The pulse at the left Cun position is surging and rapid, indicating flaming heart fire. The Guan position pulse is wiry and surging, indicating abundant liver fire. Both the right Cun and Guan positions exhibit wiry and surging pulses, suggesting that liver wood, carrying the momentum of ministerial fire, insults the metal it cannot overcome (lung) and controls the earth it can overcome (spleen).
When the heavenly cycle deviates from its regular path, the two luminaries (sun and moon) suffer eclipse; when the human body experiences disharmony, the eyes produce discharge and vision becomes dim. The eyes in the human body are like the sun and moon hanging in the sky: brightness is their normal state, while dimness is an abnormal change. Among all parts of the body, the eyes are the most precious, where the essence of the zang-fu organs gathers and the root of blood vessels connects. Li Dongyuan stated: According to the *Treatise on Yin and Yang Corresponding to Phenomena*, all meridians belong to the eyes. It also states: The eyes can see only when they receive blood nourishment. The essence of the five zang and six fu organs all ascends to the eyes to form their essence. The nest of essence is the eye; the essence of bone manifests as the pupil; the essence of sinew manifests as the black eye (iris); the essence of blood manifests as the general network of the eyelids; the essence of qi manifests as the white eye (sclera); the essence of muscle manifests as the constraints wrapping the eye. The essence of sinew, bone, blood, and qi combines with the meridians to form the optic nerve, which ascends to connect with the brain and exits posteriorly from the neck. Therefore, the pupil and black eye follow yin, while the white eye and red vessels follow yang. Thus, when yin and yang harmonize, clear vision is achieved. Consequently, the essence and qi of the five zang and six fu organs, twelve primary meridians, and three hundred and sixty-five collaterals all derive from spleen-earth, ascending to converge at the eyes to produce clarity of vision. Hence, the eyes are the messengers of the heart, and the heart is the residence of the spirit. If the spirit becomes chaotic and fails to reside properly, sudden exposure to unusual or strange sights may cause evil pathogens to invade the essence, leading to dispersion of essence, resulting in diplopia (seeing one object as two) or visual blurring. Due to mental distress, irregular diet, and excessive labor, the spleen and stomach become weak, and heart fire becomes excessive, causing the hundred vessels to boil and surge, with blood vessels flowing chaotically. The classics state: "Although the sun and moon are bright, if evil harms the empty orifices," this refers to such conditions. Dean Dong stated: The head and tail of the eyes, if red, belong to the heart; the entire white sclera belongs to the lung; the large round black pupil belongs to the liver; the upper and lower eyelid flesh belongs to the spleen; and the small black dot in the center resembling lacquer is actually governed by the kidney. This corresponds to the manifestations of the five zang organs. However, regarding their primary governance, the relationship of the pupil is the most significant. Why? Because the eyes are external indicators of the liver and kidney. Liver corresponds to wood, and kidney corresponds to water; water generates wood, with liver as the child and kidney as the mother. How can child and mother be separated? Therefore, when liver and kidney qi are abundant, the gaze is bright and vivid; when deficient, vision is blurred and dizzy. Red halos around the black eye, stinging pain, and floating exudate indicate liver heat. Clear tears secreted by the gallbladder, with yellowish dryness surrounding the eyes, indicate liver deficiency. Dilated pupils, pale and deviated, indicate kidney deficiency. Scorching small pupils or those with a slight yellow tint indicate kidney heat. Furthermore, white sclera with redness or red veins indicates heat in the lung. Redness or acne-like spots on the upper and lower eyelids or between the eyes indicate heat in the spleen. Moreover, the eyes are wrapped in thin membranes containing water, illuminating all directions. Tracing back to the source, without the water of Tianyi (kidney water), who would govern the eyes? Analyzing further: Tension and traction, blue-black eyes with white eyelids, itching with clear tears, no redness or pain, is called Wind Eye. Protrusion of the black eye, hard and swollen red eyelids, sticky discharge and tears, internal heat with stinging pain, is called Heat Eye. Turbid tearing, soft and swollen eyelids, upper congestion and blurriness, soreness and slight redness, is called Qi Eye. If wind and heat combine, there is itching and floating redness; if wind and qi clash, there is itching, astringency, and heaviness. Blood heat aggregation produces conditions like skin overgrowth, red filaments, and hordeolum. When qi and blood do not reach, conditions like monocular blindness, ptosis, night blindness, and visual obstruction occur. Pale purple with hidden red indicates deficiency heat; bright red with crimson indicates excess heat. Exposed canthi growing pterygium indicate abundant heart blood and heat. White sclera with red membranes like paper umbrellas indicate qi stagnation and blood coagulation. In heat syndromes, the pupil bulges inward, the white sclera is moist, and the color floats and is red. In cold syndromes, the pupil is green-blue, the white sclera is withered, and the qi sinks and is turbid. If eye heat persists and is invaded by wind-cold, redness and ulceration occur. If the eye is not red but painful due to phlegm-fluid retention, it causes pain. Unsmooth liver qi combined with heat causes photophobia. Accumulated heat damaging the eyelids causes them to close. Liu Hejian stated: Disease in the fu organs is exterior syndrome, requiring wind-dispelling and heat-clearing; disease in the zang organs is interior syndrome, requiring blood-nourishing and spirit-calming. Sudden blindness, blurriness, films, discharge, and spots entering the eye indicate wind-heat. Another view holds that spots entering the eye indicate abundant liver qi manifesting at the superficial level, suitable for dispersing via exterior-releasing methods. Blurred weakness, unwillingness to look, internal cataracts with black flowers, and dilated pupils are interior syndromes caused by blood deficiency and spiritual fatigue/kidney deficiency, suitable for nourishing blood, replenishing water, and calming the spirit. Observing previous physicians' treatments, the general approach prioritizes cooling the liver, clearing the heart, regulating blood, and smoothing qi. One should not rigidly adhere to "water generates wood" and over-supplement. Kidney water dislikes dryness; even in deficiency, one merely uses herbs like Dang Gui and Shu Di Huang to moisten. Mild cases can be warmed and tonified. Moreover, spleen generates dryness, while liver prefers moisture. Ancient formulas often use Xing Ren, Gan Shi, Yi Tang, and Sha Mi as assistants, clearly intending to moisten and benefit. Since the spleen is the head of all yin organs and the eyes are the masters of blood vessels, spleen deficiency causes the essence of the five zang and six fu organs to lose their direction and fail to illuminate the eyes. The heart is the monarch fire, governing the spirit, preferring stillness and peace. Ministerial fire acts on its behalf; it is the fire of the pericardium, governing the hundred vessels, all nourishing the eyes. Labor and reckless movement, combined with evil invasion damaging the vessels, give rise to various diseases. Treating leukoma requires nourishing blood and calming the spirit, but must also regulate the spleen and stomach, which is the excellent method. Regarding removing leukoma, it is particularly critical. Leukoma arises from the lung; lung heat causes mild blurriness or severe leukoma. Some say Pearl Leukoma, shaped like broken rice, is easy to disperse; Plum Blossom Leukoma, shaped like plum blossoms, is difficult to eliminate. Although leukoma arises from heat, treatment应先 remove leukoma then clear heat, because extreme heat generates leukoma; if heat is cleared first, blood coagulates and leukoma cannot be removed. Patients with red eyes given too many cooling medicines and washed with water find the water congeals quickly; the eye is merely a mass of water. Water is clear, so one should not rigidly rely on washing. Causes include emotional imbalance, excessive desires, eye strain, excessive crying, exposure to cold and wind, sun exposure in summer, avoiding smoke and fire, and spicy foods. These diseases originate in the zang-fu organs. Can one rely solely on topical washing? One must investigate the cause, understand the root and branch, analyze the vital energy to regulate it, and balance it with appropriate medications. Patients should sit quietly, clarify their spirit, control diet, abstain from sexual desire, focus inward, reduce external distractions, and cherish their eyesight, ensuring health.
