Xu Zhongguang treated a child suffering from blood-heat type variola (smallpox). Initially, cold and cool-natured herbs (such as Huangqin/Coptis Rhizome, Lianlian/Coptis Chinensis Rhizome, Xijiao/Rhinoceros Horn) were used to promote the eruption and suppuration of the pustules. Consequently, pus formed on the head, face, trunk, and back, but the surfaces of both legs remained sunken and did not rise.
After using Ginseng-based formulas to support the expulsion of toxins, the child instead developed heat sensation with thirst, restlessness, frequent loose stools, and sore throat with swelling. The treatment was then changed to clearing the pharynx and benefiting the diaphragm, using Shandougen/Sophora Flavescens Root, Maidong/Ophiopogon Tuber, Chuanbeimu/Fritillaria Cirrhosa Bulb, Jiegeng/Platycodon Root, Xuanshen/Scrophularia Root, Fangfeng/Saposhnikovia Root, Jiangcan/Bombyx Batryticatus, Shanji/Pangolin Scales, Danggui/Angelica Sinensis Radix, Baishao/Paeonia Lactiflora Radix, Honghua/Safflower, Shengdihuang/Rehmannia Glutinosa Root, Shanzha/Crataegus Fruit, Chenpi/Tangerine Peel, Dilong/Earthworm, Niubangzi/Arctium Fruit, Chuanlian/Coptis Rhizome, Huangqin/Scutellaria Root, and Gancao/Licorice Root, resulting in recovery.
Another child suffered from sore throat and obstruction after variola, caused by residual toxins lingering in the Heart and Lung, unexpelled pathogenic heat, and abundant wind-phlegm. Tongyou (Tung Oil) was first used for emesis induction, followed by Tuoyuan San, Ganjie Tang, Lianqiao/Forsythia Fruit, Niubangzi, Yeyan/Belamcanda Rhizome, and Xuanshen, leading to recovery.
A third child, post-variola, had insufficient True Yin with ministerial fire ascending, causing sore throat and swelling. Treatment with cold herbs initially reduced swelling but led to recurrence upon cessation. Upon analysis, the fire was determined to be constrained by cold. Fuzi Lizhong Tang was first administered to dispel cold, followed by Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) supplemented with Zhimu/Anemarrhena Rhizome, Maidong, and Xuanshen to tonify the root (strengthening Water to control Fire), along with external application of Gejin San to the throat, resulting in recovery.
A fourth child, post-variola, presented with spleen and stomach deficiency and yin fire flaming upward, causing sore throat. Si Jun Zi Tang (Four Gentlemen Decoction) supplemented with Gancao, Jiegeng, Zhimu, Huangbai/Phellodendron Bark, Chuanlian, and Niubangzi resulted in recovery.
A fifth child developed a carbuncle in the throat post-variola. Chaihu/Bupleurum Root, Gegen/Pueraria Root, Dilong, and Wugong/Scolopendra were decocted in equal parts, mixed with ground Xijiao/Rhinoceros Horn juice, and ingested, leading to recovery.
Wan Mizhai treated Ma's son, aged five, whose variola failed to erupt. He stated: This is a favorable course of variola. When toxins are severe, the head and face swell; when toxins are mild, they do not swell, which does not indicate failure to erupt. The child also complained of sore throat. He remarked: This is a common condition in variola cases. As long as there are no sores in the throat and tongue, and the rash is sparse around the neck, there is no cause for alarm. Ganjie Tang supplemented with Niubangzi was decocted and swallowed slowly, stopping the sore throat immediately.
Wan Mizhai's daughter-in-law, Li, aged eighteen, experienced sudden onset of loss of voice (aphonia) less than a day after her menstruation began during the suppuration stage of variola, accompanied by heavy bleeding and clots. She could only shake her head and shed tears. It is said that when variola turns black and relates to the Kidney, sudden aphonia should occur. However, her pustules were full, plump, and ruddy-red, making this an adverse sign. After deep reflection, he recalled the Classic stating: "Women pregnant for nine months may lose their voice because the Shaoyin meridian fails to nourish the tongue." The Hand-Shaoyin meridian belongs to the Heart. The Heart governs Blood, and all sores and ulcers belong to the Heart. The fire of the toxin originated internally in the Heart, forcing blood downward, hence the menstrual flow. The tongue is the sprout of the Heart; loss of blood leads to Heart deficiency, preventing nourishment from reaching the tongue, causing tongue atrophy and sudden aphonia. Shengmai San (Generate the Pulse Powder) without Wuweizi/Schisandra Fruit, supplemented with Danggui and Shengdihuang, was administered, resulting in rapid recovery. Subsequently, Shiquan Dabu Tang (Complete Great Tonifying Decoction) was used for regulation and convalescence.
Xiao's son, aged three, developed variola. Just as the lesions were about to crust over, he suddenly lost his voice. Although he could cry out, his speech was hoarse and unclear. He diagnosed this as Lung heat and prescribed Ganjie Qingjin San (Ganjie Clear Metal Powder), which brought relief.
