Translation:
According to traditional theory, the Spleen (Pi) prefers dryness and dislikes dampness. If there is internal accumulation of damp-heat, combined with external invasion of wind pathogen, body fluids are consumed and become dry, while essence and blood wither and dry up. Alternatively, internal tension with external laxity may occur, leading to various eye diseases. Yang deficiency manifests as symptoms such as trichiasis (inverted eyelashes); Yin deficiency manifests as symptoms such as pupil dilation. The general principle is: if invaded by damp-heat, reconciliation (harmonizing) is key; if there is excess or deficiency of Yin or Yang, nourishing Ying (nutritive) blood and regulating Wei (defensive) qi takes precedence.
Trichiasis and entropion (curling of eyelashes inward) are all caused by indulgence in alcohol and sexual activity, and failure to avoid wind, frost, fatigue, and hardship. This leads to loose outer skin of the eyelids and tightness of the eyelid margin, causing eyelashes to turn inward. As a scraping effect gradually invades the pupil, once formed, it is difficult to treat with medication alone; applying clamps provides some relief. If regulation is lacking, blindness will surely result.
This condition often arises because eye diseases are wrongly called "fire eyes" and disregarded, or due to drinking, lust, exposure to wind and frost, and labor, without any abstinence. This results in wind pathogen invasion, loose eyelid skin, and tight eyelid margins, causing eyelashes to gradually invert. Frequent tearing and wiping inevitably follow, leading to photophobia (aversion to light). Consequently, eyelashes gradually invade the eyeball, creating cloud-like nebulae through friction. Medicinal treatment is most difficult; only when necessary is the clamping method used. If clamping is determined, topical application should be the primary method. When the clamp is about to fall off, apply medicine to the mark immediately to preserve vision; otherwise, the condition will revert, rendering previous efforts futile. Suitable formulas for oral administration and topical application are as follows:
Shigao Qianghuo San (Gypsum and Notopterygium Powder): Treats chronic eye pain, loss of vision, long-standing or recent internal and external Qi obstructions, wind-heat attacking upward, dim vision, entropion, and trichiasis.
Ingredients: Cangzhu (Atractylodis Rhizoma, soaked in rice water and stir-fried), Qianghuo (Notopterygii Rhizoma et Radix), Mimenghua (Flos Buddlejae), Baizhi (Angelicae Dahuricae Radix), Shigao (Gypsum Fibrosum), Maziren (Cannabis Fructus), Muzeicao (Equiseti Herba), Bencao (Ligustici Chuanxiong Rhizoma), Huanglian (Coptidis Rhizoma, wine-processed), Xixin (Asari Radix et Rhizoma), Jiajuhua (Chrysanthemi Flos), Jingjie (Schizonepetae Herba), Chuanxiong (Ligustici Chuanxiong Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma) in equal parts.
Preparation and Administration: Grind into fine powder. Take 2 qian per dose after meals and before sleep, mixed with honey water or clear tea.
Liuxi Yin (Flowing Qi Decoction): Treats photophobia, aversion to light, excessive discharge and tearing, difficulty opening eyes due to itching and dryness, redness and pain of the eyeball, formation of nebulae, tightness of the eyelid margin leading to entropion and trichiasis, and redness and ulceration of the eyelid margin.
Ingredients: Jingjie (Schizonepetae Herba), Zhizi (Gardeniae Fructus), Niubangzi (Arctii Fructus), Manjingzi (Viticis Semen), Xixin (Asari Radix et Rhizoma), Fangfeng (Saposhnikoviae Radix), Baijili (Tribuli Fructus), Muzeicao (Equiseti Herba), Xuanshen (Scrophulariae Radix), Renshen (Ginseng Radix), Zhen Chuanxiong (Ligustici Chuanxiong Rhizoma) in equal parts.
Preparation and Administration: Cut into pieces. Decoct with two zhong of water until reduced to eight fen. Strain and take after meals.
Jinpi Gao (Skin-Tightening Ointment)
Ingredients: Shiyan (Fossilized Elephant Bone, one pair, powdered), Shiliupi (Punicae Pericarpium), Wubeizi (Galla Chinensis) (each 3 qian), Huanglian (Coptidis Rhizoma), Mingfan (Alumen) (each 1 qian), Kuai Tonglv (Verdigris, scraped) (5 fen), Zhen Ejiao (Colla Corii Asini), Yujiao (Fish Maw Gelatin), Shuijiao (Gelatin derived from animal hide) (each 3 qian).
Preparation and Administration: Except for the gelatins, grind the other six ingredients into powder. Add three to five bowls of water into a large copper spoon. Simmer over gentle heat, stirring constantly with locust or willow branches until a thick paste forms. Just before becoming an ointment, add Bingpian (Borneolum Syntheticum) and Shexiang (Moschus), each 3 fen, finely ground and mixed evenly. Store in porcelain. Apply to upper and lower eyelids with a new brush, three to five times daily. Reapply when dry; eyelashes will naturally grow out. This method can be used in cool weather. Visible effects in three days; mild cases resolve in thirty days; severe cases show outward growth in fifty days.
Wuhui Gao (Five Ashes Ointment)
Ingredients: Qiaomai (Tartary Buckwheat Grain, burned to ash, one sheng, leached with water), Duanshi (Calculated Alum, weathered is best, 2 liang), Qingsangchai (Green Mulberry Firewood, burned to ash, one sheng, leached with water, one bowl, combined with weathered ash and boiled dry to powder for use), Baipi (Realgar, 3 qian, powdered), Bai Mingfan (White Alumen, 1 liang, smoked until complete, powdered).
Preparation and Administration: Grind together. Add ten bowls of water and boil down to one bowl. Then add weathered Duanshi and mix well. Sweep onto the eyelid margin and eyelashes with a new brush. After several applications, eyelashes will fall out. Avoid getting into the eye.
Qijie Gao (Eyelash-Raising Ointment)
Ingredients: Muguezi (Mucunae Semen, shell removed, 1 qian), Ziran Tong (Pyritum, processed, 5 fen).
Preparation and Administration: Pound into a paste, form into strips, and insert into the nose. Also, mix Shiyan powder with a small amount of Pian Nao (Borneolum Syntheticum), dilute with water, and apply to the eyelid margin.
(Li Dongyuan stated: Entropion and trichiasis with tight eyes are caused by contraction of the eyelid skin. Internal latent heat attacks, and Yin Qi moves externally. One must remove internal heat and fire pathogens. When the eyelid skin relaxes, eyelashes stand upright, and nebulae recede. Pull the inner eyelid outward manually, quickly puncture with a three-edged needle to release dark blood, and press the left fingernail against the right needle tip for immediate cure. The Mountain Dweller's Formula states: For inverted eyelashes, pluck the curled lashes, apply blood from lice several times, and it will heal.)
(Note: The condition of trichiasis is caused by stagnation in the meridians of the Spleen, Lung, and Liver, preventing mutual generation, resulting in lax eyelid skin that allows hairs to pierce inward, causing visual discomfort. Patients inevitably wipe their eyes frequently. Internal treatment fails to root out the disease; thus, clamping is necessary to ensure hairs grow outward. However, do modern people not suffer from sexual indulgence, fatigue, and improper regulation? These conditions inevitably cause nebulae, stasis, red veins, redness and ulceration of the eyelid margins, and tearing. Following this sequence of topical drops and oral medications, no case is incurable.)
Clamping Method for Eyes
Use old, brittle, thin bamboo tubes from writing brushes, split open to make clamps, about one cun long. Soak them in Danggui juice (Angelicae Sinensis Radix decoction) for a week. Then, take one turtle shell, split it, wrap the clamp in the skin, and boil until the skin rots. Remove, dry in shade, and mix with Shexiang (Moschus). Clamping becomes effective and easy. Before clamping, evert the upper eyelid to inspect. If there is stasis, drain it until blood is exhausted before proceeding. The clamp should not be high or bulky, but placed precisely on the heavy margin (eyelid edge). Carefully identify where eyelashes do not invert, then pull tightly. Apply three moxa cones (Zhuang) using small moxa rounds to the flesh outside the clamp. Do not over-moxibustion to avoid ulceration. When the dry clamp falls off, apply Guangfen (calamine) mixed with sesame oil to the marks every morning. Over time, the skin color returns to normal.
