Luoli (Scrofula) refers to tubercular masses. They may occur in the preauricular and postauricular regions, extending to the submandibular area down to the clavicular fossa; these are all termed Luoli. Those located on the chest and its lateral aspects, extending downward to connect with both hypochondria, are all termed Machdao (Horseshoe-shaped Scrofula). The Hand-Shaoyang (Sanjiao) and Foot-Shaoyang (Gallbladder) meridians govern these conditions. These meridians have abundant Qi but scarce Blood; therefore, the masses are hard rather than soft, and when suppurating, the pus is thin like rice wash water. Additionally, those that form individually and are small are called Jiehe (Tubercles); those that are continuous and linked are called Luoli; those shaped long like a clam shell are called Machdao.
(This condition originally stems from changes due to excessive consumption of rich, greasy, and hot-natured foods, or it may be caused by consumptive disease and Qi stagnation. Treatment should focus on tonifying the body's Qi and regulating the meridians. There is no need to forcibly induce ulceration; simply allowing the heat to dissipate will cause the sores to heal spontaneously.)
Acupoints: Jianyu (LI15), Quchi (LI11), Hegu (LI4) [Hand-Yangming]; Zhigou (TE6), Tianjing (TE10) [Hand-Shaoyang]; Shaohai (HT3) [Hand-Shaoyin]; Tianchi (PC1) [Hand-Jueyin]; Zusanli (ST36) [For sores arising below the cheek, select Foot-Yangming]; Yuanli (GB23), Yangfu (GB38), Zulinqi (GB41), Taichong (LR3) [For axillary swelling, Machdao, and carbuncles, select Foot-Shaoyang and Foot-Jueyin].
For cases where the pathogenic toxin is deep, after moxibustion, perform two or three additional sessions for cure.
At the initial onset, males should have Fengchi (GB20) moxa-burned on the left side, and females on the right side; this is also effective.
One method for treating Luoli: Slice a single-clove garlic into thick pieces resembling a coin. Begin moxibustion on the nuclei that appeared later, continuing until reaching the primary nucleus that appeared first. Moxa-burning more frequently yields better results. After moxibustion, one dose of decoction can be taken: seven pods of Gleditsia sinensis (Yazao, 牙皂), seven silkworms treated with vinegar (Jiangcan, 僵蚕), one whole Trichosanthes fruit (Gualou, 栝蒌) including peel and seeds (crushed), and one Wuweizi (Schisandra chinensis, 五味子) berry per year of age. Decoct in two bowls of water until cooked. Add Sanhuang (Rhei Radix et Rhizoma, 大黄) 3-5 qian, adjusted according to the patient's deficiency or excess. One dose causes resolution.
Another treatment for Luoli, regardless of whether it has ulcerated or not, involves moxa-burning the Zhousjian (Elbow Tip) acupoint. Place the hand palm-up on the shoulder and lift slightly; the tip of the elbow bone becomes visible, which is the moxa-burning site. Moxa-burn seven cones (zhuang). After three sessions, the sore disappears naturally. Treat the left side if the lesion is on the left, and the right if on the right.
Liu (Tumor) means retention. If anger stirs Liver Fire, causing Yin blood to dry up and tendons to contract, swelling arises from the tendons; palpation feels like tendon. Over time, red threads may appear, named Jinliu (Tendon Tumor). If overwork stirs fire, causing Yin blood to boil and external pathogens to collide resulting in swelling, swelling arises from the muscle layer; over time, red threads appear or the skin turns entirely red, named Xueliu (Blood Tumor). If emotional depression injures Spleen Qi, causing muscle thinning, and external pathogens collide resulting in swelling, swelling arises from the muscle layer; palpation feels solid yet soft, named Rouliu (Flesh Tumor). If overwork injairs Lung Qi, making the interstices (Couli, 腠理) loose, and external pathogens collide causing congestion and swelling, swelling arises from the skin layer; palpation feels floating and soft, named Qiliu (Qi Tumor). (Some say it involves pulling pain). If overwork injures Kidney Water, failing to nourish bones and causing swelling, swelling arises from the bone; palpation feels hard, named Guliu (Bone Tumor) (also known as Shiying, Stone Goiter). Outside of these five tumors, Fenliu (Powder Tumor) is the most common. This occurs because fluids and phlegm in the interstices occasionally stagnate, gathering without dispersing, gradually forming a tumor. This also belongs to the category of acne, differing only in depth. Deep ones lie within the skin, gradually enlarging into a tumor. Once, a person had a small tumor growing along the lower eyelid, initially grain-sized, gradually enlarging to bean-sized. Using three or four piercing needles, the eyelid was turned inside out to pierce its membrane, releasing a small amount of blood. After two or three such sessions, the tumor shrank daily and eventually dissipated. Another person had a tumor on the arm, gradually enlarging to the size of a longan. This person used small moxa cones to moxa-burn seven cones on the tumor, which gradually dissipated and did not grow again. Alternatively, garlic-separated moxibustion can be used. For anyone developing such growths, one should choose between the above two methods appropriately. Generally, tendon disorders benefit from moxibustion, while blood disorders benefit from acupuncture. Some use topical applications of Radish seeds (Luobozhongzi, 萝卜子), Arisaema cum Bile (Nanxing, 南星), and Mirabilite (Piaoxiao, 朴硝) to treat them, which can temporarily reduce swelling. To eradicate the root, there is no method superior to the aforementioned ones.
Yingliu (Goiter and Tumors): Neck masses are called Ying (Goiter). Liuqi Cheliu (Red Tumor), Dandu (Erysipelas), are all due to Heat exceeding and Qi stagnating.
Acupoints: Fengchi (GB20) [Moxa-burn 100 cones], Dazhui (GV14), Tiantu (CV22) [Moxa-burning at the initial stage of any goiter or tumor is excellent], Jianyu (LI15) [Males: Left 18 cones, Right 17 cones; Females: Right 18 cones, Left 17 cones], Qishe (ST11), Huiyin (CV24 - *Note: Text says 'Hui', likely referring to a neck point contextually, possibly Yifeng or similar, but literal translation is Hui*), Yunmen (LU2), Tianfu (LU3).
Youzhi (Warts and Moles): You (Wart), pronounced 'you', is an excrescence; Zhi (Mole), pronounced 'zhi', is a black spot. Moxa-burn three cones directly on the wart to eliminate it; some cases require only one cone, followed by dropping water to make it fall off naturally.
Additionally, moxa-burning the center of the middle finger joint (Zhongzhixie, 中指节宛宛中) is effective for both warts and moles.
