This volume discusses cold damage (Shanghan) in women. Ancient physicians, when treating diseases, first discussed the governing principles: for men, regulate Qi; for women, regulate Blood. If the Blood Chamber (Zigong/Uterus) is not stagnant, Yin and Yang Qi harmonize; if the Blood Chamber is coagulated, Water and Fire clash. When the injury-cold presents with a tight and full Kou pulse at the wrist, purgation should be used; when the Renying pulse at the neck is tight and full, diaphoresis (sweating) should be induced. For women, if the Left Guan pulse is floating and tight, purgation is contraindicated; instead, diaphoresis should be induced to rescue the Blood Chamber. Once Ying (Nutritive) and Wei (Defensive) harmonize, body fluids flow naturally, and the disease resolves through mild sweating throughout the body. Zhang Zhongjing said: "When a woman suffers from cold damage, and her menses have just ceased, she is clear-minded during the day but delirious at night, seeing ghosts as if hallucinating. This is Heat entering the Blood Chamber." Do not injure Stomach Qi or the Upper and Middle Jiao. "Do not injure Stomach Qi" means do not use purgation. Treat with Xiao Chai Hu Tang (Minor Bupleurum Decoction). If treatment is delayed, Heat enters the Stomach, causing dryness of body fluids in the Middle Jiao, and malnourishment of the Upper Jiao leading to symptoms resembling Chest Binding due to Blood coagulation; in this case, one must puncture the Qimen acupoint. The Five Elements generate and restrain each other, supporting one another to maintain balance. In health, Water nourishes Wood; Water and Wood generate each other, thus nourishing the Blood Chamber. If the Blood Chamber is not stagnant, the Spleen has no accumulation; if the Spleen has no accumulation, rigid and dry pathogenic factors do not arise. Once rigid and dry factors arise, if Stomach Qi is injured, there will be delirium day and night, forgetfulness, fullness in the lower abdomen, and free urination; this belongs to the Dang Gui Tang syndrome. (For delirium caused by excess in the Stomach due to cold damage, purgation is appropriate; for delirium in women caused by Heat entering the Blood Chamber, purgation is contraindicated.) Although so, the treatment for cold damage in women differs from men: men regulate Qi first, women regulate Blood first. This is a general statement. In essence, a tight pulse without sweat is called Cold Damage; a slow pulse with sweat is called Wind Injury; a surging and large pulse indicates Heat Disease; a fine and weak pulse indicates Summerheat. Their manifestations are similar. For instance, if Summerheat is treated with Bai Hu Tang (White Tiger Decoction), and Stomach excess with Cheng Qi Tang (Order the Qi Decoction), why must Blood be regulated before administering medicine? Zhang Zhongjing's *Treatise on Cold Damage* does not distinguish between men and women, likely for this reason. Students can flexibly select prescriptions based on the patient's condition from those intended for men. (If a pregnant woman suffers from cold damage, the medicinal properties should be cooling; one must be cautious and avoid using Guizhi Tang (Cinnamon Twig Decoction), Banxia (Pinellia), Taoren (Peach Kernel), etc. Xiao Chai Hu Tang without Banxia is named Huang Long Tang (Yellow Dragon Decoction), removed specifically for pregnant women. Generally, before delivery, stabilize the fetus first; after delivery, tonify and benefit first, then treat the cold damage. If the illness subsides slightly, stop the medication; do not complete the full course. This is the major principle. The Yellow Emperor asked about treating pregnant women with toxic drugs: how should it be done? Qibo replied: "If there is a pathogen, there will be no destruction [of the mother/fetus], nor will there be destruction." For large accumulations and聚 (masses), they can be attacked, but stop when the pathogen is reduced by more than half; over-attack leads to death.)
[Prescriptions for Women's Cold Damage]
The treatment methods for cold damage in men and women by Zhang Zhongjing are consistent. Recently, folk books on women's cold damage have appeared, claiming to be written by Zhang Zhongjing with a preface by Wang Shuhe. Verification shows some merit, but they are suspected not to be ancient formulas, merely borrowing the sage's name to gain credibility. Now, I select treatments for women's cold damage from *Jin Gui Yu Han Jing* (Golden Cabinet Jade Locket Classic) and acceptable folk prescriptions, compiling them into this volume. Though not exhaustive, they can be referenced alongside the *Bai Wen* (Hundred Questions).
When a woman suffers from cold damage with fever, and her menses are currently flowing, she is clear-minded during the day but delirious at night, seeing ghosts. This is Heat entering the Blood Chamber. Do not injure Stomach Qi or the Upper and Middle Jiao; use Xiao Chai Hu Tang (see Vol 10).
When a woman suffers from Wind Strike for seven to eight days, continuing to have alternating chills and fever at specific times, and her menses have just ceased, this is Heat entering the Blood Chamber, causing Blood coagulation. Thus, symptoms resemble malaria; use Xiao Chai Hu Tang (see Vol 10).
When a woman suffers from cold damage with fever and chills, menses are flowing. After seven to eight days, the heat subsides, pulse becomes slow, body temperature normalizes, but there is fullness under the chest and hypochondrium resembling Chest Binding, accompanied by delirium. This is Heat entering the Blood Chamber. Puncture the Qimen acupoint and drain according to the excess pattern (Qimen acupoint see Vol 2).
When a woman suffers from cold damage for six or seven days, with dry feces in the Stomach, difficulty defecating, irritability, delirium, red eyes, and blocked toxic qi preventing circulation, use Xie Xin San Huang Tang (Drain the Epigastrium Three Yellows Decoction).
Shu Da Huang (Rheum officinale root), Shu Wei Huang Qin (Scutellaria baicalensis root), Ji Zhua Huang Lian (Coptis chinensis rhizome) (equal parts).
Grind into coarse powder. Take 4 qian per dose, decoct in one sheng of water until 0.8 sheng remains, strain, and drink. Aim for mild laxation. If eyes are red and painful, add Bai Fu Ling (Poria cocos) and tender Zhu Ye (Bamboo leaf) to drain residual Liver Qi.
When a woman suffers from cold damage with fever, chills, limb spasms, dry mouth and tongue, stagnant meridians, and impaired circulation, use Guizhi Honghua Tang (Cinnamon Twig and Safflower Decoction).

Treating postpartum blood loss and excessive sweating leading to Yu Mao (Depression and Vertigo). The pulse is faint and weak, unable to eat, stool is hard, but only the head sweats. This is because blood deficiency leads to Jue (reversal/collapse), which causes Mao (vertigo/darkening). Resolution requires profuse sweating. Due to blood deficiency and lower collapse, solitary Yang rises outward, causing only head sweating. Postpartum women welcome sweating because it expels Yin; blood deficiency makes Yang relatively excessive, so sweating restores Yin-Yang balance. The cause of vomiting and inability to eat is related to this. Treat with Xiao Chai Hu Tang (Minor Bupleurum Decoction).
(Ganjiang Chaihu Tang) Treats women with cold damage, menses just starting/stopping, chills and fever like malaria, delirious speech, and hallucinations.

Cut into hemp-seed sized pieces. Take 5 qian per dose, decoct in 1.5 sheng of water until 0.7 sheng remains, strain, and take warm. Initial dose may cause slight irritability; second dose induces sweating and cure.
