[Liang] Qibo said: When a girl reaches seven years of age, her kidney Qi becomes abundant, her teeth change, and her hair grows. At fourteen (two sevens), Tian Gui (a substance promoting reproductive maturity) arrives, the Ren Mai (Conception Vessel) becomes unobstructed, the Chong Mai (Penetrating Vessel) becomes flourishing, and menstruation occurs on time. "Tian" refers to Tian Zhen Qi (innate vital energy); "Gui" refers to the water of Ren and Gui (Heavenly Stems associated with Water in Five Elements theory). Ren is Yang Water, pairing with Ding to transform into Wood Qi. Gui is Yin Water, combining with Wu to transform into Fire Qi. Therefore it is said: Water and Fire are the manifestations of Yin and Yang. Women belong to the Yin category, so they receive the Qi of Gui. The Chong Mai is called the Sea of Blood, and the Ren Mai governs the uterus. When these two vessels have smooth circulation, menstrual blood gradually fills up and flows down on schedule as Tian Zhen Qi descends and interacts with it; thus it is called "Tian Gui." Typically, menstruation occurs once every thirty days, resembling the waxing and waning of the moon, never missing its timing, hence also called "Yue Xin" (Monthly Trust). During menstruation, utmost caution is required; otherwise, symptoms may resemble those of postpartum conditions. If subjected to fright or overexertion, Qi and Blood become disordered, meridians fail to circulate, often leading to consumptive diseases such as Lao Zhai (consumption). If menstrual blood rebels upward between the head, face, and limbs, it causes severe pain and restlessness. If anger injures the Liver, dizziness, hypochondriac pain, hematemesis, Luoli (scrofula), and carbuncles may occur. If menstrual blood渗 (seeps) inward, it leads to continuous dripping from orifices. All these scenarios involve invasion by the Six Evils (Wind, Cold, Summer-heat, Dampness, Dryness, Fire), resulting in myriad complications. Though the onset may be slight as an autumn hair, the resulting illness weighs heavy as a mountain; how can one not revere this?
[Xue] Blood is the essence extracted from food and drink; it harmonizes the Five Zang organs and spreads throughout the Six Fu organs. In men, it transforms into Essence; in women, it transforms upward into milk and downward into the Sea of Blood. Thus, although the Heart governs Blood and the Liver stores Blood, both are controlled by the Spleen. By tonifying the Spleen and harmonizing the Stomach, Blood will naturally generate. During menstruation, bitter-cold and acrid-dispersing medicines should be strictly avoided, and diet should follow the same principle.
[Liang] Irregular menstruation in women arises when Wind Evil invades the Bao Zhong (uterus) due to deficiency, injuring the Chong and Ren vessels and damaging the Hand Taiyang Small Intestine Meridian and Hand Shaoyin Heart Meridian. Since the Chong and Ren vessels originate in the Bao Zhong and serve as the Sea of Meridians, they are exterior-interior related to the Hand Taiyang Small Intestine and Hand Shaoyin Heart meridians, transforming upward into milk and downward into menstruation. Menstrual blood is merely the surplus of the meridians; if properly regulated, menstruation will occur on time.
[Xue] The Classics state: Food enters the Stomach, generating essence and Qi, which is transported upward to the Spleen. The Spleen scatters essences upward to the Lung, which regulates water passages and transports them downward to the Bladder. Water essences distribute throughout the body, and the Five Meridians function together. Mr. Li Dongyuan stated that the Spleen is the source of transformation and production, and the Heart commands the blood of all meridians. This is truly correct. I believe that when the Heart and Spleen are peaceful, menstruation remains normal. However, internal injury from the Seven Emotions, external invasion by the Six Evils, irregular diet, or improper lifestyle can damage the Spleen and Stomach, causing Heart Fire to flare recklessly, leading to menstrual irregularities. Some arrive early, some late; detailed explanations follow later. Since Blood is born from Spleen Earth, it is said that the Spleen controls Blood. For blood disorders, bitter and sweet agents should be used to assist Yang Qi and generate Yin Blood.
[Color Analysis]
[Zhu Danxi] Menstrual blood is Yin Blood. Yin must follow Yang, hence its red color, inheriting the hue of Fire. Blood is the companion of Qi; if Qi is hot, Blood is hot; if Qi is cold, Blood is cold; if Qi ascends, Blood ascends; if Qi descends, Blood descends; if Qi coagulates, Blood coagulates; if Qi stagnates, Blood stagnates; if Qi is clear, Blood is clear; if Qi is turbid, Blood is turbid. Often, clots appear due to Qi coagulation. Pain before menstruation indicates Qi stagnation. Pain after menstruation indicates deficiency of both Qi and Blood. Pale color indicates deficiency mixed with dampness. Erratic flow indicates Qi disorder. Purple color indicates Qi heat. Black color indicates extreme heat. Nowadays, people see purple or black blood, pain, or clots and uniformly attribute it to Wind-Cold, administering warm-hot remedies, bringing disaster swiftly. This stems from *Zhubing Yuanhou Lun* attributing all menstrual diseases to Wind-Cold, making it a common custom. One might argue: Black is the color of Water in the North. Purple is lighter than black; is it not cold? I say: The Classics state that excess brings harm, and control restores balance. Extreme heat necessarily involves Water transformation; thus, heat yields purple, and extreme heat yields black. Moreover, women tend to be stubborn and short-sighted, doubling their desires, causing Jue Yang Fire in the Zang-Fu to rise daily; is this not heat? If it were Wind-Cold, it must come from outside. Even if it does, it is rare (one in a thousand). (Cold syndromes from external pathogens cause pain upon entering the meridians. If there is no pain, stagnation eventually transforms into heat. Since cold causes coagulation, but the flowing blood is purple and black, it is not cold.)

Decoct and take with 30 pills of Bao He Wan and 20 pills of Yi Qing Wan.
[Assessing Quantity]
Women's diseases often involve fluctuating menstrual volume, timing irregularities, and pre-menstrual pain. Physicians who do not examine carefully label all as menstrual disorders. Ignoring whether Yang overcomes Yin or Yin overcomes Yang renders medication ineffective. If Yin Qi overcomes Yang, cold accumulates in the uterus, blood fails to circulate, akin to "freezing water," causing scanty periods occurring late. If Yang Qi overcomes Yin, blood flows excessively, akin to "boiling water in heat," causing profuse periods occurring early. One must harmonize Qi and Blood and balance Yin and Yang for health.
Yang overcoming Yin, excessive menstruation: Dang Gui Yin (Angelica Drink).
Dang Gui Yin suppresses Yang heat, assists Yin, and regulates meridians.
Dang Gui (slightly stir-fried), Di Huang (Rehmannia Root, steamed in wine and dried), Chuan Xiong, Bai Zhu, Bai Shao Yao, Huang Qin (equal parts).

Grind into powder, make pills with wine paste, and swallow 50-60 pills with warm water on an empty stomach.
[Zhu Danxi] A woman had a wiry and large pulse, not rapid, with a fat physique. In early summer, she felt fatigued and had heavy menstruation. This indicates weak constitution and insufficient Qi to control Blood, causing heavy flow.
Bai Zhu (1.5 qian), Huang Qi (Astragalus Root, raw), Chen Pi (1 qian each), Ren Shen (Ginseng Radix) (0.5 qian), Gan Cao (Licorice Root, roasted) (0.3 qian).
Scanty and sluggish menstruation indicates Deficiency or Sluggishness; tonify deficiency, moisten sluggishness.
[Shi] Si Wu Tang added with Kui Hua (Sunflower) treats scanty menstruation.
Si Wu Tang (4 liang), Kui Hua (1 liang). Alternative: add Hong Hua (Carthamus Flower) and Xue Jian Chou (Speranskia Tuberculata Seed).
Si Wu Tang added with Shu Di Huang and Dang Gui treats scanty menstruation with normal color.
Si Wu Tang (4 liang), Shu Di Huang, Dang Gui (1 liang each).
[Pulse Diagnosis] The Master said: A woman consulted, saying her menstruation is less than before. Why? The Master said: Did she previously suffer from diarrhea, sweating, or frequent urination? These are acceptable reasons. Why? Because loss of Jin Ye (Body Fluids) causes reduced menstruation. If menstruation were heavier than before, she would feel exhausted and complain of difficulty defecating and lack of sweat.
[Xian Qi Hou Qi - Early and Late Onset]
Wang Ziheng said: Menstruation is a regular sign, reflecting the state of Yin and Yang in the body to indicate safety or danger, hence occurring monthly. Excess or deficiency constitutes irregularity. Yang excess causes early onset; Yin deficiency causes late onset. Fluctuations, cessation, metrorrhagia, and metrostaxis are all caused by Yin-Yang imbalance.
[Zhu Danxi] Early onset indicates Blood Heat; use Si Wu Tang added with Huang Lian. In obese individuals with early onset and phlegm, it indicates Blood Deficiency with Heat; use Nan Xing (Arisaema Rhizoma), Bai Zhu, Cang Zhu (Atractylodes Lancea Rhizome), Huang Lian, Xiang Fu, Chuan Xiong made into pills.
[Xue] Early onset may be due to:
1. Spleen Meridian Blood Dryness: Use Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder).
2. Spleen Meridian Stagnation: Use Gui Pi Tang (Restore the Spleen Decoction) (see Miscellaneous Diseases: Forgetfulness section).
3. Liver Meridian Anger Fire: Use Jia Wei Xiao Chai Hu Tang (Modified Minor Bupleurum Decoction) (see Shang Han: Shaoyang Disease section).

Grind the four ingredients into powder, take 1 cun bi (measure) with wine, three times daily.
Note: Zhang Zhongjing's formula breaks hardness and promotes blood discharge, addressing cases of early menstruation with bleeding from skin/mucosa, which previous discussions missed. Differentiate presence of bleeding by checking for lower abdominal fullness/pain versus absence thereof.
[Zhu Danxi] Delayed menstruation with scanty volume: Use Chuan Xiong, Dang Gui, Ren Shen, Bai Zhu combined with phlegm-resolving drugs. Delayed with pale color indicates much phlegm; use Er Chen Tang (Two Aged Things Decoction) added with Chuan Xiong and Dang Gui. Delayed with purple color and clots indicates Blood Heat with pain; use Si Wu Tang added with Xiang Fu and Huang Lian.
[Xue] Delayed onset may be due to:
1. Spleen Meridian Blood Deficiency: Use Ren Shen Yang Rong Tang (Ginseng Nourishing Courage Decoction).
2. Liver Meridian Blood Scarcity: Use Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia).
3. Qi Deficiency and Blood Weakness: Use Ba Zhen Tang (Eight Treasures Decoction). (All three formulas see Miscellaneous Diseases: Consumption section).

One dose, decoct 2 bowls of water to 1 bowl, take before meals.
[Pulse Diagnosis] The Master said: Micro pulse indicates Qi and Blood deficiency. In young women, this may result from Blood loss. Diarrhea during lactation is acceptable. Otherwise, this is Ju Jing (Resident Menses), occurring once every three months. The Master said: Cun Kou (Inch position) pulse micro and choppy indicates insufficient Wei Qi and deficient Blood/Qi. Insufficient Wei Qi causes short breath and restlessness. Deficient Blood causes physical inversion, verbal errors, and combined Ying/Wei deficiency. Zu Yang (Foot Yangming) pulse floating and choppy indicates Wei Qi deficiency, short breath, dry throat, bitter mouth, and loss of fluids due to stagnant Stomach Qi. Shao Yin pulse micro and slow indicates lack of Essence, cold in the vagina, and stopped Blood, constituting Ju Jing (three-month cycle). Question: A pregnant woman at three months has a pulse indicating she is not pregnant, and menstruation should occur. What is the pulse pattern? How to distinguish? The Master said: Cun Kou pulse shows Wei floating and large, Ying reverse and weak. Floating/large indicates strong Qi; reverse/weak indicates little Blood. Solitary Yang calls out, Yin cannot absorb. Qi fails to interact. Wei descends, Ying exhausts. Yin becomes accumulated cold, Yang becomes gathered heat. Yang excess lacks moisture, meridians are insufficient. Yin deficiency allows Yang to go... (alternative text: solid). Thus little Blood, alternating chills and fever, dry throat, sweating, thick/frequent urination, excessive saliva. This is double deficiency with fluid leakage, proving non-pregnancy. Accumulated烦满 (fullness) blood follows one meridian per month, coming every three months. Yin excess causes discharge, named Ju Jing. (Right pulse floating/large, left pulse reverse/weak).

Qi disease affects both sexes, but is worse in women due to Blood/Qi issues. Blood follows Qi; Qi stagnation causes Blood/Qi binding, leading to irregular menstruation, abdominal pain, pre-menstrual pain, prolonged dripping with pain, radiating pain to waist/hypochondrium/back, vomiting, convulsions resembling epilepsy, chills/fever, masses, emaciation. It prevents conception and can turn into consumption if untreated.
Dai Fu'an said: Women's menstruation should arrive on time without excess, like the waning moon. Variations in timing or volume, or interruption, are called irregular.
