Pediatric patients in autumn and summer often suffer from heat-induced vomiting (Re Tu) due to latent summerheat (Fu Shu). They may vomit yellow saliva, have a warm forehead, experience heat in the five centers (Wu Xin Re - heat sensation in palms, soles, and chest), and have scanty, reddish urine. This constitutes heat-induced vomiting. Alternatively, they may present with dry retching without expulsion of matter, for which Xiangru Yin (Elsholtzia Decoction) is indicated.
[Xue Ji] Heat-induced vomiting in children arises from excessive consumption of sweet, fried, or roasted foods, or from the nursing mother consuming rich and greasy foods, leading to accumulated heat in the Stomach Meridian. It may also result from summerheat pathogens lying latent within the Stomach during summer. If muscle twitching occurs accompanied by irritability, fever, and thirst, this indicates summerheat damaging Stomach Qi; treat first with Xiangru San (Elsholtia Powder), followed by Zhuru Tang (Bamboo Shavings Decoction). If vomited milk appears yellow and the child cannot tolerate nourishment, this indicates heat in the Stomach Meridian; treat first with Xiehuang San (Drain the Yellow Powder), followed by Renshen Anwei San (Ginseng Stomach-Calming Powder). If sour and foul matter is vomited, this indicates retention of milk and food within; use Baohe Wan (Preserve Harmony Pill). If undigested milk is vomited, this indicates weak Stomach Qi; use Yigong San (Extraordinary Skill Powder). If vomiting is accompanied by poor appetite, abdominal pain relieved by pressure, this indicates Spleen Qi Deficiency; use Liujunzi Tang (Six Gentlemen Decoction) added with Muxiang (Aucklandiae Radix). For all such syndromes, examine the temperature of the fingers: if hot, it indicates Stomach Heat, treated with Xiehuang San; if cold, it indicates Stomach Cold, treated with Lizhong Tang (Regulate the Middle Decoction); if neither hot nor cold, regulate with Yigong San.
