For those with Blood Heat, menstruation often arrives early; however, one must examine the deficiency or excess of Yin Qi. If the color is predominantly red, or purple and thick, or if the flow is excessive, and the pulse is surging and slippery, and the organ qi favors cold drinks while fearing heat, these all belong to the category of Fire. For treating Blood Heat with Fire, Qinghua Yin (Clearing and Transforming Decoction) should be used as the principal formula. If the Fire is severe, formulas such as Chouxin Yin (Drawing Out the Firewood Decoction) may also be used temporarily. However, false Fire must not be mistaken for true Fire, nor deficient Fire for excess Fire.
Qinghua Yin (See *New Formulas Eight Arrays · Indication Array*) treats postpartum fever due to Fire in women, as well as reckless bleeding due to Blood Heat, Yin deficiency, and various conditions of unclear Fire.
Ingredients: Shaoyao (Paeoniae Radix Alba), Maidong (Ophiopogonis Tuber) (each 2 qian); Danpi (Moutan Cortex), Fuling (Poria), Huangqin (Scutellariae Radix), Shengdi (Rehmanniae Radix) (each 2-3 qian); Shihu (Dendrobii Caulis) (1 qian).
Add 1.5 zhong of water, decoct until 0.7 zhong remains, and take warm shortly after meals. If bone-steaming fever and profuse sweating are felt, add Digupi (Lycopersici Caulis) 1.5 qian; if heat is severe with thirst or headache, add Shigao (Gypsum Fibrosum) 1-3 qian; if lower jiao heat causes painful and difficult urination, add Mutong (Akebiae Caulis) 1-2 qian, or Huangbai (Phellodendri Cortex), Zhizi (Gardeniae Fructus), which can be used according to the syndrome. If accompanied by external pathogen fever, add Chaihu (Bupleuri Radix) 1-2 qian.
Chouxin Yin (See *New Formulas Eight Arrays · Cold Array*) treats various conditions where Fire is blazing and tonification is contraindicated.
Ingredients: Huangqin (Scutellariae Radix), Shihu (Dendrobii Caulis), Mutong (Akebiae Caulis), Zhizi (Gardeniae Fructus) (stir-fried), Huangbai (Phellodendri Cortex) (each 1-2 qian); Zhiqiao (Aurantii Fructus) (1.5 qian); Zexie (Alismatis Rhizoma) (1.5 qian); Xi Gancao (Glycyrrhizae Radix et Rhizoma Preparatum) (0.3 fen).
Add 1.5 zhong of water, decoct until 0.7 zhong remains, and take warm shortly after meals. If internal heat is severe, taking it cold is better. If heat is in the meridians and skin/muscle, add Lianqiao (Forsythiae Fructus) and Tianhuafen (Trichosanthis Radix) to resolve it; if heat is in the Blood level and Large/Small Intestines, add Huairui (Stemonae Radix) and Huanglian (Coptidis Rhizoma) to clear it; if heat is in the Yangming channel affecting the head/face or causing restlessness and constipation, add Sheng Shigao (Gypsum Fibrosum) to descend it; if heat is in the Lower Jiao causing painful and difficult urination, add Longdancao (Gentianae Radix) and Cheqian (Plantaginis Semen) to promote diuresis; if heat is in the Yin level with insufficient body fluids, add Maidong (Ophiopogonis Tuber), Shengdi (Rehmanniae Radix), Shaoyao (Paeoniae Radix Alba) etc. to nourish; if heat is in the stomach/intestines with solid accumulation, add Dahuang (Rhei Radix et Rhizoma) and Mangxiao (Natrii Sulfas) to unblock it.
The above are the original dosages; clinically, they may be appropriately increased according to the syndrome to avoid the disease being heavy and the medicine too light, failing to achieve expected results. The remaining formulas follow this example and are not annotated separately.
Generally, heat tends to cause rapid flow and prolonged menstruation; herbs such as Xuduan (Dipsaci Radix), Diyu (Sanguisorbae Radix), Danshen (Salviae Miltiorrhizae Radix et Rhizoma), Qiangen (Rubiae Radix), and Zhizi (Gardeniae Fructus) can all be used.
If there is slight Fire and Yin deficiency leading to heavy and early menstruation, treatment should focus on nourishing Yin and clearing Fire, using Baoyin Jian (Nourishing Yin Decoction) as the principal formula.
Baoyin Jian (See *New Formulas Eight Arrays · Cold Array*) treats leukorrhea, turbid discharge, spermatorrhea, stranguria in men and women, blood-stained leukorrhea, slippery pulse with much heat, continuous hematochezia, metrorrhagia, bloody stranguria, or menstruation arriving too early. It treats all conditions of Yin deficiency with internal heat disturbing the Blood.
Ingredients: Shengdi (Rehmanniae Radix), Shudi (Rehmanniae Radix Preparata), Shaoyao (Paeoniae Radix Alba) (each 2 qian); Shanyao (Dioscoreae Rhizoma), Chuan Xuduan (Dipsaci Radix), Huangqin (Scutellariae Radix), Huangbai (Phellodendri Cortex) (each 1.5 qian); Sheng Gancao (Glycyrrhizae Radix et Rhizoma) (1 qian).
Add 2 zhong of water, decoct until 0.7 zhong remains, and take warm shortly after meals. If urine is hot or accompanied by anger-fire disturbing Blood, add Jiao Zhizi (Gardeniae Fructus Preparatus) 1-2 qian; if night fever occurs, add Digupi (Lycopersici Caulis) 1.5 qian; if Lung heat causes profuse sweating, add Maidong (Ophiopogonis Tuber) and Zaoren (Ziziphi Spinosae Semen); if Blood Heat is severe, add Huanglian (Coptidis Rhizoma) 1.5 qian; if Blood deficiency with stasis causes swelling and pain in bones/joints, add Danggui (Angelicae Sinensis Radix) 2-3 qian; if Qi stagnation causes pain, remove Shudi and add Chenpi (Citri Reticulatae Pericarpium), Qingpi (Citri Reticulatae Pericarpium Viride), Danpi (Moutan Cortex), Xiangfu (Cyperi Rhizoma) etc.; if Blood loss with slipping and chronic hematochezia occur, add Diyu (Sanguisorbae Radix) 1-2 qian or Wumei (Mume Fructus) 1-2 pieces, or Baiyaojian (Fermented Medicinal Powder) 1-2 qian, Wenha (Concha Arcae) is also acceptable; for youths or those with robust Qi and Blood, Shudi and Shanyao need not be used; for joint/bone pain or swelling, add Qinjiao (Gentianae Macrophyllae Radix) and Danpi (Moutan Cortex) each 1-2 qian.
"Early menstruation" should be judged based on the general monthly pattern; "Blood Heat" should be analyzed based on the manifestations of the entire body's Zang-Fu organs. Do not label occasional early arrival in a person who usually has irregular cycles as "early"; do not diagnose "Heat" solely based on early menstruation if the pulse and symptoms show no signs of Fire. If the pulse and symptoms show no Fire but menstruation arrives early, it is due to Heart and Spleen Qi deficiency failing to secure and control the Blood. Daying Jian (Great Nourishing Decoction), Dabu Yuansheng Jian (Great Tonifying Origin Decoction), or Wufu Yin (Five Blessings Drink) plus Duzhong (Eucommiae Cortex) and Wuweizi (Schisandrae Fructus) should be used as the principal treatment. Such cases are very common; treating them as Fire will certainly lead to error. If menstruation occurs two or three times a month, or every half-month or ten days, this is a syndrome of chaotic collapse of Qi and Blood. Treatment should be adjusted according to cold or heat, and it should not be discussed simply under the category of early menstruation.
Daying Jian (See *New Formulas Eight Arrays · Tonify Array*) treats deficiency of True Yin, Essence, and Blood, as well as delayed menstruation and scanty flow in women, pain in waist/knees/bones/joints, or Qi and Blood deficiency-cold causing heart/abdominal pain.
Ingredients: Danggui (Angelicae Sinensis Radix) (2-3 qian or 5 qian), Shudi (Rehmanniae Radix Preparata) (3-5-7 qian), Gouqi (Lycii Fructus) (2 qian), Zhi Gancao (Glycyrrhizae Radix et Rhizoma Preparatum) (1-2 qian), Duzhong (Eucommiae Cortex) (2 qian), Niuxi (Achyranthis Bidentatae Radix) (1.5 qian), Rougui (Cinnamomi Cortex) (1-2 qian).
Add 2 zhong of water, decoct until 0.7 zhong remains, and take warm shortly after meals. If Cold-Stagnation in the channels prevents Qi and Blood circulation, causing severe bone/joint pain, Zhi Fuzi (Aconiti Lateralis Radix Preparata) 1-2 qian must be added for efficacy; if leukorrhea/turbid discharge and abdominal pain occur, add Guzhi (Psoraleae Fructus) 1 qian stir-fried; if Qi deficient, add Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma); if Middle Qi deficiency-cold causes vomiting/nausea, add Chao Jiao Ganjiang (Zingiberis Rhizoma Preparatus) 1-2 qian.
Dabu Yuansheng Jian (See *New Formulas Eight Arrays · Tonify Array*) treats severe collapse of Qi and Blood in men and women, loss of spiritual mastery, and critical conditions. This is the primary formula for restoring heaven-like vitality, saving the root, and cultivating the origin.
Ingredients: Renshen (Ginseng Radix) (Tonifies Qi and Yang, serves as the main herb, use 1-2 qian for small dose, 1-2 liang for large dose); Shanyao (Dioscoreae Rhizoma) (stir-fried 2 qian); Shudi (Rehmanniae Radix Preparata) (Nourishes Essence and Yin, serves as the main herb, use 2-3 qian for small dose, 2-3 liang for large dose); Duzhong (Eucommiae Cortex) (2 qian); Danggui (Angelicae Sinensis Radix) (2-3 qian, remove if diarrhea occurs); Shanzhuyu (Corni Fructus) (1 qian, remove if sour aversion/reflux occurs); Gouqi (Lycii Fructus) (2-3 qian); Zhi Gancao (Glycyrrhizae Radix et Rhizoma Preparatum) (1-2 qian).
Add 2 zhong of water, decoct until 0.7 zhong remains, and take warm shortly after meals. If Yuan Yang deficiency causes much cold, add Fuzi (Aconiti Lateralis Radix Preparata), Rougui (Cinnamomi Cortex), Pao Jiang (Zingiberis Rhizoma Preparatus) etc. as appropriate; if Qi level is particularly deficient, add Huangqi (Astragali Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma), but do not use if Stomach Qi is stagnant; if Blood stasis exists, add Chuanxiong (Ligustici Chuanxiong Rhizoma) and remove Shanzhuyu; if slippery diarrhea occurs, add Wuweizi (Schisandrae Fructus) and Guzhi (Psoraleae Fructus) etc.
Wufu Yin (See *New Formulas Eight Arrays · Tonify Array*) treats deficiency of Qi and Blood in all five Zang organs; it is worthy of being called the supreme King's Way formula.
Ingredients: Renshen (Ginseng Radix) (Use as appropriate for Heart); Shudi (Rehmanniae Radix Preparata) (Use as appropriate for Kidney); Danggui (Angelicae Sinensis Radix) (2-3 qian, for Liver); Baizhu (Atractylodis Macrocephalae Rhizoma) (stir-fried 1.5 qian, for Lung); Zhi Gancao (Glycyrrhizae Radix et Rhizoma Preparatum) (1 qian, for Spleen).
Add 2 zhong of water, decoct until 0.7 zhong remains, and take warm shortly after meals. Or add 3-5 slices of Shengjiang (Zingiberis Rhizoma Recens); for treating combined Qi and Blood deficiency, this is the main formula. If warming is needed, add Jiang (Zingiberis Rhizoma) and Fu (Aconiti Lateralis Radix Preparata); if dispersing is needed, add Shengma (Cimicifugae Rhizoma), Chai (Bupleuri Radix), Ge (Puerariae Lobatae Radix). It adapts perfectly to any situation.
