花9克，白薇9克，赤苓12克，粉丹皮9克，连翘12克，冬葵子9克，白茅根1扎，绿豆衣12克，紫地丁12克，麦冬12克，京玄参12克。
3．王小。面部透布红点，视疹为小，视痧而大，既有热为之前驱，亦是皮肤发疹病之一种。小便短少，最堪注意。
牛蒡子9克，浮萍草6克，白薇9克，银花9克，赤苓9克，通草3克，地龙9克，白茅根1扎，绿豆衣9克。
4．王幼。丹痧发于遍身，骨节酸痛异常，喉痛，此为喉痧重症。舌红起刺如杨梅，是其典型特征。
浮萍草5克，前胡5克，板蓝根9克，紫草2.4克，山栀皮9克，蒲公英9克，薄荷6克，大力子9克，射干2.4克，丹皮6克，连翘9克，六一散9克（包煎），白茅根30克（捣碎）。另：玄明粉30克，水冲多次漱口。
小蓟9克，玄参9克，麦冬9克，连翘9克，升麻2.4克，板蓝根9克，知母9克，银花9克，生山栀9克，通草3克，鳖甲24克（先煎），藏青果5枚。另：陈莱菔英120克，煎汤代茶。外吹锡类散。
三诊：再投养阴凉血之属。
鲜生地12克，小蓟9克，白薇9克，麦冬9克，夏枯草9克，梗通1.5克，玄参9克，浮萍草5克。
四诊：喉痧寻愈，一身关节疼痛，不利转侧。
浮萍草6克，西河柳9克，稀莶草9克，桃仁泥9克，丹皮9克，薄荷6克，白芍9克，汉防己12克，海桐皮6克，晚蚕沙9克（包煎）。
[按语]喉痧即现代医学所称之猩红热，临床以发热，咽喉炎和全身起细小密集的猩红色皮疹，以及杨梅舌为特征。治疗初起宜辛凉解表，解毒利咽；表解后宜清营、泄热、养阴。锡类散吹喉，能清热消肿。
山豆根5克，桔梗3克，菊花9克，大贝母9克，生草节3克，板蓝根6克，射干2.4克，前胡5克，炙僵蚕9克。
二诊：咽头白腐消失，乳蛾仍肿，尚有中度发热、便难，再予前法，兼通腑气。
板蓝根6克，射干3克，杭菊花6克，黄芩6克，紫花地丁9克，玄参9克，桔梗3克，玄明粉6克（分冲），生甘草3克。
2．高幼。发热两个时辰，呈起伏型，入暮有寒意，主因在喉蛾双发。
玄参9克，黄芩9克，大青叶6克，青蒿9克，山豆根5克，菊花9克，连翘9克，紫地丁12克，桑白皮9克，杜红花5克。另：玄明粉18克，溶于温开水漱口。
[按语]此两案均为急性扁桃体炎。方药均从散风、解毒、清热、消肿入手。其中甘桔汤为治疗咽痛之效方；山豆根、板蓝根、射干、僵蚕等均为治疗咽痛的常用药。大凡此证一有便难见症，即需投硝、黄以泻火通便，顿挫其火炎之势。高案用玄明粉溶于温开水中漱口，取其局部消炎消肿作用。
3．陈幼。其热起于昨夜，喉蛾双发，耳下腺亦发炎。风热上乘之候。
大贝9克，薄荷6克，射干6克，大力子9克，僵蚕9克，玄参9克，苏子6克，大青叶30克，板蓝根9克，白茅根30克（捣碎）。
5．谢幼。喉头掀红上罩白腐，身热，此不可忽。

Half a month after the eruption of scarlet fever (Dan Sha), careless diet led to dysentery, characterized by tenesmus (里急后重), defecating dozens of times a day, accompanied by exterior syndrome. Stir-fried Schizonepetae Herba (Jingjie) 5g, Allii Fistulosi Bulbus (Xie Bai) 9g, Prepared Rheomi Radix et Rhizoma (Shu Jinwen) 3g, Aurantii Fructus Immaturus (Zhi Shi) 9g, Arecae Semen Preparatum (Hainan Pian) 6g, Prepared Strychni Semen (Heiqiu) 5g, Black tea 6g, Medicated Leaven (Shenqu) 9g, Stir-fried Saposhnikoviae Radix (Fangfeng) 6g, Hordei Fructus Germinatus and Ziziphi Spinosae Semen (Guyamai) each 9g, Almond paste (Xingren Ni) 12g, Xianglian Wan 2.4g (taken in two doses).
29. Chen You. Initially suffered from bloody and purulent dysentery for nearly twenty days, subsequently developing loose stools mixed with mucus. Due to severe malnutrition, vision was blurred, and both feet were swollen. This indicates a severe deficiency of Spleen Yang. Prepared Aconiti Lateralis Radix (Paofupian) 9g, Codonopsis Radix (Ludangshen) 9g, Halloysit Rubrum (Chishizhi) 12g, Myristicae Semen (Roudoukou) 9g, Artemisiae Argyi Folium (Sheng Aiye) 5g, Wumei Wan 9g, Prepared Zingiberis Rhizoma Carbonis (Paojiangtan) 3g, Dioscoreae Rhizoma (Huai Shanyao) 9g, Poria (Yunling) 12g. Grind into fine powder, take 5g each time with a little white sugar, mixed into food. [Commentary] Acute bloody and purulent dysentery is mostly caused by Damp-Heat. However, if prolonged, it injures Spleen Yang, and bitter-cold intestinal-clearing herbs should not be used to further damage Yang. The formula is based on Fuzi Lizhong Tang (minus Licorice) with modifications, primarily to strongly tonify Spleen Yang and astringe to stop diarrhea; Wumei Wan harmonizes cold and heat in the intestines and is good for chronic dysentery, hence used as an adjuvant.
30. Li Dadi. Diarrhea persisted for months. Initially, white mucus was passed; red mucus appeared only in the last half-month. Zhang Zhongjing's Wumei Wan can treat chronic dysentery. [Commentary] Wumei Wan uses both cold and warm herbs, making it most suitable for chronic dysentery with mixed cold and heat.
31. Xu You. Chronic dysentery has lasted a long time, with loose and frequent stools. This formula warms the Spleen and also has the intention of killing parasites.

[Commentary] This is chronic dysentery. Inferring the syndrome from the herbs, it must be a pattern of Spleen and Kidney Yang Deficiency; adding parasite-killing herbs like Sophorae Flavescentis Radix and Toosendan Fructus suggests Amoebic Dysentery.
32. Wang You. Emaciated body with distended abdomen for a long time. Stool examination revealed amoeba protozoa. For the past few days, stool resembles turbid mucus, with a sense of straining.

[Commentary] Long-standing emaciation with abdominal distension implies existing Gan Accumulation (infantile malnutrition), so Bufonis Corneum (Chanpi) is used to treat Gan and resolve accumulation; since stool examination shows amoeba protozoa, it is undoubtedly Cold Dysentery. Warming the middle and astringing with sour flavors is comprehensive and appropriate.

1. Mo You. Whenever children cough severely when lying flat, it is always due to Phlegm. If there is much Phlegm accompanied by high fever, Phlegm-Heat interacts, Lung loses its descending function, and cough, wheezing, and convulsions are expected.

The formula uses Ceterachii Herba, Platycodonis Radix, Asteris Tatarici Radix, and Perillae Fructus to diffuse Lung, transform Phlegm, and calm wheezing; Forsythiae Fructus, Scutellariae Radix, Pheretima, and Paris Polyphyllae Rhizoma clear Heat and extinguish Wind to calm convulsions.
2. Jin You. Fever with cough, the fever fluctuates, and the cough is not continuous or severe. Treat as a common cold.

Prescribe herbs that drain Lung and expel Phlegm; once Phlegm is gone, breathing will normalize. Antitussives inhibit Phlegm discharge, so they are unsuitable.
4. Xue You. Cough with exterior syndrome persisting for a long time. San'ao Tang combined with Zhousou San is most appropriate. [Commentary] Chronic cough requires antitussives; exterior syndrome requires dispersing. The formula uses Ephedrae Herba and Schizonepetae Spica to disperse exterior Cold; Armeniacae Semen Amarum and Platycodonis Radix open Lung and stop cough; Citri Reticulatae Pericarpium and Glycyrrhizae Radix transform Phlegm and relieve urgency; Asteris Tatarici Radix, Stemona Radix, and Ceterachii Herba moisten and direct Qi downward. The whole formula combines San'ao Tang and Zhousou San to dispel pathogens and stop cough.
5. Guo Di. Cough sounds as if coming from a jar, typical of tracheal spasm, so Phlegm is present but hard to expectorate. Based on ancient experience, heavy use of Lung-opening methods is needed. One effect is opening Lung and expelling Phlegm; the other is relaxing spasms.

Using Ephedrae Herba with Morus Cortex follows the combination method of Huagai San. Using Paeoniae Radix Alba implies restraining within dispersing.
6. Chen You. Loss of voice in children is rarely due to Yin Deficiency, mostly due to inflammation. Loose green sticky stool. Ancients said green color indicates clearing Liver Heat, but actually it is because Scutellariae Radix and Coptidis Rhizoma stop diarrhea; do not be dogmatic.

[Commentary] Childhood loss of voice is mostly upper respiratory infection inflammation. Generally, treating exterior-induced loss of voice does not require heavy doses of wind-dispersing herbs. Clinical practice confirms this.
7. Zhu Di. Cough accompanied by sore throat, due to pharyngeal congestion. No need for anti-inflammatory drugs; dispersing is also a method to eliminate stasis.

Since sore throat is an accompanying symptom, dispersing is sufficient; if pharyngeal inflammation is severe, large doses of Heat-clearing and toxicity-resolving herbs are required.
8. Zhu You. Diffusing Lung is mostly Phlegm-transforming; Descending Lung is mostly antitussive.

[Commentary] Pertussis is a common pediatric respiratory infectious disease. Early stage is hard to distinguish from acute bronchitis; symptoms become obvious during spasmodic coughing. Treatment involves diffusing Lung and stopping cough, harmonizing Stomach and transforming Phlegm. Viticis Fructus and Abutili Semen are effective antitussives. Abutili Semen is toxic, dosage should not be large.
Luzi Xian Wan clears Heat, transforms Phlegm, stops cough, and calms wheezing. The master often used it for pertussis. Available as patent medicine. Sparrow decoction for pertussis is a folk prescription, capable of relieving spasms and stopping cough. One per day, can be simmered and eaten continuously for 5-7 days.
Dan Sha (Scarlet Rash)

Initial treatment should be acrid-cool to release exterior, resolve toxicity and soothe throat; after exterior release, clear Ying level, drain Heat, and nourish Yin. Xi Lei San blown into throat clears Heat and reduces swelling.

[Commentary] These two cases are acute tonsillitis. Formulas start from dispersing Wind, resolving toxicity, clearing Heat, and reducing swelling. Ganjie Tang is an effective formula for sore throat; Sophorae Tonkinensis Radix, Isatidis Radix, Belamcandae Rhizoma, and Bombyx Batryticatus are common herbs for sore throat. Generally, if difficult bowel movement occurs, Natrii Sulfatis Crystallis and Rhei Radix et Rhizoma should be used to purge Fire and unblock bowels, curbing the Fire flare-up. In Gao's case, Natrii Sulfatis Crystallis dissolved in warm water for rinsing takes local anti-inflammatory and swelling-reducing effects.
3. Chen You. Fever started last night, bilateral tonsillar enlargement, parotid gland also inflamed. Pattern of Wind-Heat attacking upward.
