Xu Zhongguang treated a child with dense smallpox. Before the lesions erupted, the lips and mouth appeared yellow and mature first, and the eyes showed signs of empty closure. The throat was swollen and painful, making eating impossible. The body-wide smallpox lesions were flat and dark. Fortunately, the forehead had an even and bright complexion. Although the eye sockets and lips were swollen, they remained red and vibrant. He prescribed Ganjie Tang (Licorice and Platycodon Decoction) added with Niu Bang Zi (Arctii Fructus), Lian Qiao (Forsythiae Fructus), Shan Zha (Crataegi Fructus), Jing Jie (Schizonepetae Herba), Chen Pi (Citri Reticulatae Pericarpium), and Ren Zhong Huang (Homo Feces Purificatus). This formula cleared and benefited the throat and diaphragm, resolving spleen toxicity, leading to recovery.
Another child presented on the fourth day of smallpox with dense lesions that had not yet swelled. The lips and mouth turned yellow and mature first, indicating a malignant sign of internal ulceration, which would inevitably lead to "lip thorns" (a severe oral lesion). Fortunately, the remaining lesions were lustrous and receding at the base. Although the spleen was affected by toxicity, the root qi remained solid. He prescribed Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) without Bai Zhu (Atractylodis Rhizoma), added with Niu Bang Zi, Lian Qiao, and Shan Zha. Subsequently, Bu Yuan Tang (Tonify the Origin Decoction) and Lizhong Tang (Regulate the Middle Decoction) were administered, resulting in recovery.
In another case, the smallpox was dense like attached millet grains, flat and tenderly red, with blood qi scattered and unattached to the lesions. The lips and mouth were swollen and hard, with skin peeling off layer by layer. The skin was thin with empty blisters, breaking open to exude fluid resembling blood juice. The patient suffered from loose stools, restlessness, and thirst, all indicative of adverse prognoses. However, the ability to eat and remain calm indicated the presence of stomach qi. He used Ren Shen (Ginseng Radix et Rhizoma), Bai Zhu (Atractylodis Rhizoma), and Huang Lian (Coptidis Rhizoma), each 2 qian, taken three times daily. On the fourteenth day, the limbs became cold, and the pulse was still and deep as if dead. The smallpox lesions inverted and crusted over, but warmth remained in the chest and heart region. After several days, the patient gradually revived and recovered.
Xu Zhongguang stated: A child suffered from blood heat. After the smallpox subsided, the body remained feverish, with bad breath leading to gan syndrome (mouth ulcers/gangrene). The upper gums, front teeth, and left cheek molar teeth ulcerated and fell out. The lips were red, dry, and cracked, and the lower left cheek was also swollen and red, resembling a toxic carbuncle. Shen Sanchun from Huzhou applied Yi Yang San (Suppress Yang Powder) mixed with scallion juice and wine paste externally to the swollen area. If facial swelling presents with a purple hue, it indicates excess heat, inevitably developing into zou ma gan (gangrenous stomatitis progressing rapidly like a running horse), leading to cheek ulceration which is untreatable. Since this case showed swelling but with red and vibrant color, it was identified as a separate carbuncle. Internally, he prescribed Xi Jiao (Rhinocerotis Cornu - *Note: now substituted with water buffalo horn in modern practice*), Yu Nao Gu (Fish Brain Bone), Ling Yang Jiao (Capricornis Saigae Cornu), Huang Lian, Huang Qin (Scutellariae Radix), Xuan Shen (Scrophulariae Radix), Sheng Di Huang (Rehmanniae Radix), Niu Bang Zi, Jie Geng (Platycodonis Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma), Bai Shao (Paeoniae Radix Alba), Hua Fen (Trichosanthis Radix), Mu Tong (Akebiae Caulis), Zi Hua Di Ding (Violae Herba), and Ren Zhong Huang San. One or two doses were given. Alternatively, Yin Hua (Lonicerae Japonicae Flos) 1 liang was decocted with Guang Jiao (Colla Corii Asini) 1 qian. Occasionally, juice from Gan Ju Hua Gen Ye (Radix et Folium Chrysanthemi Indici) was crushed and mixed in. The facial swelling gradually subsided, and body heat receded. Externally, a powder was blown into the mouth containing Niu Huang (Bovis Calculus) 1 fen, Zhen Zhu (Margarita) 3 fen, Huang Bo (Phellodendri Cortex), Qing Dai (Indigo Naturalis), Ren Zhong Bai (Homo Alumen), and Peng Sha (Borax prepared with pig bile) each 4 fen. If healing did not occur easily, Wu Mei Tan (Mume Fructus Carbonized) 3 fen, Xue Jie (Santalum Rubrum), and Long Gu (Os Draconis) 1 fen were added. Due to the patient's deficiency, Ren Shen and Xiang Pi Hui (Elephant Skin Ash) 3 fen each, and Zhi Lu Gan Shi (Calamina Preparata) 4 fen were added. Additionally, Xiong Huang (Realgar) powder mixed with vinegar was applied with a soft brush into the ulcerated cavities. Recovery took over a month.
