Black qi rising from above the ears, eyes, and nose, gradually entering the mouth, indicates death. The same applies to white qi. Red qi appearing above the ears and eyes indicates death after five days. Mouth open like a fish's, with exhaled breath not returning, indicates death. Picking at clothes and seams indicates death. Delirium without fever indicates death. Enuresis with loss of consciousness indicates death. Cyanosis of the fingernails indicates death. Blackening of the nails and surrounding flesh indicates death. Tongue retraction and testicular retraction indicate death. Eyebrow loss and stiff hair indicate death. Everted lips and full philtrum indicate death. Closure of both Yin and Yang Qi with loss of voice indicates death. Shen Qi (Spirit Qi) failing to reside within, with hoarse voice, indicates death. Sweating with non-flowing sweat indicates death. Foul breath that cannot be approached indicates death. Rolling eyes, fixed gaze, and shoulder breathing (Shanxi) indicate death. Sudden blackening of teeth, cyanotic face with black eyes, cyanotic face with yellow eyes, cyanotic face with white eyes, and cyanotic face with black lips are all signs of death. White face with black eyes, and white face with white eyes, are both signs of death. Red face with yellow eyes, and red face with white eyes, lead to death. If black resembles burning fire, white resembles dry bone, red resembles fresh blood, and green resembles grass, these are true signs of death.
Heart Qi exhaustion presents with shoulder breathing, backward glance with fixed eyes, leading to death in one day. Lung Qi exhaustion presents with slow and difficult exhalation, mouth open like a fish, leading to death in three days. Bone Qi exhaustion presents with lower back and spine pain, inability to turn over, leading to death in five days. Spleen Qi exhaustion presents with cold mouth, swollen feet, abdominal distension, and diarrhea, leading to death in twelve days. Kidney Qi exhaustion presents with dark and scanty bloody stool, dry ears, swollen feet, and swollen tongue, leading to death in six days. Tendon Qi exhaustion presents with fright and fear of the Hun (Ethereal Soul), cyanosis of hands, feet, and nails, frequent shouting, and incessant cursing, leading to death in nine days. Intestine Qi exhaustion presents with stiff hair, continuous sweating, and inability to flex or extend limbs, leading to death in six days. Liver Qi exhaustion presents with fear, crouching prone, fixed gaze, and cyanotic face, leading to death in eight days; some die immediately. Stomach Qi exhaustion presents with falling teeth and yellow eyes, leading to death in seven days.
Pulse: Deep, minute, fine, and weak pulses contraindicate diaphoresis (sweating therapy). Deep and fine pulse indicates interior syndrome; weak and minute pulse indicates deficiency of Qi and Blood. Floating and tight pulse typically presents with body pain and should be resolved by diaphoresis. However, if the Chi (Cun Kou) pulse is slow, diaphoresis is contraindicated due to slight blood deficiency. Yin disease with fine, deep, and rapid pulse is contraindicated for diaphoresis because the pathogen is in the interior. In Taiyang-Shanghan complicated by Wind-Dampness, where the patient was previously injured by Wind and subsequently by Heat, presenting with flaccid limbs, headache, fever, and persistent sweating despite treatment, the focus should be on Shaoyin and Jueyin meridians; diaphoresis is contraindicated. Diaphoresis would cause delirium, internal vexation, insomnia, fright, and scattered gaze lacking luster. In Taiyang-Shanghan Wet-Warmth, where the patient was previously injured by Cold and then infected, presenting with cold calves, abdominal distension, headache, and delirium, the focus should be on the Foot-Taiyin meridian; diaphoresis is contraindicated. Diaphoresis would lead to aphonia, deafness, unawareness of pain location, cyanosis of the body, and facial changes resulting in death. For Taiyang-Shanghan headache resembling Zhongfeng (Wind Strike), with mild sweating and spontaneous vomiting, causing mental restlessness (Ao Nao), diaphoresis leads to Jing Disease (Spasm). For Taiyang-Shanghan with wiry and fine pulse, headache, and counter-current heat, this belongs to Shaoyang; diaphoresis is contraindicated. In concurrent Taiyang and Shaoyang disease, with stiffness and pain in the head and neck, dizziness, and hardness/fullness below the heart, diaphoresis is contraindicated. In Shaoyin disease, cough with diarrhea and delirium results from forced diaphoresis. Throat obstruction is contraindicated for diaphoresis; diaphoresis causes hematemesis. Jueyin disease is contraindicated for diaphoresis; diaphoresis causes chaotic speech and hoarseness. Patients with blood loss (Wang Xue Jia) are contraindicated for diaphoresis; diaphoresis causes chills and trembling. Nasal bleeding (Nu) patients are contraindicated for diaphoresis; diaphoresis causes sunken forehead and fixed gaze. Lin Jia (patients with strangury) are contraindicated for diaphoresis; diaphoresis causes hematuria. Chuang Jia (patients with sores/ulcers) are contraindicated for diaphoresis; diaphoresis causes spasm. Han Jia (patients with chronic sweating) must not undergo heavy diaphoresis; diaphoresis causes mental confusion, and a short pulse indicates death. Winter diaphoresis causes vomiting, diarrhea, and mouth ulcers. Diarrhea with undigested food (Qing Gu) is contraindicated for diaphoresis; diaphoresis causes abdominal distension. Cough with clear urination, or misapplied diaphoresis, leads to Jue Ni (Cold Reversal). For various reverse conditions, diaphoresis makes mild cases difficult to cure; severe cases with chaotic speech and dizzy eyes result in death. Dong Qi (Moving Qi/Pulsation) present anywhere (left, right, up, down) contraindicates diaphoresis universally.
Pulse: Floating and large pulses may undergo diaphoresis. Inquiry: If there is diarrhea, it indicates deficiency, and diaphoresis is contraindicated. Floating and tight pulses may undergo diaphoresis. In Taiyang disease, floating and weak pulses may undergo diaphoresis. Floating and rapid pulses may also undergo diaphoresis. Slow pulse with profuse sweating and slight aversion to cold, indicating unresolved exterior, may undergo diaphoresis. Fever resembling malaria, belonging to Yangming, with floating and deficient pulse, may undergo diaphoresis. Body pain with normal bowel movements may undergo diaphoresis.
