黄丸加青铅五钱,俾清浊定位,斯不治痛而痛自止矣。所以然者,以阳气居上,体本虚也,而浊气干之则实;

Xu Lingtai said: Tou Feng (Head Wind) is often characterized by heat in the root and cold on the surface. Medicines that clear fire can indeed cure mild cases of wind-fire. However, if cold pathogens invade the brain or wind-cold comes from outside, warming and dispersing methods must not be overlooked, and methods to lift yang and expel pathogens should be extensively studied.
For all diseases of the physical body, external treatments should be combined. If one does not understand external treatment methods, even if internal medicine hits the disease, it only represents half of medical skill.
He also said: Mild Tou Feng is easy to cure. Severe cases involve wind-toxin attacking upwards and collaterals' blood flowing recklessly. Severe cases lead to syncope or dizziness; prolonged cases damage the eyes. Only heavy-dose medicines combined with various external treatments can be effective.
Someone asked Peng Zu why three generations of his family had short lifespans. Peng Zu observed their dwelling and found a hole directly facing Naohu (GV17). He instructed them to block it, and they subsequently lived long lives. This is likely because wind entering through gaps into the ears blows directly into the brain, causing the dispersion of Yang Qi. The head is the gathering place of all Yang meridians and governs life. (From *Yanshu*.)
Dou Cai treated a patient who lived normally but suffered occasional headaches. This was caused by retained food in the stomach. Ten pills of Dingxiang Wan (Clove Pill) cured him. (Yangming Food Stagnation Headache.)
Zhang Zihe treated Chen Jun of Nanqing, who was about to take the autumn imperial examination. He suffered from headache with unilateral swelling extending to one eye, resembling half a pot, and his pulse was flooding and large. Zhang consulted the *Neijing*, which states that facial swelling is due to wind; this is wind invading the Yangming Meridian. Since Yangming has abundant Qi and Blood, wind-induced swelling is suitable for sweating therapy. He prescribed Tongsheng San, decocted with ginger, scallion roots, and fermented soybeans in a large bowl. After taking it, the patient sweated slightly. The next day, a grass stem was inserted into the nose, causing massive bleeding, which immediately resolved the swelling. (Yangming Wind-Heat Headache.)
Wang Dingguo suffered from wind headache and sought the famous doctor Yang Jie Lao in Duliang. After three pills, the illness vanished instantly. Requesting the prescription, he found it consisted solely of Xiang Baizhi (Angelica Dahurica), washed, dried, powdered, and made into pill-sized balls with refined honey. One pill was chewed each time and taken down with tea water or Jingjie Decoction. It was named Duliang Wan. This medicine is effective for Tou Feng with vertigo, postpartum headache in women, and blood-wind headache. (*Baiyi Xuanfang*. Note: This formula is only suitable for Yangming Wind-Heat; it should not be used for other patterns.)
Zhang Dafu said: The suffering of Biandou Feng (Hemicrania) is indescribable by patients, and prescriptions vary with little efficacy. In the Wushen year, I suddenly contracted this illness. While feeling depressed, Zhou Shuming sent me a cake-based method, which I did not use initially. On the fifth day of the fifth lunar month, Gu Minfu gave me two cakes. Applied to Taiyang (EX-HN5), the condition improved overnight. The method uses Nanxing (Arisaema), Banxia (Pinellia), and Baizhi (Angelica Dahurica) ground equally, mixed with mashed ginger and scallion bulbs to form cakes. It is applied externally without ingestion, purgation, or fumigation, making it simpler than other formulas. (*Bitan*. Note: Suitable for Wind-Phlegm.)
Yao Yingfeng treated Shi Shengyu from Yanzhou, who suffered unbearable headaches for three years. Yao said: According to the method, bone from the center of the head should be removed. However, it is August, a time of contraction, making sudden treatment difficult. We scheduled it for the following spring. The forehead was incised, the bone removed, and several liters of stasis blood were drained, resulting in immediate recovery. (*Qiantang County Annals*. Note: This case resembles "stone crabs" in the brain. Comment: It seems somewhat absurd.)
Gong Zicai treated Du Shiyu, who suffered from knife-like splitting headaches, unable to move, fearing wind and speech, with tinnitus and sensations of fire in the eyes. His six pulses were tight, rapid, and forceful. He was given Dahuang (Rhubarb) soaked in wine, nine-steamed and nine-sun-dried, powdered. Three qian taken with tea water cured him in one dose. (This is also Yangming Blood-Heat pathology. Because the disease is at the highest point, Dahuang must be processed this way.)
Sun Wen Yuan treated the wife of Cai Lechuan, who suffered from knife-splitting headaches. Slight movement of the hair roots caused pain radiating over the whole head, leading to fainting and loss of consciousness, recovering after half an hour. Her whole body ached, chest and diaphragm felt full and stifled, and fluids stopped in the diaphragm. The day before, she vomited several times due to anger, expelling three roundworms. She now alternated between aversion to wind and heat, thirst or no thirst, constipation, and her six pulses were slippery, large, and forceful. This was Phlegm-Jue Headache. First, Huoxiang Zhengqi San was used to stop vomiting, followed by Niuhuang Heihu Dan to clear the mind, but the headache persisted. Then, Tianma (Gastrodia), Gaoben (Ligusticum), Banxia (Pinellia), Chenpi (Tangerine Peel), Baizhi (Angelica), Bohe (Mint), Mahuang (Ephedra), Shengjiang (Ginger), and Congbai (Scallion White) were decocted and taken. Slight sweating reduced the headache. Taking it again at night stopped most of the pain, but consciousness remained unclear. This indicated excessive phlegm in the Middle Jiao, requiring purgation. Banxia (5 qian) and Bashuang (Croton seed extract, 1 fen) were made into pills with flour paste. Thirty pills taken with ginger soup resulted in three bowel movements of thick, sticky phlegm accumulation. Diet was slightly restored, other symptoms improved, but body aches remained. Erchen Tang was modified with Qianhu (Peucedanum), Shigao (Gypsum), Gaoben, Bohe, Zhike (Aurantium), Huangqin (Scutellaria), and Shichangpu (Acorus) for regulation and recovery. (Wood excess, Earth deficiency.)
Zhu Danxi treated a person who developed fever, headache, and a tight pulse after bathing in cold water. This was Cold-Dampness. Warm medicines to induce sweating were appropriate: Cangzhu (Atractylodes), Mahuang (Ephedra), Gange (Pueraria), Gancao (Licorice), Chenpi (Tangerine Peel), and Chuanxiong (Ligusticum). After two doses induced sweating, knowing the pathogen was retreating, tonifying medicines were given: Chenpi, Chuanxiong, Gange, Baizhu (Atractylodes macrocephala), Cangzhu, Renshen (Ginseng), Mutong (Akebia), and Gancao. Four doses decocted in ginger water cured him. (Damp-Heat.)
A woman with headache, fever, and thirst was treated with Baizhu, Chenpi, Chuanxiong, Gange, Mutong, and Gancao, decocted in water and taken warm. (Yangming Disease.)
Lou Quanshan treated an elderly woman with chronic headaches lasting years. Observing her hands and feet, he saw purple-black blood vessels. He pricked all these vessels with a three-edged needle, draining several cups of dark, ink-like blood. Later, treating the affected meridians with acupuncture and moxibustion led to complete recovery. This aligns with the classic text: "Great Bi becomes malignant," and "Chronic headache and Bi stagnation not leaving the body: observe blood vessels and drain all blood." (Three-Yang Wind-Heat.)
Li Shizhen treated a person with Qi-Stagnation Hemicrania. Castor seeds, Frankincense, and salt were pounded together and applied topically. Pain ceased overnight. (Excellent symptomatic treatment.)
Li Shicai treated Jiang Shozai, who suffered from splitting headaches, heaviness, and restlessness. Wind and blood-invigorating medicines, as well as phlegm and stasis-resolving medicines, failed after long-term use. Pulse: Chi (Kidney) position weak, Cun (Lung) position slippery. This was Kidney Deficiency causing Water Overflow transforming into Phlegm. Dihuang (Rehmannia) 4 qian, Shanyao (Dioscorea), Danpi (Moutan), Zexie (Alisma) 1 qian each, Fuling (Poria) 3 qian, Chenxiang (Aquilaria) 8 fen. Taken four times daily. Reduced by 60-70% in two days. Followed by Qiwei Wan (Six-Ingredient Pill) and Renshen Tang (Ginseng Decoction). Pain disappeared in five days. (Many recent cases involve Upper Excess; this illustrates Shaoyin diseases.)
Wu Fuxian treated a person with intolerable headache, short and choppy pulse. Wu said: The head is the summit of all Yang. If invaded by external pathogens, the pulse should be floating, tight, and wiry. Now it is short and choppy. Short indicates Yang collapse above; choppy indicates Yin decline below. With cold limbs, this is called True Headache, identical to True Heart Pain, considered incurable. However, aggressive Renshen (Ginseng) and Fuzi (Aconite) were administered to save the situation. Following this method, he recovered.
Li Chengzhang, Commander of Lu'an Guard, was skilled in acupuncture. A patient suffered unbearable headache, deaf to thunder. Li diagnosed: "Insects devouring the brain." Powdered insecticides were insufflated into the nose. Within two to three days, insects exited via eyes, ears, mouth, and nose, curing the patient. (*History of Ming*. Note: Though rare, this disease exists. People in Wu region suffer from it, calling it "Sky White Ants," caused by Wind-Heat.)
A person with chronic jaundice suddenly suffered persistent headache. Dispersing and cooling methods failed. Pulse was large and slow, with generalized body pain and nasal congestion. This was Dampness-Home Headache. Genti San (Thistle Powder) insufflated into the nose expelled a cup of yellow fluid, curing the patient.
Zhang Sanxi frequently encountered refractory headaches. Inquiry revealed a history of Yameichuang (Syphilis/Kaposi's varicella). Tufuling (Smilax), Baixianpi (Dictamnus), Kushen (Sophora), Jinyinhua (Honeysuckle), Huangbai (Phellodendron), Saojiaozizi (Gleditsia Seed), Yiren (Coix Seed), Mutong, Fangfeng (Saposhnikovia) were used. Ginseng and Astragalus added for Qi deficiency; Siwu Tang added for Blood deficiency. Significant results achieved, including relief of body pain. (*Zhifa Hui*. Damp-Toxin.)
A person with headache had a slippery and rapid pulse, indicating Phlegm-Fire attacking upward. Erchen Tang, Jingjie, Qianghuo, and Wine-fried Huangqin were ineffective. Adding Shigao (Gypsum) for two doses helped slightly but didn't resolve it. Adding Shu Dahuang (Prepared Rhubarb) 3 qian to the previous formula, taken after meals, cured the patient completely. (Same source. Yangming Phlegm-Fire.)
A person suffered headache, treated as external contraction. Pulse: Right Cun was four times larger than the left; both Chi positions were flooding and full. This was Internal Injury Qi-Blood Headache. Accompanied by spontaneous sweating and fatigue, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with fried Huangbai was used. Improved in one dose, cured in two. (Qi Deficiency.)
A person with headache had a blue-black complexion and emaciation. Left Cun and Guan pulses were imperceptible; both Chi pulses were flooding and full. Treated as Yin Deficiency using Ziyin Siwu Tang (Nourish Yin Four Substances Decoction) plus Huangbai, Zhimu (Anemarrhena), and Yuanshen (Scrophularia). Halved in two doses, cured in ten days. (Blood Deficiency.)
A woman suffered headache, misdiagnosed as external contraction. Dispersing and digesting herbs worsened it. Pulse: Qikou (Radial artery) urgent, large, and rapid; soft upon pressure; Right pulse empty and large. Inquiry revealed no initial fever, only after medication. Wind-Cold should cause fever within one to two days. This is Qi Deficiency Headache. Shortness of breath confirmed this. Over-dispersion depleted Qi; purgation depleted Blood. Both Qi and Blood injured, leading to isolated deficient fire and apparent heat. Massive tonification was needed. Buzhong Yiqi Tang in large dose with one slice of Shu Fuzi (Prepared Aconite) as a guide was used. (Note: This medicine has flaws.) Patient slept deeply, woke with pain gone, but consciousness unclear, with muscle twitching and tremors. Blamed tonics for extreme deficiency. Another doctor used Chaihu (Bupleurum) dispersants, causing fire to wander externally, resulting in macules. Believed to be Shanghan (Cold Damage). Huaban Chengqi Tang (Resolve Macules and Descend Urgency Decoction) administered daily led to death. (Same source.)
A person with headache, stiff body, aversion to cold, constipation, and nausea was treated for food stagnation without effect. Pulse: Qikou peaceful, Chi rapid and thin. Difficulty walking indicated Hongjiujiao (Gout/Podagra) impending. Treated as Hongjiujiao and cured. (Same source.)
A person with headache treated as external contraction failed. Left pulse peaceful, Qikou strong. Symptoms included fullness, nausea. This was Food Stagnation. Resolved with digestion-promoting methods. (Same source, Yangming Disease.)
A person had alternating tooth and temple pain due to excess in Shaoyang and Yangming meridians. Qingwei San (Clear the Stomach Powder) combined with Xiaochaihu Tang (Minor Bupleurum Decoction) minus Banxia and Renshen, plus Bohe and Shigao, cured in two doses. (Same source.)
An old woman had headache extending to the forehead. Dispersing and cooling failed. Small red blisters with fluid appeared on the face, not ulcerating. After a month, pain shifted to the right, left eyelid swollen and red, lethargic, poor appetite. Acrid-cool herbs worsened it. Six pulses were soggy and weak like silk threads, slightly wiry on light pressure. Treated as Qi Deficiency: Liu Jun Zi Tang (Six Gentlemen Decoction) with doubled Huangqi (Astragalus) and Manjingzi (Vitex). Gradually improved. Palpitations and insomnia worsened. Tiaozhong Yiqi Tang (Regulate Middle and Augment Qi Decoction) plus Fushen (Poria with wood), Yuanshen, Suanzaoren (Ziziphus), and Baiziren (Biota) cured it rapidly. (Same source.)
Dong Yuan often suffered headache, with green-yellow cheeks, vertigo, unwillingness to open eyes, reluctance to speak, heavy body, and nausea. Jiegu said: This is Jueyin-Taiyin combined disease, named Wind-Phlegm. Juzhu Wan (Jade Pot Pill) from *Ju Fang* was used, and Moxibustion at Xiaxi (GB44) cured it. Thus, formulas are the substance, methods are the application. Clinging to substance without understanding application is flawed. Balancing substance and application makes one a superior physician. (*Yishuo Xubian*.)
Yan Situi, Military Adjutant of Hunan, treated Tou Feng pulling pain. Two jin of wax and half a jin of salt were melted together in a tin pot, molded into a helmet shape fitting the head, placed on the head until the forehead. Headache stopped immediately. (*Experience Formula*. Same source.)
Wang Shuquan said: In my later years, reading at night in the cold, I felt cold in the brain. Excessive drinking caused severe brain pain. Moxibustion at Xinhui (GV22) cured it. A soldier with persistent epistaxis was taught to moxa this acupoint and cured. Someone with chronic Tou Feng was also cured by moxa at this point. *Tongren Mingtang Jing* only mentions treating nasal congestion and loss of smell. I record this to supplement its therapeutic gaps. Given Naohu (GV17) should not be needled, Xinhui should also avoid needling. *Needle Classic* says children under eight should not be needled here, but this may be incomplete.
Shuquan's mother accompanied him to his post, blown by river winds, causing body and head shaking as if on a boat for half a year. Profuse phlegm vomiting occurred. Various phlegm medicines and moxa at Tou Feng points finally cured her.
A scholar suffered brain heat-pain, severe enough to press his head against the floor. Cold water provided slight relief but pain persisted. Medicines failed. Moxa at Xinhui cured him. Heat-pain can be moxa-ed, let alone cold-pain. For brain pain and spleen diarrhea, Xinhui should be moxa-ed first, followed by Jiangjian (GV19), etc. (All from *Zisheng Jing*.)
Xue Lizhai treated a scholar with excessive alcohol and sex, causing pain in the head and flanks. Diagnosed as Stomach Deficiency with Liver Pathology. Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) plus Chaihu and Danggui (Angelica Sinensis) cured him in one dose.
Shang Yibu suffered headache when fatigued, diagnosed as Yang Deficiency failing to ascend. Buzhong Yiqi Tang plus Manjingzi cured him.
Weng Kentang treated a person traveling between Yanjing in winter, contracting wind-cold, causing headache persisting for months. All Tou Feng medicines failed, pain worsened, body emaciated. Thinking: Clearly external pathogen, why unresolved? Saying: "Treat wind by treating blood; when blood moves, wind extinguishes." Root cause was Blood Deficiency allowing wind-cold invasion, exacerbated by excessive dispersal. How could it heal? Also: "Pain implies blockage; unblocked means no pain." Used Danggui (Angelica Sinensis) to induce sweating and invigorate blood, Mutong to open orifices and vessels. Since the patient drank wine, one dou of wine was used to soak the two herbs for three days, then boiled in double boiler. Drunk hot, then slept without pillow. However, for pain due to Fire Stagnation above, wine combined with Gypsum is suitable. Another formula: Xiong (Chuanxiong), Gui (Danggui), Shudi (Rehmannia), Lianqiao (Forsythia) 2 qian each. Bohe (Mint) 2 qian in a bowl, poured with boiling water. Inhale vapor, drink when warm. Cured instantly. But this is set for Blood Deficiency cases.
Ma Yuanyi treated a person with headache persisting for a year. Clear-headed in morning, confused and restless from noon to dusk. Treated as wind without effect. Pulses: Both deep and slippery. Taiyin-Yangming Phlegm-Jue Headache. Mengshi Guntan Wan (Flake Stone Rolling Phlegm Pill) used, alternating with Daotan Tang (Guide Phlegm Decoction) to purge. Then Liujun Zi Tang (Six Gentlemen Decoction) with minor amounts of Qinjiao (Magnolia Flower) and Quanxie (Scorpion) for regulation. Recovery.
Zhang Shuzi's sister suffered monthly headaches. Pulse: Yang large, Yin choppy. Diagnosis: Yin decline below, Yang hyperactivity above, Upper Excess Lower Deficiency pattern. Liuwei Dihuang Wan plus 5 qian of Qingqian (Lead) was prescribed to settle clear and turbid. Pain ceased without direct analgesia. Reason: Yang resides above, originally deficient, but turbid Qi makes it seem excess. Yin resides below, originally solid, but rebellious Qi makes it seem deficient. Head is the seat of Clear Yang, invaded by Turbid Yin. Chaos of Yin-Yang leads to disease. Physicians ignoring pulse treat all headaches as Wind-Fire, using penetrating/resolving agents, worsening deficiency.
Zhu suffered monthly headache, unbearable. All used wind-scattering and fire-clearing agents. Pulse: Floating, empty, non-forceful. Lazy speech, aversion to cold. Fatigue injuring Middle, Clear Yang failing to ascend, Turbid Yin failing to descend. Sweating damaged exterior; clearing damaged middle. Aversion to cold was Qi Deficiency failing to nourish exterior. Floating-empty pulse and fatigue confirm weak Middle Qi. Buzhong Yiqi Tang was given. (Note: This decoction raises Clear, but how does it descend Turbid? Adding Manjingzi aids reaching the summit.) Improved in one dose, cured in two.
A person with Tou Feng feared cold, wrapped head in heavy cotton for thirty years. Buckwheat flour 2 sheng mixed with water formed two cakes, alternately applied to head. Slight sweating cured it. (Li Lou *Guai Zheng Qi Fang*. Wang Daicun notes: Ancestor Xi Bo used this successfully on clan elder.)
Miu Zhongchun treated a young woman from Liangxi with headache and vomiting, unable to swallow rice water. Caused by Blood Heat/Blood Deficiency with Fire flaming upward. Maidong (Ophiopogon) 5 qian, Juhong (Tangerine Pith), Mugua (Chaenomeles), Fuling 2 qian each, Baishao (White Peony) 3 qian, Pipaye (Loquat Leaf) 3 large leaves, Suzi (Perilla Seed) 1.5 qian, Ganju (Chrysanthemum) 1.5 qian, Wumei (Mume Fruit) 2 pieces, Zhuli (Bamboo Sap) 1 cup, Lugenzhi (Reed Rhizome Juice) half bowl. Vomiting stopped in two doses. Headache remained. Added Tianmaidong and Maidong 2 qian. Headache stopped. Added Tufuling 2 liang, Black Soybeans a pinch. Fully cured. (*Guang Biji*.)
Shen Guanyi Zhongcheng transmitted a divine formula for Tou Feng from a Daoist priest. My servant girl suffered this, pain so severe she attempted suicide. Two doses prevented recurrence for years. Tufuling 4 liang (Avoid Iron), Jinyinhua 3 qian, Manjingzi, Fangfeng, Tianma 1 qian each, Yuanshen 8 fen, Xinyi (Magnolia Bud), Chuanxiong 5 fen each, Black Soybeans 49 grains, Dengxin (Juncus) 20 stems, Yacha (Sprouted Tea) 5 qian. River water and well water 1.5 bowls each, decocted to 1 bowl. Drink. (*Guang Biji*.)
*Guang Biji* treats Half-Sided Tou Feng of Fire type: Dahuang powder 3 fen, Huangqin powder 1 qian. Mixed in raw white wine 1 bowl, heated and stirred evenly. Drink once, cured.
Lizhai treated a woman with left brain swelling/pain, pus from left nostril, persisting over a year, occasional vertigo as if on a boat. As Xu Shimiao said: Liver Deficiency with Wind-Evil invasion. Chuanxiong 1 liang, Danggui 3 qian, Qianghuo, Juanhua (Inula), Xixin (Asarum), Manjingzi, Fangfeng, Shigao, Gaoben, Jingjiesui (Schizonepeta), Banxiaqu (Pinellia Fermented), Shudihuang, Gancao 5 qian each. Prepared as one batch, 1 liang per dose, decocted in ginger water. Cured.
Liu Yunmi said: A woman caught cold in late winter. By mid-spring, vertex and left rear head hurt. Original disease in Foot-Taiyang Cold-Water. Cold stagnated, transformed into Heat ascending. Involved Hand-Taiyang, blocked by Wind rising transformation, thus affecting Left Jueyin. Classics say: "Excess in Juyang and Jueyin." Physician said: Hand-Taiyang Heat exceeds Wind; Foot-Jueyin Heat exceeds Dampness; Spleen/Lung also have stagnant Heat. I only treated Hand-Taiyang slightly involving Lung, as Hand-Taiyang Qi transformation resides in Lung, which governs Qi. Heart had slight Heat, treated Foot-Jueyin. Once Wind rising transformation reached, Hand-Taiyang Qi flow smoothed. Slightly purged Small Intestine to unblock vessels and harmonize Qi, removing Heart Heat. Thus, no need to treat other meridians much. Note: This is one method for vertex headache; seeing cold patients, warming treatment was incomplete. Wine-processed Huangqin 2.5 fen, Wine-dried Huangqin 1.5 fen, Manjingzi 2.5 fen, Fangfeng 1.5 fen, Yulian (Coptis) 2.5 fen, Chaihu 3 fen, Gaoben 3 fen, Shengma (Cimicifuga) 2 fen, Chuanxiong 2 fen, Wine-processed Huangbai 3 fen, Danggui 3 fen, Mutong 4 fen, Niuxi (Achyranthes) 3 fen. Decocted in water, one dose cured instantly.
