Boil one dose into a decoction, and mix with two qian for oral administration. (The above discusses Slippage Syndrome). Li Dongyuan treated an old servant who had suffered from rectal prolapse for a long time, and recently developed diarrhea with tenesmus, where the stool was predominantly white with little red, unable to bear the suffering. This person did not consume meat or rich foods, but likely ate excessive vegetables or had irregular diet. The weather was already cold, and clothing/bedding was thin, allowing cold pathogenic factors to invade the body without resistance, causing the rectum to prolapse. This is a Cold syndrome and a Slippery syndrome. The True Qi fails to secure, and the physical form fails to retract, constituting Blood depletion. This is Cold-Slipperiness, with Qi leakage and insecurity, hence the physical form descends and prolapses. One should use astringent herbs to remove the prolapse and eliminate slipperiness, use large-hot agents to dispel cold and tonify Yang, use Qi-tonifying herbs to raise Yang and benefit Qi, and use slightly sour flavors to secure Qi and draw it upward. The formula is named Hezi Pi San (Persimmon Sepal Powder). Take half the usual dose for the first administration, and full recovery after the second dose. Other options include Yangzang Tang (Nourish the Viscera Decoction) and Diyü Shaoyao Tang (Sanguisorba and Peony Decoction). Dai Yuanli stated: Rectal prolapse is difficult to treat with medicine; Heat causes anal closure, while Cold causes anal prolapse. Internally, grind Magnetitum (Cishi) into powder, take two qian each time, mixed with rice water before meals. Externally, grind rust of iron (Tiexiu) into water and wash the area with warm solution. (The above discusses Rectal Prolapse). In Dripping Down (Dysentery), if there is fecal incontinence and the anus opens like a hollow cavity that does not close, crush Scallion (Congbai) and Sichuan Pepper (Huajiao) into powder and insert into the anus. Simultaneously administer astringent and intestinal-firming agents to consolidate, such as Yumiqiao (Pericarpium Papaveris Rubrum) and Hezi Pi (Pericarpium Diospyrotis), which are highly effective. (The above discusses Open Anus). If dysentery persists for a long time causing anal pain, it can be either Cold or Heat. Smoke-treat with prepared Mugwort (Shuai Ai), yellow wax, and Persimmon Sepal (Hezi Pi). If painful due to Heat, use Areca Seed (Binglang), Aucklandia (Muxiang), Coptis (Huanglian), and Scutellaria (Huangqin) plus dried Ginger (Ganjiang). If painful due to Cold, apply fried salt with heat compression. Or apply roasted Ziziphus Spinosae Semen (Zhishi). Zhu Danxi used a tile fragment rounded like a copper coin, heated until red, then dipped into child's urine. Quickly retrieve, wrap in dry paper, and place on the painful area. Because the external cold might invade deficiency at that time, drink a strong decoction of Ginseng (Renshen), Angelica Sinensis (Danggui), and Tangerine Peel (Chenpi), and maintain a bland diet for natural peace. (The above discusses Anal Pain). Dysentery patients who cannot eat, or vomit immediately after swallowing liquids/medicines, are commonly known as Jin Kou Li (Jin Kou Dysentery). Etiologies include: pathogens lingering, causing Stomach Qi to stagnate and fail to diffuse, and Spleen Qi to be obstructed and fail to distribute, leading to vomiting and inability to ingest food; insufficient Yang Qi, causing retained food in the Stomach to remain undigested, resulting in belching and sudden inability to swallow food; Liver Qi invading the Spleen and Stomach triggering vomiting, preventing food intake or causing regurgitation; retention of fluid-retention, causing急促 breathing and vomiting, preventing grain intake; blazing Fire Qi causing internal rejection and vomiting, preventing food intake; deficient-cold Stomach Qi causing food to be regurgitated after ingestion; evil-heat in the Stomach causing lack of appetite; weak Spleen and Stomach causing lack of appetite; foul accumulation below, with malignant Qi steaming upwards causing vomiting and inability to ingest food. Treatment must follow the specific cause. Differentiate by pulse and symptoms: if Spleen and Stomach are not weak, and the patient reports headache, vexation, warm hands and feet, and has not taken many cooling medicines, this is toxic vapors rushing up to the Heart and Lungs, causing vomiting and anorexia. Use Baidu San (Eliminate Toxics Powder), four qian per dose, with one hundred grains of aged stored rice, three slices of ginger, and one jujube, boiled in 1.5 cups of water to 0.8 cups, taken warm. If the pulse is weak/faint, or there is abdominal distension, cold extremities, no initial vomiting, but vomiting upon smelling food after excessive use of astringent-cool-bitter medicines like Pericarpium Papaveris Rubrum or Chinese Olive (Wumei), this indicates Spleen-Stomach weakness. Use single herb Dioscorea Opposita (Shanyao), cut into mung-bean-sized pieces, half stir-fried in a silver pot until cooked, half raw, ground together, taken with rice gruel. Another method: use Stone Lotus (Shilian), remove shell, keep heart and flesh, grind to powder. Take two qian, mixed with aged rice water. This disease is likely toxic vapors rushing to Heart/Lungs; this remedy helps open Heart Qi, inducing appetite. Zhu Danxi used Ginseng (Renshen) and Scutellaria (Huanglian) stir-fried with ginger juice, decocted strongly, sipped slowly throughout the day. If vomiting occurs, take again. If even one sip goes down, the throat opens, and dysentery stops spontaneously. Yang Renzhai used Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) with powder of Acorus Tatarinowii (Shichangpu), mixed hot with high-quality japonica rice water. Or use Ginseng, Poria, and Stone Lotus flesh with some Acorus. Dai Fu'an used Zhizhong Tang (Regulate the Middle Decoction) with half a qian of Aucklandia (Muxiang) or one qian of Amomum Villosum (Suisha). (The above discusses Jin Kou Dysentery). If pathology is below causing stagnation, and above causing vomiting of food, this indicates untransformed toxic accumulation and unpacified Stomach Qi. Identify Cold and warm it, Heat and clear it, Deficiency and tonify with Ginseng and Atractylodes. Once toxins resolve and accumulations descend, appetite returns. For chronic loose stools/dysentery with pus/blood, thick/sticky stool, tenesmus, occurring day and night without limit, use Dahuang Tang (Rhubarb Decoction). Use one liang of Rhubarb (Dahuang), cut, soak in two large cups of good wine for half a day, boil together to 1.5 cups, strain, divide into two doses, take all at once. Stop if dysentery ceases, continue if not, stopping when purgation occurs. Then take Shaoyao Tang (Peony Decoction) to harmonize and thoroughly eliminate toxins. After the previous medicine clears dysentery, use Baizhu Huangqin Tang (Atractylodes and Scutellaria Decoction) to harmonize. Zhu Danxi treated a man with dysentery for over 100 days with no effect from various methods. Six pulses were rapid/hurried, deep, wiry, fine, weak, and hollow, more pronounced on the left. Ten bowel movements day and night, with very little excrement, though purple-black blood streaks appeared in clear mucus. Food was completely refused. This was not dysentery but Blood Stasis. Treat with Peach Kernel (Taoren), Frankincense (Ruxiang), Myrrh (Moyao), and Talcum (Huashi), assisted by Areca Seed (Binglang), Aucklandia (Muxiang), and Medicated Leaven (Shenqu) made into pills with paste. Take over one hundred pills with rice water. At midnight, no movement. Administer another two hundred pills according to the previous method. By dawn, excreted two and a half sheng of filth resembling rotten fish intestines. Exhausted all day, gradually given congee for recovery. Note: This formula likely contains Rhubarb; without it, expulsion would be difficult. He also treated his paternal uncle, age 70, constitutionally robust but emaciated, suffering from loose stools/dysentery from late summer to autumn. Various treatments failed. Though ill for a long time, spirit was not exhausted. Urination was scant and not red. Both wrists showed slippery/wiry pulses. Patient reported slight chest stuffiness and reduced appetite. Realized this must be long-standing accumulation, located in intestines/stomach. Asked about dietary preferences: replied he loved carp, eating it daily for three years. This phlegm accumulation is in the Lung. Since Lung is the exterior organ of the Large Intestine, it affects Large Intestine security. Clear the source, and the flow will naturally clarify. Decoct Cornus (Zhuyu), Tangerine Peel (Chenpi), Green Onion (Qingcong), Millet Root (Mugen), and Ginger (Shengjiang) into a strong decoction, mix with brown sugar, drink about one bowl. Induce vomiting by stimulating the throat with finger. Half a sheng of glue-like phlegm was vomited in half an hour. Symptoms halved that night. Took again next morning, vomited another half sheng, and dysentery stopped spontaneously. Then prescribed Pingwei San (Calm the Stomach Powder) plus Atractylodes (Baizhu) and Scutellaria (Huanglian), recovered in ten days. For astringency, use Muxiang San or Helile Wan. (The above discusses Chronic Dysentery). Resting Dysentery often results from premature astringency, leaving residual accumulation. Or poor management after recovery, causing intermittent onset. Use Sijunzi Tang (Four Gentlemen Decoction) plus one qian Tangerine Peel (Chenpi) and half a qian Aucklandia (Muxiang), swallowing Zhuche Wan. It arises because astringents trapped the accumulation, creating resting dysentery. Re-administer elimination formulas, then use astringents. Zhang Wenzhong used roasted Tiger Bone (Hugu) ground to powder, mixed and taken orally, three spoons daily, effective.
For chronic/resting dysentery with severe deficiency-slipperiness, use White Bark of Toona Sinensis (Chungen) from the southeast direction, soaked in running water for three days, remove yellow bark, slice. Match one liang with one liang Ginseng (Renshen), add two qian baked Aucklandia (Muxiang), and a pinch of japonica rice (Jingmi). Decoct and drink. (The above discusses Resting Dysentery). Labor Dysentery results from prolonged dysentery consuming essence and blood, emptying intestines/stomach, generating other syndromes, or five-palm heat resembling consumption. Use Yulian Yin (Dioscorea and Lotus Decoction); double Lotus Seed (Lianrou) if red predominates, double Dioscorea (Shanyao) if white predominates. Post-dysentery regulation/tonification: use Sijunzi Tang plus 1.5 qian Tangerine Peel (Chenpi), i.e., Yigong San. Or Qizhen San. Those aversive to sweetness: use Shengliao Pingwei San plus half a qian each of Ginseng (Renshen) and Poria (Fuling). For deteriorated syndromes from various diseases, prolonged passage of pus/blood, or stool like dead pig liver color, or mixed five colors, frequent uncontrollable discharge resembling dripping down, commonly called "Scraping Intestines." This is deficiency of all viscera, impending failure of Spleen Qi, causing intestinal prolapse. Using dysentery medicines would be a mistake. Use Liuzhu Yin minus Aconite (Fuzi), plus Alpinia Oxyphylla (Yizhiren) and White Peony Root (Baishao), perhaps offering a slim chance. Post-dysentery Wind results from post-dysentery lower deficiency, poor nursing, excessive sexual activity, or external pathogen invasion, causing flaccidity of both legs, painful or numb, becoming Wind-Dysentery. Use Duhuo Jisheng Tang, swallowing Hugu Sijin Wan. Or use Dafangfeng Tang. Or grind one-third Gynura Segetum (Gusuibu) to extract juice, taken with wine. Externally, fumigate/wash with Achyranthes Bidentata (Niuxi), Bamboo Joint (Shanjiejie), Angelica Dahurica (Baizhi), Arisaema (Nanxing), and Dioscorea Hypoglauca (Bixie) decoction. Zhu Danxi said: Post-dysentery wind is due to blood entering viscera, not fully expelled, returning to meridians and failing to circulate. Use Pine Knot (Songmingjie) one liang, stir-fried with two qian Frankincense (Ruxiang) until charred but retaining nature, Atractylodes Lancea (Cangzhu) and Phellodendron (Huangbo) one liang each, Tecoma Stans (Ziwei) one and a half liang, Licorice (Gancao) half liang, Peach Kernel (Taoren) peeled but tip kept one liang. All ground to powder. Take three qian each time, pounded finely with ginger, boiled in water for two-three boils, then taken. Neighbor Bao Zi, over twenty years old, suffered bloody dysentery, cured with astringents, later developed gouty pain, screaming shaking neighbors. I examined him and said: This is Evil Blood entering meridians. Blood affected by damp-heat, long-term turbid coagulation, incomplete expulsion, remaining in tunnels, thus causing pain. If untreated long-term, fear wasting/emaciation. Prescribed Siwu Tang plus Peach Kernel (Taoren), Safflower (Honghua), Achyranthes (Niuxi), Scutellaria (Huangqin), Tangerine Peel (Chenpi), Licorice (Gancao) decoction. Grind Qianxing San with ginger juice, add little wine, drink dozens of doses. Also punctured Weizhong (BL40) to release black blood, nearly three he, and recovered. *Baojian* states: Even if diarrhea/dysentery stops, if Spleen/Stomach are weak and cannot tolerate food, do not uniformly use attacking/clearing agents. If Spleen/Stomach Qi is sufficient through tonification, appetite will naturally return. Use Qian Yi Gong San from Qian's formulas. If indulgence leads to excessive eating, damaging Spleen/Stomach, causing epigastric/ch abdominal fullness, vomiting, nausea, this exception applies. Temporarily use JuPi Zhishi Wan (Tangerine Peel and Immature Bitter Orange Pill) one dose, stop if rapid relief occurs. If diet is regulated without injury, Stomach Qi becomes peaceful.
