Ya Zhang Hou Feng (Mute Miasma Throat Wind). Its symptoms include lockjaw, inability to open the mouth, and aphonia. The treatment is the same as for Jin Hou Feng (Tight Throat Wind). Rinse with Gan Cao Tang (Licorice Decoction) to resolve the odor of Tong You (Camellia oil).
Nong She Hou Feng (Playing Tongue Throat Wind). It also presents with aphonia, but the distinguishing feature is the protrusion of the tongue, often manipulated by the patient's hand. Urgently puncture Shao Shang (LU-11). Hold Chan Su Wan (Toad Venom Pill) in the mouth. Guide out residual Tong You (Camellia oil). Blow in Jin Suo Chi (Golden Key Powder). Administer Jin Chan Su Du Wan (Golden Toad Venom Detoxification Pill). If the interior of the throat resembles pine nuts or fish scales, use a large slice of Fu Zi (Aconite Root), hold it in the mouth, and swallow the saliva. If sores like soybeans form in the throat, grind Wu Yao (Lindera Root) with vinegar and rinse with it.
Hou Ji (Throat Boil). Initially resembling a plum pit, it cannot be expectorated nor swallowed. Over time, it gradually ascends to the region of the laryngeal prominence. Apply Jin Suo Chi (Golden Key Powder). Administer Jin Chan Jie Du Wan (Golden Cicada Detoxification Pill) and Si Qi Tang (Four Seven Decoction).
Hou Liu (Throat Tumor). Resembling a longan seed, wrapped with blood vessels. Caused by heat in the Lung Meridian. Needles and knives are contraindicated. Administer Yi Qi Shu Feng Tang (Benefit Qi and Disperse Wind Decoction). Blow in Huang Lian Bing She San (Coptis, Borneol, and Musk Powder).
Suo Hou Du (Lock-Throat Toxicity). Arises at the end of Ting Hui (GB-2) and spreads to the side of Xuan Ying (Ren-22). Resembles Luo Li (Scrofula). Hinders eating and drinking. Caused by heat accumulation in the Heart Meridian. Administer Qing Xin Niu Huang Wan (Clear Heart and Calculus Bovis Pill).
Infants suffering from colds where wind-heat attacks the throat, or where heat from pox diseases (such as smallpox) accumulates and attacks upward, resulting in sore throat, swelling, obstructed phlegm and qi, cough, and aphonia, should be treated with: Ren Shen (Ginseng Root, 5 qian), Jie Geng (Platycodon Root, honey-fried, 1 liang), Gan Cao (Licorice Root, half raw half roasted, 3 qian). Decoct in water and administer.
Qing Yan Jie Du Tang (Clear Throat and Detoxify Decoction): Treats sore throat in infants during pox eruptions; can be used from onset to resolution.
Composition: Shan Dou Gen (Sophorae Tonkinensis Radix), Mai Dong (Ophiopogonis Tuber, each 1 qian), Niu Bang Zi (Arctii Fructus), Xuan Shen (Scrophulariae Radix), Jie Geng (Platycodon Root, each 7 fen), Fang Feng (Saposhnikoviae Radix, 5 fen), Gan Cao (Licorice Root). Decoct in water and administer.
"Indiscriminate location" means they can arise anywhere. They are classified separately because they do not follow specific acupoint sequences.
For carbuncles and swellings arising on the exterior of the back, observe the Shu (Transport) points. For internal carbuncles arising within the abdomen, observe the Mu (Alarm) points. One should differentiate the meridians according to these Mu points. If the point exhibits lingering dull pain, and the skin slightly elevates, it is a Yang abscess (Yong); if the skin does not elevate, it is a Yin ulcer (Ju). The pulse of the affected meridian will necessarily be full and excess.
In cases of doubt, test using the "Dian Yin Jiao Method" (Applying Yin Gum). (Yin Jiao refers to the carbonaceous residue accumulated inside a steamer over long periods). Place a small amount in the mouth; one can then identify the originating zang-fu organ and trace it to the lesion site. Knowing the precise location of pain is sufficient for treatment.
Lei Gong stated: To locate the sore, apply Yin Jiao orally, or drink raw soybean milk. If the patient does not reject this "vital energy," it is a Yang abscess (Yong). Alternatively, raise the patient's hands above their head; if sudden pulling pain radiates from the hypochondrium through the heart, it is a Yang abscess. Spit sputum into water; if it sinks, it contains pus, indicating a Yang abscess.
A rapid pulse without fever indicates an internal carbuncle. Floating and tight pulses indicate exterior conditions; deep and solid pulses indicate interior conditions; neither floating nor deep indicates involvement of the meridians. Tightness primarily indicates pain. A tight and rapid pulse suggests pus has not yet formed; if the tightness disappears while the pulse remains rapid, pus has formed. If the pulse is slippery, rapid, full, and hurried, apply purgation. If the pulse is hollow, slow, and weak, apply tonification. A long and moderate pulse is easy to treat, indicating robust Stomach Qi. A short, scattered, knotted, or intermittent pulse that does not respond to tonification is untreatable.
In the early stages, combining external moxibustion is highly effective. Use a three-qian thick slice of single-clove garlic placed thickly over the acupoint for moxibustion. Replace the garlic every three moxa cones. Continue until the area becomes insensitive to pain, which indicates the toxin has dissipated. If the internal toxin has already ulcerated, or if the lesion is located in the Liver/Gallbladder regions or at the umbilicus, only oral medication for attack/clearance is suitable; acupuncture and moxibustion are strictly contraindicated.
General Treatment Principles: Lesions in the chest/diaphragm region belong to the Upper Jiao Qi level; administer formulas such as Shen Gua Lou San (Divine Efficacy Trichosanthes Powder). Lesions in the abdominal/rib region belong to the Middle and Lower Jiao Qi-Blood levels; administer formulas such as Yi Yi Ren Tang (Coix Seed Decoction). A single swollen spot in the abdomen with fixed, unshifting pain is a Belly Carbuncle (Du Yong); use Hui Nong San (Meeting Pus Powder). Abdominal pain like needle insertion, intolerable to touch, with six pulses being flooding and rapid, indicates Yang Toxicity; use Jia Wei Gui Jin Tang (Modified Golden Precious Decoction). Internal carbuncles persisting for a long time with foul-smelling, putrid pus and cold pain in the umbilical/abdominal region require Huai Zhong Dan (Cherish Loyalty Pill). A perforation in the chest/heart region that fails to heal for a long time is a Fistula (Lou); use Bu Lou Dan (Tonify Fistula Pill).
