亏少,惟六味加二冬、人乳、牛乳,滋阴润燥,而便自如常,又云∶肾为阴,主藏精,阴虚则精不藏;

Smoke is the foremost of pungent and hot substances, and wine is the ultimate of damp-heat. All things such as ginger, pepper, mustard, garlic, and all other pungent, spicy, and hot foods can severely damage Yin fluids and must absolutely be avoided. Similarly, all cold, raw, slippery-intestine-inducing, and hard foods should also be abstained from to prevent injury to the Spleen and Stomach. Furthermore, one should stay away from sexual activities, avoid anger, release worries and thoughts, and avoid overwork; this is the primary principle. The Classics state that the Kidney governs closure and storage, while the Liver governs dispersion and excretion. Both organs contain Ministerial Fire (Xiang Huo), and their meridians connect upwards to the Heart. Therefore, whenever desire arises, Ministerial Fire rises abruptly. Even without sexual intercourse, essence is secretly consumed, let alone when engaging in sexual activity which leads to essence depletion and Kidney exhaustion. The Classics say: "Anger causes Qi to rebel; in severe cases, it leads to vomiting blood or diarrhea with undigested food." It also says: "Worry causes Qi to become blocked and cease to flow." It further states: "Anger causes Qi to knot." And again: "When Yang Qi is subjected to excessive mental and physical labor, it flares outward while Essence is exhausted inward." This means that excessive labor causes Qi to flare externally and Essence to deplete internally, resulting in Yang disturbance and Yin deficiency. As mentioned above, patients with Xu Lao (Consumptive Disease) should strictly avoid these factors and absolutely contraindicate moxibustion. Zhang Zhongjing said: "In cases of a faint and rapid pulse, moxibustion must be慎 (carefully avoided). Although the fire energy is slight, its internal attacking power is strong; it can scorch bones and injure sinews, making blood difficult to restore." In treatment, selecting a physician is particularly important. A patient with deficiency is like a nation with empty reserves and scattered populace, making pacification and governance difficult. Without appointing wise and virtuous individuals to allow for quiet rest and recuperation, it is hard to ensure safety. Thus, if a physician is not enlightened, who can calm the condition with stillness, maintain stability over time, and ultimately achieve success?
Baoyin Decoction (Self-made formula). Treats True Yin deficiency with flourishing Ministerial Fire causing fever, specifically occurring before noon. (From Wu hour to Hai hour belongs to Yin.) Or presenting only with cold skin but steaming bone sensation. (Empty marrow allows fire to sink into bones, causing continuous steaming heat. Those with sweat indicate San Jiao Ministerial Fire pathology; those without sweat indicate Pericardium Ministerial Fire pathology.) Constant heat in the five centers (palms, soles, and chest), dry nose, red lips, and malar flush, bitter taste in mouth and dry tongue (all signs of internal heat). Tinnitus, vertigo, soreness and weakness of waist and knees, limb weakness, fatigue, and desire to lie down (all due to essence and blood deficiency). Dry stool and yellow-red urine (caused by internal heat). Wiry and rapid pulse, or deficient and rapid with weak force (these are definitive symptoms of Xu Lao; hematemesis or coughing blood are not necessarily present. Therefore, if there are symptoms of bleeding or coughing but without the aforementioned internal heat, deficiency signs, and rapid pulse, it is still not Xu Lao; differentiation is essential). If the disease has lasted long, with poor appetite and loose stools (indicating Spleen and Stomach injury), chills followed by fever in the afternoon, or fever extending until the hours of Mao and Chen (3-7 AM), night sweats with cool body temperature, use this formula, or modify Liuwei Dihuang Wan or Zuogui Wan.
Shu Di Huang (Prepared Rehmannia Root) [3 qian to 1 liang]. Sheng Di Huang (Raw Rehmannia Root) [Both Rehmannias tonify Kidney and benefit Yin, cultivating the root.] Mai Dong (Ophiopogon Tuber) [2-3 qian each]. Tian Dong (Asparagus Tuber) [The two Dong herbs clear Lung and descend fire, preserving the Mother element. 2 qian.] Niu Xi (Achyranthes Root) [Steeped in wine, 2-3 qian.] Fu Ling (Poria) [Guides fire downward, 2 qian.] Shan Yao (Chinese Yam) [Together with Poria to tonify Spleen, steamed, 2-3 qian. If loose stools, remove Sheng Di Huang and Tian Dong.] Yu Zhu (Polygonatum Odoratum Rhizome) [Treats deficiency-related alternating chills and fever, and all insufficiencies; used as a substitute for Ren Shen and Huang Qi.] Bie Jia (Turtle Carapace) [Superior herb for retreating labor-fever in bones and yin-deficiency alternating chills and fever.] Gui Jia (Turtle Plastron) [Tonifies Kidney Yin, retreats bone-steaming heat, roasted with酥 (crisp texture), 4-5 qian each.]
Add Yuan Rou (Longan Aril) [10 pieces]. For bone-steaming internal heat with sweat, add Gu Pi (Cortex Lycii) [2 qian]. Without sweat, add Dan Pi (Moutan Cortex) [1 qian]. For lower back pain, add Gou Qi Zi (Goji Berry) [3-5 qian] and Du Zhong (Eucommia Bark) [2 qian]. I once used pig kidney (one piece) and spinal cord (4-5 strips) decocted in water to boil the medicine, treating lumbar and spinal soreness with miraculous effect. For night sweats, add Zao Ren (Semen Ziziphi Spinosae) [Stir-fried and finely ground, 2-8 qian] and Wu Wei Zi (Schisandra Fruit) [2 fen to 1 qian]. For cough, add fresh Bai He (Lily Bulb) [1-2 liang], Kuan Dong Hua (Flos Farfarae) [2-3 qian], and Pi Pa Ye (Loquat Leaf) [3 large slices]. With phlegm, add Bei Mu (Fritillaria Bulb) [2-3 qian]. With blood, add Ou Zhi (Lotus Rhizome Juice) and Tong Bian (Child's Urine) [1 cup each]. With poor appetite, add Yi Yi Ren (Coix Seed) [Stir-fried, 5 qian to 1 liang]. If Lung channel has no heat and Lung pulse is weak upon pressure, add Ren Shen (Ginseng Root) according to dosage.
This formula uses sweet and cold herbs that nourish Water and replenish Essence as the Sovereign drug, consistent with the Nan Jing saying: "For damage to the Kidney, benefit the Essence." The Kidney meridian ascends from the Kidney, passes through the Liver and Diaphragm, enters the Lung, follows the throat, and flanks the root of the tongue. When Kidney Fire moves, Liver Fire attacks it; then it enters the Lung. The Ministers include the two Dong herbs to protect Metal (Lung) and nourish the source of generation. To prevent being too heavy and cloying, they are assisted by sweet and neutral Spleen-tonifying agents. Because of the Middle Jiao, methods for modification are prepared to perfect its function. For Yin deficiency with fire hyperactivity, this formula yields divine effects. Like the humid summer heat, when the Shang wind blows, the intense heat vanishes.
Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) [If made into a decoction, reduce the dosage]. Treats insufficient Kidney Water. (All subsequent symptoms are due to Yin deficiency.) Fever and thirst. (Yin deficiency causes fever; fluid scarcity causes thirst.) Qi stagnation with phlegm and cough. (Kidney deficiency fails to grasp Qi and return it to its root, causing Qi to stagnate upward; fire steams fluids, condensing into phlegm. Coughing indicates Water deficiency where Fire insults Metal.) Vertigo and blurred vision, deafness. (Dragon Fire flares, triggering Thunder Fire; Liver Blood deficiency causes dizziness and blurred vision; Yin deficiency with phlegm-fire rising causes deafness.) Dry throat and painful tongue, loose teeth. (Throat and tongue root are pathways of the Kidney meridian. Kidney governs bones; teeth are the surplus of bones. Withered essence and loss of moisture in gums cause tooth mobility, like bamboo shoots loosening when the root rots, but firm when wet.) Soreness and pain in waist, spine, legs, and calves. (Waist is the residence of the Kidney; the meridian runs up the inner ankle, along the medial thigh, through the spine. Exhausted essence causes soreness and pain.) Epistaxis, bright red stool, hematemesis. (Danxi said: All bleeding indicates heat syndromes caused by excessive Imperial and Ministerial Fire forcing blood out through orifices.) Night sweats and aphonia. (Awake is Yang, asleep is Yin; Yin deficiency causes sweat during sleep, indicating failure of closure. Kidney meridian does not ascend to throat/tongue, causing aphonia.) Water overflowing into phlegm. (Yin deficiency with fire movement causes boiling water to rise as phlegm.) Dysuria and retention. (Lin syndrome involves painful, dribbling urination; Bi syndrome is obstruction. Kidney controls opening and closing; deficiency leads to dysfunction. Also, Jin Gui says: Heat in Lower Jiao causes hematuria and dysuria.) Nocturnal emission and spermatorrhea. (Yin deficiency with reckless movement of Imperial Fire.) Dry and hot soles, heel pain. (Kidney meridian travels to sole and enters heel.) Irregular menstruation, amenorrhea due to blood exhaustion. (Damage to Chong and Ren vessels.) Shu Di Huang (Replenishes marrow and fills essence, 8 liang.) Yu Rou (Cornus Fruit) [Tonifies Kidney Qi, secures primordial essence; 4 liang.] Shan Yao (Enters Hand Taiyin Lung, moistens skin, clears deficient heat, supplements upper source of Water, as Metal is Mother of Water; 4 liang.) Dan Pi (Treats latent fire in Hand/Foot Shaoyin and Jueyin, 3 liang.) Fu Ling [Lightly leaching to descend Yang within Yin.] Ze Xie [Salty and leaching to descend Yin within Yin, 3 qian each.] Add Mai Dong [Moistens dry Metal and clears Water source. 6 liang.] Wu Wei Zi [Upper gathers dissipating Lung Qi, lower nourishes deficient Kidney Water, 3 liang.] Named Baxian Changshou Wan (Eight Immortals Longevity Pill); adding Ren Shen makes it a combination with Shengmai San. (Metal is source of Water; for Water deficiency, combine with Shengmai San to nourish the source of transformation. Honey pills sized like paulownia seeds, taken on an empty stomach with warm light salt water, 4-5 qian.) For fever and thirst, add Bie Jia, Hua Fen, and Er Dong. For Qi stagnation, add Chen Xiang, Sha Ren, Mai Dong. For phlegm-cough, add Bei Mu, Bai He, Mai Dong. For dry throat/painful tongue, add raw egg, Xuan Shen, Er Dong. For loose teeth, add Baxian plus Lu Rong, Pig Marrow, Long Chi, Mu Li. For leg/calf soreness, add Niu Xi, Du Zhong, Gou Qi, Gui Jia. For gum bleeding, add Mai Dong, Tong Bian, or add Ren Zhong Bai, Mai Dong, decoct and take frequently. For painful hematuria (Blood Lin), remove Yu Rou, add Er Dong, Niu Xi, Ou Zhi, Gan Cao Shao. For frequent erections and painful urethra, add Huang Bo, Zhi Mu. For painless hematuria, add Baxian plus Bai Shao, Lian Xu, Ou Zhi, Zao Lian Cao. For aphonia, add Mai Dong, raw egg. For hot soles, add Er Dong, Niu Xi, Gui Jia. The above modifications apply similarly to Baoyin Jian and Zuogui Wan.
This is a purely Yin, heavy-tasting, descending-moistening formula. Pure Yin corresponds to Kidney Qi, heavy taste to Kidney Substance, and descending-moistening to Kidney Nature. Song Dynasty Qian Zhongyang used this for children with delayed dentition, speech, walking, soft limbs, and unclosed fontanelles, and various Yin-deficiency fevers, as all belong to Kidney deficiency. Since children have tender Yang and pure Qi, he removed Gui Zhi and Fu Zi from Zhang Zhongjing's Babai Wan to solely tonify True Yin, achieving immediate results. Ming Dynasty Xue Lizhai realized that for large-formula Yin deficiency, Danxi's Bu Yin Fang was ineffective, so he substituted this, yielding immediate results. Xue's modifications were numerous; for example, removing Ze Xie and adding Huang Qi and Dang Gui to harmonize blood-nourishing properties. Because fever/thirst and urinary issues imply no further depletion, Ze Xie is removed. Adding Shengmai San generates Metal to nourish Water, following the principle of tonifying the Mother when deficient. Combining with Yigong San strengthens Earth to generate Metal, supplementing the Mother's external home, renamed Renshen Buqi Tang. Modifications are infinite, like a dragon playing in the sea. Students should understand the method and apply it creatively for endless utility. Zhao Yangkui's *Yi Guan* benefited from Xue's case records, expounding principles, connecting内外 (internal/external) and miscellaneous diseases, broadening the use of Liuwei. Examples of Yin-deficiency fever usage: Most people with True Yin deficiency fever resemble exterior patterns. Seeing high fever, red face, thirst, irritability, I treated with large doses of Liuwei Tang successfully. Yin-deficiency fire movement boils water into phlegm (heavy, turbid white foam), different from Fire-decay water overflow (clear water). Kidney-originating fire is like Dragon Fire from the sea; Liver-originating is like Thunder Fire from earth. Use Liuwei Tang to nourish Yin and descend fire (double Fu Ling, add Bei Mu, Tian Dong, Niu Xi). This treats the root of phlegm, not the branch; after treating the root, tonify Spleen to control Water. Cough is blamed on Lung, treated via Spleen, ultimately returning to Kidney. Violent cough with Qi rushing up from below navel indicates Kidney failing to grasp Qi; treat with Liuwei Tang (add Bai He, Mai Dong,少许 Chen Sha). Do not merely treat Lung. Lung governs exhalation (Master of Qi); Kidney governs inhalation (Root of Qi). Kidney is Child of Lung; tonify Child when deficient. Yin-deficiency cough starts from sexual excess damaging True Yin; Fire insults Metal. Preemptively strengthen Water with Liuwei to raise Water and descend Fire, then use Ren Shen/Huang Qi to rescue damaged Lung, combining tonification of Earth Mother. If Water is not strengthened first to suppress Fire, using Ren Shen/Huang Qi to tonify Yang will worsen Fire and damage Metal further. This is ignorance of priority. If Fire does not descend, Ren Shen/Huang Qi cannot be used, explaining the difficulty of treating Yin deficiency. Kidney Water deficiency creates Fire, creating Phlegm, creating Cough, creating Asthma. Use Liuwei with Mai Dong, Wu Wei, Niu Bei (Bei Mu, Pi Pa Ye) in large doses. Yin-deficiency asthma is near death; hope lies in Mingmen root. Good treatment uses Yuan-supporting, Zhen-connecting, settling drugs to return to origin, but not abruptly. Yin-deficiency sore throat belongs to Shaoyin disease. Shaoyin Fire rushes up like a galloping horse, blocking throat Qi, causing pain/swelling. Symptoms: internal heat, dry mouth, red face, surging saliva, wiry-rapid-weak chi pulse. Use Liuwei with Mai Dong, Wu Wei (or raw egg, Xuan Shen) in large doses. This is Chu Shi's "treat upper disease by lowering." Bleeding syndromes: Hematemesis/vomiting blood from Stomach/Liver; spitting blood from Kidney/Heart; coughing blood from Lung; saliva blood from Kidney/Stomach; epistaxis from Lung/Stomach; phlegm-blood from Spleen. Collectively, all belong to Kidney. Dry True Water causes True Fire to flare, boiling blood, causing erratic flow from orifices. Chu Shi said cold-bitter herbs kill; drinking urine saves. I believe Liuwei Tang uniquely tonifies Kidney Water, is not cold/bitter, does not damage Spleen/Stomach; long-term use raises Water and descends Fire to cure. Then use Ren Shen to rescue Lung/Stomach. Initially avoid Ren Shen to prevent aiding Yang; once Fire returns to origin, Ren Shen is permissible, especially with Yin-nourishing herbs. Yin-deficiency urinary retention: Excessive sweating dries interior; Bladder has no water. Forcing diuresis is like begging for food. Only Liuwei nourishes Water, allowing natural urination; avoid bland leaching herbs. Yin-deficiency constipation: Due to scant Kidney fluids. Use Liuwei with Er Dong, human milk, cow milk to nourish Yin and moisten dryness. Kidney is Yin, stores Essence; Yin deficiency fails to store. Liver is Yang, disperses; Yang excess fails to seal. Unsealed Fire + unstored Essence = dream emission. Use Liuwei to nourish Water; Water nourishes Wood, raising Water extinguishes Wood-Fire. Shen Shi said heart movement triggers Ministerial Fire, causing dreams and essence leakage. Treatment focuses on tonifying Kidney Water, securing Essence, calming Spirit, clearing Ministerial Fire. I detail this because people use Liuwei without understanding its purpose.
Zuogui Wan (Left-Restoring Pill). Symptoms same as Liuwei.
Shu Di Huang (Self-made, 8 liang.) Yu Rou (Steamed, 4 liang.) Gou Qi Zi, Tu Si Zi (Wine-boiled, dried.) Niu Xi (Wine-steamed, 3 liang each.) Shan Yao (Steamed with human milk.) Gui Jia Jiao (4 liang each.) Lu Jiao Jiao (Deer horn tonifies Yang, suitable for Right Kidney Qi/Essence deficiency; Elk horn tonifies Yin, suitable for Left Kidney Blood deficiency. This ingredient tonifies both. Jiezai said: Left Chi is often deficient, Right Chi often vigorous. Tonifying both keeps Fire > Water. Tonify Left, restrain Right to balance Water/Fire. I treat True Yin deficiency with Elk Horn Gel instead. 3 liang dissolved in wine. Make pills size of paulownia seeds, take on empty stomach, or make paste.) For True Yin loss with floating deficient fire (use pure Yin, tranquil agents), remove Gou Qi Zi, Lu Jiao Jiao, add Nu Zhen Zi, Mai Dong (3 liang each.). For Fire insulting Lung Metal causing dry cough, add Bai He (3 liang.). For night fever/bone steaming, add Gu Pi (3 liang.). For difficult urination, add Fu Ling (3 liang.). For dry constipation, remove Tu Si Zi, add Rong Zhong (4 liang.). For Blood deficiency with stasis, add Dang Gui (4 liang.).
This formula strengthens the Master of Water to cultivate Left Kidney Primordial Yin. For severe essence/Qi damage, aging, internal lack of True Yin failing to nourish Ying/Wei, leading to emaciation, even pure tonification may seem insufficient. Adding Fu Ling/Ze Xie (leaching) reduces tonifying power. Thus, in a group of Yin-tonics, add Gui/Luo jiao (blood/qi substances) for swift effect. Jing Yue said: I understood Yin tonification theory in middle age, expanded its meaning, created Zuogui Wan, using Liuwei's principle but not its formula, achieving immediate results. All five fluids are governed by Kidney; thus all Yin herbs can reach Kidney. Claims requiring guiding herbs show misunderstanding.
Zuogui Yin (Left-Restoring Decoction). Symptoms same as Liuwei.
Shu Di Huang (3 qian to 2 liang.) Yu Rou (1-2 qian, use less if acid-sensitive.) Gou Qi Zi (2 qian, remove if Ministerial Fire is vigorous.) Shan Yao (2 qian.) Fu Ling (1.5 qian.) Gan Cao (Crucial here, as Classics say "harmonize with sweet herbs." 1 qian.) For Lung heat with irritability, add Mai Dong (3 qian.). For Lung heat with cough, add Bai He (2 qian.). For low Blood, add Dang Gui (2 qian.). For Blood stasis with heat, add Dan Pi (2 qian.).
