Postpartum hematuria with lower abdominal pain is due to retained blood flowing into the bladder. Lower abdominal pain-free but painful and difficult urination is due to internal heat. Both are primarily treated with Xiaojie Tang (Little Thistle Decoction). *Zunsheng* uses Jiawei Shenqi Wan (Modified Kidney Qi Pill) with Cinnamon Twig (*Guizhi*) and Aconite Root (*Fuzi*) removed, plus Rehmannia Root (*Shengdi*) and burnt hair (*Fahui*). As for hematochezia (blood in stool), it may be caused by improper diet and lifestyle, or by extreme excess of the six climatic factors (*Liuyin*) and seven emotions (*Qiqing*), leading to depletion of Yuan Qi (Original Qi) and injury to the Yin collaterals. It is treated with Sijunzi Tang (Four Gentlemen Decoction) plus Rehmannia Root (*Shengdi*), Cimicifuga Rhizome (*Shengma*), Angelica Sinensis Root (*Guishen*), White Peony Root (*Baishao*), and burnt hair (*Fahui*).
Xiaojie Tang: Little Thistle Root (*Xiaoji Gen*), Rehmannia Root (*Shengdi*), Red Peony Root (*Chishao*), Akebia Stem (*Mutong*), Polygonum orientale Seed (*Puhuang*), Lophatherum Herb (*Danzhuye*), Licorice Root tip (*Gancao Shao*, raw) each 1 qian, Talcum (*Huashi*) 2 qian, Juncus Effusus Stem (*Dengxin*) 49 cun, decocted in water and taken orally. For retained blood, add Angelica Sinensis Tail (*Guiwei*) and Safflower (*Honghua*) each 1 qian; for concurrent internal heat, add Scutellaria Root (*Huangqin*) and Ophiopogon Tuber (*Maidong*, heart removed) each 1 qian.
Postpartum amenorrhea does not necessarily require medication. Milk flows upward, menstruation flows downward. If excessive blood loss occurs postpartum, amenorrhea is common. For nursing mothers, the absence of menstruation within half a year to one year is still a normal condition. If menstruation returns around half a year postpartum, the woman must be young, vigorous, and abundant in blood and qi. If menstruation does not return one to two years postpartum without other discomforts, there is no need to take medicines to promote menstruation. This is because labor has injured Ying and Wei (Nutritive and Defensive Qi), the Chong and Ren vessels are deficient, and blood and qi are depleted. Simply taking medicines to strengthen the spleen and stomach and nourish blood and qi will naturally restore flow. Forcing menstruation is like pulling shoots to help them grow.
Around half a month after childbirth, sudden carbuncles and swellings on the limbs, chest, and abdomen are due to incomplete expulsion of retained blood stagnating in the collaterals, or due to deficiency of blood and qi causing Ying Qi to fail to follow its course and reverse into the muscle tissue. If due to retained blood stasis, there is swelling, redness, pain, and a wiry, flooding, and forceful pulse. Treatment should focus on nourishing and moving blood, assisted by guiding out stasis and dispersing qi. If due to blood and qi deficiency with stagnant Ying and reversed Wei, the lesion is flat, diffuse, and the pulse is deficient, minute, and weak. Treatment should primarily greatly tonify blood and qi, such as using Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) or Bazhen Tang (Eight Treasures Decoction), to consolidate the root, support stomach qi, strengthen qi, harmonize blood, and allow the toxin to resolve spontaneously. If treated with toxic substances or cooling detoxification methods, it will inevitably fail to suppurate and ulcerate, turning into a disastrous condition.
