If the stool is smooth and harmonious, yet there are symptoms of panting, coughing, and expectoration of phlegm, this is due to Liver Fire insulting the Lung (Liver Fire invading the Lung). It should be treated with Xiao Chai Hu Tang (Minor Bupleurum Decoction) added with Qing Pi (Green Tangerine Peel) and Shan Zhi (Gardenia Fruit) to clear the heat. If there is distending pain in the chest and abdomen, unsmooth defecation, accompanied by panting, coughing, and hemoptysis, this is due to the stagnation of Blood Stasis. It should be treated with Dang Gui Dao Zhi San (Angelica Dahurica Decoction for Unblocking Stagnation) to promote the discharge of stasis. The *Nei Jing* (Inner Canon) states: "The Liver stores blood, and the Spleen controls blood." Generally, the Liver belongs to Wood, which generates Fire; Fire insults Earth (Spleen/Stomach). Once Liver Fire is blazing, Liver Blood must be damaged, and Spleen Qi must be deficient. One should first clear Liver Fire and nourish Liver Blood, so that Blood Stasis will not coagulate, and muscles will not ulcerate extensively; subsequently, strengthen the Spleen and invigorate the Stomach, so that necrotic flesh will easily slough off and new flesh will easily grow. If one uses harsh purgative or attacking methods, the deficient will become more deficient, and the stagnant will become more stagnant, bringing disaster swiftly.
A patient presented with distending pain at the affected site, fever with a desire to vomit, hot distension in both hypochondria, and a surging pulse in the Liver position. Xue Ji said: "This is a symptom of Liver Fire. Let him drink child's urine and take Xiao Chai Hu Tang (Minor Bupleurum Decoction) added with Huang Lian (Coptis Rhizome), Shan Zhi (Gardenia Fruit), Gui Shao (Tip of Angelica Root), and Hong Hua (Carthamus Flower). All symptoms indeed subsided." In such cases, if the pulse at the left Guan position is floating and weak, and pressing the abdomen does not cause distension, this is Liver distension due to Blood deficiency. It should be treated with Si Wu Tang (Four Substances Decoction) plus Ren Shen (Ginseng), Fu Ling (Poria), Qing Pi (Green Tangerine Peel), Gan Cao (Licorice Root), and similar herbs. If the pulse at the left Guan position is surging and forceful, and the chest and hypochondria are distending and painful upon pressure, this is a condition of Liver injury caused by anger. It should be treated primarily with Xiao Chai Hu Tang (Minor Bupleurum Decoction) plus Xiong (Ligusticum Chuanxiong Rhizome), Dang Gui (Angelica Sinensis Root), Qing Pi (Green Tangerine Peel), Shao Yao (Peony Root), Jie Geng (Platycodon Root), and Zhi Ke (Bitter Orange). Generally, for this condition, it is unnecessary to discuss the severity of the beating or whether bleeding occurred at the site. As long as one was twisted or pressed heavily, exerted oneself greatly, or became angry, thereby damaging Qi and Blood, causing Blood Stasis to return to the Liver, it often leads to the aforementioned symptoms. In severe cases, there may be fullness and distension in the chest and hypochondria, counterflow of Qi and obstruction, or bleeding from the mouth and nose, leading to sudden death without rescue.
Another patient, after recovery, experienced bitter taste in the mouth and distending pain in the lower back and hypochondria. He took tonifying Kidney and moving Qi medicines without effect. Xue Ji palpated his Liver pulse, finding it floating and weak. This belonged to deficiency of Qi and Blood in the Liver and Gallbladder. It was treated with Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala Rhizome), Xiong (Ligusticum Chuanxiong Rhizome), Dang Gui (Angelica Sinensis Root), Di Huang (Rehmannia Root), Bai Zhu (Atractylodes Macrocephala Rhizome), Mai Men Dong (Ophiopogon Tuber), and Wu Wei Zi (Schisandra Berry), resulting in a cure.
Li Jinshi injured his hand in late summer. Bleeding did not stop, accompanied by fever, thirst, and distension in both hypochondria, which ceased upon pressure. This was Blood deficiency. It was treated with Ba Zhen Tang (Eight Treasures Decoction) added with Ruan Chai Hu (Soft Bupleurum Root) and Tian Hua Fen (Trichosanthes Root), resulting in immediate recovery. Subsequently, he took medicines to nourish Qi and Blood, and recovered through regulation.
Xiao Chai Hu Tang (Minor Bupleurum Decoction): Treats all kinds of blunt trauma injuries, etc., where pain is caused by excessive fire in the Liver and Gallbladder meridians, spontaneous sweating with bleeding, alternating chills and fever, fever in the afternoon (Shi hour), tidal fever with body heat, cough with fever, pain under the hypochondria, and fullness/stuffiness in both hypochondria.

Preparation: Decoct with ginger water and take orally.
Po Xue San (Blood-Breaking Powder): Treats injuries from horse riding, such as falling and fracturing the spine, where foul blood flows into the hypochondria, causing severe pain making turning difficult.
Ingredients: Qiang Huo (Notopterygium Rhizome), Fang Feng (Saposhnikovia Root), Rou Gui (Cinnamon Bark) (each 1 qian), Shui Zhi (Leech) (stir-fried until smoke disappears, separately ground, 0.5 qian), Chai Hu (Bupleurum Root), Dang Gui Shao (Tip of Angelica Sinensis Root), Lian Qiao (Forsythia Fruit) (each 2 qian), She Xiang (Musk) (separately ground, a small amount).
Preparation: Combine as one dose. Decoct with one cup of water and one cup of wine until reduced to one cup. Strain. Add the powders of Shui Zhi (Leech) and She Xiang (Musk), mix well. Take warm at any time.
