When the Lung is affected by Wind, there is dryness of the mouth and panting; the body feels dizzy and heavy; there is a sensation of being muddled (head fogginess) and abdominal distension.
When the Lung is affected by Cold, turbid nasal discharge is expectorated.
The pulse of the dying Lung: superficially it is empty, deeply it is weak; it feels like a scallion leaf—hollow in the middle with no root at the bottom—this indicates death.
When the Liver is affected by Wind, there is dizziness and trembling of the head and eyes, pain in both hypochondria, habitual stooping while walking, and a craving for sweet flavors. (Qianjin Fang adds four characters under "sweet": "like a pregnant woman with obstruction.")
When the Liver is affected by Cold, the arms cannot be raised, the base of the tongue is dry, there is frequent sighing, chest pain preventing turning over, vomiting after eating accompanied by sweating. ([Original Note] Mai Jing and Qianjin Fang state: occasional night sweats and coughing, vomiting juice after eating. ○ In Qianjin Fang, "base of the tongue" is written as "large tongue".)
The pulse of the dying Liver: superficially it is weak; deeply it feels like a cord that does not return, or curved like a snake moving—this indicates death.
In Liver Stagnation (Gan Zhuo), the patient constantly desires someone to tread on their chest. Before suffering, they only desire hot drinks. Xuanfu Dahong Tang (Cineraria Flower and Honey Pill) is the primary formula. ([Original Note] Chen Yi et al. collated various editions; Xuanfu Dahong Tang records are identical. ○ Note on twelve characters: Cheng Lin writes "Formula seen in Women's Miscellaneous Diseases Chapter 6", which is incorrect. There may be missing characters or errors. Qianjin Fang lacks the six characters "Xuanfu Dahong Tang is the main formula". Xu Lingtai and Shen Mingzong change "tread" to "step", which is incorrect.)
When the Heart is affected by Wind, there is mild fever (Xi Xi Fa Re), inability to get up, a feeling of hunger in the heart/chest, and vomiting immediately after eating. (Qianjin Fang adds two characters under "hunger": "then drink"; adds two characters before "immediately": "food and drink".)
When the Heart is affected by Cold, the patient suffers as if eating garlic. In severe cases, heart pain radiates to the back, and back pain radiates to the heart, resembling Gu Zhu disease. If the pulse is superficial, spontaneous vomiting leads to recovery. (Under "garlic", Qianjin Fang has the character "minced". Gu, Xu Lingtai reads as "insect", suggesting "Zhu" might be "boring/rotting", which is incorrect. Shen Mingzong, Wei Litong, and You Zaijing also read as "insect injection/note".)
If the Heart Qi is damaged, the person becomes fatigued easily; upon fatigue, the head and face turn red while the lower body feels heavy; there is heart pain with spontaneous vexation and fever; pulsation occurs at the umbilicus; the pulse is wiry. This is caused by damage to the Heart organ. (Under "pulsation", Qianjin Fang has the character "hand".)
The pulse of the dying Heart: superficially it is solid like pills or beans; deeply it becomes increasingly restless and rapid—this indicates death. ("Pill", Zhao Yi, Xu Lingtai, Shen Mingzong, and You Zaijing all write as "hemp seed". Qianjin Fang adds two characters under "bean": "striking the hand".)
Evil weeping causing insecurity of the Hun and Po spirits is due to deficiency of Blood and Qi. Deficiency of Blood and Qi belongs to the Heart. When Heart Qi is deficient, the person fears easily, closes their eyes wanting to sleep, dreams of traveling far away, spirit and essence scatter, and Hun and Po wander recklessly. Deficiency of Yin Qi leads to Dian (depressive psychosis); deficiency of Yang Qi leads to Kuang (manic psychosis). (Note: Xu Lingtai says "weeping" is likely "entering". Shen Mingzong agrees. Jin Jian says Dian and Kuang are swapped, neither should be followed.)
When the Spleen is affected by Wind, there is mild fever (Xi Xi Fa Re), the form resembles that of a drunk person, abdominal vexation and heaviness, twitching of the eyelids, and shortness of breath. ("Eye", Qianjin Fang writes as "flesh/muscle", which is correct.)
The pulse of the dying Spleen: superficially it is large and hard; deeply it feels like an inverted cup, empty and shaking—this indicates death. ([Original Note] Chen Yi et al. detailed that each of the five zang organs has Wind and Cold afflictions, but now only Spleen Wind is recorded, while Kidney Wind and Cold are not recorded, because the ancient texts are extremely brief and chaotic. Having been distant from antiquity, there is no text to supplement it. ○ Jie Jie, Qianjin Fang writes as "Jie Jie". Qianjin Fang labels "Spleen Cold" but does not record symptoms, indicating long-term loss.)
Fu Yang pulse is floating and choppy. Floating indicates strong Stomach Qi; choppy indicates frequent urination. The interaction of floating and choppy causes hardness of the stool; this is Spleen Constraint (Pi Yue). Maziren Wan (Hemp Seed Pill) is the primary formula. (Qianjin Fang adds thirteen characters under "Constraint": "In Spleen Constraint, stool is hard, urination is convenient, and there is no thirst.")
Maziren Wan Formula (Ming Li Lun calls it Pi Yue Wan.)
Hemp Seed (Er Sheng)
Peony Root (Ban Jin)
Immature Bitter Orange (Yi Jin)
Rhubarb (Yi Jin)
Magnolia Bark (Yi Jin)
Apricot Seed (Yi Sheng; in Yangming Chapter, Immature Bitter Orange is Ban Jin, Magnolia Bark is one chi)
These six herbs are powdered, mixed with refined honey into pills the size of wu zi (walnut seeds). Take ten pills with water, three times a day. Stop when effective.
When Kidney is affected by Cold-Dampness adhering (Shen Zhuo), the body feels heavy, the waist feels cold as if sitting in water, the appearance is like having water, yet there is no thirst, urination is normal, and appetite is unchanged. The disease belongs to the Lower Jiao. It arises from physical labor with sweating, wearing cold and damp clothes inside, developing over time. The lower waist is cold and painful, and the waist feels heavy as if carrying five thousand coins. Gan Jiang Ling Zhu Tang (Licorice, Dried Ginger, Poria, and Atractylodes Macrocephala Decoction) is the primary formula. (As if having water, Qianjin Fang writes "as if washed by water". Body character, Qianjin Fang and Waitai Miyao write as "cong" or "zuo". Long time to develop, Waitai Miyao writes "jiu zhi gu de ye". Waist heavy, original text and Waitai Miyao write as "abdomen heavy", now corrected according to Zhao Kai Mei edition. Qianjin Fang Kidney Pulse Theory writes "waist", Pain in Waist Gate writes "abdomen". Xu Lingtai, Cheng Lin, etc., all annotate as "abdomen".)
Gan Jiang Ling Zhu Tang (Qianjin Fang names it Shen Zhuo Tang. Waitai Miyao cites Gu Jin Lu Yan, naming it Gan Cao Tang.)
Licorice (Liang Er)
Atractylodes Macrocephala (Liang Er ○ Qianjin Fang and Waitai Miyao use Liang Si)
Dried Ginger (Liang Si ○ Qianjin Fang and Waitai Miyao use Liang San)
Poria (Liang Si)
These four ingredients are boiled in five sheng of water to obtain three sheng. Divide into three warm doses. The waist will become warm.
Qianjin Fang Shen Zhuo San (Waitai Miyao cites Jing Xin Lu, with no indication of treatment, listed after the above formula.)
Eucommia Bark
Cinnamon Twig
Licorice
Alisma Rhizome
Achyranthes Root
Dried Ginger (each Liang San)
Atractylodes Macrocephala
Poria (each Liang Si)
These eight ingredients are processed and sifted into a coarse powder. Take one square-cun spoonful, boil in one sheng of wine for five or six boils, remove dregs, take once, twice a day.
Qianjin Yi Fang Wen Shen Tang primarily treats edema and immobility of the lumbar spine, knees, and feet. (From Hong Jiao/Foot Gout formulas.)
Poria
Dried Ginger
Alisma Rhizome (each Liang Er)
Cinnamon Twig (Liang San)
These four ingredients are cut and boiled in six sheng of water to obtain two sheng. Divide into three doses.
Another formula for treating Water-Qi between the Kidneys, lumbar and spinal pain, and cramping pain in the abdomen and back:
Take the above formula, remove Licorice, add Alisma Rhizome.
San Yin Fang Fu Ling Bai Zhu Tang treats exposure to summer-heat toxicity combined with dampness, or bathing before sweat dries, resulting in Summer-Heat-Dampness.
Take the above formula, add Cinnamon Twig, each Liang Yi.
Another Chu Shi Tang treats exposure to rain and dampness stagnating in the meridians, causing blood extravasation leading to epistaxis, or Spleen disharmony where dampness sticks to meridians, blood flows into the Stomach, stomach fullness leads to hematemesis.
This is the same formula. For headache, add Chuanxiong Ergian. Most effective for stopping epistaxis occurring in bathrooms.
The pulse of the dying Kidney: superficially it is hard; deeply it is chaotic like a rolling ball, increasing downward into the Chi position—this indicates death. ("Increasing", Qianjin Fang writes as "overflowing".)
Question: Regarding the exhaustion of the San Jiao departments, upper Jiao exhaustion leads to belching. What does this mean? The Master said: The Upper Jiao receives from the Middle Jiao. If Qi is not harmonized and cannot digest grains, belaching occurs. Lower Jiao exhaustion leads to enuresis and fecal incontinence. Because Qi is not harmonized and cannot self-control, no treatment is needed; it will heal itself after a long time.
The Master said: Heat in the Upper Jiao leads to Fei Wei (Lung Withering) due to coughing. Heat in the Middle Jiao leads to hardness (constipation). Heat in the Lower Jiao leads to hematuria and also causes Lin syndrome and urinary retention. Cold in the Large Intestine leads to loose stools; Heat leads to intestinal mucus. Cold in the Small Intestine leads to heaviness below and bloody stools; Heat leads to hemorrhoids.
